  • Streptococcus parasanguinis is a Viridans group bacteria that is most often discovered in the oral cavity and causes dental plaque and endocarditis in a rat model. It has low virulence but an unknown relationship to human respiratory infections. We report on a 61-year-old woman who developed hemoptysis followed by pleuritic chest pain after conscious sedation during a gastroscopic polypectomy and was diagnosed with pneumonia and parapneumonic effusion from Streptococcus parasanguinis isolated in pleural fluid. Microaspiration during the procedure was presumed to play a role in the pathogenesis.

  • Careful examination of the oral cavity may reveal findings indicative of an underlying systemic condition, and allow for early diagnosis and treatment. Examination should include evaluation for mucosal changes, periodontal inflammation and bleeding, and general condition of the teeth. A 12-year-old man visited for molar pain during 3 months. He was diagnosed with having a possibility of hematopoietic malignancy, showing the loss of lamina dura, destruction of bony crypt, and high attenuation in the bone marrow. He was referred to department of pediatrics, additional study, including peripheral cell morphology and bone marrow exam, were performed, and diagnosed as acute lymphoblastic anemia. Despite chemotherapy to cure leukemia, he was expired 8 months after initial diagnosis. The purpose of this report is to promote and evoke the awareness regarding an initial examination of the dentist to make an effort to acquire accurate knowledge and information about life-threatening disease in usual dental practice.

Factors influencing oral health-related quality of life in health allied college students (일부 보건계열 대학생들의 구강건강관련 삶의 질에 영향을 미치는 요인)

  • Objectives: The purpose of this study was to investigate the possible impact factors(oral health level, oral health promotion behaviors, health level, health behaviors, and mental health) on oral health related quality of life using OHIP-14 of health allied college students. Methods: A total of 363 self-administered questionnaires were collected from university student in Seoul. To investigate the casual relationship between each variable presented in the research model, descriptive statistics, t-test, one-way ANOVA(Scheffe's test), man-whitney, kruskal wallis, multiple regression analysis were carried out by using SPSS ver. 21.0 Results: The study shows that the students reported mean score of OHIP-14 ($8.32{\pm}7.51$), of which physical pain was the highest score($1.88{\pm}1.45$) and social disability was the lowest score($0.69{\pm}1.13$). Multiple regression revealed that the score of OHIP-14 was shown to be significantly higher for the following people: who were get more self-reported symptom of periodontitis, halitosis, negative self-perceived general health and oral health, no received dental scaling treatment, and participants who had no experience awareness of distress in two weeks. The explanatory power was 18.2%. The most powerful factor regarding to self-reported symptom of periodontitis was shown to be negatively relations oral health-related quality of life. Conclusions: In order to enhance the students' life quality, there need to be considered for a comprehensive oral health-related quality of life program for the students through health education policy.

A maxillary canine with two separated root canals: a case report (두 개의 근관을 가진 상악 견치: 증례보고)

  • Maxillary canines have less anatomical diversities than other teeth. They usually have a single root and root canal. This report describes an endodontic treatment of a maxillary canine with two separated root canals which have not been reported through the demonstration of radiography and computerized tomography (CT). Even though appropriated endodontic treatment has been performed, the severe pain could happen due to lack of consideration of anatomical variations of the teeth. Therefore, the clinicians should be well aware of the possibility of anatomical variations in the root canal system during endodontic treatment even if the number of root canals is obvious such as in this case.

  • Tumors of the parotid gland are the most frequently encountered salivary gland tumors. Knowledge of the histology and anatomy of the salivary gland is important when considering the histiogenesis of salivary gland tumors, requiring close cooperation between the pathologist and the surgeon. Most tumors are benign epithelial formations. Pleomorphic adenomas predominate. Superficial lobectomy is adequate treatment. When the tumor involves a deep lobe, total parotidectomy is indicated. Treatment of malignant tumors depends on the histology, its TNM stage and other factors. Total parotidectomy with lymph adectomy and radiotherapy are needed in case of high grade malignancy. In children, vascular neoplasias are the most frequent, followed by malignant tumors. Their histological features and treatment are the same as for adults. We reviewed 64 cases of the parotid tumors at Department of surgery, Dong-A University Hospital from July. 1990 to Jan. 1999 for the purpose of apprehension of parotid gland tumor by the clinical study and review. Over all sex ratio was 1:1.13(M:F), mean age was 38.9 years, mean size was 3.53cm. According to histologic findings of 64 cases, pleomorphic adenoma was 55(85.9%), Warthin's tumor was 3(4.7%), mucoepidermoid carcinoma was 3(4.7%), squamous cell carcinoma was 2(3.1%), acinic cell carcinoma was 1(1,6%). Post op. facial nerve palsy 16(25%), Frey's syndrome 11(17.2%) cases were happened. Hence, the clinical manifestation of pain, tenderness, facial N. palsy suggest malignant tumors.

THE INFLUENCE OF DENTIN DESENSITIZER TO SHEAR BOND STRENGTH OF DENIAL CEMENTS (상아질 지각완화제가 치과용 시멘트의 결합강도에 미치는 영향)

  • Statement of problem: A tenderness of the dentin after tapering of teeth for dental prosthesis is a common phenomenon. In practice, the alternative desensitizer may be used for minor pain after tapering of teeth. Purpose: In this study, the desensitizers were used to investigate the affect decreasing of shear bond strength according to the use of various cement, such as resin, Glass Ionomer, and phosphate cement. Material and method: Three different desensitizers were used on this study Compositions of two dentin desensitizers were HEMA(hydroxyethylmethacrylate) and glutaraldehyde. The other one is oxalic acid. Three dentin desensitizers applied on 12 degrees taper teeth. Then, Ni-Cr crowns were bonded with Resin cement, Zinc Phosphate (ZPC) cement and Glass Ionomer (GIC) cement. 120 human premolar teeth were used for specimens. The specimens were divided into four group as the reference and the empirical each with thirty specimens, then further divided into 12 group according to type of desensitizers and cement types. The shear bond strength were measured by Instron multi task instrument. Results: According to the result, the measured shear bond strength in order from the weakest to the strongest in general was ZPC, Resin, and GIC. And it is found that the application of desensitizers on dentin surface does not affect the shear bond strength. Conclusion: Dentin desensitizers that alleviate or prevent a dentin tenderness, usually contains HEMA and glutaraldehyde compounds. Such desensitizers are widely used in clinical studies. By applying the dentin desensitizer on the exposed dentin surface, the dentin capillary are blocked and periodontal membrane and cementum can not be drawn in pulp cavity. Since HEMA and glutaraldehyde may cause harm to the pulp cavity, an alternative desensitizer was developed.

  • The radicular cyst is the most common odontogenic cyst which is caused by pulpal inflammation, pulp death, and secondary to trauma or dental caries. Usually, the radicular cyst is asymptomatic, but a secondary inflammation can cause pain, swelling and redness. Getting larger, the radicular cyst can cause facial asymmetry and paresthesia by pressure on nerves. It requires conservative endodontic treatment or surgical approach. When the size of cyst is large or invasion of the adjacent tissue is not expected, cyst enucleation is carried out. And most of the case can be completely cured and shows low recurrence. In these radicular cysts cases, by cyst enucleation or apicoectomy after root canal treatment simultaneously, the infected teeth can be preserved successfully.

Medication for Benign Prostatic Hyperplasin in a Dog (개에서 발생한 양성 전립선 비대증의 약물 치료 1례)

  • An 8-year-old, male Shih-tzu dog with clinical signs of hemorrhage from penile orifice and pain on lifting hindlimb was brought to the Veterinary Medicine Teaching Hospital, Chungnam National University. The patient was shown polyuria/polydipsia (PU/PD), more than 10 times per day, and gasterointestinal signs such as intermittent vomiting and melena. On laboratory examination, BUN, creatinine and amylase level were all highly increased. Radiographic and ultrasonographic findings included that both kidneys were abnormal, and both prostates were hyperplastic bilaterally and cysts were presented in parenchyma. Additionally, hyperechoic pancreas and intact urinary bladder were observed from ultrasonographic examination. The dog was diagnosed as pancreatitis, acute renal failure and benign prostatic hyperplasia (BPH). After the appropriate treatment for pancreatitis and acute renal failure, He dog was treated with administration of azasteroid for BPH. On day 71 after azasteroid treatment, good response to the treatment was defined by ultrasonography and clinical signs cured completely.

Conservative approach of a symptomatic carious immature permanent tooth using a tricalcium silicate cement (Biodentine): a case report

  • The restorative management of deep carious lesions and the preservation of pulp vitality of immature teeth present real challenges for dental practitioners. New tricalcium silicate cements are of interest in the treatment of such cases. This case describes the immediate management and the follow-up of an extensive carious lesion on an immature second right mandibular premolar. Following anesthesia and rubber dam isolation, the carious lesion was removed and a partial pulpotomy was performed. After obtaining hemostasis, the exposed pulp was covered with a tricalcium silicate cement (Biodentine, Septodont) and a glass ionomer cement (Fuji IX extra, GC Corp.) restoration was placed over the tricalcium silicate cement. A review appointment was arranged after seven days, where the tooth was asymptomatic with the patient reporting no pain during the intervening period. At both 3 and 6 mon follow up, it was noted that the tooth was vital, with normal responses to thermal tests. Radiographic examination of the tooth indicated dentin-bridge formation in the pulp chamber and the continuous root formation. This case report demonstrates a fast tissue response both at the pulpal and root dentin level. The use of tricalcium silicate cement should be considered as a conservative intervention in the treatment of symptomatic immature teeth.

A Comparison of the Effects of Concomitant Analgesics with Midazolam for Sedative Dental Therapy

  • Purpose: Intravenous sedation with midazolam is common in contemporary dentistry. That is effective for anxious patients, but additional analgesic agent needs to be used, because midazolam alone doesn't have an analgesic effect. This study was performed to select an analgesic agent between an opioid agent, and nonsteroidal anti-inflammatory drugs as adjunctives in intravenous sedation with midazolam. Methods: The subjects were 60 patients who visited the Department of Oral and Maxillofacial Surgery, Sacred Heart Hospital, Hallym University, between August 2009 and February 2010. Conscious sedation was performed on 20 patients of 3 groups (control group, ketorolac group, and fentanyl group), who were divided randomly. The analgesic agent was administrated preoperatively. For sedation, vital signs were recorded. After sedation and operation, subjective questionnaires of the patient and operator were implemented. Results: All of the $SPO_2$, blood pressure, and heart rates stayed within the normal range for sedation. The sedation depth and analgesic effect of the ketorolac group and fentanyl group were similar. In the case of sedation depth, 12 patients in the ketorolac group and 14 patients in the fentanyl group had no memory of surgery. In the case of analgesic effect, the visual analogue scale of pain scored 2~3 in 13 patients in the ketorolac group, and 0~2 in 12 patients in the fentanyl group. The satisfaction of patients and doctors was also similar. Conclusion: Considering the management and complication of an opioid agent, non-steroidal anti-inflammatory drugs is more effective than an opioid agent.
