A study on Measurement and Improvement of Indoor Air Quality in Dental Clinic

  • The purpose of this study is to propose a management method to maintain the pleasant indoor air quality of the dental clinic by measuring and analyzing the indoor air quality of the dental clinic. The measurement was conducted in two rooms, a lobby where many residents stay in the reception room for waiting for medical treatment, and a VIP room where treatment activities are mainly performed. Measurement items are Temperature, Humidity, $CO_2$, CO, $NO_2$, $CH_2O$, VOC, $PM_{10}$ and measurement were taken on April 27, 2018. As a result of analyzing the temperature and humidity of the dental clinic, it was analyzed that the average indoor temperature was maintained at $25^{\circ}C$ and the humidity was kept at around 50%, maintaining proper indoor temperature and humidity environment. $CO_2$ was 855ppm in the VIP Room, which satisfied the maintenance standard. In the case of the lobby, it was analyzed to be 1,160ppm, which exceeded the maintenance standard and it is judged that the carbon dioxide generated by the respiration of the people staying in the lobby is the main reason. The mean concentration of formaldehyde in the VIP room was analyzed as $436{\mu}g/m^3$, exceeding the maintenance standard, and $2,100{\mu}g/m^3$ for the VOC exceeded the recommended standard. It was analyzed that the concentration was relatively higher due to the use of disinfectant and other drugs. The mean concentration of PM-10 in the lobby was analyzed as $65{\mu}g/m^3$ and it was analyzed that it satisfied the maintenance standard. To maintain a pleasant indoor air quality in a dental clinic it is necessary to minimize the effects of formaldehyde, VOC, $CO_2$ in the VIP rooms and lobby. For this purpose, the entire ventilation system and air purification system of the dental clinic should be installed. In case of the VIP room, local exhaust ventilation should be installed and workers should wear personal protective equipment.

치위생과 학생의 인성과 대인관계 (Personalities and interpersonal relationships of dental hygiene students)

  • 본 연구는 치위생과 학생을 대상으로 인성과 대인관계가 어느 정도 인지 파악하고자 조사하였다. 조사는 2021년 4월 1일부터 6월 30일까지 진행되었다. 연구결과 인성은 3.42점, 대인관계는 3.46점이었으며, 일반적으로 대인관계 만족도와 대학생활 만족도에서 인성과 대인관계 하위영역 모두에서 통계적으로 유의한 차이를 보였다(p<.05, p<.001). 임상실습 경험이 있을수록 대인관계 중 친구관계와 교수관계에 대한 점수는 낮았다. 인성의 하위영역 중 도덕성과 사회성이 가장 높은 양의 상관관계(r=.674), 대인관계에서는 친구관계와 교수관계가 높은 상관관계(r=.606)로 분석되었다. 따라서 예비 치과위생사인 치위생과 학생들의 전인적 성장과 치과의료 현장 적응을 위해서는 가장 기본이 되는 인성과 대인관계 함양에 대해 대학에서도 장기적인 전략과 지속적인 교육적 접근이 필요하며, 치과위생사의 직무와 연계한 다양한 프로그램 개발 및 적용될 수 있는 방안이 모색되어야 할 것이다.

Characteristics of noise generated during treatment in dental clinic

  • 본 연구는 치과병원에서 진료 시 발생하는 소음특성을 NR-곡선, PSIL에 적용하여 소음수준 및 적절한 대화거리를 산정한 결과를 제시하고자 실시하였다. 치과병원에서 치료 시 소음특성을 분석한 결과 소음에 의한 건강 보존한계 값인 60dB(A)을 초과하고 고주파 성분의 소음도가 높은 것으로 분석되었다. NR곡선에 적용하여 평가한 결과 일부 치료 시 작업장 소음기준을 초과하고 근무자와 환자 사이에 이루어지는 대화에 대한 회화방해도 수준을 분석한 결과 보통크기의 소리일 경우 0.13m~0.42m, 큰 소리일 경우 0.26m~0.85m 정도 인 것으로 분석되어 환자와 근무자 간의 정확한 의사전달을 위하여 가급적 1m 이내의 거리에서 대화가 이루어지는 것이 바람직 할 것으로 판단된다. 병원의 의료서비스 질의 향상을 도모하고 치과공포를 감소시키기 위하여 근무자에게 방음보호구를 지급하고 소음 발생원(진료 시 사용되는 치료기기) 및 수음원(환자 및 근무자)에 대한 방음대책도 필요 할 것으로 판단된다.

치과위생사의 스트레스 탄력성, 분노표출, 직무만족이 삶의 질에 융합적으로 미치는 영향 (Dental Hygienist's Stress Resilience, Anger Expression, Job Satisfaction Convergence Effect on Quality of Life)

  • 본 연구는 치괴위생사의 분노표출, 스트레스 탄력성, 직무만족, 삶의 질의 융합적 관계를 규명하기 위해 대구, 경북지역의 치과위생사 202명을 조사하였다. 자료는 SPSS WIN 18.0통계프로그램으로 기술통계, t-test, ANOVA, 상관분석을 이용하여 분석하였다. 주요결과는 분노표출은 평균 1.50이며 병원급, 직업동기유발이 있는 경우, 자부심이 없는 경우, 높은 수입, 금주가 높았다. 스트레스 탄력성은 평균 3.09이며 음주를 하지 않은 경우 높았으며 직무만족은 평균 3.24이며 자부심이 있는 경우, 금주가 높았다. 삶의 질, 분노표출, 스트레스 탄력성, 직무만족의 상관분석 결과 삶의 질과 스트레스 탄력성과 직무만족은 양의 상관관계(p<.05). 스트레스 탄력성과 직무만족은 양의 상관관계이다(p<.05). 총괄적으로 치괴위생사의 삶의 질을 높이고 분노표출을 줄이기 위해서 직무만족과 스트레스 탄력성을 높이기 위한 정보제공과 환경적인 지원이 되어야 할 것이다.

치과위생사의 이직과 직무스트레스와의 관련성 (Relationship between Turnover and Job Stress of Dental Hygienists)

  • 본 연구는 치과진료기관의 주된 근무인력인 치과위생사들이 지각하는 직무스트레스를 파악하고 이직행태가 직무스트레스에 미치는 영향을 살펴봄으로써 직무만족도 향상을 위한 기초자료를 제공하고자 2009년 3월에서 5월까지 울산 경남 지역권내 치과병 의원에 근무하는 치과위생사 214명을 대상으로 분석한 결과 다음과 같다. 대상자의 업무 환경적 특성은 진료기관은 의원이 57.5%, 병원이 42.5%였고, 경력은 1~3년이 42.5%로 가장 많았고, 이직경험이 있는 치과위생사가 38.3%였으며 이직횟수는 1회가 45.1%로 가장 많았다. 보수는 130~149만원이 33.6%로 가장 많았고, 근무시간은 8~9시간이 49.5%로 가장 많으며 주된 업무는 일반진료업무보조가 70.1%로 가장 많았다. 연령이 낮을수록 부적절한 보상에 스트레스가 많았으며 학력이 높을수록, 기혼인 경우 직무요구에 따른 스트레스가 많았다. 이직경험에 따라서는 이직횟수와 직장문화가 관련성이 있었으며 업무환경적 특성에서는 진료기관별 직무요구와 관계갈등이, 보수에서는 관계갈등이, 근무시간에서 조직체계가, 주5일제 근무여부에서 직무요구가 관련성이 있었다. 치과위생사의 이직경험과 내부인력과의 관계갈등 스트레스에 영향을 미치는 것으로 나타났으며 보수가 적을수록, 치과위생사의 수가 많을수록 스트레스가 많은 것으로 나타났다.

Relationship among necessity, importance of creative teaching methods, and strengthening core competencies

  • 본 연구는 대학에서 창의적 교수법의 필요도와 중요도에 대한 인식과 핵심역량과의 관련성을 알아보고자 실시하였다. 네이버 온라인 설문지를 이용하여 자발적으로 연구 참여에 동의한 총 142명을 최종 연구대상자로 선정하여 조사하였다. 창의적 교수법의 중요도, 핵심역량의 하위요인인 현장실무역량, 문제해결역량, 융복합역량, 의사소통역량, 직업윤리, 공동체의식과 창의적 교수법 필요도와 관련성이 있는 것으로 나타났다. 대학교육은 현장에서 실제 할 수 있는 능력인 핵심역량 향상에 초점을 맞추어 액션러닝, PBL 등과 같은 창의적 교수법을 적극적으로 도입하여 적용해야 할 것이다.

  • A comparative study was conducted to evaluate the relationship of investing medium to the amount of vertical occlusal changes and to the differences of surface smoothness during denture construction. Three groups of 20 dentures, 30 sets of upper and lower were fabricated of conventional heatcuring acrylic denture base resin, using silicone-gypsum molding techniques, with or without covering the occlusal surfaces of the teeth by artificial stone and all-gypsum molding techniques. The distance between the two reference points indented by 1/2 round bur on the upper and lower frontal surfaces of each articulator were measured and recorded before processing and again after processing and remounting of each denture on the articulator. The differences between the two recordings indicated the amount of vertical opening during denture processing. The difference of surface smoothness were investigated and determined by 3 observers continual comparing of the two randomly selected dentures with each other, which were seperately selected as pairs from the different two groups of 20. The results obtained were as follows: 1. During resin processing no statistically significant differences of the amount of vertical occlusal changes were detected between any of the two groups of two silicone-gypsum and one allgypsum molding techniques, although the amount of vertical opening was somewhat increased when silicone-gypsum molding technique was used. 2. Surface smoothness of the processed denture was makedly by increased when silicone-gypsum molding technique was used.

미산성 차아염소산수의 S. mutans와 A. actinomycetemcomitans에 대한 살균 효과 (Antimicrobial and Cell Viability Measurement of Hypochlorous Acid against Streptococcus. mutans and Aggregatibacter. actinomycetemcomitans)

  • Purpose : Hypochlorous acid (HOCl), a major inorganic bactericidal compound of innate immunity, is effective against a broad range of microorganisms. In particular, HOCl is well-known as a non-antibiotic antimicrobial substance. However, effects of HOCl as an antimicrobial agent are still needed to study these functions against various specific type of microorganisms. In this study, we investigated the antimicrobial effect of hypochlorous acid (HOCl) in S. mutans and A. actinomycetemcomitans to cause dental caries and periodontal disease. Experiments were conducted to observe whether HOCl become effective replacement of disinfectant. Methods : To observe antimicrobial effect of HOCl, stabilized HOCl is prepared in the form of a physiologically balanced solution in pre-conditioned and post-conditioned HOCl solution. As a control, commercially available disinfectant MAXCLEAN was used as positive control. Moreover, S. mutans and A. actinomycetemcomitans distribution in gagrin, filtered tap water, and culture media. Cell viability were measured by viable cell count methods and disk diffusion test. Results : Our results showed that treatment of HOCl have no effect against antimicrobial effect compare to control group especially gagrin in disk diffusion test. HOCl tended to reduced viability against S. mutans in group of post-conditioned than pre-conditioned of HOCl solution however, there was no significant difference as well as no effect in A. actinomycetemcomitans. Conclusion : HOCl showed tendency to reduce viability against S. mutans in group of post-conditioned of HOCl solution and no effect of antimicrobial effect. Although HOCl is well known as effective against a broad range of microorganisms, HOCl seems to have diversity following type of species to be used as antimicrobial drug following our results. Therefore, it is necessary to be rigidly controlled and regulated in using HOCl solution clinically.

도재-금속의 결합 강도에 미치는 비금속 합금의 열처리 효과 (The effect of preheat treatment on ceramic to metal bond strength)

  • In dental prosthetics, the application of metal-ceramic restorations has steadily increased since their introduction. This is due to excellent esthetics in combination with high mechanical stability. In order to optimum bond strength between metal and ceramics, controlled oxidation of metal substructure is essential factor. Beryllium containing and beryllium free Ni-Cr alloys for metal-ceramic restorations were evaluated for the metal-ceramic bond strength by changing heat treatment for oxide formation. A mechanical three-point bending test was employed to evaluate the interfacial bond strength of metal-ceramic. In each metal, plate type specimens were used for mechanical three-point bending test. With Ni-Cr alloys for metal ceramics, mechanical three-point bending test showed that double degassing was more available preheat treatment method than another. It was found that beryllium containing Ni-Cr alloys are more effective than beryllium-free for metal-ceramic bond strength.

  • 3-dimensional information for anatomic stucture plays a role as integral part in clinical aspect of dental practice. CBCT(cone beam computed tomography) has been accepted as useful diagnostic tool offering Volume data and images for evaluating teeth and jaws in lower radiation dose than conventional CT. CBCT equipment is essential for the quality assurance of it to ensure continued satisfactory performance and result of adequate images. Dental practitioner and oral and maxillofacial radiologist should have a responsibility and critical thinking to deliver this technology to patients in a responsible way, so that diaganostic value is maximised and radiation doses kept as low as resonably achievable. CBCT imaging modality should be used only after a review of the patient's health and imaging history and the completion of a thorough clinical examination. Clinical guidelines are systematically developed statements to assist practitioner and patient decisions about appropriate health care for specific clinical circumstances Dental practitioners should prescribe CBCT imaging only when they expect that the diagnostic yield will benefit patient care, enhance patient safety or improve clinical outcomes significantly. Knowledge of patient dose is essential for clinicians who are making the decision regarding the justification of the exposure. There are some limitation in the measurement of patient dose in CBCT for the approval and adaptation of conventinal methodolgy in CT. It is also important to ensure that doses are optimised and in line with any national and international guidelines. The higher radiation doses of CBCT compared with conventional radiography, mean that high standards must be maintained. The Quality Assurance(QA) programme should entail surveys and checks that are performed according to a regular timetable. QA programme should be maintained by staff to ensure adherence to the programme and to raise its importance among staff.
