맞춤형 당뇨교육이 인슐린요법을 하는 제2형 당뇨환자의 혈당조절과 자가간호에 미치는 효과 (The Effects of Tailored Diabetes Education on Blood Glucose Control and Self-Care)

  • Purpose: The purpose of this study was to test the effects of tailored diabetic education on blood glucose control and self-care for patients with type 2 diabetes on insulin therapy. Methods: The participants were 60 patients (experimental group: 30, control group: 30) with type 2 diabetes on insulin therapy. The patients were being seen at a university hospital in Seoul, Korea. Group diabetic education and tailored diabetic education were given to the experiment group while group diabetic education only was given to the control group. Data were collected before and three months after the education. $X^2$ test, t-test, and ANCOVA were used to analyze the data. Results: No significant differences in postprandial (PP2hrs) glucose and HbA1c levels were found between the two groups. Participants in the experiment group showed statistically significant differences in the area of self-glucose test, management of insulin injection, and life style change compared to those in the control group. Conclusion: The results indicate that tailored education for patients with diabetes on insulin therapy improve self-glucose test, management of insulin injection, and life style. Therefore it is suggested that tailored education can be applied in diabetic education to improve self-care.

  • Objectives: This study is based on Public Care Service for Chronic Diseases in Incheon Metropolitan City featuring Health-Point Service. It is objective to evaluate whether the service could improve self-management ability of hypertension and diabetes patients. Methods: The knowledge about diabetes and hypertension, which can be improved by educational intervention, is considered as one of the most important factor for self-management ability. While the education service have been conducted, 194 patients who have been diagnosed as hypertension and diabetes within 3 months were included. They completed questionnaires two times within 4 months period, which give information about the knowledge, service registration and education experiences. For further inspection, 396 patients who had been diagnosed formerly were studied by cross-sectional analysis. Results: Regardless of service registration or educational intervention, there wasn't statistically meaningful difference between the initial and the follow-up assessments on the knowledge. However, when investigating formerly diagnosed patients, educated patients had better knowledge on hypertension compared with non-educated patients and registered patients, educated patients had the better knowledge on diabetes compared with non-registered patients, non-educated patients. Additionally, analyzing each questions, educated patients got higher grades in questions about definition of hypertension, diabetes managements which they can practice in daily lives. Conclusions: It is probable that long-term service registration and education experiences could improve the knowledge and self-management ability.

  • Purpose: This study was performed to explore the effects of an admission-education program on knowledge, self-efficacy, self-care and glucose control in type 2 diabetes patients. Method: A single group pre-post test design was used. Twenty-one patients participated in this program during 6 days which consisted of education and practice about exercise, diet, self-care and prevention of complications. Variables at baseline, 3 months, and 6 months were measured. Results: Knowledge level, self-care level and self-efficacy were significantly increased over time after the admission-education program. The $HbA_1C$ level and glucose level in urine were significantly decreased over time after the admission-education program. Conclusion: The admission-education program can increase levels of knowledge, self-care, self-efficacy and decrease glucose levels. Therefore, this admissioneducation program was shown to have a positive effect on the management of diabetes.

수중운동 프로그램이 지역사회 당뇨병 환자의 자기효능감과 자가간호행위에 미치는 효과 (Effects of the aquatic exercise program on diabetes self efficacy and self care behaviors in diabetic patients in community)

  • 본 연구는 당뇨병 자가관리교육이 포함된 수중운동 프로그램이 지역사회 당뇨병 환자의 자가간호행위와 자기효능감에 미치는 효과를 알아보기 위한 비동등성 대조군 유사실험연구이다. 연구대상자는 D광역시 W 보건진료소에 등록된 당뇨병 환자 중 수중운동 프로그램에 참여한 실험군 26명과 참여하지 않은 대조군 25명이다. 프로그램은 주 2회, 80분으로, 당뇨병 자가관리교육 30분과 50분의 수중운동 프로그램으로 구성되었다. 본 프로그램 후 공복혈당(p=.047), 당화혈색소(p<.001)는 유의하게 감소되었고, 당뇨병 자기효능감(p<.001), 당뇨병 자가간호행위(p<.001)는 유의하게 증가하였다. 본 연구결과로 자가관리 교육을 포함한 수중운동 프로그램이 자기효능감과 자가간호행위를 향상시켜 당뇨병 관리에 중요한 지표인 공복혈당과 당화혈색소를 완화하는데 효율적임을 알 수 있었다. 지역사회에서 당뇨병 유병율을 낮추고 합병증을 예방하기 위해 자가관리 교육이 포함된 수중운동 프로그램 상시운영이 필요한 것으로 사료된다.

노인 당뇨병 환자를 위한 동기강화상담 당뇨병 자기관리 프로그램의 개발 및 효과 (Development and Effects of a Motivational Interviewing Self-management Program for Elderly Patients with Diabetes Mellitus)

  • Purpose: This study was conducted to develop and test the effects of a motivational interviewing self-management program for use with elderly patients with diabetes mellitus. Methods: A non-equivalent control group pretest-posttest design was used. The participants were 42 elderly diabetic patients (experimental group: 21, control group: 21). The motivational interviewing self-management program for elders with diabetes mellitus developed in this study consisted of a 12-week program in total (8 weeks for group motivational interviewing and education and 4 weeks for individual motivational interviewing on the phone). Data were collected between February 13 and May 3, 2013 and were analyzed using t-test, paired t-test, and repeated measure ANOVA with SPSS/WIN 18.0. Results: For the experimental group, significant improvement was found for self-efficacy, self-care behavior, glycemic control and quality of life (daily life satisfaction, influence of disease) as compared to the control group. Conclusion: The study findings indicate that the motivational interviewing self-management program is effective and can be recommended as a nursing intervention for elderly patients with diabetes mellitus.

노인의 당뇨병 관리 자기효능감 측정도구 개발 및 평가 (Development and Validation of the Diabetes Management Self-efficacy Scale for Older Adults (DMSES-O))

  • Purpose: The purpose of this study was to develop and validate a diabetes management self-efficacy scale for older adults (DMSES-O). Methods: A preliminary DMSES-O of 22 items was derived from a literature review and seven domains of self-management behaviors. Content validity was confirmed by experts in diabetes self-management education. To test the reliability and validity of the DMSES-O, data were collected from 150 older adults with type 2 diabetes. The data were analyzed using exploratory factor analysis, and Cronbach's ${\alpha}$ and Pearson's correlation coefficients were calculated. Results: From the exploratory factor analysis, 17 significant items in six subscales were derived. Factors derived were named "problem solving for hypoglycemia and self-monitoring blood glucose," "problem solving for hyperglycemia," "coping with psychological distress and taking medication," "reducing risks of diabetes complications," "appropriate exercise," and "healthy eating." The criterion-related validity of the DMSES-O was established by its correlation with the Summary of Diabetes Self-care Activities Questionnaire. Cronbach's ${\alpha}$, a measure of internal consistency, was .84 for the overall scale and ranged from .54 to .80 for the subscales. Conclusion: The DMSES-O is a reliable and valid instrument to measure selfefficacy for diabetes self-management among older adults.

농촌 지역 노인 당뇨병 환자의 자기관리 지식, 사회적 지지, 교육요구도에 관한 융합연구 (Convergence Study on Diabetes Self-Management Knowledge, Social Support, Educational Needs of the diabetes in rural elderly)

  • 본 연구는 농촌 지역 노인 당뇨병 환자의 자기관리 지식, 사회적 지지, 교육요구도의 융합적인 요인을 파악하여 노인 당뇨병 자기관리를 위한 중재방안을 모색하기 위한 서술적 조사연구이다. 연구대상은 U시 소재 군, 읍면, 보건소 및 보건지소에 당뇨병 관리 대상자로 등록되어 약물 치료 기간 6개월을 경과한 만 65세 이상 노인 115명을 대상으로 조사기간은 2016년 8월 24일부터 10월 31일까지였다. 연구결과 당뇨병 자기관리 지식은 성별, 교육수준, 당뇨병 유병기간, 당뇨교육 경험, 가족의 당뇨교육 참여 경험, 지각된 건강상태에 유의한 차이가 있었고(p<.05), 사회적 지지는 결혼 상태, 당뇨교육 경험, 당뇨 교육요구도는 성별, 당뇨병 유병기간, 당뇨병 자기관리 어려움의 인식정도에서 유의한 차이가 있는 것으로 나타났다(p<.05). 당뇨병 자기관리 지식과 사회적 지지, 사회적 지지와 교육요구도는 유의한 정(+)의 상관관계(p<.05)가 있는 것으로 나타났다. 본 연구결과를 바탕으로 노인 당뇨병 환자의 요구가 반영된 융합적인 자기관리 프로그램이 개발되기를 기대해 본다.

노인 당뇨병 환자의 자가간호, 가족지지, 우울 (Self-care, Family Support and Depression in Elderly Patients with Diabetes Mellitus)

  • Purpose: This study was done to investigate the degree of self-care, family support and depression, and relationship among these variables for elder with diabetes mellitus. Method: Participants were 202 diabetic patients, 65 years or over, living in Seoul, Korea. Data were collected using the self-care tool for diabetic patients by Kim (1996), the family support tool for diabetic patients by Park (1984), and Korea's BDI scale by Lee (1995). Results: Of the patients, 43.1% showed HbAlc levels than higher 7%. The highest mean score was for self-care for medication compliance, and the lowest for blood glucose testing compliance. Factors affecting self-care were employment, education, HbAlc level, diabetic self-care education and complications. Factors affecting family support were living with family, diabetic self-care education, hospitalization and complications. Factors affecting depression were gender, living with family and complications. All of these factors were significant. Patients experiencing depression were 16.8% of patients. There was a significant positive correlation between self-care and family support, and significant negative correlations between self-care and depression, and family support and depression. Conclusion: For more effective management of diabetes mellitus in elders, improvement in self-care compliance, and family support are needed.

한국 성인 2형 당뇨병 환자를 위한 당뇨 중재 프로그램 효과에 대한 메타 분석 (Meta-Analysis of the Effect of Diabetes Intervention Programs for Korean Adults with Type 2 Diabetes)

  • This study aimed to provide directions and implications for a future program by analyzing studies on diabetes programs from 2000 to 2020. Among the studies with control and experimental groups, the selected studies included ones that provided intervention to patients with diabetes and ones that contained descriptive statistics. Sixteen studies were selected to verify the effectiveness and homogeneity of the data coding meta-analysis. The overall effect size in the diabetes program combined estimate was 0.398 (95% CI: 0.268, 0.425, p=0.000). Among the dependent variables, fasting blood glucose (-0.616) and glycated hemoglobin (-0.442) showed median effect sizes, but the effect of fasting blood glucose was not statistically significant. In terms of the study design, non-randomized control trials (NRCTs) (-0.543) was more effective than randomized control trials (RCTs) (0.719). Among, the counseling and self-management program (-3.241) showed a very large effect size. Furthermore, the cognitive-behavioral (-0.828) and self-management (-0.482) programs were also found to have a positive effect on lowering fasting blood glucose. As the importance of diabetes management increases, further studies based on RCT should be actively performed, and differentiated and specialized diabetes intervention plans need to be established.

당뇨병 환자 교육에 따른 지식, 자기효능감 및 자기관리행위의 변화: 세종특별자치시 고혈압·당뇨병 등록교육센터 등록자를 중심으로 (Improvement of Knowledge, Self-Efficacy and Self-Care Behaviors among Diabetic Patients participated in the Education Program of Sejong Center for Hypertension and Diabetes Management)

  • 본 연구는 2014년 5월부터 2015년 5월까지 세종특별자치시 고혈압 당뇨병 등록교육센터에서 교육 받은 당뇨병 환자 39명을 대상으로 교육 프로그램의 효과를 평가하고자 실시되었다. 프로그램은 제2형 당뇨병에 대한 질환, 영양, 운동 등에 관한 내용으로 2일 동안 2회의 세션(세션 당 45 분)으로 구성되었다. 교육 전에 당뇨병에 관한 지식, 자기효능감, 자기관리행위 등을 평가하였고, 교육 직후에 지식과 자기효능감 평가, 교육 후 12주째에 자기관리행위를 평가하여 다음과 같은 결과를 얻었다. 당뇨병 지식 점수(10점 만점)는 프로그램 전후 평균 5.72점에서 8.21점으로 유의하게 증가하였고(p<0.001), 자기효능감 점수(10점 만점)는 프로그램 전후 6.88점에서 8.16점으로 유의하게 증가하였다(p<0.001). 자기관리행위 점수는 프로그램 전 5점 만점 중 3.59점이었던 것이 프로그램 종료 12주째에 4.35점으로 유의하게 증가하였다(p<0.001). 본 프로그램은 당뇨병 지식, 자기효능감, 자기관리행위 등의 향상을 통해 등록환자의 효과적인 당뇨병 관리에 기여할 수 있을 것으로 사료된다.
