The purpose of this study was to develop Nurse's Suffering Experience Scale and to test the reliability and validity of the instrument. Method: The subjects used to verify the scale's reliability and validity were 220 nurses who were taking care of the end stage cancer patients, while working at university and general hospitals in Daegu and Kyungbuk province from April 20. to July 10, 2001. The data was analyzed by the SPSS/WIN 8.0 program. Results: A factor analysis was conducted, and items that had a factor loading more than .40, and an eigen value more than 1.0. were selected. The factor analysis classified a total of seven factors statistically, and it's communality was 44%. The explanation of factors based on the conceptual framework and item content are as follows: The first factor was expanding self consciousness, the second factor was forming empathy with family, the third factor was professional challenge, the fourth factor was change of values, the fifth factor was spiritual sublimation, the sixth factor was helplessness, and finally the seventh factor was rejection to death. Cronbach's coefficient to test reliability of the scale was .8665 for total of 44 items. The Scale for Nurse's Suffering Experience developed in the study was identified as a tool with a high degree of reliability and validity. Therefore this scale can be effectively utilized for the evaluation of the degree of nurse's suffering experience in clinical settings.

Background: Preconditioning with inflammatory stimuli is used to improve the secretion of anti-inflammatory agents in stem cells from variant species such as mouse, human, and dog. However, there are only few studies on feline stem cells. Objectives: This study aimed to evaluate the immune regulatory capacity of feline adipose tissue-derived (fAT) mesenchymal stem cells (MSCs) pretreated with interferon-gamma (IFN-γ). Methods: To assess the interaction of lymphocytes and macrophages with IFN-γ-pretreated fAT-MSCs, mouse splenocytes and RAW 264.7 cells were cultured with the conditioned media from IFN-γ-pretreated MSCs. Results: Pretreatment with IFN-γ increased the gene expression levels of cyclooxygenase-2, indoleamine 2,3-dioxygenase, hepatocyte growth factor, and transforming growth factor-beta 1 in the MSCs. The conditioned media from IFN-γ-pretreated MSCs increased the expression levels of M2 macrophage markers and regulatory T-cell markers compared to those in the conditioned media from naive MSCs. Further, prostaglandin E2 (PGE2) inhibitor NS-398 attenuated the immunoregulatory potential of MSCs, suggesting that the increased PGE2 levels induced by IFN-γ stimulation is a crucial factor in the immune regulatory capacity of MSCs pretreated with IFN-γ. Conclusions: IFN-γ pretreatment improves the immune regulatory profile of fAT-MSCs mainly via the secretion of PGE2, which induces macrophage polarization and increases regulatory T-cell numbers.
This study is attempted for the purpose of clearly understanding the stressors which emergency medical technology students experience during clinical practice, giving guidance in desirable direction and improvement of clinical practice, and giving the basic materials so that emergency medical technology students may practice effectively. The design of this study was a descriptive survey. The subjects in this study were 59 emergency medical technology students who were 2nd grade in D college. The collection of questionnaires took 3 days from September 1, 1999 to September 3, 1999. This researcher explained the purpose of this study to students, after they agreed to this survey, the researcher gave the questionnaire to them, had them answer the questions and collected the questionnaire immediately. Data analysis included percentage, mean and standard deviation, t-test and ANOVA trough SPSS computer program. The results of this study are, briefly, as follows; 1. In 'satisfaction-degree to specializing in the science of emergency medical technology' 27.1% of the subjects is revealed as 'satisfied' and in 'satisfaction-degree of clinical practice', 8.5% of the subjects is revealed as 'satisfied'. 2. Viewed from each of the categories of stressors which emergency medical technology students experience during clinical practice, 'ideals and values' is the category in which emergency medical technology students experience most severe stress at a mean 3.25 and 'personal relations' is the category in which emergency medical technology students experience mildest stress at a mean 2.82. The total stress degree mean is 3.06, which emergency medical technology students experience during clinical practice is revealed as 'moderate' stress. 3. Considering the stressor involved in each category, the results are as follows; (1) In 'environment', 'orientation to the hospital before clinical practice' is revealed as the most severe stressor at a menu 3.44. (2) In 'personal relations', 'relations with doctor and nurse' is revealed as the most severe stressor at a mean 3.59. (3) In 'role', 'unilaterally when their role is seems to be obscure' is revealed as the most severe stressor at a mean 4.10. (4) In 'ideals and values', 'difference between college practice and hospital practice' is revealed as the most severe stressor at a mean 3.88. (5) In 'activity', 'repetition of simple and functional work during clinical practice' is revealed as the most severe stressor at a mean 3.98. (6) In 'total stress factors', 'unilaterally when their role is seems to be obscure' is revealed as the most severe stress factor at a mean 4.10 and 'relations with comrade students' is revealed as the stress factor causing mildest stress at a mean 2.20. 4. Considering the relationship between the categories of stressors and the general characteristics, the results are as follows; (1) 'Environment' is revealed as being positively correlated with 'satisfaction degree of clinical practice'(F=3.53, p= .0359). (2) 'Ideals and values' is revealed as being correlated with 'sex'(t=-2.31, p= .0240) and 'satisfaction degree of clinical practice'(F=3.31, p= .0438).

Background: Oxidation plays an important role in acute lung injury. This study was conducted in order to elucidate the effect of repetitive post-treatment of N-acetylcysteine (NAC) in lipopolysaccaride (LPS)-induced acute lung injury (ALI) of rats. Methods: Six-week-old male Sprague-Dawley rats were divided into 4 groups. LPS (Escherichia coli 5 mg/kg) was administered intravenously via the tail vein. NAC (20 mg/kg) was injected intraperitoneally 3, 6, and 12 hours after LPS injection. Broncho-alveolar lavage fluid (BALF) and lung tissues were obtained to evaluate the ALI at 24 hours after LPS injection. The concentration of tumor necrosis factor ${\alpha}$ (TNF-${\alpha}$) and interleukin $1{\beta}$ (IL-$1{\beta}$) were measured in BALF. Nuclear factor ${\kappa}B$ (NF-${\kappa}B$), lipid peroxidation (LPO), and myeloperoxidase (MPO) were measured using lung tissues. Micro-computed tomography (micro-CT) images were examined in each group at 72 hours apart from the main experiments in order to observe the delayed effects of NAC. Results: TNF-${\alpha}$ and IL-$1{\beta}$ concentration in BALF were not different between LPS and NAC treatment groups. The concentration of LPO in NAC treatment group was significantly lower than that of LPS group ($5.5{\pm}2.8$ nmol/mL vs. $16.5{\pm}1.6$ nmol/mL) (p=0.001). The activity of MPO in NAC treatment group was significantly lower than that of LPS group ($6.4{\pm}1.8$ unit/g vs. $11.2{\pm}6.3$ unit/g, tissue) (p<0.048). The concentration of NF-${\kappa}B$ in NAC treatment group was significantly lower than that of LPS group ($0.3{\pm}0.1\;ng/{\mu}L$ vs. $0.4{\pm}0.2\;ng/{\mu}L$) (p=0.0001). Micro-CT showed less extent of lung injury in NAC treatment than LPS group. Conclusion: After induction of ALI with lipopolysaccharide, the therapeutic administration of NAC partially attenuated the extent of ALI through the inhibition of NF-${\kappa}B$ activation.
Purpose: The purpose of this study was to develop the Job Esteem Scale for Korean Nurses (JES-KN) and verify its validity and reliability. Methods: Preliminary items were based on the attributes and indicators elicited from a concept analysis study on Korean nurses' job-esteem. The final preliminary tool for the main survey was confirmed through the content validity test of 10 experts and preliminary survey of 20 hospital nurses. The final preliminary scale was used on 350 hospital nurses in the scale testing phase for the main survey designed to test the validity and reliability of the scale. Results: The final scale consisted of 28 items and 6 factors, these factor explained 66.6% of the total variance. The correlation between the total score and factors ranged from .64 and .84, validating that each sub-factor is suitable to explain job esteem. The correlation coefficient between this scale and the Job Satisfaction Scale for Clinical Nurses ranged from .41 to .70, and the internal consistency for the scale using Cronbach's α for the total items was .94. Conclusion: The JES-KN is a valid and reliable tool that reflects the reality of clinical sites accordingly. The JES-KN may well be used effectively to assess and evaluate the job esteem of Korean nurses.
Purpose: This study was conducted to evaluate the validity and reliability of the Korean version of the Sexuality Attitudes and Beliefs Survey (SABS) and to assess SABS for Korean nurses. Methods: The Korean version of SABS was developed through forward-backward translation techniques. Internal consistency reliability and construct validity using confirmatory factor analysis were conducted using PASW+ PC Win (18.0) and AMOS (18.0). Data were collected from 567 nurses who worked in one of six general hospitals across the country. Results: The Korean version of SABS showed a reliable internal consistency with Cronbach's ${\alpha}$ of subscales ranging from .59 to .73. Factor loadings of the 10 items of three subscales ranged from .38 to .83. The three subscales model were validated by confirmatory factor analysis (GFI>.97, RMSEA<.05). Sexuality attitudes and beliefs for Korean nurses were more negative than that of European or American nurses. The SABS scores for Korean nurses were significantly different according to age, marriage, education, clinical experiences, and feeling about sexuality. Conclusion: The Korean version of SABS has satisfactory construct validity and reliability to measure Korean nurses' attitudes and belief toward sexuality. Education is essential to enhance importance and self-efficacy and to relieve barriers to addressing patients' sexuality.
Purpose Pulmonary complications continue to be the major cause of morbidity and mortality after esophageal resection. The aim of this study was to compare and analyze retrospectively the factors which effect for postoperative pulmonary complications in patients who underwent curative resection for esophageal cancer. Material and Method A total of 118 patients were enrolled in the study from January 1994 to March 2009, and patients with previous neoadjuvant chemotherapy or radiotherapy were excluded. Of the total 118 patients, 27 patients developed pulmonary complications within 30 days of their operation. the factors which effect for postoperative pulmonary complications were compared and analyzed. Results There were 7 patients in-hospital deaths. 51 patients (43.2%) developed complications, and of them, the most common complication was pulmonary complication and occurred in 27 patients (22.9%). In univariate analysis, diabetes mellitus, cervical anastomosis through the retrosternal route, old age and poor lung function were risk factors contributing to postoperative pulmonary complications (p<0.05). In multivariate analysis, statistically significant factor was old age (65 years or older). Conclusion Clinical factor for the pulmonary complications after esophagectomy of esophageal cancer was significantly associated with diabetes mellitus, cervical anastomosis through the retrosternal route, old age (65 years or older) and poor lung function (FEV1<80%). Of these, old age was the most significant factor.
Purpose: This study aims to assess the reliability and validity of a Korean version of the satisfaction with simulation experience (SSE) scale for low-, medium-, and high-fidelity simulations among paramedic students. Methods: Exploratory factor analysis with varimax rotation was used to determine construct validity. Cronbach's alpha was calculated to assess internal consistency reliability. Results: Cronbach's alpha for the overall scale was .841. Exploratory factor analysis yielded a three-factor structure composed of clinical learning, clinical reasoning and debriefing and reflection. Each of the subscales had high internal consistency, with Cronbach's alphas of .852, .790 and .913, respectively. With respect to year, freshmen scored the highest on the SSE scale. With respect to type of simulation, the medium-fidelity group reported the highest satisfaction. Generally, the students were satisfied with debriefing and reflection. Conclusion: The Korean version of the SSE scale was shown to be a reliable and valid instrument for assessing satisfaction with simulation learning in paramedic students.
The statistical regression model is one of the most frequently used clinical analysis methods. It has basic assumption of linearity, additivity and normal distribution of data. However, most of biological data in medical field are nonlinear and unevenly distributed. To overcome the discrepancy between the basic assumption of statistical model and actual biological data, we propose a new analytical method based on artificial neural network. The newly developed multilayer perceptron(MLP) is trained with 120 data set (60 normal, 60 patient). On applying test data, it shows the discrimination power of 0.76. The diabetic risk factors were also identified from the MLP neural network model and the logistic regression model. The signigicant risk factors identified by MLP model were post prandial glucose level(PP2), sex(male), fasting blood sugar(FBS) level, age, SBP, AC and WHR. Those from the regression model are sex(male), PP2, age and FBS. The combined risk factors can be identified using the MLP model. Those are total cholesterol and body weight, which is consistent with the result of other clinical studies. From this experiment we have learned that MLP can be applied to the combined risk factor analysis of biological data which can not be provided by the conventional statistical method.
Purpose: The purpose of this study was to develop an instrument to evaluate the needs satisfaction of nurses and examine its validity and reliability. Methods: The initial items for the instrument were developed through a literature review and interviews, using the conceptual framework of Maslow's hierarchy of needs theory. The initial items were evaluated for content validity by 14 experts. Four hundred and eighty-six clinical nurses participated in this study through offline and online surveys to test the reliability and validity of the instrument. The first evaluation (n = 256) was used for item analysis and exploratory factor analysis, and the second evaluation (n = 230) was used to conduct a confirmatory factor analysis and to assess the criterion-related validity and internal consistency of the instrument. Test-retest reliability was analyzed using data from 30 nurses. Results: The final instrument consisted of 30 items with two sub-factors for five needs that were identified through the confirmatory factor analysis. The criterion-related validity was established using the five need satisfaction measures (r = .56). Cronbach's α for total items was .90, and test-retest reliability was .89. Conclusion: The findings from this study indicate that this instrument has sufficient validity and reliability. This instrument can be used for the development of nursing interventions to improve the needs satisfaction of clinical nurses.
