  • Objectives This study aimed to propose biomarkers for diagnosing Chuna manual therapy (CMT) based on X-ray images in the thoracic and lumbar spines. Methods Through a literature review and expert consensus process, diagnostic biomarkers for CMT were selected based on the listing system in thoracic and lumbar radiograph anterior-posterior (AP) and lateral views. Results 1. Diagnostic biomarkers were derived from four points on the outer contour of the vertebral body in the thoracic and lumbar spine radiograph lateral view, enabling the diagnosis of flexion and extension malposition. 2. Additional diagnostic biomarkers were identified in the thoracic and lumbar radiographAP view, utilizing points on the outer contour of the vertebral body. These biomarkers facilitate the diagnosis of lateral bending. Moreover, biomarkers derived from the innermost point of the pedicle contour allow for the diagnosis of rotation malposition. 3. Furthermore, through the biomarkers proposed in this study, all malpositions of the thoracolumbar spines and complex Type I and II malpositions can be diagnosed in CMT. Conclusions The biomarkers reported in this study consist of minimal points to determine the position of the vertebral body, providing the advantage of simplicity while minimizing potential errors during the CMT diagnostic process. Further clinical research and the development of related programs should be pursued to expand the evidence for CMT.

  • Objectives The aim of this study was to systematically review and analyze the clinical efficacy of Chuna Manual Therapy (CMT) for the treatment of Sacroiliac Joint Syndrome (SIJS). Methods A comprehensive literature search was conducted up to April 2024, utilizing databases such as PubMed, EMBASE, The Cochrane Library, CNKI, ScienceON, Kmbase, OASIS, RISS, and KISS. Studies were selected based on the PICOS criteria: Participants (patients with SIJS), Intervention (CMT), Control (various treatments), Outcomes (pain and functional improvements), and Study design (randomized controlled trials, RCTs). The Risk of Bias (ROB) was assessed using the Cochrane ROB tool, and data were analyzed using Review Manager 5.4.1. The primary outcomes were pain reduction measured by Visual Analogue Scale (VAS) and treatment efficacy rates. Results Out of 330 identified studies, 10 RCTs involving 776 patients met the inclusion criteria. The selected studies compared CMT with acupuncture, pharmacotherapy (celecoxib), and physical therapy. Meta-analysis showed that CMT significantly reduced pain compared to physical therapy (p<0.00001). When compared with pharmacotherapy, the results indicated no statistically significant difference (p >0.05). Combined treatments of CMT with other modalities such as acupotomy and cupping showed enhanced efficacy rates and pain reduction, suggesting synergistic effects. Conclusions The review indicates that CMT is effective in reducing pain and improving function in patients with SIJS, especially when compared to physical therapy. However, the study also highlights the need for larger sample sizes and longer follow-up periods to validate these findings. Additionally, future studies should address the challenges of blinding in manual therapy trials to mitigate bias risks. Despite these limitations, CMT appears to be a viable option for the treatment of SIJS, with potential for enhanced outcomes when combined with other therapeutic modalities.

  • Objectives This study aims to evaluate the effectiveness of cervical chuna manipulative treatment (CCMT) in patients with peripheral facial paralysis (PFP). Methods We conducted a comprehensive search of MEDLINE, CENTRAL, EMBASE, KMbase, OASIS and ScienceOn, and CNKI for randomized controlled trials (RCTs) comparing CCMT for PFP up to April 25, 2024. Risk of Bias was assessed using the Cochrane RoB tool. Results Among the 462 studies retrieved, 5 RCTs with a total of 340 patients were included in the systematic review. Of these, 4 RCTs that reported the total effectiveness rate as the primary outcome were included in the meta-analysis. CCMT combined with acupuncture showed significant improvement in total effectiveness rate compared to acupuncture alone (Relative Ratio [RR] 1.14, 95% CI 1.04 to 1.26, I2=49, P<0.008). However, the included studies had some limitations including uncertain risk of bias and short follow-up periods. Conclusions CCMT may be an effective treatment for PFP. Due to the low quality of evidence from the included studies, high-quality, well-designed RCTs are needed to further verify the effectiveness and safety of CCMT.

  • Objectives : The objectives of this report is to introduce PTA. Methods : The examination of the leg length inequality gives us a useful tool for diagnosis of body imbalance. especially pelvic distortion. There are four steps in the process of the leg analysis, according to "The Standard Manual of Chuna Society (2nd ed., Seoul:KCA Press. 2001)". In the last step of the analysis, knee-flexing over $90^{\circ}$, we have often experienced a specific sign that the lower legs are attracted toward one side spontaneously. The authors call it 'Lower Leg Lateral Attraction'. This is a very significant sign that gives us which is the major part between pelvis and the upper parts over sacrum. Thus it is definded as "Pelvic Twist Analysis, PTA" by the authors. With PTA, first, you must check the side of short leg and next, check the side of lateral attraction in lower leg over-flexing. If both sides coincide with each other, then the major part you can correct first is pelvic distortion. If not, you must find another part for primary correction, instead of pelvis. Conclusions : PTA becomes a useful complement to the examination of the leg length inequality.
