방문건강관리인력의 구강건강지식과 실천 (Oral health knowledge and behavior of visiting health care personnel)

  • 본 논문은 공공의료보건기관에서 근무하는 방문건강인력의 구강보건에 대한 전문성 제고의 목적에서 구강건강 지식 및 실천에 대해 조사하였다. 연구 결과는 다음과 같다. 첫째, 구강건강지식은 치주질환 67%, 치아우식증 68%, 일반치과지식은 68.4%이상의 정답률을 보였다. 둘째, 구강건강실천은 예방목적의 치과방문에서 50%이상, 치료를 위해서 60.1%가 치과방문을 하는 것으로 조사되었다. 치석제거는 46.6%가 정기적으로 받는 것으로 나타났고, 치과치료이용을 89.6%가 종합병원·치과(병)의원을 이용하는 것으로 조사되었다. 칫솔질의 경우 3회 이상은 89.5%, 구강위생용품으로는 치실을 58%로 가장 많이 사용하였으며, 칫솔질 방법으로 회전법을 시행하는 대상자는 46%로 조사되었다. 셋째, 일반적 특성에 따른 구강건강지식은 연령(F=5.25, p<0.01), 보건의료직 경력(F=3.94, p<0.01)에 따라, 구강건강실천은 방문보건경력에 따라 유의한 관련성을 보였다(F=3.20, p<0.05).

임상치과위생사의 내부마케팅, 직무만족도와 조직몰입간에 관련성 (Relationship Between Internal Marketing, Job Satisfaction and Organizational Commitment of Clinical Dental Hygienists)

  • 본 연구는 임상치과위생사의 조직몰입에 영향 요인을 파악하여 인력관리에 기여하고자 시도하였다. 연구대상자는 195명으로 하였고, 2020년 3월 10일부터 4월 20일까지 자기기입식 설문법으로 자료 수집하여 분석한 결과 다음과 같은 결과를 얻었다. 일반적 특성에 따른 조직몰입, 직무만족, 내부마케팅 정도는 t-검정과 one-way ANOVA로 분석하였고, 사후검정은 Scheffe'로 하였다. 조직몰입, 직무만족, 내부마케팅 상관관계는 Pearson's correlation을 실시하였다고 직무만족(r=0.689), 내부마케팅(r=0.678)과 유의한 정의 상관관계를 나타냈다. 조직몰입에 영향요인은 다중회귀분석을 하였고 조직몰입에 영향요인은 내부마케팅, 직무만족도였다. 가장 큰 영향 요인은 직무만족도 이었으며 설명력은 56.1%이었다. 이상의 결과, 임상치과위생사의 내부마케팅과 직무만족도 및 조직몰입과의 관련이 있는 것으로 나타났고, 가장 영향을 미치는 요인은 직무만족도이었다. 따라서 임상치과위생사의 조직몰입에 영향을 미치는 직무만족도를 향상시키기 위한 치과 의료기관의 제도적·행정적 지원이 필요하다.

치과의사의 아동 학대 및 방임에 대한 인식과 신고 의무 태도 (Dentists' Recognition of Child Abuse and Neglect and Mandatory Attitude to Report)

  • 이 연구의 목적은 우리나라의 치과의사 및 소아치과 의사들의 아동학대에 대한 인식과 신고 의무 태도를 조사하는 것이다. 치과병원 수련기관의 인턴 및 전공의를 포함하여 개인의원 또는 대학병원에서 근무 중인 치과의사를 대상으로 설문조사를 실시하였다. 총 문항 수는 32문항과 세부 문항으로 구성되어 있으며, '설문 응답자의 일반적인 특성', '아동학대', '아동학대 신고', '아동학대 관련법', '아동학대 대처방안'에 대한 인식의 내용을 포함한다. 설문은 메신저를 이용하여 전송하였고 이에 응답한 176명의 답변 기록을 분석 대상으로 하였다. 아동학대의 인식은 신체적 학대, 방임, 정서적 학대 순으로 높은 응답을 나타냈다. 전체 응답자 중 19명이 아동학대를 의심하여 신고했던 경험이 있다고 응답했고, 18명은 아동학대 의심 정황이 있었으나 신고를 망설였다고 하였다. 아동학대 관련법 중에서 아동학대 신고자 보호조치와 아동학대 신고 프로토콜에 대해 낮은 인식도를 보였다. 이후 아동학대 관련 교육에서 학대 아동의 발견 방법과 이에 대한 대처방안을 가장 알고 싶어 했다. 이 연구는 향후, 아동학대에 대한 치과의사의 인식 및 의식향상을 위한 자료로 활용될 수 있을 것이라고 사료된다.

  • We studied 1809 oral cancer patients who visited and were treated in 2002 at the 148 institutions certified as training facilities by the Japanese Society of Oral and Maxillofacial Surgeons, which is composed of 39 dental university hospitals, 44 medical university hospitals, 64 general hospitals, and 1 unknown institution. The patients consisted of 1071 (59.2%) males and 738 (40.8%) females (male:female ratio, 1.45:1), who had a mean age of 65.2 years old. The tongue (40.2%) was the most common site affected, followed by the gingiva (32.7%), buccal mucosa (10.1%), and oral floor (9.0%). There were 6 cases of intraoral multiple cancer. In histopathological examinations, squamous cell carcinoma (88.7%) was the most common type found, followed by adenoid cystic carcinoma (2.1%), and mucoepidermoid carcinoma (1.7%). In addition, non-epithelial tumors comprised 1.8%, among which malignant melanoma was the most common type. Cases classified as T2N0 were the most common (32.1%), followed by T1N0 (21.4%), T4N0 (8.0%), and T2N1 (7.6%). Distant metastasis occurred in 17 patients (1.0%). The sizes of the non-epithelial malignant tumors ranged from 1.0 to 7.0 cm, with a mean size of 3.7 cm.

Publication patterns in Restorative Dentistry and Endodontics

  • Objectives: Restorative Dentistry and Endodontics (Restor Dent Endod; RDE) is an English-language journal published by the Korean Academy of Conservative Dentistry, and it has been online since 2012 with quarterly publications. The purpose of this paper was to review and analyze the publications in this journal since its inception and over the 7-year period from 2012 to 2018. Materials and Methods: This paper assessed the number, type, and subject of articles published, as well as authorship patterns and article citations of the journal over a 7-year period. The citation indicator for the journal (h-index) was assessed using Google Scholar. Results: The number of articles per issue has remained relatively consistent in the 7 years that were analyzed. An analysis of the article types revealed various categories of review articles. Original research articles accounted for the most articles per volume. Twice as many articles per volume were on endodontic topics than on restorative subjects. Articles published in RDE have been widely cited in Synapse, Crossref, and PubMed Central. A country-wise mapping of authors' institutions revealed significant contributions from authors around the world. With an h-index of 24, RDE ranks third among journals in its specialty. The most cited articles were open lectures on statistics and research articles on recent concepts, technology, and materials. Conclusion: Over the last 7 years, RDE has served as a platform for a large number of manuscripts in the field of restorative dentistry and endodontics.

치과의사와의 관계에서 치과위생사와 간호조무사의 직종 간 갈등 연구 (A study on conflicts between different occupational categories of dental hygienists and nursing assistances in terms of relationships with dentist)

  • The purpose of this study was to examine the state of conflicts among dental health care workers. A survey was conducted on 266 dental hygienists and nursing assistants who worked in dental institutions from September 12 to November 13, 2017, and SPSS(statistical package for the social science) version 20.0 was employed to analyze the collected data. The findings of the study were as follows: 1. The most common reason of the dental hygienists for turnover was working hours and heavy workload(24.6%), followed by pay (22.6%), conflicts with dentists(16.0%) and conflicts with colleagues (11.3%). The most dominant reason of the nursing assistants for turnover was pay(31.1%), followed by working hours(24.4%), heavy workload(17.8%), conflicts with dentists(15.6%) and conflicts with colleagues(8.9%). 2. The largest reason for unsuccessful communication with dentists was that heavy workload reduced the opportunity to communicate well(54.5%). The second biggest reason was that they couldn't communicate well though they had the opportunity(24.0%), and the third greatest reason was that they tended to lag behind dentists in terms of professional knowledge(16.9%). 3. The biggest reason for unsuccessful communication among the dental health care workers was that they didn't have a lot of chances to communicate well on account of heavy workload(41.0%). The second largest reason was the differences in professional knowledge(24.9%), and the third greatest reason was that they couldn't communicate well though they had the chance(23.7%). 4. The most dominant reason for conflicts with dentists was the difference in power(24.0%), followed by poor communication skills(22.1%) and a lack of mutual respect(18.1%). But the opinions of the nursing assistants were different from those of the dental hygienists, as they cited poor communication skills as the most common reason, which was followed by the difference in power and a shortage of understanding of each other's work. 5. The most common reason for conflicts among the dental health care workers was a shortage of communication and communication skills(22.9%), and the second most dominant reasons were a lack of mutual respect and poor understanding of each other's work(17.5%), followed by a lack of mutual respect(17.2%). 6. As to the ways of resolving conflicts with dentists, the most common case was making some mutual concessions to compromise (28.9%), followed by delivering opinions through the staff meeting (23.9%), resolving conflicts by candidly exchanging opinions(15.8%), avoiding each other in moderation(11.7%) and following the opinions or assertions of dentists(1.3%). 7. Concerning the conflict resolution methods among the dental health care workers, the most prevalent way was making some mutual concessions to compromise(36.4%), followed by resolving conflicts by candidly exchanging opinions(23.0%) and conveying opinions through the staff meeting(18.5%). 8. Regarding communication among the dental health care workers, the dental hygienists(3.53±.729) considered themselves to be better at communicating than the nursing assistants(3.29±.745) did(p<0.05), and the dental hygienists(3.45±.809) who thought there was respectful treatment among workers who were different in occupational categories found themselves to be better than the nursing assistants(3.21±.952) who had the same thought did(p<0.05). As a result of analyzing whether frequent job-related meetings occurred among the workers whose occupational categories were different, the dental hygienists(3.05±.975) perceived that there were more frequent meetings than the nursing assistants(2.67±.955) did (p<0.01).

교정환자의 치과의료서비스 만족도에 영향을 미치는 요인 (Factors Affecting the Satisfaction Level of Orthodontic Patients with Dental Service)

  • 본 연구는 교정환자를 대상으로 의료서비스에 대한 만족도를 조사하여 교정환자의 다양한 요구와 치과의료기관의 경쟁력 강화를 위한 정보를 제공하고자 2008년 12월 3일부터 20일까지 부산지역 Y치과의원, 대구지역 B치과의원의 교정내원환자 226명을 대상으로 설문조사를 실시하였으며 SPSSWIN 14.0을 이용하여 분석한 결과 다음과 같은 결론을 얻었다. 1. 치과의원을 선택한 이유로는 '주위의 소개'가 50.7%로 가장 많았으며, 전체의 절반정도인 47.8%가 불만사항이 있을 때 '이야기 하는 편이다'라고 응답하였다. 교정치료 후 기대하는 것은 '가지런한 치아배열' 58.3%로 가장 많았으며, 교정치료 후 치아건강에 대한 기대로는 80.9%가 많이 혹은 아주 많이 나아질 것으로 기대하고 있었다. 교정치료 후 가장 하고 싶은 치료로는 미백이 70.6%로 가장 많았다. 2. 진료서비스 만족도에 유의한 영향을 미친 요인으로는 직원(치과위생사) 만족도가 가장 크고, 편의시설 만족도, 진료비 만족도, 치과의사 만족도, 친절성 만족도 순이었다. 3. 다른 사람에 대한 권유의사에 영향을 미친 요인으로는 진료비 만족도가 가장 크고, 치과의사 만족도, 직원(치과위생사) 만족도 순이었다. 교정환자가 원하는 치과의료서비스를 제공하기 위해서는 치과의료진의 지속적인 치과진료서비스 교육과 친절교육이 이루어져 교정환자의 요구도를 만족시켜야 할 것으로 사료된다.

중년층의 구강건강지식, 실천 및 보철치료 만족실태 조사 (A study for middle-aged on oral health knowledge, oral health care and satisfaction with prosthetic treatment)

  • Objectives : The purpose of this study was to examine the oral health knowledge and actual oral health care of the selected subjects, their decision making about prosthetic treatment, the state and characteristics of their prosthetic treatment and their satisfaction with prosthetic treatment in an attempt to provide some information on the improvement of the quality of life related to oral health and the promotion of oral health. Methods : The subjects in this study were 250 people who received education in two different lifelong education institutions in the city of Busan. After a survey was conducted from May 23 to June 15, 2011, the answer sheets from 217 respondents were analyzed. Results : 1. As for the general characteristics of the respondents, the men(52.1%) outnumbered the women. Those who were in their 60 and up(47.5%) made up the largest age group, and the married people(65.4%) outnumbered the unmarried ones. By occupation, the company employees(20.3%) made up the biggest group. By education, the greatest group was high-school graduates(36.1%). By monthly mean income, the biggest group gained an income of 2.01 million won or more(36.9%). As to medical security, community-based insurance was most prevailing (36.9%). In terms of health promotion, the largest group worked out to stay fit(52.4%). 2. Regarding oral health knowledge, they had a good knowledge about the cause of dental caries(56.7%), but they weren't well aware of the right time for regular dental checkup(47.9%). In relation to oral health care, regular scaling wasn't prevailing(16.9%). 3. Regarding connections between the general characteristics and satisfaction with prosthesis, the less-educated respondents expressed significantly more dissatisfaction(p=0.015). By monthly mean income, those who had a smaller income were dissatisfied in general(p=0.028). Conclusions : The findings of the study illustrated that it's required to spread awareness of the importance of oral health among people in general, and that differentiated incremental oral health care should be provided for different age groups. In order to raise the satisfaction of patients with prosthesis, how to relieve their pain and anxiety and how to adjust prosthetic treatment cost properly should carefully be considered. In addition, the government should take measures to offer assistance for the low-income classes in preparation for an increase in the elderly population.

치과위생사의 감정노동이 직무스트레스와 불안, 수면에 미치는 영향 (The effects of emotional labor of dental hygienist on the job stress, anxiety and sleep)

  • Objectives: The objective of this study was to identify the relationship between emotional labor and occupational stress of the dental hygienists, and the effects of emotional labor on their occupational stress and health problems including depression, anxiety and sleep. Methods: The survey was conducted using the questionnaires about dental hygienists' working in medical institutions in Busan, Gyeongsangnam-do from August 19th and October 7th, 2016. Total of 225 participants were selected for data analyses. Frequency analysis, t-test, ANOVA, Pearson's correlation coefficient analysis, and regression analysis were conducted using SPSS Windows ver. 21.0 program(SPSS Inc., Chicago, IL. USA). Results: The analysis of the factors affecting emotional labor, occupational stress, anxiety, and sleep by participants' characteristics showed that those with higher education level, work position and annual salary had higher emotional labor. In addition, participants who worked 5 days every other week and were responsible for patient consultation had higher emotional labor. The analysis of correlations among emotional labor occupational stress, anxiety, and sleep showed positive correlations between emotional experience and emotional expression, occupational stress and emotional expression, and anxiety and emotional expression. On the other hand, negative correlation was found between anxiety and emotional experience. Regression analysis was conducted to examine the effect of emotional labor on job stress. Emotional labor was a factor affecting job stress and anxiety. Conclusions: In order to improve emotional control and vulnerability to stress among dental hygienists, intra-organizational training opportunities and mental health care strategies are needed. Additional broad-based studies are required to identify the factors affecting the occupational stress and emotional labor and to develop relevant intervention measures.

전문가잇솔질에 의한 치주상태 변화와 환자만족도 (Change in Periodontal Status by Professional Toothbrushing, and Patients' Satisfaction)

  • 본 연구는 잇솔질 방법 중에 하나인 전문가 잇솔질법(Toothpick Method)을 이용하여 치주질환의 예방효과 및 환자들의 만족도를 파악하기 위하여 2008년 1월 9일부터 2월 28일까지 치주질환으로 치과의원에 내원한 환자 33명을 대상으로 전문가잇솔질법과 설문조사를 실시하였으며, SPSS WIN 12.0 프로그램을 이용하여 분석한 결과 다음과 같은 결론을 얻었다. 1. 치면세균막지수와 탐침시 출혈을 비교한 결과 치면 세균막지수에서는 최종검사를 시행한 후 초기치료직후와 비교한 결과 초기정밀검진 시 보다 0.16점으로 감소하였으며, 탐침 시 출혈은 2.96점으로 감소하여 통계적으로 유의한 차이를 보였다(p < .001). 2. 전문가잇솔질에 대한 환자만족도는 5점 만점에서 4.20으로 만족도가 높은 것으로 조사되었으며, 전반적인 프로그램 만족 항목에서 4.67로 가장 높게 조사되었으며, 치석제거와 비교 시 만족정도에서는 3.97로 가장 낮게 조사되었다. 일반적인 특성에 대한 환자만족도는 모두 통계적으로 유의한 차이는 없었다(p > 0.05). 3. 전문가 잇솔질 적용 후 성별에 따른 구강 내 자각인 지도를 살펴본 결과 '잇몸이 좋아졌다'가 남성의 경우 16.4%, 여성은 25.5%로 가장 높게 조사되었고, '출혈감소'는 남성은 14.5%, 여성은 10.9%로 나타났고, '입냄새 감소'는 남성이 10.9%, 여성이 7.3% 순으로 조사되었다. 전문가 잇솔질을 이용해 환자구강건강관리를 한 결과 치면세균막지수와 출혈율이 감소한 것으로 조사되었으며, 전문가잇솔질에 대한 환자만족도는 5점 만점에서 4.20으로 만족도가 비교적 높은 것으로 조사되었다. 위의 결과로 미루어 볼 때, 치과진료실 내에 환자의 치주건강을 유지할 수 있는 구강건강관리 인력의 요구와 교육이 필요하며 구강건강증진 프로그램 개발이 절실히 필요하다고 생각한다.
