간호·간병통합서비스병동과 일반병동 환자의 간호요구도, 간호만족도 및 간호사의 의사소통 유형 비교 (Comparison of Patient Needs, Patient Satisfaction, and Communication Type of Nurses between Integrated Nursing Care Service Ward and General Ward Patients)

  • Purpose: The purpose of this study is to provide patient-centered nursing by comparing the patient needs, patient satisfaction, and communication type of nurses for patients admitted to integrated nursing care service ward and general ward. Methods: Data were collected by self-report questionnaires between September 7 and October 9, 2021. Patients were patients recruited from a hospital in D city, 100 each from comprehensive nursing care service ward and general ward. The data were analyzed using descriptive statistics, 𝜒2-test, t-test, ANOVA, and Pearson's correlation. Results: There was no significant difference in patient needs between the patients of integrated nursing care service ward and the general ward (t=-1.12, p=.263). However, patient satisfaction was higher among the patients in integrated nursing care service ward than in the general ward (t=-3.55, p<.001). Pertaining to the communication type of nurses, the informational communication type (t=-4.02, p<.001) and the friendly communication type (t=-3.92, p<.001) were stronger in the integrated nursing care service ward than the general ward. It was, hence, confirmed that the educational requirements were positively correlated with patient satisfaction in the reliability domain and technical-professional domain, and that the nurse informational communication type and friendly communication type had a significantly positive correlation with patient satisfaction. Conclusion: The expansion of the integrated nursing care service ward may be considered in the future, with an informational and friendly communication method as an effective response that can be adapted in practice as a reflection the type of communication among nurses.

급성기병원에서의 간호 · 간병통합서비스 운영 전후 낙상 및 욕창 발생 변화 (Performance of Comprehensive Nursing Care Service in an Acute Care Hospital: Focusing on Accidental Falls and Pressure Injuries)

  • Purpose: This study aimed to investigate whether the comprehensive nursing care service positively affected accidental falls and pressure injuries. Methods: This study was a retrospective study that analyzed the accidental falls and pressure injuries cases in an acute care hospital located in Seoul and compared the rates of accidental falls and pressure injuries before and after the comprehensive nursing care service was operated. Results: Comparing the accidental fall incidence rates per 100 person-months between a comprehensive nursing care ward and a general ward, it showed fewer accidental falls by 0.44 in comprehensive nursing care wards, but the result was not statistically significant. In the case of pressure ulcers, the incident rate per 100 person-month was 6.17 in general wards and 4.77 in comprehensive nursing care wards, which showed that the number of pressure ulcer patients was lower in comprehensive nursing care wards, however it was also not statistically significant. Conclusion: It is not confirmed that the operation of the comprehensive nursing care service contributes to the reduction of accidental hospital falls or pressure injuries. Follow-up studies are recommended to determine the effectiveness of comprehensive nursing services in quality indicators.

  • Purpose: This study aims to compare and analysis job stress, patient safety culture, and patient safety management activities of nurses in comprehensive nursing care service wards and general wards. Methods: Through an online survey conducted on nurses with more than three months of working experience at five general hospitals in the metropolitan area, 156 participants's data were used for 𝜒2-test, t-test, ANOVA using SPSS ver. 26.0. Result: There were no significant differences in the nurses' job stress and patient safety management activities between the comprehensive nursing care service wards and general wards. However, the patient safety culture of nurses in the comprehensive nursing care service ward was significantly lower than that of general ward. Conclusion: Patient safety culture should be promoted through continuous regular patient safety education, training, and organizational support to increase patient safety management activities. Additionally, institutional programs should be prepared to reduce job stress of nurses at the frontline of patient safety.

간호조직문화, 셀프리더십이 공공병원 간호·간병통합서비스 병동 간호사의 직무만족에 미치는 영향 (Effects of Nursing Organizational Culture and Self-leadership on Job Satisfaction for Nurses Working in the Comprehensive Nursing Care Service Ward in Public Hospitals)

  • Purpose: This study investigated the effects of nursing organizational culture and self-leadership on job satisfaction for nurses working in the comprehensive nursing care service ward in public hospitals. Methods: The participants were 138 nurses working in Gyeonggi-do medical center. The collected data were analyzed using descriptive statistics, t-test, ANOVA, Pearson's correlation coefficients, and regression analysis with SPSS/WIN 23.0. Results: Factors affecting job satisfaction were innovation-oriented culture (${\beta}=.29$, p<.001), want to work in this department (${\beta}=-.25$, p<.001), the cognitive strategy of self-leadership (${\beta}=.23$, p=.002), hierarchical-oriented culture (${\beta}=-.21$, p=.002), and marital status (${\beta}=.18$, p=.005). These factors accounted for 42.0% of the variability in job satisfaction. Conclusion: In order to improve the job satisfaction of nurses in the comprehensive nursing care service ward in public hospitals, institutional-level efforts should be made to encourage an innovation-oriented culture and avoid a hierarchy-oriented culture. In addition, education and program development are needed to improve the cognitive strategy of self-leadership.

  • 본 연구의 목적은 간호·간병통합서비스병동과 일반병동 간호사를 대상으로 직무만족, 직무스트레스, 간호업무수행을 조사하고 비교하여 간호·간병통합서비스병동의 효율적인 간호인력 관리를 위한 기초자료를 제공하기 위한 비교조사 연구이다. 대상자는 D광역시 일개 종합병원의 간호·간병통합서비스병동 간호사와 일반병동 간호사 136명으로, 자료수집은 구조화된 설문지를 이용하여 SPSS WIN 23.0 program 으로 분석하였다. 간호·간병통합서비스병동 간호사와 일반병동 간호사의 직무만족, 직무스트레스, 간호업무수행에서 집단 간의 유의한 차이는 없었고 간호·간병통합서비스병동 간호사와 일방병동 간호사의 직무만족과 직무스트레스가 높을수록 간호업무수행이 높은 것으로 나타났다. 간호업무수행에 영향을 미치는 요인은 직무만족과 직무스트레스로 간호·간병통합서비스병동 간호사는 20.6%, 일반병동 간호사는 47.5%의 설명력을 보였다. 따라서 직무만족을 높이고 직무스트레스를 적정 유지시킬 수 있는 간호업무수행 능력증진을 위한 지원이 필요하다.

A Study on Relations among the Job Stress, Job Satisfaction, Emotional Labor, and Turnover Intention of Nurses at the Comprehensive Nursing Care Service and General Wards

  • 본 연구는 간호·간병통합서비스병동과 일반병동 간호사의 직무스트레스, 직무만족, 감정노동과 이직의도와의 상관관계를 파악하기 위한 서술적 조사연구이다. 대상자는 간호·간병통합서비스병동 간호사 93명과 일반병동 간호사 92명이다. 연구결과 집단 간 직무스트레스, 직무만족, 감정노동, 이직의도는 유의한 차이는 없었다. 간호·간병통합서비스병동 간호사와 일반병동 간호사의 직무스트레스, 직무만족, 감정노동, 이직의도의 상관관계를 분석한 결과 두 집단 모두 직무스트레스와 감정노동, 직무스트레스와 이직의도, 감정노동과 이직의도 간 유의한 양의 상관관계로 나타났다. 간호사의 직무스트레스와 감정노동을 줄일 수 있는 적정인력 배치, 간호의 명확한 업무경계, 적절한 보상 등을 통해 직무만족을 향상시킴으로써 숙련된 간호인력의 이직을 감소시킬 수 있을 것이다.

  • Objectives: The purpose of this study is to evaluate the factors affecting patient satisfaction for the nursing service of comprehensive nursing care service unit in Tertiary hospital Methods: This study employed a descriptive survey to investigate the nursing satisfaction and the study data was collected from 74 patients among 83 patient who were admitted to the nursing care integration service ward of an tertiary hospital for 10 days from May 9, 2018 to May 19, 2018. Data was analyzed by descriptive statistics, t-test, ANOVA, and multiple linear regression using SPSS 21 program. Results: The average with nursing satisfaction was significantly different according to marriage type and KPCS level. It was found that physical factor and educational factor have statistically significant effect on nursing satisfaction. As physical care increase by one unit, patient satisfaction increase by 0.226(95% CI; 0.061-0.390) and also satisfaction increase by 0.443(95% CI; 0.070-0.816) as education care increase by one unit. Conclusions: As a result of study, it was found that it is necessary to provide patients with more careful nursing service in physical and educational perspective, in order to improve the service quality of comprehensive nursing care service unit.

간호·간병통합서비스 병동 간호사의 직무만족에 미치는 영향 요인 -간호전문직관과 근무환경을 중심으로- (The factors influencing on Compliance with Job Satisfaction of the Nurses in Comprehensive Nursing Care Service Wards - Focusing on Nursing professionalism and Nursing work environment)

  • 본 연구는 간호 간병 통합서비스병동의 간호사의 직무만족에 미치는 영향요인을 파악하고자 하였다. 임상간호사 127명을 대상으로 설문조사하였고, 5점 Likert척도를 이용하여 간호직무만족, 간호전문직관, 간호근무환경을 측정하였다. 자료분석은 SPSS WIN 23.0프로그램을 이용하여 t-test, ANOVA, correlation, Regression분석을 실시하였다. 연구결과는 간호 간병 통합서비스병동 직무만족과 독립변수간 상관계수는 간호전문직관(r=.90, p<.001), 간호근무환경(r=.75, p<.001)으로 유의하였다. 직무만족에 영향을 미치는 요인은 간호전문직관, 간호근무환경 순으로 나타났으며(F=31.516, p<.001), 설명력은 84.1%였다. 따라서 직무만족을 향상시키는 중재방안의 중요성과 올바른 간호전문직관 확립, 간호근무환경의 향상을 위한 융합적 전략의 필요성을 제시하였다.

간호·간병통합서비스 병동 간호사의 감정노동, 강인성이 직무만족도에 미치는 영향 (Influences of Emotional Labor, Hardiness on Job Satisfaction of Nurses in Comprehensive Nursing Care Service Units)

  • 본 연구는 간호·간병통합서비스 병동 간호사의 직무만족도에 미치는 영향을 파악하기 위해 수행된 서술적 조사연구이다. 본 연구는 3개 도시 250병상이상 종합병원 2개와 800병상이상 상급종합병원 3개로 총 5개 병원에서 수행하였다. 연구 대상자는 간호·간병통합서비스 병동에 근무하고 있는 간호사로150부를 배포하여 최종 146부를 회수하였다. 자료수집 기간은 2019년 8월 1일부터 2019년 11월 15일까지였고, x2-test, t-test, ANOVA, Pearson 상관관계, Multiple linear regression으로 분석하였다. 감정노동과 강인성은 음의 상관관계(r=-.20, p=.018), 감정노동과 직무만족도는 양의 상관관계가 나타났다(r=.25, p=.004). 대상자의 직무만족도에 영향을 미치는 예측변인은 병동업무의 적성여부(β=-.35, p=<.001)와 강직성(β=.21, p=.009)으로 나타났다. 직무만족도의 설명력은 18.2%였다(F=3.54, p=.009). 본 연구 결과 간호·간병통합서비스 병동 간호사의 직무만족도에 영향을 미치는 예측변인은 강직성과 병동업무의 적성여부로 나타났다. 따라서 해당 병동 간호사의 직무만족도를 높이기 위해서는 간호사의 강직성 향상과 적성을 고려한 인력배치 등의 행정적인 뒷받침이 요구된다.

종합병원 병동부 간호 이동 동선을 고려한 가시 특성 분석 모델에 관한 연구 (A Study on the Visibility Analysis Model for the Ward of General Hospital Considering Nurse's Circulation)

  • The path and visibility of nurses in general hospital wards have been treated as architectural planning factors. However, the analysis approach of existing studies shows limitations that only fixed physical elements are considered without considering the behavior of users using space. Consider factors for analysis of ward and models based on this study model. Select a case hospital to apply the analysis technique and conduct the nurse questionnaire and route survey of the hospital. Establish a framework for analysis model applied with the path of nurse movements. The analysis model applies to the case hospital. The analysis results are aggregated to derive design suggestion for reference to the spatial improvement of the ward. Visible visibility to observe the bedside in the nursing station, visibility to observe the beds in the nurse's path, and visibility to observe patients moving in the nurse's path were derived from visual access frequency and exposure frequency. The survey of nurses' movements at the site allowed the nurses to calculate the distance required to move. Reflecting the path of nurse movement, a model was presented for a comprehensive analysis of nursing distance and nursing visibility, which could lead to improvement in the observation and visibility of nurses and the layout of patient rooms or day rooms.
