Analysis on Clinical Characteristics and Influencing Factors of Patients with Locoregionally Advanced Nasopharyngeal Carcinoma

  • Background: To explore the independent prognostic factors for the recurrence/metastasis of patients with locoregionally advanced nasopharyngeal carcinoma (LANPC). Materials and Methods: A total of 604 patients initially diagnosed as LANPC by pathohistology in Fujian Provincial Cancer Hospital were selected to analyze the relationship between the clinical pathological patterns, therapeutic protocols and clinical stages with the recurrence/metastasis of LANPC. Results: The 1-, 3- and 5-year locoregionally recurrent rates of LANPC patients were 2.0%, 9.5% and 12.9% respectively, with average recurrent period being 78 months. Univariate analysis results indicated that clinical stages had certain influence on the recurrent period of LANPC patients. However, COX regression models showed that ages, genders and clinical stages were not the independent prognostic factors influencing the recurrence. The 1-, 3- and 5-year metastatic rates of LANPC patients were 6.6%, 17.5% and 18.8% respectively, with average metastatic period of 73 months. Univariate analysis results demonstrated that ages, N stages, clinical stages, locations of lymph node, retropharyngeal lymph node and extracapsular invasion of lymph node had certain influence on the metastatic period of LANPC patients. Additionally, further COX regression analysis results suggested that T stages, reduction protocols and extracapsular invasion of lymph node were the independent prognostic factors influencing the metastasis of patients with LANPC, in which T stages and extracapsular invasion of lymph node were the pestilent factors while reduction protocols the protective factor. Conclusions: Induction chemotherapy is beneficial to LANPC patients with initial treatment, and the metastatic rate decreases greatly after the application of reduction chemotherapy.

A CLINICAL STUDY ON THE PROGNOSIS AND SUCCESS IN CONVENTIONAL ENDODONTIC TREATMENT (재래식 근관치료의 예후와 성공에 관한 임상적 연구)

  • The purpose of this study was to determine the incidence and the degree of interappointment pain and post obturation pain associated with the pretreated clinical factors or conditions, and to examine the correlation between the success or failure and the pretreated clinical factors or conditions and postoperative pain. The author experienced the conventional root canal therapy in one hundred fifty-one teeth after recording the following clinical factors or conditions - sex, age, pulp vitality status, presence or absence of periapical rarefaction, single or multirooted teeth, pretreatment pain, and investigated the pain during and after treatment for seven days. After six months all the cases were re-evaluated through the clinical sign, symptom, and the recalled radiograph. The following results were obtained; 1. Of the 151 teeth (73.5%) had no interappointment pain, 31 teeth (20.5%) slight pain; and 9 teeth (6.0%) moderate to severe pain. 2. The presence of the pretreatment pain significantly increased the incidence and degree of interappointment, and there were no significant relationship between interappointment pain and other clinical factors or conditions. 3. Of the 151 teeth, 142 teeth (4.0%) experienced no post obturation pain, 3 teeth (2.0%) slight pain, 6 teeth (4.0%) moderate to severe pain. 4. There were no statistical correlation between postobturation pain and any of the clinical factor or conditions. 5. 141 teeth (3.4%) of 151 teeth were evaluated as success in this study, and success rate was decreased significantly in the cases of periapical rarefaction before treatment and postobturation pain. But there were no significant relationship between success or failure and other clinical factors or conditions.

The Effects of Temperament and Character, Emotional Intelligence on Self-leadership of Clinical Nurses (임상간호사의 기질·성격, 감성지능이 셀프 리더십에 미치는 영향)

  • Purpose: This descriptive research study investigated the effects of temperament and character, emotional intelligence on self-leadership of clinical nurses. Methods: This study recruited clinical nurses at a tertiary general hospital located in S city through convenience sampling. A survey was conducted from April 10 to May 5, 2020, using offline questionnaires. A total of 343 answers were analyzed using descriptive statistics and a three-step hierarchical regression analysis with the SPSS 22.0 program. Results: Factors influencing self-leadership of clinical nurses at a tertiary general hospital in S city were persistent among temperament (β=.331, p<.001), personal maturity (β=.408, p<.001), and total emotional intelligence (β=1.062, p<.001). These factors contributed 58% of self-leadership (F=29.20, p<.001). Conclusion: It was confirmed that persistent among temperament and personal maturity among character, emotional intelligence were factors affecting self-leadership of clinical nurses. Therefore, it is necessary to develop and provide programs to control the factors affecting self-leadership of clinical nurses to increase self-leadership.

  • Methicillin-resistant Staphylococcus aureus (MRSA) is a bacterial pathogen capable of causing human diseases, such as soft tissue infection, bacteremia, endocarditis, toxic shock syndrome, pneumonia, and sepsis. Although the incidence rate of diseases caused by MRSA has declined in recent years, these diseases still pose a clinical threat due to their consistently high morbidity and mortality rates. However, the role of virulence factors in staphylococcal infections remains incompletely understood. Methicillin resistance, which confers resistance to all β-lactam antibiotics in cellular islets, is mediated by the mecA gene in the staphylococcal cassette chromosome mec (SCCmec). Differences in SCCmec types and differences in their sizes and structures serve epidemiological purposes and are used to differentiate between hospital-associated (HA)-MRSA and community-associated (CA)-MRSA. Some virulence factors of S. aureus are also providing a distinction between HA-MRSA and CA-MRSA. These factors vary depending on the presence of toxins, adhesion, immune evasion, and other virulence determinants. In this review, we summarized an overview of MRSA such as resistance mechanisms, SCCmec types, HA- and CA-MRSA, and virulence factors that enhance pathogenicity or MRSA epidemiology, transmission, and genetic diversity.

Factors affecting Clinical Practice-related Fatigue among Nursing Students (간호 대학생의 임상실습 관련 피로에 영향을 미치는 요인)

  • The purpose of this study was to identify factors affecting clinical practice-related fatigue among undergraduate nursing students. The data were collected from a questionnaire distributed among 129 juniors and seniors (3rd and 4th year students) in nursing college at the G city, who were at the end of clinical practice. Multiple regression analysis found that the significant major predictors of clinical practice-related fatigue among nursing students were sleep quality (${\beta}=.30$), anxiety (${\beta}=.26$), exercise frequency (${\beta}=-.21$) and clinical practice stress (${\beta}=.19$). These variable factors could explain 33.6% of clinical practice-related fatigue among nursing students. This study suggests that sleep quality, anxiety, exercise frequency, and clinical practice stress are significant factors of clinical practice-related fatigue among nursing students. Therefore, strategies improving sleep quality and exercise and managing anxiety and stress should be developed to reduce the fatigue for nursing students during clinical exposure.

Development of Clinical Competency Self-Report Scale for Clinical Satisfaction of Occupational Therapy Student (작업치료대학생의 실습만족을 위한 임상수행능력 자기보고식 척도 개발)

  • This study is aimed to develop and validate the clinical competence scale of occupational therapy student. The development of clinical competence scale analyzed the definition of clinical performance and previous studies. preliminary examinations were conducted on 203 occupational therapy departments in 3rd and 4th grade to verify item analysis and job validity. After exploratory factor analysis, eight factors of professional consciousness, 11 items of occupational therapy evaluation factors, 4 items of occupational therapy intervention factors, and 4 items of communication factors were extracted into a total of 27 factors. As a result of verifying the reliability of each factor through the internal consistency coefficient Cronbach's α, it was found to be .87~.94 and the overall reliability was .96. The correlation between the total score and the factors of the clinical competence scale was statistically significant. Through the confirmatory factor analysis, the model fit test of the factor structure for 27 items of 4 factors (χ2=.76, df = .31, CFI = .81, TLI = .80, RMSEA = .79) is considered acceptable. Through this study, The clinical competence scale is a valid and reliable scale that can be useful for objectively assessing.

The Effect of Clinical Practice Satisfaction and Self-esteem on Nursing Professionalism of Nursing College Students (간호대학생의 임상실습만족도와 자아존중감이 간호전문직관에 미치는 영향)

  • Purpose: This study involeved a descriptive survey to identify clinical practice satisfaction, self-esteem, and nursing professionalism of nursing college students and determined the factors that affected their nursing professionalism. Methods: Data were collected through a survey of conducted from December 4 to December 20, 2019 (N=186). The relationship among variables was analyzed using Pearson's coefficient correlation and affecting factors for turnover intention were identified by using multiple linear regression. Results: The research results demonstrate that the mean of nursing students' clinical practice satisfaction mean was 3.64 (5 total), the mean of self-esteem was 3.84 (4 total), and the mean of nursing professionalism mean was 3.89 (5 total). Nursing professionalism was significantly different by age and satisfaction with major. Nursing professionalism had positive relationships with clinical practice satisfaction, self-esteem, and satisfaction with major, but had a negative relationships with age. Clinical practice satisfaction, self-esteem, satisfaction with major, and age were identified as influencing factors in turnover intention, and the explanatory power was 43.3%. Conclusion: A strategy to increase clinical practice satisfaction and to enhance self-esteem in nursing college students is necessary. Likewise, further analysis and programs to improve clinical practice environments and training systems should be implemeted.

Effects of Grit and Critical Thinking Disposition on Nursing Students' Clinical Competence (간호대학생의 그릿(Grit)과 비판적 사고성향이 임상수행능력에 미치는 영향)

  • Objectives: A descriptive survey was used in this study to identify the relationship between nursing students' grit, critical thinking disposition, and clinical competence and to examine factors that influence clinical competence. Methods: Data were collected from 241 nursing students and analyzed using t-test, ANOVA, Scheffé test, Pearson correlation, and stepwise multiple regression analysis with the SPSS/WIN 24.0 PC program. Results: Clinical competence was found to have significant pure correlations with critical thinking disposition (r=.49, p<.001), and grit (r=.31, p<.001). The most influential factors influencing nursing students' clinical competence were critical thinking disposition (β=.35, p<.001), and grit (β=.28, p<.001), and these factors explained 33% of the variance in clinical competence. Conclusions: The findings indicate the need to develop and apply strategies and educational programs to enhance individual nurse's critical thinking disposition and grit. Furthermore, exploration is needed on ways to enhance clinical competence.

Influence of Perfectionism and Coping Styles on Burnout in Clinical Nurses (임상간호사의 완벽주의 성향과 대처유형이 소진에 미치는 영향)

  • Purpose: The purpose of this study was to identify the influencing factors on burnout after analyzing the relationship among clinical nurses' various perfectionism and coping styles. Methods: A descriptive, cross-sectional study was conducted with 200 clinical nurses who had six months or more years of clinical experiences from two general hospitals located in D city. The data were analyzed by t-test, one-way ANOVA, Pearson's correlation coefficients, and stepwise regression analysis using IBM SPSS statistics 23.0 program. Results: The burnout of clinical nurses was positively correlated with socially prescribed perfectionism and negatively correlated with active coping style. Socially prescribed perfectionism was positively correlated with active coping style and passive coping style. Influencing factors on the burnout of clinical nurses were socially prescribed perfectionism (${\beta}=.37$), satisfied work unit (${\beta}=-.64$) and neutral satisfaction of work unit (${\beta}=-.27$), over 40 years of age (${\beta}=-.21$), and active coping style (${\beta}=-.14$). The model consisting of these variables explained 42% of variance of burnout in clinical nurses. Conclusion: Based on the findings of this study developing intervention programs that consider influencing factors such as perfectionism and coping styles is needed to reduce the level of burnout of clinical nurses.

Influencing factors of and satisfaction with clinical practice between college and university dental hygiene students (학제에 따른 치위생과 학생의 임상실습 만족도와 관련 요인)

  • Objectives: This research aimed to identify the influential factors that affect the satisfaction of dental hygiene students' clinical training between college and university and contribute to improvement in clinical practice quality. Methods: A self-reported questionnaire was completed by 210 dental clinical hygienists from June 10 to August 30, 2018. Data were analyzed using SPSS 22.0. An independent t-test, one-way ANOVA, Scheffe's post-hoc test, and Pearson's correlation coefficient were employed, and a stepwise multiple regression analysis was performed. Results: Regarding the influential factors of satisfaction with clinical practice among dental hygiene students, in the case of college students, dental hygienist practice instructor dummy, self-efficacy, satisfaction of school life, interpersonal relation was the increasing order. In the case of university students, dental hygienist practice instructors, subjective health, satisfaction with school life, and interpersonal relations was the increasing order. Conclusions: The most influential factor of dental hygiene students' clinical practice satisfaction between college and university was the ability to form interpersonal relationships. Therefore, to increase the satisfaction of dental hygiene students with clinical practice between college and university, external sources and efforts to emphasize internal sources are required.
