정맥내 카테터 조각의 엑스선, 초음파 및 CT 영상의 실험적 비교 연구 (An Experimental Comparative Study of Radiography, Ultrasonography and CT Imaging in the IV Catheter Fragment)

  • 조영증강을 위해 조영제를 정맥내 카테터로 주입하는 과정에서 정맥태 카테터 조각을 검출하고 제거하기 위해 식염수와 18 게이지 테프론 IV 카테터의 1cm 조각을 삽입하여 팬텀을 제작하여 엑스선, 초음파 및 CT 영상을 비교하고자 한다. 엑스선, 초음파 및 CT에서 정맥내 카테터 조각을 임상적으로 유용하게 보였고, 조각의 묘출 능력에는 차이가 유의한 차이가 없었다.(p >.05). 방사선 선량은 엑스선에서 DAP ($0.93{\mu}Gy{\cdot}m^2$) 및 유효선량 ($0.2139mSv{\cdot}Gy^{-1}{\cdot}cm^{-2}$), CT의 DLP ($201mGy{\cdot}cm$), 유효선량은 0.483 mSv로 산출되었다. 엑스선, 초음파 및 CT에서 정맥내 카테터 조각을 임상적으로 검출하였고, 조각의 묘출 능력에는 CT가 우수하였다. 그러나 엑스선 및 CT는 방사선 피폭을 수반하여 정맥내 카테터 조각의 위치 및 정보를 확인하는 데는 초음파 검사를 추천한다.

소아환자에서 대복재정맥을 통한 하대정맥도관법의 유용성 (Usefulness of Tunneled Trans-saphenous IVC Catheters for Long Term Venous Access in Pediatric Patients)

  • Central venous catheter (CVC) for long-term venous access is indispensable for various reasons including hyperalimentation, frequent blood sampling, frequent IV drug use in pediatric patients. We report clinical experience of surgical neonates in whom CVC was inserted primarily via great saphenous vein into suprarenal inferior vena cava. From March 2004 to March 2006, we performed CVC insertion via saphenous vein - contralateral side to main wound - into suprarenal inferior vena cava in surgeries of neonates. 2.7Fr or 4.2Fr, single lumen, tunneled Broviac catheters (Bard Access system, Inc, Salt Lake City, Utah) were used. Skin exit site of tunneled catheter was located in ipsilateral flank area just below edge lower rib. At the end of the procedure, location of the catheter tip was confirmed by plain radiography of abdomen. We retrospectively reviewed the admission records of the patients including nursing staff charts. Nine (50.0 %) patients were male and nine (50.0%) were female. Median gestational age was 38 weeks (range, 29-42 weeks) and median birth weight was 3,105 gm (range, 1,040-3,720 gm). Median age at catheter insertion was 38.5 days (range, 1-236 days). The purpose of CVC insertion was short-and long-term hyperalimentation in nine (50.0 %) patients. CVC insertion was performed in operation room under general anesthesia in sixteen (88.9 %) patients (in these cases, CVC insertion was performed just prior to concurrent operation) and neonatal intensive care unit (NICU) under local anesthesia with adequate sedation in two (11.2%). During the admission period (total catheter-indwelling time: 553 days), CVC functioned well without any significant side effects. Transient swelling of the ipsilateral leg (n=1, 5.6 %) and transient migration of catheter tip (n=1, 5.6 %) were noted, which did not affect function of the indwelled CVC. Mean catheter-indwelling time was 30.7days (range, 3-72 days). All catheters were removed electively except two mortality case. Complications, such as thrombosis, infection, kinking or extravasation of drugs, were not observed in our study period. Tunneled trans-great saphenous vein inferior vena cava catheters are not only comparable to cervical CVCs in terms of function and complication rates, but also very beneficial in selected patients, especially those in whom cervical approach is technically impossible or contraindicated.

안저골절 정복술 후 풍선 달린 카테터와 방사선조영제의 이용 (Reconstruction of the Orbital Floor Fracture using the Antral Balloon Catheter with Radiopaque Dye)

  • Purpose: Blow-out fractures are reduced through transcutaneous or transconjunctival incisions. But the field of orbital surgery is difficult due to lack of visualization of fracture site, blind dissection of orbital floor, susceptibility of injury of orbital structures. In these situations, the former technique of using an antral balloon catheter has advantages over other methods for reconstruction because of its rapidity, simplicity, and inexpensiveness. Furthermore, the antral balloon catheter allows not only elevation of the orbital bone fragment but also expansion of the maxillary sinus in cases where there is a fracture of its walls. But postoperative follow-up method using computed tomography is expensive. Hence, we report a simple and inexpensive follow-up method using radiopaque dye inflation. Methods: We performed endoscopic transantral approach in 5 cases of blow-out fracture under general anesthesia. To accomplish this technique, a rigid 4 mm, 0 or 30 degree angled endoscopy was inserted into the maxillary sinus. Inflation of the catheter started gradually, with 10 to 15 mL of saline mixed radiopaque dye (saline: dye, 5 : 1) by syringe and while observing the elevation of the fracture site with endoscope until a proper contour was reached. For the maintain of the position of fractured site, 12 French urinary balloon foley catheter were used in fracture site for 7 - 10 days. Results: Postoperative assessment was performed by means of clinical and simple radiographic examination to secure the catheter under the inferior orbital wall and in the maxillary sinus. No specific complications occurred related to this procedure. Results of the surgery and follow-up in all cases were satisfactory. Conclusion: It may be a better alternative to the conventional follow-up method, with less cost and effectiveness of the catheter patency. The advantages of using the urinary balloon foley catheter with the radiopaque dye include the following : it is safe, efficacy, simple, and especially low cost. On drawback of this method is the discomfort to the patient caused by the catheter during the treatment.

경막외 카테터의 적합한 위치지표로서의 주입공기의 흡인 (The Aspiration of Injected Air via an Epidural Catheter as an Indicator for Appropriate Placement of the Catheter in the Epidural Space)

  • Background: The "gold standard" for proper epidural catheter positioning is a clinical response, as assessed by a pinprick test. Yet it may take time or it may be difficult to perform this test after epidural catheter placement in sedated or uncooperative patients or during general anesthesia. We assessed the usefulness of aspirating injected air via an epidural catheter as an indicator of correct epidural catheter placement. Methods: We surveyed 200 patients who underwent surgery under general or epidural anesthesia. A Tuohy needle was inserted into the epidural space with using the hanging drop technique. After placement of the epidural catheter, 3 ml of air was injected via the catheter, and then the volume of aspirated air was measured. Results: The mean volume of aspirated air was $2.3{\pm}0.7ml$ (75% of the injected air volume) and this ranged from 0 to 3 ml. Conclusions: Aspiration of injected air is a simple alternative method for identifying the appropriate placement of epidural catheters in the epidural space.

Adjustable Ghajar Guide Technique for Accurate Placement of Ventricular Catheters : A Pilot Study

  • Objective : An adjustable Ghajar guide is presented to improve the accuracy of the original Ghajar guide technique. The accuracy of the adjustable Ghajar guide technique is also investigated. Methods : The coronal adjustment angle from the orthogonal catheter trajectory at Kocher's point is determined based on coronal head images using an electronic picture archiving and communication system. For the adjustable Ghajar guide, a protractor is mounted on a C-shaped basal plate that is placed in contact with the margin of a burrhole, keeping the central $0^{\circ}$ line of the protractor orthogonal to the calvarial surface. A catheter guide, which is moved along the protractor and fixed at the pre-determined adjustment angle, is then used to guide the ventricular catheter into the frontal horn adjacent to the foramen of Monro. The adjustable Ghajar guide technique was applied to 20 patients, while a freehand technique based on the surface anatomy of the head was applied to another 47 patients. The accuracy of the ventricular catheter placement was then evaluated using postoperative computed tomography scans. Results : For the adjustable Ghajar guide technique (AGT) patients, the bicaudate index ranged from 0.23 to 0.33 ($mean{\pm}standard$ deviation [SD] : $0.27{\pm}0.03$) and the adjustment angle ranged from $0^{\circ}$ to $10^{\circ}$ ($mean{\pm}SD:5.2^{\circ}{\pm}3.2^{\circ}$). All the AGT patients experienced successful cerebrospinal fluid diversion with only one pass of the catheter. Optimal placement of the ventricular catheter in the ipsilateral frontal horn approximating the foramen of Monro (grade 1) was achieved in 19 patients (95.0%), while a suboptimal trajectory into a lateral corner of the frontal horn passing along a lateral wall of the frontal horn (grade 3) occurred in 1 patient (5.0%). Thus, the AGT patients experienced a significantly higher incidence of optimal catheter placement than the freehand catheterized patients (95.0% vs. 68.3%, p=0.024). Moreover, none of the AGT patients experienced any tract hemorrhages along the catheter or procedure-related complications. Conclusion : The proposed adjustable Ghajar guide technique, using angular adjustment in the coronal plane from the orthogonal trajectory at Kocher's point, facilitates accurate freehand placement of a ventricular catheter for hydrocephalic patients.

Initial Experience of ACE68 Reperfusion Catheter in Patients with Acute Ischemic Stroke Related to Internal Carotid Artery Occlusion

  • Objective : Penumbra ACE68 reperfusion catheter is a new large bore aspiration catheter used for reperfusion of large vessel occlusion. The objective of this study was to investigate the efficacy of this catheter in comparison to that of previous Penumbra catheters in patients with acute ischemic stroke related to internal carotid artery (ICA) occlusion. Methods : Data of all eligible patients who received endovascular treatment (EVT) for ICA occlusion using Penumbra aspiration catheters between January 2015 and December 2018 were retrospectively reviewed. After dividing into two groups according to use of penumbra ACE68, baseline characteristics of patients, successful recanalization rate, puncture to recanalization time, and switch to stent base technique rate were assessed. Successful recanalization was defined by a thrombolysis in cerebral infarction (TICI) score ${\geq}2b$ and favorable functional outcome was defined according to modified Rankin scale (score, 0-2). Results : ACE68 reperfusion catheter was used in 29 of 75 eligible patients (39%). The puncture to recanalization time was significantly shorter ($26{\pm}18.2$ minutes vs. $40{\pm}24.9$ minutes, p=0.011) and the rate of switch to stent-based retrieval was significantly lower (3% vs. 20%, p=0.046) in ACE68 catheter group. Moreover, although not statistically significant, the successful recanalization rate was higher (83% vs. 76%, p=0.492) in ACE68 catheter group. Favorable functional outcome was observed in 48% of patients treated with ACE68 reperfusion catheter and in 30% of patients treated using other Penumbra systems (p=0.120). Baseline Alberta Stroke Program Early CT Scores ${\geq}8$ (odds ratio [OR], 9.74; 95% confidence interval [CI], 1.72-54.99; p=0.010) and successful recanalization (OR, 10.20; 95% CI, 1.13-92.46; p=0.039) were independent predictors of favorable outcome. Conclusion : EVT using ACE68 reperfusion catheter can be considered a first-line therapy in patients with acute ICA occlusion as it can achieve rapid recanalization and reduce the frequency of conversion to stent-retrieve therapy.

완전이식형 정맥 접근 포트 삽입 시 카테터 팁을 심벽에 닿지 않고 우심방 내 위치시키는 방법의 타당성과 안전성 (Feasibility and Safety of a Technique Intended to Place the Catheter Tip in the Right Atrium without Abutment Against the Cardiac Wall during Implantation of the Totally Implantable Venous Access Port)

  • 목적 완전이식형 정맥 접근 포트 이식 시 카테터 팁을 심벽에 닿지 않으면서 우심방 내 위치시키는 기술의 타당성과 안전성에 대하여 평가하였다. 대상과 방법 2016년 1월부터 12월까지 완전이식형 정맥 접근 포트를 삽입하고 추적관찰 CT를 시행한 환자 총 330명이 연구에 포함되었다. 정맥 접근 포트는 절개선에서 직접 액와정맥으로 접근하는 단일절개법을 적용하여 이식하였다. 카테터 팁을 자르기 전 정맥 환류를 확인하여 우심방 내벽에 닿지 않도록 위치를 확인하였다. 카테터의 길이 및 연관된 합병증은 의료영상 그리고 의료기록을 통해 후향적으로 평가하였다. 결과 모든 환자에서 카테터 팁은 심벽과의 닿음과 기능장애 없이 성공적으로 위치 조정되었다. 카테터 팁의 위치는 cavoatrial junction 기준으로 fluoroscopy에서 15.3 mm, 첫 번째 CT에서 6 mm 원위부에 있었다(중앙값). 카테터 팁은 fluoroscopy 대비 첫 번째 CT에서 10.4 mm 머리 방향으로 이동하였다(중앙값). 추적 CT에서 우심방과 상대정맥에서 각각 1건씩의 혈전증이 발견되었다. 결론 카테터 팁을 심벽에 닿지 않으면서 우심방 내 의도적으로 위치시키는 것은 혈전증 발생률이 낮고 기능장애가 없는 기술적으로 가능하고 안전한 방법이다.

Cerebral Air Embolism Following Pigtail Catheter Insertion for Pleural Fluid Drainage

  • Pigtail catheter drainage is a common procedure for the treatment of pleural effusion and pneumothorax. The most common complications of pigtail catheter insertion are pneumothorax, hemorrhage and chest pains. Cerebral air embolism is rare, but often fatal. In this paper, we report a case of cerebral air embolism in association with the insertion of a pigtail catheter for the drainage of a pleural effusion. A 67-year-old man is being presented with dyspnea, cough and right-side chest pains and was administered antibiotics for the treatment of pneumonia. The pneumonia failed to resolve and a loculated parapneumonic pleural effusion developed. A pigtail catheter was inserted in order to drain the pleural effusion, which resulted in cerebral air embolism. The patient was administered high-flow oxygen therapy and recovered without any neurologic complications.

The Predictive Strength of Students' Self-Efficacy, Problem Solving Skills to Perform Catheter Care

  • Purpose: The aim of this study was to determine the predictive strength of students' self-efficacy, problem-solving skills, and other characteristics in performing intravenous practices and monitor phlebitis and infiltration. Methods: This cross-sectional study was carried out with 736 third and fourth-year students studying at the Health Sciences Faculties of Balikesir and Sakarya universities. The data were collected using the Self-Efficacy Scale, Problem-Solving Inventory and Catheter Care and Infiltration Monitoring Questionnaire. Results: The participants' mean Catheter Care and Infiltration Monitoring Questionnaire score significantly and positively correlated with their mean Self-Efficacy Scale score on a moderate level (r=.25; p<.001) but significantly and negatively correlated with their mean Problem-Solving Inventory score on a moderate level (r=-.21; p<.001). In other words, because a low Problem-Solving Inventory score indicates that the person's problem-solving skill is high, the Care and Infiltration Monitoring Questionnaire score increased as the problem-solving skill increased. While the Self-Efficacy Scale predicted the year of study and catheter care and infiltration monitoring variables positively, the Problem-Solving Inventory predicted the satisfaction with the profession variable negatively. Conclusion: Self-efficacy, problem-solving, liking the profession, and year of study predict success in catheter care and infiltration monitoring. For this reason, guidance may be provided in the development of a comprehensive education system toward increasing students' problem-solving skills, self-efficacy, and professional knowledge and skills.

Prevention of Catheter-related Infections (CRIs) using Ciprofloxacin

  • Microbial infection provokes one of the most serious complications to the patients with indwelling catheters. Ciprofloxacin (CFX) was added into the catheter materials (polyurethane or silicone) during the manufacturing process to avoid the microbial infection. Efficacy of the catheters containing CFX was investigated by using the in vitro zone of growth inhibition test method. The catheters made of polyurethane or silicone exhibited a strong antimicrobial activity against the major catheter-related microorganisms (S. aureus, S. epidermidis, P. aeruginosa and E. coli), when CFX was incorporated into the catheters. Fetal bovine serum (FBS) did not affected antimicrobial activities of the polyurethane catheters with CFX loading of 0.5 and 1.0% (W/W) against S. aureus and S. epidermidis. However, the polyurethane catheters with 1.0% (W/W) of CFX loading showed a significantly (P<0.05) reduced antimicrobial activity against E. coli when the catheters were exposed to FBS. Silicone catheters with 1.0 and 1.5% (W/W) of CFX loading demonstrated effective antimicrobial activity against S. epidermidis for at least 2 weeks. These results suggest that the use of catheters containing ciprofloxacin could be effective in preventing catheter-related infections.
