The Impact of an Increased Application of Critical Pathway for Gastrectomy on the Length of Stay and Cost

  • Purpose: We developed a standardized critical pathway for gastric cancer surgery and then determined the increase of application, which resulted in an improvement in terms of measurable indices, such as hospital stay and cost. Materials and Methods: A critical pathway was revised and used widely from the 2nd quarter of 2009. We collected clinical data, such as length of stay and complication rate, as clinical indices of quality prospectively. The total cost paid at the patient's discharge, as well as the daily hospital income, were calculated and compared by each quarter from January 2008 to December 2009. Results: The application rate of critical pathway was 11.8% and 87.8% in 2008 and 2009, respectively. There were no perioperative deaths. There was no difference in the complication rates between 2008 and 2009 (P=0.45). However, the mean length of stay was significantly different between the 2 years (P<0.05). Although the total cost was not different, the daily hospital income was significantly higher in the latter year (P<0.05). Conclusions: An increase in the application of critical pathway for gastrectomy resulted in significant decreases in length of stay and increases in the daily hospital income without a compromise on the clinical indices.

  • In this paper, we studies EMTP analysis and characteristics of critical voltage of pulse corona and load impedance on variable electrode length. To obtain a stable Pulse voltage, we designed a compact pulse generator switched MOSFET and tested their characteristics by adjusting electrode length and pulse repetition. As a result, critical voltage of pulse corona and load impedance on increasing electrode length were decreased. These results indicate we can control critical voltage of pulse corona and suppress arc discharging between two electrodes.

Large deflections of variable-arc-length beams under uniform self weight: Analytical and experimental

  • This paper presents the solution of large static deflection due to uniformly distributed self weight and the critical or maximum applied uniform loading that a simply supported beam with variable-arc-length can resist. Two analytical approaches are presented and validated experimentally. The first approach is a finite-element discretization of the span length based on the variational formulation, which gives the solution of large static sag deflections for the stable equilibrium case. The second approach is the shooting method based on an elastica theory formulation. This method gives the results of the stable and unstable equilibrium configurations, and the critical uniform loading. Experimental studies were conducted to complement the analytical results for the stable equilibrium case. The measured large static configurations are found to be in good agreement with the two analytical approaches, and the critical uniform self weight obtained experimentally also shows good correlation with the shooting method.

  • Purpose: This study was undertaken in order to investigate the relationships between critical thinking disposition, professional nursing competence and job satisfaction in clinical nurses. Method: A convenience sample of 221 registered nurses were obtained from one hospital in Seoul. The data were analyzed by descriptive statistics, t-test, ANOVA, Scheffe' test, Pearson correlation coefficient using SPSS WIN 14.0 program. Results: There were a statistically significant difference in the critical thinking disposition according to age, marital status, career length, and department. There were a statistically significant difference in professional nursing competence according to role, age, career length, current position, and department. There were a statistically significant difference in job satisfaction according to age, marital status, career length, and department. A significant positive correlation between critical thinking disposition and professional nursing competence, and job satisfaction was found. Conclusion: Critical thinking disposition, professional nursing competence and job satisfaction of clinical nurses are significantly related. To improve professional nursing competence and job satisfaction of clinical nurses, it need to use actively the critical thinking disposition with resonable salary. And it need to prepare the nursing education and strategy to increase the job satisfaction of clinical nurses.

Development of a Flexible Critical Pathway with Electronic Medical Record for Gastrectomy Patients in a University Hospital (위 절제술 환자의 진료계획표 개발 및 전자 의무 기록화)

  • Objectives : This study was conducted to evaluate the effect of fixed critical pathway with emr (electronic medical record) on the length of hospital stay, the cost and quality of care provided to gastrectomy patients in a university hospital and to develop flexible critical pathway with emr which can be used excluded or drop-out patients. Methods : Thirty-eight patients with gastrectomy were included as case group and Thirty-four patients included as control group. The comparison between control and case with using fixed critical pathway were done. To develop and to evaluate usefulness of flexible critical pathway with flexible data base, simulation was done for flexible critical pathway with drop-out patients. Result : The major results of this study were as follows: There were no significant differences in patient clinical conditions and no sign of deterioration of quality from critical pathway. The length of hospital stay was 11 days in control group, 8 days in path group(P<0.01). The total costs during the hospital stay were reduced in path group. However the cost per day was significantly increased from reduction of hospital stay(554,352 won in control, 645,669 won in path group). One hundred percentage of drop out patients(60) in the simulation of flexible critical pathway was successful. Conclusion : Computerized critical pathway reduced the length of hospital stay, total hospital costs and resource utilization without harming quality of patient care. The flexible critical pathway program can be used as one of the powerful management tools for reducing the practice variations and increasing the efficiency of care process and decreasing the workload of doctors and nurses in Korean hospital settings.

Notched Strength and Fracture Criterion of Glass/Epoxy Plain Woven Composites Containing Circular Holes (원공을 가진 Glass/Epoxy 복합재료의 노치강도 및 파괴조건)

  • The fracture behavior of glass/epoxy plain woven composite plates containing circular holes is experimentally investigated to examine the effects of hole size and specimen width on notched tensile strength. It is shown in this paper that the characteristic length according to the point stress criterion depends on the hole size and specimen width. For predicting the notched tensile strength, a modified failure criterion is developed. An excellent agreement is found between the experimental results and the analytical prediction of modified failure criterion. The notched strength and the characteristic length have an increase and decrease relations. When the unstable fracture occured, the critical crack length equivalent for the damage zone size at the edge of hole is about twice the characteristic length. The critical energy release rate G$_{c}$ is independent of hole size(0.03 .leq. 2R/W .leq. 0.5) under the same specimen width. However G$_{c}$ increases with an increase in specimen width which can be explained by stress relaxation due to the notch insensitivity.ity.

  • The conventional grounding impedance of a counterpoise is calculated as a function of the length of the counterpoise by use of the distributed parameter circuit model with an application of the EMTP(Electromagnetic Transient Program). The adequacy of the distributed parameter circuit model is examined and verified by comparison of the simulated and the measured results. The conventional grounding impedance of the counterpoise is analyzed for the first short stroke and subsequent short stroke currents. As a result, the simulated results show that the minimum conventional grounding impedance gives at a specified length of the counterpoise. The shorter the time taken to reach the peak of injected currents, the shorter the length of the counterpoise having the minimum conventional grounding impedance. We also present the critical lengths of the counterpoise for short stroke currents as a function of soil resistivity. Based on these results, it is necessary to compute the length of the counterpoise in a specified soil resistivity which satisfies both the low conventional grounding impedance requirement whilst also providing a suitable ground resistance in order to obtain an economical design and installation of the counterpoise.

  • C- V method is a means to determine the effective channel length for miniaturized MOSFET's. This method achieves L$_{eff}$ by extracting a unique channel length independent extrinsic overlap length($\Delta$L) at a critical gate bias point. In this paper, we conducted an experiment on two different C-V methods. L$_{eff}$ extracted from experiment is compared with L$_{eff}$ simulated from a two-dimensional (2-D) device simulator, and the accuracy of C-V method for L$_{eff}$ extraction is analyzed.

Influence of Thermal Expansion on Eccentricity and Critical Speed in Dry Submersible Induction Motors

  • Rotor eccentricity is one of the major factors that directly influence the security of horizontal electrical machines, and the critical speed of the shaft has a close relationship with vibration. This paper deals with the influence of thermal expansion on the rotor eccentricity and critical speed in large dry submersible motors. The dynamic eccentricity (where the rotor is still turning around the stator bore centre but not its own centre) and critical speed of a three-phase squirrel-cage submersible induction motor are calculated via hybrid analytical/finite element method. Then the influence of thermal expansion is investigated by simulation. It is predicted from the study that the thermal expansion of the rotor and stator gives rise to a significant air-gap length decrement and an inconspicuous slower critical speed. The results show that the thermal expansion should be considered as an impact factor when designing the air gap length.
