Psychiatric understanding and treatment of patients with amputations

  • Amputation changes the lives of patients and their families. Consequently, the patient must adapt to altered body function and image. During this adaptation process, psychological problems, such as depression, anxiety, and posttraumatic stress disorder, can occur. The psychological difficulties of patients with amputation are often accepted as normal responses that are often poorly recognized by patients, family members, and their primary physicians. Psychological problems can interfere with rehabilitation and cause additional psychosocial problems. Therefore, their early detection and treatment are important. A multidisciplinary team approach, including mental health professionals, is ideal for comprehensive and biopsychosocial management. Mental health professionals could help patients set realistic goals and use adaptive coping styles. Psychiatric approaches should consider the physical, cognitive, psychological, social, and spiritual functions and social support systems before and after amputation. The abilities and limitations of physical, cognitive, psychological, and social functions should also be considered. To improve the patient's adaptation, psychological interventions such as short-term psychotherapy, cognitive behavioral therapy, mindfulness meditation, biofeedback, and group psychotherapy can be helpful.

The Mediation Effect of Job Burnout on the Relationship between Role Stress and Depression among Local Clinic Medical Doctor : Gender's Moderated Mediation Effect (개원의 역할스트레스와 우울 간 관계에서 직무소진의 매개효과 : 성별의 조절된 매개효과)

  • Objectives: The objective of the present study was to investigate whether gender mederates the mediating effect of job burnout on the relationship between role stress and depression among medical doctors in Local clinics in South Korea. Methods: Questionnaires that assess role stress, job burnout, and depression were administered to. 265 medical doctors who worke in local clinics in Seoul, Busan, Daejeon, and Gyeonggi provinces. Data analysis was conducted using the PROCESS macro in Statistical Package for the Social Sciences(SPSS). Results: Job burnout mediated the relationship between role stress and depression. Further, gender moderated the relationship between role stress and job burnout. Finally, gender moderated the mediating effect of job burnout on the relationship between role stress and depression. Conclusions: On the basis of the emergent findings, effective coping strategies that can alleviate role stress are articulated. However, the findings also suggest that the selection of a suitable intervention must be based on the gender of the medical doctor.

Transfer anxiety in parents of children transferred from pediatric intensive care units to general wards in South Korea: a hybrid concept analysis

  • Purpose: This study aimed to analyze the concept of transfer anxiety in parents of children transferred from pediatric intensive care units to general wards. Methods: The hybrid model by Schwarz-Barcott and Kim was used to analyze the characteristics of transfer anxiety in parents of children transferred from pediatric intensive care units to general wards. Results: Transfer anxiety was defined by the following attributes: 1) stress concerning the adaptation process, 2) concern about the child's condition worsening due to the parent's caregiving, and 3) involuntary changes in daily life due to the treatment. Transfer anxiety has the following antecedents: 1) uncertainty; 2) a lack of knowledge about the illness, medical devices, and caregiving; and 3) a lack of social support. It resulted in 1) caregiver burden, 2) a decrease in the capacity for coping with caregiving, 3) delays in the child's physical and psychological recovery, and 4) decreased quality of life. Conclusion: It is necessary to develop an assessment scale that considers the attributes of transfer anxiety in parents of children transferred from pediatric intensive care units to general wards. Furthermore, an effective nursing intervention should be developed to reduce transfer anxiety.

A Qualitative Study on Dismissed Workers' Psychological Experiences (해고근로자의 심리적 경험에 대한 질적 연구)

  • This study purposed to reveal the process of change in dismissed workers' experiences at dismissal and the subsequent period of unemployment. For this purpose, six dismissed workers' psychological experiences were explored using the phenomenological method as one of qualitative research methods. Data were collected through in-depth interviews with the workers on their psychological experiences at dismissal, continuous psychological or physical changes after dismissal, current state, etc. and from the analysis of the data were derived 12 sub-themes and they were categorized into four main themes. The four main themes were 'emotions experienced with dismissal,' 'psychological and physical symptoms,' 'patterns of social perception,' 'and ways of coping with dismissal.' The main themes and their sub-categories were described, and dismissed workers' specific experiences related to them were explained. Lastly, dismissed workers' changes and experiences were discussed based on previous studies.

  • The purpose of this study is to diminish the risk through identifying the risk factors occurring in the process of real estate development projects, and analyzing and coping with the risk factors, and enable stable advance in the development process by improving the roles and work distribution of the project investor. The results of this study are as follows : 1) identifying the risks of contractions from the step of selecting the sites must be made possible through supplemental human resources expertise in plan and construction of the operation 2) as an improvement scheme, heightening the penetration wall of development projects from the investors, proposing a registration law and a selection standard of construction companies to investors' cooperating companies enabling the growth of superior investors.

The Meaning of Menopause Experienced by Women (여성이 경험한 폐경의 의미)

  • The purpose of this study was attempted to understand the substance and meaning of menopause experienced by women through informal interviews with oral consent. The informants were 6 perimenopausal women of 50-55 years old who are executing menopause. Colaizz's analytical method, a type of phenomenological analysis, was used to analyze data recorded by audiotape. One professor and a master's degree student who understand phenomenology, and the one who has a master of arts examined the validity between the meanings composed of the clusters of themes. Findings were turned out to be valid through validation process as the last step. The meaning of menopause implied both 'concept about menopause' and 'menopause as a time of change'. Menopause was mostly considered as cessation of menstruation as a physiological, natural, and normal process by aging. However, some people regarded menopause as a loss of youth and womanhood and lessening of every function of the body. Menopause as the time of changes means 'the period of' 'hormonal changes' such as change of menstruation, hot flushes, perspiration, and palpitation ; 'body function changes' of visual acuity, physical strength, sleeping, digestion, thoughts, bone and joints, skin sensibility, sexual pattern and intelligence ; 'emotional changes' such as anxiety, loneliness, gloominess, and nervousness. Menopause is a turning point on the women's life cycle accompanying various kind of changes and health problems. Therefore it is inevitable to develop strategy helping menopausal women pass through the critical successfully by adapting and coping with their critical period toward the healthy and better quality of life individually rather than putting them all into the standardized hormonal replacement protocol.

Development of an Optimal Operation Model of Residual Chlorine Concentration in Water Supply System (송·배수시스템의 최적 잔류염소농도 관리 모델 개발)

  • This study aimed to develop a method to optimize residual chlorine concentrations in the process of providing water supply. To this end, this study developed a model capable of optimizing the chlorine input into the clearwell in the purification plant and the optimal installation location of rechlorination facilities, and chlorine input. This study applied genetic algorithms finding the optimal point with appropriate residual chlorine concentrations and deriving a cost-optimal solution. The developed model was applied to SN purification plant supply area. As a result, it was possible to meet the target residual chlorine concentration with the minimum cost. Also, the optimal operation method in target area according to the water temperature and volume of supply was suggested. On the basis of the results, this study derived the most economical operational method of coping with water pollution in the process of providing water supply and satisfying the service level required by consumers in the aspects of cost effectiveness. It is considered possible to appropriately respond to increasing service level required by consumers in the future and to use the study results to establish an operational management plan in a short-term perspective.

A Qualitative Study on the Self-determination Experience of Youth with Disabilities (장애청소년의 자기결정 경험에 대한 질적 연구)

  • This study explored how young people with disabilities experience self-determination in their lives. This study employed a grounded theory approach involving 10 undergraduate students with cerebral palsy or physical disabilities, in their early 20's, and individual interviews. The experience relating self-determination of the participants seemed to start with "wanting to do things at their will as they grow up". The participants reported experiencing "yearning to attain self-actualization overcoming disabilities" in the context of "disability-related social limitation", "divided opinions between the participants and their parents", and "limitation in financial and psychological resources". The emerging strategies to attain self-actualization included "active coping", "avoiding confrontation", and "conditional acceptance". Intervening conditions to formation of the strategies included ”challenging spirits towards prejudice about disability", "autonomous tendencies", "taking the most of social support" and "parenting attitudes of their parents". In the process of seeking to attain self-actualization, these participants experienced "self-maturation" and "the sense of guilt towards their parents". The findings emerged from the study was that there are three stages in the process young people went through in self-determination. The stages consist of "perceiving their needs and obstacles for self-determination", "problem solving", and "perceiving their growth". In addition, four self-determination patterns are derived from this data: "breaking through", "multi-strategic", "moderate", and "acceptive". This study suggested some theoretical and prctical implications needed to facilitate self-determination of youth with disabilities.

Implementation of Business Integration Platform using Web Services (웹서비스를 이용한 비즈니스 통합 플랫폼의 구현)

  • Not only Enterprise Application Integration (EAI) within a company but also business-to-business integration (B2Bi) should be achieved for companies to collaborate seamlessly through the Internet. There have been great efforts in this area of EAI and B2Bi, resulting in lots of solutions in the market. However, EAI and B2Bi systems have been designed and operated in independent platforms and environments causing a serious problem of integrating EAI and B2Bi systems. In this paper we have proposed a business integration platform for connecting EAI and B2Bi systems in a uniform architecture and also coping with changes in business environments. In the integration platform proposed in this work, collaboration processes between B2B companies can be performed using Web Services technology widely considered as B2B integration platform. Especially, we have defined basic business execution elements based on a thorough analysis of various business integration standards and developed an efficient and extensible business process engine for B2B collaboration process management. A Web-based order processing system is developed for validation of our integration platform. It is expected that the level of B2B collaboration can be enhanced using the integration platform and furthermore it will contribute to more collaborative B2B commerce.

A study on the quality of medical social work influenced by organizational culture of hospital (조직문화가 의료사회사업서비스의 질에 미치는 영향에 관한 연구)

  • In this study, the impact of organizational culture on the quality of social work services in hospitals were empirically analysed. A mailed questionnaire survey was conducted between February 19 and April 10, 2001. A sample of total 70 hospitals, including general hospitals with one social worker at least and single-department hospital with two social workers or more, were identified nationwide through the registry of Korean Association of Medical Social Workers and Korean Association of Hospitals. According to coping strategy and reacting pattern with the environmental changes, four types of organizational culture in each hospital, classified as group culture, developmental culture, hierarchial culture, and rational culture, were adopted for the independent variables. Three dimensional aspects of quality of social work service - structure, process, and outcome were selected as dependent variables in this study. Also the quality of social work service was distributed into provider-perceiving quality and consumer-perceiving quality The major findings were as following in summary; First, most social workers reported that the characteristic of culture in their hospitals are group culture the first, hierarchial culture the second, developmental culture the third, and rational culture finally in order of comparing the level of quality perceived between social worker's recognition. Second, service provider and consumer, The provider-perceiving quality showed less score than that of consumer, especially the lowest was the score of quality of outcome perceived by provider. Third, according to the types of organizational culture, there were significantly different levels of quality in total social work services, structure dimension and process dimension. The quality of outcome dimension did not show significant differences among the type of organizational culture. Finally, the most influential variables to the quality of social work service ice proved departmental form of social work unit, leader of social work unit, and developmental culture of hospital To assure quality services, accordingly, social work unit in hospital is required to be organized as a single unit, that means to be an independent department of which qualified social worker is supposed to control the unit. It is strongly recommended to develop leadership for the leaders of social work unit.
