Retrospective Study for Safety of Xie-xia Fa (瀉下法) by Euphorbia kansui Radix: Analysis of Clinical Features, Liver and Kidney Functions (감수(甘遂)를 이용한 사하법(瀉下法)의 안전성에 대한 후향적 연구: 임상반응, 간기능, 신기능을 중심으로 한 평가)

  • Objectives: The purpose of this study was to report the safety of diarrhea-inducing treatment (Xie-xia Fa, 瀉下法) by Euphorbia kansui Radix, especially in the respect of clinical features, liver and kidney functions. Methods: Patients who were treated with Euphorbia kansui Radix powder between September 2013 and September 2014 were assessed retrospectively. Results: Less than 6 grams dosage of Euphorbia kansui Radix can induce several mild intestinal reactions but have no harmful effects on liver and kidney functions. Conclusions: Through this study, diarrhea-inducing treatment by less than 6 grams of Euphorbia kansui Radix can considered safe enough to be used in local clinics, but a cautious approach is needed.

Single Oral Dose Toxicity Test of Areca catechu Aqueous Extracts in Mice (빈랑자(檳榔子) 추출물의 마우스 경구 단회 투여독성 평가)

  • This study was to evaluate the single dose toxicity of Arecae Semen (AS) in male and female ICR mice. Aqueous extracts of AS (Yield = 13.15%) were administered as an oral dose of 2,000, 1,000 and 500 mg/kg (body weight) according to the recommendation of Korea Food and Drug Administration (KFDA) guidelines. Animals were monitored for the mortality and changes in body weight, clinical signs and gross observation during 14 days after dosing, upon necropsy; organ weight and histopathology of 12 principle organs were examined. We could not find any mortality, clinical signs, and changes in the body and organ weight except for diarrhea. Diarrhea were observed in all three different dosage groups of male mice, and in 2000 mg/kg groups of female mice within 48hrs after administration. In addition, no AS extract related abnormal gross findings and changes in histopathology of principle organs were detected except for some sporadic accidental findings. Although the 50% lethal dose and approximate lethal dose of AS aqueous extracts in female and male mice were detected as over 2,000 mg/kg - the limited highest dosage recommended by KFDA guidelines. It should be carefully used in clinics because AS may be induced severe digestive tract disorders.

Epidemiology of the transmissible gastroenteritis of pigs in Gyeongbuk province (경북지방 돼지에서 발생한TGE의 역학적 특성)

  • Five pigs industry with outbreaks of transmissible gastroenteritis(TGE) in Gyeongbuk province were investigated during the period from January to December 2000. The typical signs of TGE in piglets had transient vomiting and a watery yellowish diarrhea, rapid loss of weight, dehydration and high mortality in pigs under 2 weeks of age. Clinical signs of TGE in growing and finishing pigs and sows were usually limited to inappetence and diarrhea for one or a few days, with vomiting observed in an occasional animal. The detection of TGE viral antigen in epithelial cells of the small intestine had been used in indirect fluorescent antibody test (IFA) for diagnosing TGE in young pigs. WかR had been successfully used to detect the DNA derived from TGEV in specimen of intestinal swabs. Among 5 pigs industry, four showed typical signs of epizootic TGE and one progressing enzootic TGE. It was 22~53 days that the duration of initial clinical disease in TGE outbreaks of pigs investigated in Gyeongbuk province in 2000. However the duration related directly to herd size. Mortality of piglets under 2 weeks of age for duration was 53.2~88.2%, but that of piglets 2~5 weeks of age was 2.5~6.5%. The piglets of 1 weeks of age died mostly during duration of TGE, but varied considerably with husbandry and other environmental factors.

Economic Evaluation of Ramosetron in Male Patients with Diarrhea-predominant Irritable Bowel Syndrome in Korea (설사형 과민성 장 증후군 남성 환자에게 사용되는 ramosetron 의 경제성 평가)

  • This study was conducted to analyze the cost-utility of ramosetron monotherapy, trimebutine monotherapy and trimebutine+loperamide combination therapy in male diarrhea-predominant patients with irritable bowel syndrome (IBS) in Korean healthcare setting. We constructed a decision-analytical model to estimate both total costs for each state of health and outcomes such as IBS-symptoms improvement for 3 and 6 months time horizon. Base analysis found that for ramosetron treatment with the price of KW910 for 5 ${\mu}g$ tablet, incremental cost effectiveness ratios (ICERs, cost per quality-adjusted life day) were KW85,000 and KW62,000 for 3 months and 6 months, respectively, compared with trimebutine. But ramosetron was a dominant strategy when compared with trimebutine+loperamide for both 3 months and 6 months. Sensitivity analyses showed robust results for drug acquisition costs till ramosetron price of KW950/tablet. In conclusion, ramosetron was a cost-effective regimen compared with trimebutine or trimebutine+loperamide from the societal perspective.

Clinical Study of Oriental Medical Treatments for Neonates in Oriental Medicine Hospital Postpartum Care Center (한방병원 산후조리센터 내 신생아의 한방적 처치에 대한 임상적 고찰)

  • Objective : The aim of this study is to investigate the effects of oriental medical treatments for neonates in the Postpartum care center. Methods : This clinical study was carried out with the 186 neonates who admitted to the Postpartum Care Center of Conmaul Oriental Medicine Hospital, from November, 2003 to May, 2004. The data of sex, delivery method, gestation age, birthweight, symptoms, herbal prescription, improved condition and other treatments was collected via the medical charts. Results : Among 186 newborn babies, 48 cases had diarrhea. We used Gwakhyangjeonggi-san for 31 cases, Bulwhangumjeongnggi-san for 5 cases, compound prescription of the two for 8 cases. The effect of treatment was excellent for 25 cases, good for 11 cases, not good for 8 cases. In fever, there were 39 cases. We used Gwakhyangjeonggi-san and venesection of blood for 17 cases, only venesection of blood for 20 cases, and only Gwakhyangjeongg-isan for 3 cases. The effect of treatment was excellent for 37 cases, good for 3 cases. In cold, there were 4 cases. We used Samsoum for 1 case, Socheongryong-tang for 3 cases. The effect of treatment was excellent for 2 cases, good for 1 case, not good for 1 case. Conclusions : This study shows oriental medical treatments like acupucture, herbal medicine, and venesection of blood for neonates have significant effects. we will need further prospective studies about effects of these oriental medical treatments.

Pathological changes on naturally occuring necrotic enteritis in chicken (닭의 괴사성 장염에 대한 병리학적 연구)

  • From January of 1991 to December of 1992, 42 chickens collected from 21 poultry farms and also diagnosed as necrotic enteritis(NE) was examined clinical signs, gross and histopathological findings. Main clinical signs were characterized by decreased appetite, mild to severe depression, reductance to move, ruffled feathers, greenish to yellow-browinish diarrhea sometimes including blood. As progressed, diseased chickens showed feces mixed with necrotic debris which detached from the intestinal mucosa and mostly resulted in the death. In chronic cases, there were dirty feathers around cloaca due to diarrhea and notably retarded growth. Principle gross lesions were usually confined to the jejunum and ileum, especially toward the lower part of Meckel's diverticulum. The part of small intestine was frequently distended with gas, and also showed mucosal congestion and hemorrhages with varying degrees. Sometimes, the intestinal mucosa was thickened, and also covered with fibronecrotic psuedomembrane. In addition, there were focal necrosis and severely multifocal ulcreation in the mucosa of small intestine. Major histopathological findings included villous necrosis and erosion of the small intestine covering with lots of bacterial colonies, inflammatory cell infiltration in the lamina propria, and dilatation and hyperplasia of crypts. Luminal exudate contained bacterial colonies, fibrin, erythrocytes, and desquamated epithelium. Thirteen(61.9%) out of 21 NE-occurring farms were complicated with intestinal coccidiosis.

Salmonellosis in swine: Clinical perspectives

  • Salmonella is one of the most important food-borne zoonotic pathogens, causing acute or chronic digestive diseases such as enteritis. The acute form of enteritis is common in young pigs of 2 - 4 months of age. The main symptoms include high fever ($41-42^{\circ}C$), loss of appetite, and increased mortality within 2 - 4 days of onset of the disease. It is often the cause of increasing mortality, decreasing growth rate and reducing feed efficiency of piglets. In the case of chronic enteritis in pigs, the main symptom is weight loss due to the continuing severe diarrhea. Salmonella enterica serovar Typhimurium and Salmonella enterica serovar Choleraesuis are typical pig adapted serotypes, which cause one of four major syndromes: enteric fever, enterocolitis/diarrhea, bacteremia and chronic asymptomatic carriage. These syndromes cause a huge economic burden to swine industry by reducing production. Therefore, it is necessary that swine industries should strive to decrease Salmonellosis in pigs in order to reduce economic losses. There are several measures, such as vaccination to prevent salmonellosis, that are implemented differently from country to country. For the treatment of Salmonella, ongoing antibiotic treatment is needed. However constant doses of antibiotics can be a problem because of antibiotic resistance. Therefore, the focus should be made more on prevention than treatment. In this review, we addressed the basic information about Salmonella, route of infection, clinical symptoms, and prevention of Salmonellosis.

Epidemiological and Clinical Characteristics of Confirmed Cases of Severe Fever With Thrombocytopenia Syndrome in Jeju Province, Korea, 2014-2018

  • Objectives: Jeju Province is well known as the region showing the highest incidence of severe fever with thrombocytopenia syndrome (SFTS) in South Korea. The aim of this study was to evaluate the epidemiological and clinical characteristics of SFTS patients in Jeju Province. Methods: The primary data for this study were obtained from the Integrated Diseases and Health Control System of the Korea Centers for Disease Control and Prevention (KCDCIS). The selection criteria were confirmed cases of SFTS with a residence listed in Jeju Province at the time of diagnosis, reported to the KCDCIS between July 16, 2014 and November 30, 2018. Results: Of 55 confirmed cases of SFTS, the case fatality rate was 10.9% (95% confidence interval [CI], 4.1 to 22.2). The most common presenting symptoms at diagnosis of severe fever, myalgia, and diarrhea had incidences of 83.6% (95% Cl, 71.2 to 92.2), 45.5% (95% Cl, 32.0 to 59.5), and 40.0% (95% CI, 27.0 to 54.1), respectively. Conclusions: Compared to SFTS patients nationwide in 2013-2015, the subjects of this study exhibited a lower case fatality rate and had a lower incidence of severe fever, myalgia, and confusion.

An analysis of Clinical Studies on Galgeun-tang (갈근탕의 임상연구 논문 분석)

  • Objective : This study's purpose was to review the clinical studies of Galgeun-tang. Method : We searched papers about Galgeun-tang using KISS, RISS, OASIS, PUBMED and J-stage. The key words we used were "Galgeuntang", "Kakkonto", "Ge gen tang", and "Pueraria Decoction". Papers not matched with inclusion criteria were excluded. Results : Until today, there have been 223 studies on Galgeun-tang. Of these, 25 studies were classified as clinical research papers. There were 4 cases of fever, 5 cases of inflammation on respiratory system, 3 cases of head, neck and shoulder disorder, 2 cases of diarrhea, 6 cases of pharmacokinetics and interaction, 5 cases of side effect. Conclusion : It can be seen that Galgeun-tang has established the basis for application to the purpose of treating fever (common cold, influenza), inflammation on respiratory system (nasal obstruction, maxillary sinus retention cyst, mucoid pseudomonas aeruginosa chronic lower respiratory tract infection), head, neck and shoulder disorder (temporomandibular disorders, shoulder stiffness, tetanus), and diarrhea. On the other hand, considering 4 cases of side effect on drug eruption, caution should be exercised when observing the progress of the patient taking Galgeun-tang.

Clinical manifestation of Campylobacter enteritis in children

  • Purpose: Timely antibiotic therapy in selected cases of diarrhea associated with bacterial infections can reduce the duration and severity of illness and prevent complications. The availability of a predictive index before identification of causative bacteria would aid in the choice of a therapeutic agent. Methods: The study included patients admitted to the pediatrics unit at Konyang University Hospital for acute inflammatory diarrhea from August 1, 2015 to July 31, 2016 who underwent multiplex polymerase chain reaction testing. Of 248 patients, 83 had positive results. The clinical symptoms and blood test results were examined in 61 patients with Campylobacter spp. (25 patients), Salmonella spp. (18 patients), and Clostridium perfringens (18 patients) infections. The mean age of the 61 patients (male:femal=31:30) was $84.0{\pm}54.8months$, and the mean hospital stay was $4.6{\pm}1.7days$. Results: There were no statistical differences in sex, age, clinical symptoms, or signs. Patients with Campylobacter infection were significantly older (P=0.00). C-reactive protein (CRP) levels in patients with Campylobacter infection were higher than those in the other 2 groups, at $9.6{\pm}6.1mg/dL$. The results of receiver-operating characteristic curve analysis showed that the cutoff age was ${\geq}103.5months$ (sensitivity, 72%; specificity, 86%) and the CRP cutoff level was ${\geq}4.55mg/dL$ (sensitivity, 80%; specificity, 69%). Conclusion: Age (${\geq}103.5months$) and higher CRP level (${\geq}4.55mg/dL$) were good predictors of Campylobacter enterocolitis. If neither criterion was met, Campylobacter enterocolitis was unlikely (negative predictive value 97.2%). When both criteria were met, Campylobacter enterocolitis was highly likely.
