완전 디지털 시스템을 이용한 상악동 거상술 및 구치부 임플란트 고정성 보철 수복 증례 (Sinus floor elevation and implant-supported fixed dental prosthesis in the posterior area, with full-digital system: a case report)

  • 진단, 수술, 인상, 보철물 제작 등 임플란트 치료 전 과정에서 디지털 기술이 활용되고 있다. 본 증례에서는 디지털 수술 가이드를 이용하여 합병증 없이 상악동 거상술을 시행하고, 계획된 위치에 임플란트를 식립하였다. 골유착을 위한 치유 기간 이후, CAD-CAM(Computer-aided design/Computer-aided Manufacturing)으로 맞춤형 지대주 및 임시 보철물을 제작하여 장착하고, 환자의 적응도와 교합을 평가하였다. 임시 보철물상에서 교합 변화가 관찰되어, 광중합형 컴포지트 레진으로 수리하였다. 최종 보철물 제작 시, 지대주의 수직 침하, 임시 보철물의 형태와 적응된 교합 관계를 반영하기 위해, 이중 스캔과 지대주 수준의 디지털 인상을 채득하였다. 치은 압배 없이 치은 연하 변연을 인기하기 위해, CAD-CAM 소프트웨어상에서 라이브러리화된 지대주 데이터를 중첩하고, 지르코니아 최종보철물 제작하여 장착하였다. 구치부 임플란트 수복 시, 디지털 시스템을 이용하여 전통적인 방법에서 겪는 어려움을 줄이고, 수술부터 보철물 제작까지 효율적인 치료 과정과 안정적이고 예지성 있는 결과를 얻어 보고하는 바이다.

  • 본 증례는 기존의 국소의치에서 파절된 metal-ceramic surveyed crown을 complete contour zirconia surveyed crown으로 전환하는 간단하고 편리한 CAD/CAM 기술을 보고하고 있다. 항암치료를 받고 있는 환자의 기존 metal-ceramic surveyed crown 외형, 형태학적 특성, 레스트 시트를 complete contour zirconia surveyed crown으로 제작하는 복제 기법에 대해 보고한다.

  • 최근 CAD-CAM과 지르코니아를 이용한 보철물의 제작이 증가하고 있으며 구강스캐너의 발달로 임상 및 기공과정이 단순화되고 있다. 균열치 혹은 치아의 일부가 파절된 경우, 구강스캐너를 사용하여 치아 삭제 전에 치아의 형태를 미리 스캔하고, 추후 수복물 제작 시 중첩을 통해 단일체 지르코니아 전장관을 제작하면 원래 환자의 자연치 형태와 교합을 재현할 수 있다. 본 증례에서는 균열 및 파절된 치아에서 구강 스캐너, CAD-CAM 및 단일체 지르코니아 전장관을 사용하여 삭제 전 치아의 형태와 교합을 재현한 수복물을 제작하였으며, 교합조정을 최소화할 수 있었다. 또한 임상적으로 기능적이고 심미적인 결과를 얻을 수 있었다.

Influence of abutment height and convergence angle on the retrievability of cement-retained implant prostheses with a lingual slot

  • PURPOSE. Cement-retained implant prostheses can lack proper retrievability during repair, and residual cement can cause peri-implantitis. The purpose of this in vitro study was to evaluate the influence of abutment height and convergence angle on the retrievability of cement-retained implant prostheses with lingual slots, known as retrievable cement-type slots (RCS). MATERIALS AND METHODS. We fabricated six types of titanium abutments (10 of each type) with two different heights (4 mm and 6 mm), three different convergence angles ($8^{\circ}$, $10^{\circ}$, and $12^{\circ}$), a sloped shoulder margin (0.6 mm depth), a rectangular shape ($6mm{\times}6.5mm$) with rounded edges, and a rectangular ledge ($2mm{\times}1mm$) for the RCS. One monolithic zirconia crown was fabricated for each abutment using a dental computer-aided design/computer-aided manufacturing system. The abutments and crowns were permanently cemented together with dual-curing resin cement, followed by 24 hours in demineralized water at room temperature. Using a custom-made device with a slot driver and torque gauge, we recorded the torque ($N{\cdot}cm$) required to remove the crowns. Statistical analysis was conducted using multiple regression analysis and Mann-Whitney U tests (${\alpha}=.05$). RESULTS. Removal torques significantly decreased as convergence angles increased. Multiple regression analysis showed no significant interaction between the abutment height and the convergence angle (Durbin-Watson ratio: 2.186). CONCLUSION. Within the limitations of this in vitro study, we suggest that the retrievability of cement-retained implant prostheses with RCS can be maintained by adjusting the abutment height and convergence angle, even when they are permanently cemented together.

CAD/CAM splint based on soft tissue 3D simulation for treatment of facial asymmetry

  • Background: Most cases of facial asymmetry involve yaw deformity, and determination of the yaw correction level is very difficult. Methods: We use three-dimensional soft tissue simulation to determine the yaw correction level. This three-dimensional simulation is based on the addition of cephalometric prediction to gradual yaw correction. Optimal yaw correction is determined visually, and an intermediate splint is fabricated with computer-aided design and computer-aided manufacturing. Application of positioning devices and the performance of horseshoe osteotomy are advisable. Results: With this procedure, accurate repositioning of jaws was confirmed and patients obtained fairly good facial contour. Conclusions: This procedure is a promising method for a widespread, predictable treatment of facial asymmetry.

The reliability of an easy measuring method for abutment convergence angle with a computer-aided design (CAD) system

  • PURPOSE. The purpose of this study was to evaluate the intra-rater reliability and inter-rater reliability of three different methods using a drawing protractor, a digital protractor after tracing, and a CAD system. MATERIALS AND METHODS. Twenty-four artificial abutments that had been prepared by dental students were used in this study. Three dental students measured the convergence angles by each method three times. Bland-Altman plots were applied to examine the overall reliability by comparing the traditional tracing method with a new method using the CAD system. Intraclass Correlation Coefficients (ICC) evaluated intra-rater reliability and inter-rater reliability. RESULTS. All three methods exhibited high intra-rater and inter-rater reliability (ICC>0.80, P<.05). Measurements with the CAD system showed the highest intra-rater reliability. In addition, it showed improved inter-rater reliability compared with the traditional tracing methods. CONCLUSION. Based on the results of this study, the CAD system may be an easy and reliable tool for measuring the abutment convergence angle.

A Digital Approach to a Definitive Immediate Denture: A Clinical Report

  • Even though an immediate denture (ID) is a practical prosthesis, fabricating an ID may be challenging, as unexpected removals of periodontally compromised teeth may occur during an impression procedure. This clinical report introduces a digital approach to a maxillary ID. An intraoral scanner was applied to prevent accidental extraction. A physical cast and a resin pattern of a framework were fabricated with rapid prototyping technology. A proper border and retention was also achieved by an altered cast impression.

  • Procera system으로 제작한 금관은 여러 연구들을 통해 강도, 적합도, 심미성, 시멘트 결합강도, 생체적합성 등에서 우수한 것으로 나타났고 오랜 기간 임상 연구를 통해서도 확인되었다. 따라서 주의 깊고 신중하게 임상에 적용시 만족스러운 보철물 제작이 가능할 것이다.

  • Digital technology has changed various aspects of the clinical dentistry. The intraoral scanner and Computer-aided design / Computer-aided manufacturing (CAD-CAM) technology are widely used in fabricating fixed prostheses and in implant surgery. These technologies greatly improved the efficiency of clinical and laboratory procedures. With all newly introduced software, devices, and clinical studies, digital technology has been actively applied in removable prostheses. It is now possible to fabricate the removable prostheses more quickly and easily through subtractive and additive manufacturing. Various clinical and laboratory protocols were introduced by many manufacturers. The purpose of this review is to summarize the literature on digital technology for fabricating complete denture with current status and future perspectives.

치과용 지르코니아로 제작된 심미보철물의 임상적 예후에 관한 문헌고찰 (Review of clinical studies applying yttrium tetragonal zirconia polycrystal-based esthetic dental restoration)

  • Application of ceramic materials for fabrication of dental restoration materials has been a focus of interest in the field of esthetic dentistry. The ceramic materials of choice are glass ceramics, spinel, alumina, and zirconia. The development of yttrium tetragonal zirconia polycrystal (YTZP)-based systems is a recent addition to all-ceramic systems that have high strength and are used for crowns and fixed partial dentures. Computer-aided design/computer-aided manufacturing (CAD/CAM)-produced, YTZP-based systems are popular with respect to their esthetic appeal for use in stress-bearing regions. The highly esthetic nature of zirconia and its superior physical properties and biocompatibility have enabled the development of restorative systems that meet the demands of today's patients. Many in vitro trials have been performed on the use of zirconia; however, relatively fewer long-term clinical studies have been published on this subject. The use of zirconia frameworks for long-span fixed partial dentures is currently being evaluated; in the future, more in vivo research and long-term clinical studies are required to provide scientific evidence for drawing solid guidelines. Further clinical and in vitro studies are required to obtain data regarding the long-term clinical use of zirconia-based restorations.
