어머니의 개인·사회적 대처자원이 성인발달장애인 자녀의 돌봄스트레스에 미치는 영향 (The Effects of Personal and Social Coping Resources of Mothers on the Stress of Caring Adult Children with Developmental Disabilities)

  • 본 연구는 성인발달장애인 자녀를 돌보는 어머니의 대처자원과 돌봄스트레스와의 관계를 파악하고 그에 따른 실천적 차원의 개선방안을 제시하고자 하는 데 있다. 연구를 위해 만18세 이상에 속한 성인발달장애인 자녀를 돌보고 있는 주돌봄자인 어머니 119명을 대상으로 조사한 자료를 위계적 중다회귀분석방법을 통해 분석하였다. 연구결과 첫째, 돌봄스트레스는 어머니의 연령이 높은 50세 ~ 54세 집단이 연령이 낮은 49세 이하 집단보다 높게 나타났고, 가구 소득은 200만원 이하 집단이 550만원 이상집단보다 스트레스가 높게 나타났으며, 또한 취업 중인 경우가 그렇지 않은 경우보다 돌봄스트레스를 더 많이 받는 것으로 나타났다. 둘째, 어머니의 대처자원 중 사회적 대처자원인 가족지지와 친구지지가 돌봄스트레스에 영향을 미치는 것으로 나타났다. 즉 가족지지와 친구지지가 어머니의 돌봄스트레스를 완화시키는 것으로 나타났다. 이 같은 결과에 근거하여 주 돌봄자의 돌봄을 완화를 위해 정부정책 중 장애인활동지원제도와의 연계를 통한 돌봄부담의 경감과 어머니의 연령, 경제수준, 취업여부 등 개별적인 특성에 입각한 돌봄지원계획의 수립을 지원하고 가족 내 외의 다양한 자원들을 활용한 어머니를 직 간접적으로 지원해 줄 수 있는 사회적 지지망 구축 및 동일한 성인발달장애인 자녀를 둔 어머니들과의 자조모임 등 관련 프로그램 개발을 제안하였다.

Identification of 1p36 deletion syndrome in patients with facial dysmorphism and developmental delay

  • Purpose: The 1p36 deletion syndrome is a microdeletion syndrome characterized by developmental delays/intellectual disability, craniofacial dysmorphism, and other congenital anomalies. To date, many cases of this syndrome have been reported worldwide. However, cases with this syndrome have not been reported in Korean populations anywhere. This study was performed to report the clinical and molecular characteristics of five Korean patients with the 1p36 deletion syndrome. Methods: The clinical characteristics of the 5 patients were reviewed. Karyotyping and multiplex ligation-dependent probe amplification (MLPA) analyses were performed for genetic diagnoses. Results: All 5 patients had typical dysmorphic features including frontal bossing, flat right parietal bone, low-set ears, straight eyebrows, down-slanting palpebral fissure, hypotelorism, flat nasal roots, midface hypoplasia, pointed chins, small lips, and variable degrees of developmental delay. Each patient had multiple and variable anomalies such as a congenital heart defect including ventricular septal defect, atrial septal defect, and patent duct arteriosus, ventriculomegaly, cryptorchism, or hearing loss. Karyotyping revealed the 1p36 deletion in only 1 patient, although it was confirmed in all 5 patients by MLPA analyses. Conclusion: All the patients had the typical features of 1p36 deletion. These hallmarks can be used to identify other patients with this condition in their early years in order to provide more appropriate care.

경도의 발달장애 아동의 시지각 및 주의력 향상을 위한 시기능 훈련의 임상사례 (Clinical cases of vision therapy for visual perception and attention enhancement of children with mild developmental disabilities)

  • Purpose. The purpose of this study was to investigate the effect of visual field training on visual function, visual perception and attention enhancement. Methods. We examined the changes of visual function and visual perception tests for children with mild developmental disabilities through vision therapy. Results. At the time of the initial visit of the child, the ability to control not only distant and near visual function, but also accommodation, was also very limited. Also, the visual perception score was measured at 80 points. Through vision therapy, it was confirmed that the improvement of control ability as well as the visual function and visual perception function test results were also improved to 108 points. Conclusion. It was confirmed that vision therapy results in an improvement of visual function, improved visual perception ability, as well as visual perception ability, It was concluded that visual function affected visual perception, and that vision therapy used in order to prevent visual function deterioration was found to be very helpful.

Teaching Indigenous Students With Developmental Disabilities: Embedding the Cultural Practices of Dance, Movement, and Music in Pedagogy

  • The rich cultural backgrounds and practices of children from native cultures are often absent in classroom instruction, as teachers might feel that they have inadequate knowledge and backgrounds in the unique practices of these populations. Historically, children from native cultures have had challenging educational experiences and poor educational outcomes. To address these challenges, we propose a Family and Culture Based (FCB) framework that draws from family-centered practice, asset-based practice, and culturally responsive pedagogy. This article describes the three steps of the FCB framework, which uses a teacher-as-learner approach to instruct students from native cultures by engaging teachers in reflection, gaining knowledge about the cultural practices of the family and community, and integrating the new knowledge into practice. We use a Pacific Islands case (Native Hawaiian family) and a Pacific Northwest case (First Nations family) to illustrate the potential benefits of using the FCB framework with indigenous students.

  • 본 연구는 우리나라 어린이집에서의 다양한 유아를 위한 보육(장애아보육, 다문화가정 유아보육, 영아보육, 시간연장형보육)의 발전방안을 모색하는 데 그 목적을 두고 있다. 이를 위해 먼저 어린이집에서의 장애아 보육과 통합보육 및 다문화가정 보육과 영아 보육의 현황 및 정책과 이에 따른 연구동향에 대해 살펴본 후, 문제점 및 개선점을 제시하고, 발전방안을 제안하였다. 이러한 기초자료 제공 및 관련 논의를 통해 장애아 보육과 특수보육 등의 다양한 보육 대상자를 위한 포괄적 서비스 제공을 도모함으로써 보육의 질적 수준을 제고하는 데 기여하고자 한다.

  • Developmental language disorder is the most common developmental disability in childhood, occurring in 5-8% of preschool children. Children learn language in early childhood, and later they use language to learn. Children with language disorders are at increased risk for difficulties with reading and written language when they enter school. These problems often persist through adolescence or adulthood. Early intervention may prevent the more serious consequences of later academic problems, including learning disabilities. A child's performance in specific speech and language areas, such as phonological ability, vocabulary comprehension, and grammatical usage, is measured objectively using the most recently standardized, norm-referenced tests for a particular age group. Observation and qualitative analysis of a child's performance supplement objective test results are essential for making a diagnosis and devising a treatment plan. Emphasis on the team approach system in the evaluation of children with speech and language impairments has been increasing. Evidence-based therapeutic interventions with short-term, long-term, and functional outcome goals should be applied, because there are many examples of controversial practices that have not been validated in large, controlled trials. Following treatment intervention, periodic follow-up monitoring by a doctor is also important. In addition, a systematized national health policy for children with speech and language disorders should be provided.

일본 연하장애 어린이의 치과적 접근 (DENTAL APPROACHES OF CHILDREN WITH DYSPHAGIA IN JAPAN)

  • I participated in Academic Exchange Program(Action plan II) between KADH(Korean Association for Disability and Oral Health) and JSDH(Japanses Society for Disability and Oral Health) for 2 months from 3rd July 2012 to 2nd september 2012 in the Department of Hygiene and Oral Health, School of Dentistry, Showa University at Tokyo, Japan. I have observed their operation process and learned what dysphagia is and how it is consulted and taken care of as a therapy for patients with eating and swallowing disorders for two months in The department of special needs dentistry at Showa University Dental Hospital, Jonan Branch of Tokyo Metropolitan Kita Medical Rehabilitation Center for the Disabled, Smile Nakano Center, Tokyo metropolitan center for persons with disabilities in Lidabashi for one week, Eating and swallowing functional therapy workshop for disabled children, Tokyo metropolitan Tobu medical center for Persons with Developmental/Multiple Disabilities located in Minamisunamitchi for one week and on The 17-18th JSDR(Japanese Society of Dysphagia rehabilitation) in Sapporo. Through Action Plan II program, I learned how precious eating, drinking and swallowing with ease are and observed how they do and what they do as a dentist or a dental hygienist in Japan for dysphagia patients. Therefore, I want to present the dental approaches of children with dysphagia in Japan, based on my experience for two months.

K 대학 소아치과에 내원한 장애인의 구강진료 실태 (State of dental treatment among disabled patients at K university hospital pediatric dentistry)

  • The purpose of this study was to examine the state of dental treatment among disabled patients by the type of disability. After the medical records of 531 disabled patients who received treatment at the pediatric dentistry in K university hospital, the following findings were given: 1. As for age distribution by year, the rate of patients aged 10 or down rose to 42.5 from 5.1 percent, and the 16-20 age group increased from 16.7 to 24.8 percent. But the rates of patients aged between 11 and 15 and aged 21 and up were on the rise(p<.05). 2. Concerning the type of disability by year, there was an increase in the number of patients with brain lesions, mental retardation, developmental disorder and Down's syndrome(p<.05). As to the number of dental caries by the type of disability, the patients with heart diseases had the most dental caries that numbered 8.49, followed by Down's syndrome, metal retardation, brain lesions, the other disabilities and developmental disorder. 3. In relation to dental treatment experiences by the type of disability, the patients with developmental disorder(57.5%) received the most dental treatment, followed by mental retardation, the other disabilities, brain lesions, Down's syndrome and heart diseases(p<.05). 4. Regarding general anesthesia experience by the type of disability, the patients with mental retardation(31.6%) were put under general anesthesia the most, followed by developmental disorder, brain lesions, the other disabilities, heart diseases and Down's syndrome(p<.05). In conclusion, nationwide efforts to nurture separate dental personnels responsible for the disabled, to expand relevant facilities and to improve the health care insurance are required to promote the oral health of disabled children.

발달장애 아동의 사회적 기술 향상을 위한 현장체험학습 지원 시스템의 설계 및 구현 (Design and Implementation of a Field Experience Activity Support System for Improving Social Skills of Children with Developmental Disabilities)

  • 본 연구에서는 발달장애 아동의 사회적 기술 향상을 위하여 지역사회 기반의 현장체험학습을 지원하는 시스템을 설계하고 구현하였다. 본 시스템의 특징은 다음과 같다. 첫째, 구체적이고 실제적인 사전 학습을 통해 발달 장애 아동의 현장 적응력을 높이고 현장체험학습의 효과성 및 효율성을 향상시킬 수 있다. 둘째, 현장학습이 단순한 체험 및 지식의 습득 차원에서 머무르는 것이 아니라 실제적인 문제 상황에서 지식을 적용하고 활용할 수 있는 능력을 키울 수 있다. 셋째, 시스템 설계시 지역사회 활용성 및 관리의 용이성을 고려하여 설계함으로써 학습자의 실생활에 맞게 컨텐츠를 변형하여 사용할 수 있다. 넷째, 학교에서의 현장체험학습 뿐 아니라 가정에서의 생활 교육 등에 시스템을 활용함으로써 발달장애 아동의 사회적 기술 및 기초학습 능력 향상에도 긍정적인 영향을 제공한다. 본 시스템을 구현하여 적용한 결과, 발달장애 아동의 현장체험 학습시 현장 적응력을 높이고 학습 결과에 긍정적인 영향을 끼치는 것으로 나타났다. 첫째, 본 시스템을 이용하여 사전학습을 실시한 결과 학습이나 사물에 대한 집중도가 낮은 발달장애 아동들에게 매우 높은 관심과 흥미를 유발하였다. 둘째, 발달장애 아동들이 실제 현장학습장소에서도 현장 적응력이 높았으며, 관심 및 주의집중이 유지되었다. 셋째, 현장체험학습 후 실시한 사후지도 및 평가에서도 많은 학습 내용을 기억하고 있는 것으로 나타났다.

장애 아동의 발달재활서비스가 부모의 이용만족도에 미치는 영향 (The Development and Rehabilitation Services for Children with Disabilities The Effect of Parents' Service Satisfaction)

  • 본 연구의 목적은 장애 아동의 발달재활서비스 부모의 이용만족도를 파악하는 것이다. 이를 위해 강원도의 9개 시, 군 지역 복지관, 사설기관 등 17개소에서 장애 아동 120명과 부모 120명을 설문대상으로 하였다. 자료 분석은 SPSS Statistics 18을 사용하였으며, 주요 연구결과는 다음과 같다. 첫째, 발달재활서비스를 이용하는 부모의 일반적 특성에서 수급형태가 이용만족도에 유의한 차이가 나타났다. 둘째, 발달재활서비스의 서비스 이용특성에서 사설치료기관이 이용만족도에 통계적으로 유의한 차이가 나타났다, 셋째, 발달재활서비스의 서비스 제공에서 과정과 효과가 이용만족도에 유의하게 나타났다. 넷째, 발달재활서비스의 서비스 접근성에서 선택이 이용만족도에 미치는 영향은 유의하게 나타났다. 연구결과의 의의는 발달재활서비스의 서비스 제공과 서비스 접근성이 높을수록 장애 아동 부모의 이용만족도가 높아진다는 것을 확인한 것이다. 이에 따라 발달재활서비스를 이용하는 부모의 이용만족도를 향상시키기 위한 논의와 함의를 제시하였다.
