Nutrition Education for the Elderly in the US

  • Eating behavior change as a result of nutrition education interventions as secondary prevention strategies can contribute to an increase in life expectancy and better health for older adults in the United States (U.S.). Many of the chronic conditions prevalent in older adults are modifiable by dietary changes, including heart disease, diabetes mellitus, hypertension, obesity and osteoporosis. Important demographic observations in the U.S. including the projected large increase in number of older adults by 2030 have implications for nutrition education focus and services. A comprehensive review of nutrition education interventions for older adults in the U.S. published in 1995 identified elements from adult education theories that contribute to the effectiveness of nutrition education. These elements have been the focus of more recent studies with older adults providing additional evidence for relationships between concepts from commonly used behavior change theories and dietary patterns or change. In the U.S, an important program contributing to nutritional adequacy of the diet for older adults is the Elderly Nutrition Program which provides resources for congregate dining and includes a mandatory nutrition education component. Nutrition education is also provided through clinic based programs, and print and broadcast media. Application of the Transtheoretical Model has shown that the level of interest or motivation to comply with dietary guidance may be greater for some older adults due to an increasing burden of chronic disease and poorer quality of life, while others may not feel a need to change lifestyle habits.

Development and Evaluation of Preventive Mental Health Program for Depression among the Elderly in Community : Focused on the Community Welfare Center User (재가노인의 우울증 예방 프로그램 개발과 효과성 연구 : 사회복지관 이용노인을 중심으로)

  • The purpose of this research was to develop and evaluate a preventive mental health program for the depression among the elderly in community. Based on the review of literature, intervention program for preventing depression among the elderly was developed. In particular, the program focuses on enhancing the sense of control which is from social cognitive theory. The preventive mental health program consists of two parts: Tan-Hak and Social intervention program. Tan-Hak was oriented to cognitive change and activities, while social intervention program dealt with some social skills for relationship, stress management, motivation of social activities (volunteer activities), positive future designing, etc. For the evaluation of this program, we employed depression scale and medical measurement such as hormone test before and after the program, and in-depth interview of subjects. Overall, this program was successful in some points. First, through the pre and post test, this program has positive effect on strengthening the sense of control, and consequently, lowering depression level. Secondly, 18 people started this program called "smile class" at first time, and none of them turned out until the end of the program. This implies that the program was interesting to the elderly.

  • If we may practice the nutrition education planned on the basis which carefully grasped the inappropriate behavioral determinants of middle-school students, it might be an effective method achieving the change in perception and behavior improving the distorted perception about the ideal body shape, so we are to suggest the 8 week program of body shape perception improvement for successful nutrition education as follows. The body shape perception improvement program is a step-by-step group consulting program. At the introduction stage, we let them understand the meaning of true beauty and body change of teenage period and forming of sexual identity. At the stage of perception conversion, we let them have the opportunity to observe the status of body perception of the teenager and self-observation. At the stage of correction, we let them criticize the distorted body image in the society with mass media at the same time with the self-reflection. At the stage of maintenance and evaluation, we suggested the behavior guidance while preparing it. Setting this as the basis, we applied the contents such as the evaluations through cultural sharing events making somethings while directly participating. As the target groups to practice education were middle school students, we considered the learning level and behavioral features of the middle school students, and composed the programs including the methods such as role play, watching real things, media production, discussions and experiences. If the program of body shape perception improvement developed at this study could be utilized at the field of schools, the teenagers can change their ways of thought naturally avoiding the view about unified appearance rightly perceiving negative self-image that the teenagers can have and if the group consulting can be practiced regularly at each school, many students may experience the change in perception, so it might solicit the improvement of health of the families and local societies as well as that of the individual student.

Planning Health Promotion Program in Workplace (산업장 건강증진 프로그램 개발에 관한 연구 -건강행위의 변화의도 예측요인 및 대상의 유형 분류를 중심으로-)

  • In recent years, interests in health promotion have been stimulated by the epidemiological transition from infectious to chronic diseases as lead ing causes of death, the aging of the population, rapidly escalating health care costs, and epidemiologic findings linking individual risk to morbidity and mortality. It is not surprising that the workplace has been targeted as a promising setting for health promotion. In Korea, national attention to the opportunities for workplace health promotion began in the first 1990s. But there is no in depth study to identify the relating factors to the health promotion program in the workplace. The objective of this study is behavioral and physical characteristics to find that in crease the person's risk for a range of health problems and to analysis other characteristics to influence the degree of his/her intention to change health behavior. In addition, this study is purposed to present the process of planning health promotion program in the workplace. To accomplish these objectives, one workplaces was selected. And 363 employees in those workplaces were served as subjects for the study. Major findings in this study are as follows. (1) They have many risk factors such as smoking, drinking, lack of sleep, law rate of regular exercise, irregular eating, stress. (2) Some of the health risk factors such as smoking, drinking, and stress have the negative correlation to the intention to change. (3) Among cognitive and socio-environmental factors, significant predictors to the intention are attitude and social support. (4) In the cluster analysis to segment the target population in to homogeneous unit, three clusters of lifestyle are specified. (5) Smoking cessation and exercise program are planned for the risk group to change their behavior.

The Isokinetic exercise and effects of combined exercises on the change of postural control, functional status in patients with osteoarthritis of the knee (등속성 운동과 복합 운동이 퇴행성 슬관절염 여성의 근기능, 균형능력, 기능적 능력 및 관절기능상태에 미치는 영향)

  • Objective: The purpose of this study was to investigate the effects of combined exercise on the change of postural control, functional status in patients with osteoarthritis of the knee. Methods : The subjects were consisted of 24 women patients fifties and sixties with knee osteoarthritis. Each group had a exercise for 30 minutes per day and three times a week during 8 weeks period. Was used to measure muscular function(60, $180^{\circ}/sec$), postural control. functional capacity(15 M walking test, Rising form a chair test, stair climbing and descending test) and the functional state of a joint(WOMAC). Results: This study results in following 1. In case of muscular function have significantly increased in both groups. 2. There was no significant difference in the postural control of the bilateral between two groups. However, the postural control of a isokinetic exercise program group showed a significant difference in the bilateral side(overall, $M{\cdot}L$ stability index). Especially, the change of postural control in the isokinetic plus postural control exercise program group decreased more significantly in the bilateral(overall, $A{\cdot}P$ stability index). 3 It was significantly decreased in climbing and descending the stairs, and rising from the chair, although functional capacity decreased in lam walking in both groups. 4. The functional state of a joint(WOMAC) showed a significant decrease in both groups. Conclusion : In order to improve the functional status and postural control of patients with osteoarthritis of the knee, performing exercise programming of isokinetic plus balance will be more effective than the exercise program composed of only the isokinetic exercise program.

The Influence of the Laughing Programs on Self-esteem and Stress Coping Behaviors in Elementary School Student (일 초등학교 학생들의 웃음활동이 자아존중감과 스트레스 대처행동에 미치는 영향)

  • Purpose: The purpose of this study is to survey on the influence of the laughing program on elementary school students' self-esteem and stress coping behaviors. Methods: In total, 43 children (grade 4~6 students) were recruited for self administered questionnaire. Questionnaire was composed of general characteristics, self-esteem, and stress coping behaviors. Ten week course of laughing program was scheduled to the Experimental group, meanwhile no program was given to the control group. Results: The median self-esteem scores in the experimental group was increased from 30.05 to 34 (Z=-2.375, p<.05). Meanwhile control group did not show significant change. The median inactive/avoidant coping scores in the experimental group was decreased from 10 to 9 (Z=-2.028, p<.05). Meanwhile control group did not show significant change in any items of stress coping behaviors. Conclusion: Laughing program may be helpful to improve stress coping behaviors through reducing inactive/avoidance coping as well as to strengthen self-esteem, especially among participants who had low self-esteem before the intervention.

  • Software system is a hierarchical structure with collection of program units. Software system can import external packages globally or locally depending on the usage within a system. If the imported package is used globally, the soft-ware system can be influenced globally by any change of package and programmer's debugging time for the program maintenance will be greater. To solve these problems, it is desirable to use the imported package locally right on the usage point within the system. The model presented in this paper analyzed entity usage of package in structure of program, identified the usage level to obtain localization and provided information for restructure of the program to localize package usage. To obtain localization, it identified declared entities inside the imported package and analyzed the specification and body part of program unit to identify entities referenced from the imported package. The proposed model can be used to improve the maintainability of software system and contributed to reduction of programmer's debugging time in program maintenance.

  • The purpose of this study is to analyze the program development process of a nonformal environmental education(EE) program in detail. For the purpose, following research questions were answered in "the Nature School in the Forest" program in Eco-Club 1) What is the program development process? 2) What is the role of staffs, program developers, in the program development process? What are the meanings of their pedagogical approach? 3) With the findings of this study, how is the researcher able to develop ground theory for community-based nonformal EE, and to promote theoretical discussion for field improvement? The data were mainly gathered through participation observation and unstructured interview. And the data were analyzed by qualitative techniques such as clustering, factoring, noting pattern and themes, seeing plausibility, making metaphors, and building logical chain of evidence. The following conclusion comes out of the findings of this study. "The Nature School in the Forest" program is a educational device which the community-based NGO chose as a strategy to change individuals and community with its ideological purpose. And the program development process was the contiuous group decision-making process among staffs and volunteers. Consequently "the Nature School in the Forest" program is a circulated process of the voluntary activists training and their participation in program operation.

The effects of 8-week spinal stabilization exercise program on NDI, postural balance and body shape change in patients with chronic neck pain (8주간의 척추 안정화 운동 프로그램이 만성 경부통 환자의 NDI, 균형 능력 및 자세 변화에 미치는 영향)

  • This study examined how the eight-week spinal stabilization exercise program affects the cervical disability index, postural balance ability, and body shape change. The exercise program performed 60 minutes of spinal stabilization exercise three times a week for eight weeks. Sixteen patients with chronic neck pain, who complained of neck pain for six months, were classified into exercise group (n=8 patients) and control group (n=8 patients). The results before and after the eight-week exercise program were observed. Significant differences were observed in the time, group, and interaction of the neck disability index (p<.05). The balance ability showed significant interaction effects between the groups and periods (p<.05). Significant differences were noted in the timing and interaction in the pelvic inclination angle in posture change (p<.05), and there were significant differences in the group, timing, group, and interaction in the cervical and shoulder position angles (p<.05). The above results showed that the spinal stabilization exercise significantly improved the cervical disability index, balance ability change, and body shape change in patients with chronic neck pain. Future studies will analyze the specific changes in spinal structure through radiographic imaging to increase the validity of spinal stabilization exercise.

The Effects of the Thera Band Exercise Program on Bone Mineral Density and Health Promotion Behaviors in Elderly Women (세라밴드 운동 프로그램이 여성노인의 골밀도와 건강증진행위에 미치는 효과)

  • Purpose: This study was conducted in order to closely examine the effects on bone mineral density and health promotion behaviors following the intervention of a 20-week Thera band exercise program targeting elderly women. Methods: This study is a pseudo-experimental research with a pre-and-post design in a single group that examined the effects after the intervention of Thera Band Exercise for 20 weeks targeting 31 elderly women over 60 years of age who visit a Senior Welfare Center located in K city. It analyzed subjects' general characteristics using error and percentage, health promotion behavior, and a bone density aspect using the mean and standard deviation, as well as a change in the health promotion behavior and bone density, before and after exercise with Paired t-test by using the SPSS 18.0 program. Results: The effect of the Thera band exercise program on bone mineral density was reduced (t=8.140, p<.001) gradually after 20 weeks of exercise compared to the pre-exercise period. The health promotion behavior before and after exercise showed the significantly increasing outcomes (t=3.26, p=.003). Conclusion: There is a limitation to a direct rise in bone mineral density with the use of Thera band exercise intervention in the elderly. A change in health promotion behavior before and after exercise increased significantly. Thus, it was confirmed that taking exercise leads to a gradual rise in health promotion behavior. Hence, it understood that continuous exercise is useful and brings about a positive change in promoting health maintenance in elderly women.
