Korean Migrant Health Professionals with Clinical Experience in UAE Hospitals (UAE로 취업한 한인 의료직 종사자의 임상 경험)

  • This study was intented as an investigation of the meaning of clinical experience for gapproach was used to investigate clinical experience for Korean health professionals, A total of 10 health professional immigrants working in UAE hospitals were selected who has experience in Korea over 2 years and UAE for 1 year. Data were collected through in-depth interviews, participant observation and questionnaire. The findings were created with four essential themes; struggling to adapt at workplace at the beginning, feeling frustrated to communicate with different languages, professional reputations as a specialist, a variety of opportunities for self-development supported from hospitals and caring system of staff's health. The results indicate that the Korean health professionals were able to develop capabilities and competency with clinical experience in UAE hospitals. The results of this study suggest that lived experimental evidence for Korean migrants of health professionals. A further direction of this study, will be expected to provide more experimental information for health professional immigrants to middle east.

Ursolic Acid Promotes Apoptosis of SGC-7901 Gastric Cancer Cells through ROCK/PTEN Mediated Mitochondrial Translocation of Cofilin-1

  • Ursolic acid, extracted from the traditional Chinese medicine bearberry, can induce apoptosis of gastric cancer cells. However, its pro-apoptotic mechanism still needs further investigation. More and more evidence demonstrates that mitochondrial translocation of cofilin-1 appears necessary for the regulation of apoptosis. Here, we report that ursolic acid (UA) potently induces the apoptosis of gastric cancer SGC-7901 cells. Further mechanistic studies revealed that the ROCK1/PTEN signaling pathway plays a critical role in UA-mediated mitochondrial translocation of cofilin-1 and apoptosis. These findings imply that induction of apoptosis by ursolic acid stems primarily from the activation of ROCK1 and PTEN, resulting in the translocation of cofilin-1 from cytoplasm to mitochondria, release of cytochrome c, activation of caspase-3 and caspase-9, and finally inducing apoptosis of gastric cancer SGC-7901 cells.

Factors in Dry Period Associated with Intramammary Infection and Subsequent Clinical Mastitis in Early Postpartum Cows

  • The objective of this study was to determine cow characteristics and farm management factors during the dry period associated with early postparturient intramammary infection (IMI) and subsequent clinical mastitis (CM). Data were collected three times: before drying off (P1), during the dry period (P2), and 5 to 14 days after calving (P3), using questionnaires and farm investigation. Milk samples were aseptically collected for bacterial identification at P1 and P3. Factors associated with IMI and CM were analyzed using multiple logistic regression models. The final model showed that IMI in early postpartum was significantly associated with full insertion of dry cow antibiotic, dry cows in barns with a combination of tie and free stalls, body condition score (BCS) in dry period and after calving, and milk yield before drying off. For IMI cows, factors significantly associated with clinical expression of mastitis were having daily barn cleaning, teat disinfected with alcohol before administration of dry cow therapy, BCS before drying off, milk yield before drying off, and days in milk at drying off. In conclusion, both cow and farm management factors are associated with the IMI rate and subsequent expression of clinical signs of mastitis in early postpartum cows.

Epidemiological Investigation of Onychomycosis and Tinea Pedis in Children (소아의 손·발톱 및 족부백선증에 관한 역학적 조사)

  • The number of children patients with tinea pedis and onychomycosis diagnosed on clinical findings and culture at the Catholic Dermatological Clinic in Daegu City were 144,446 for 2 years from January 2005 to December 2006. 120 children were suspected of having clinical onychomycosis or tinea pedis. KOH smears and cultures on the Potato corn meal dextrose agars (PDCA) were done for the suspected toe webs, nails and uninvolved neighbor toe webs. Socks were dusted and cultured to check contamination of clothes. Family infections were checked by questionnaire. The incidence was 92 (0.06%) out of 144,446 outpatients in tinea pedis, 28 (0.02%) in onychomycosis. Trichophyton(T.) rubrums were isolated from all of the 7 cases of onychomycosis. T. mentagrophytes was isolated from 5 cases of the tinea pedis patients and T. rubrum was isolated from 50 (92.5%) cases of the tinea pedis. The right side was predominant for onychomycosis of both toe nails (23 of 25) and finger nails (3 of 3). No fungal elements were detected from normal looking neighbor toe webs by KOH examinations. However, culture on PDCA agars showed T. rubrum isolates from 3 toe webs. 7 showed positive cultures from the socks. From the questionnaire, 94 had family infections.

Clinical Assessment on the Safety of Chunghyul-dan (Qingwie-dan) (청혈단(淸血丹)의 임상적인 부작용에 대한 연구)

  • Background and Purpose : Chunghyul-dan has been widely used in the Department of Cardiovascular & Neurologic Diseases, Kyung Hee Oriental Medical Center to prevent stroke by lowering serum cholesterol level. Previous experimental and clinical studies revealed that Chunghyul-dan had therapeutic effects on hyperlipidemia by inhibiting HMG-CoA reductase and pancreatic lipase. It was also reported that Chunghyul-dan showed an anti-oxidation effect by scavenging free radicals and inhibiting nitric oxide synthesis. Therefore, we examined the safety of Chunghyul-dan on all subjects who had been treated with it. Methods : We performed a retrospective study by reviewing the medical records of those who had been administrated Chunghyul-dan at Kyung Hee Oriental Medical Center from February 8,2001 to December 31,2002. The subjects' general characteristics (gender, age, medical history, and present illness), recorded adverse effects, and the results of laboratory findings were obtained and analyzed to assess the clinical safety of Chunghyul-dan. Results : Six hundred fifty six subjects were treated with Chunghyul-dan. Clinical adverse effects appeared in 13 subjects, the major symptom being indigestion (8 subjects). The apparent frequency of adverse effects was much lower than that in previous reports on the safety of certain medications. On investigation of laboratory findings, we could not find any hepatic or renal toxicity. Conclusion : We suggest that our results contribute towards confirming the safety of Chunghyul-dan by offering clinical evidence.

Effect of Aromatherapy on Anxiety and Fatigue in Students Nurses Experiencing their First Clinical Practice (아로마테라피가 첫 임상실습을 경험하는 간호학생의 불안과 피로에 미치는 영향)

  • Purpose: The purpose of this study was to investigate the effects of aromatherapy on the level of anxiety and fatigue in student nurses who were undergoing their first clinical practice. Method: A non-equivalent. control group. pretest-posttest design was used. A convenience sample of 42 student nurses were selected. They were assigned to experimental and control groups. The aromatherapy using lavender, geranium and camomile was given to the experimental group for four consecutive days while the control group only rested. Result: 1. Anxiety scores for the experimental group were significantly lower than those of the control group on the $2^{nd},\;3^{rd}$ and $4^{th}$ days of clinical practice. 2. Fatigue scores for the experimental group were significantly lower than those of the control group on the $2^{nd},\;3^{rd}$ and $4^{th}$ day of clinical practice. Conclusion: The results support the conclusion that aromatherapy using selected essential oils is an effective intervention for relieving anxiety and fatigue in student nurses experiencing their first clinical practice. However differences between aromatherapy and massage were not clear. Further investigation needs to be done.

The Clinical Study on ShrinKage Rate of Graft following Strip Gingival Autografts (Strip 치은자가이식술후 이식편의 수축률에 관한 임상적 연구)

  • The purpose of this study was to evaluate clinical changes in graft size after treatment with strip gingival autograft in human. 57 premolar teeth in 27 patients having the following mucogingival problems were selected. The width of extension, attached gingiva including free marginal gingiva, width of transplant and clinical sulcus depth were measured at the initial examination, 2, 12 and 24 weeks following the strip gingival autograft and free gingival autograft. The change of width of extension, attached gingiva including free marginal gingiva, width of transplant and clinical sulcus depth according to healing process in both graft procedures was statistically analyzed by repeated measure ANOVA test and independent t-test using SPSS program. The results were as follows : 1. The change of keratinized gingiva in both graft procedures was increased significantly at 24 weeks post-op. 2. The clinical sulcus depth exhibited no marked changes throughout the entire investigation in both graft procedures. 3. No dimensional variation was seen in graft size in both graft procedures. 4. Shrinkage did not differ significantly in both graft procedures. From the day of grafting to 24 weeks after surgery the percentages of shrinkage were : strip gingival autograft 28% and free gingival autograft 29%.

Review of Registration of Interventional Studies of Children and Adolescents in Korea (소아청소년 대상 중재연구의 등록 현황 - CRIS 등록부를 중심으로 -)

  • Objectives The purpose of this study is to investigate the current status of registration of interventional studies focusing on children and adolescents in South Korea and the status of Korean medicine interventional studies through Clinical Research Information Service (CRIS). Methods CRIS was used to search interventional clinical researches which were registered from May, 2010 to October, 2018. Key words of 'child', 'children', 'adolescents', 'infant', 'toddler', 'elementary school student', 'middle and high school', 'middle school student', and 'high school student' were used. Results A total of 37 intervention clinical studies were selected. To identify the trends, years of registration in the CRIS, intervention types, disease categories, recruitment status, phase, institutions responsible for the research, consent, compensation and safety investigation were analyzed. Conclusions This study shows the current state of registration of interventional studies in children and adolescents in Korea. Since interventional studies in children and adolescents are essential for providing safe and effective treatment, further interventional studies, Korean medicine interventional studies and ethical considerations are needed.

Proactive Consultation by a Clinical Pathologist Prevents Diagnostic Delay in Hematological Malignancies

  • Background: Diagnostic difficulties in hematological malignancies may lead to unacceptably prolonged help-seeking to diagnostic interval as well as increased complications and poor outcomes. Proactive consultation by a clinical pathologist (PCCP) may help clinical diagnosis and therapeutic strategy. Hence, the aim of this investigation was to evaluate the effect of PCCP on the help-seeking to diagnostic interval in hematological cancer cases. Materials and Methods: From January to November, 2015, abnormal results of hematological laboratory testing with added laboratory comment were selectively screened out, and patients with such abnormalities in hematological laboratory testing and accompanied laboratory comment with PCCP were enrolled. Results: A total of 125 aberrant results of hematological laboratory testing were given with accompanied laboratory comments with PCCP and 40.8% (n=51) of these patient-oriented comments had an effect on clinical diagnosis and therapeutic strategy. Twelve of the subjects belonged to newly diagnosed hematological malignancies with the assistance of PCCP, and the help-seeking to diagnostic interval was also shortened from 42 days to 26 days in chronic lymphoid leukemia (CLL), from 83 days to 11 days in multiple myeloma (MM), and from 128 days to 15 days in myelodysplastic syndrome/myeloproliferative neoplasm (MDS/MPN). During the monitoring interval, neither complication events nor deaths were reported in the study group. Conclusions: It was seemingly that PCCP prevented diagnostic delay in hematological malignancies via shortening the help-seeking to diagnostic interval, particularly in CLL, MM and MDS/MPN cases. PCCP can be considered to play an essential role in prompt establishment of diagnosis in hematological malignancies for those who newly present.

Perception and Inner Struggle Experienced by Nursing Students in Relation with Infection Management through Observation and Performance of Infection Control Activities (임상실습을 경험한 간호대학생의 감염관리 활동의 관찰과 수행을 통한 감염관리에 대한 인식 및 심리적 갈등)

  • Purpose: To study the internal psychological conflicts among nursing students during an infection control protocol carried out in the hospital by measuring their observation skills and performance during clinical training. Methods: Investigation of both pre- and post- infection control was conducted using questionnaires for clinical infection practices. We identified and evaluated the students' observation skills, clinical performance, clinical perception, and internal conflict regarding clinical infection control. We also interviewed the students as part of our study. Results: Among parameters such as clinical performance, observation skills, clinical perception, and internal conflict, the average observation skills (t=5.49, p<.001) were significantly lower, while internal conflict among students (t=-7.23, p<.001) was significantly higher than expected prior to clinical training. Generally, there was a negative correlation between observation skills and internal conflict in every aspect of infection control practice (r=-.281, p=.031). Internal conflict was significantly higher than expected in the context of hand hygiene (t=-2.135, p=.037), personal hygiene (t=-3.48, p=.002), and ventilator management (t=-3.69, p<.001). Clinical performance of students in the context of hand hygiene (t=4.69, p<.001), personal hygiene (t=2.06, p=.044), and ventilator management (t=2.68, p<.001) was significantly lower than expected prior to clinical training. Conclusion: Our findings showed that internal psychological conflict is higher when infection control practices are observed or performed to a lesser degree. Therefore, reinforcing education regarding infection control among students, such as developing a systematic program, or consecutive training and monitoring, is suggested.
