초산부의 간호요구에 관한 연구 (A Study on Nursing Needs during Labor Pain)

  • The purpose of this study was to explore the structure of nursing needs during labor pain as perceived by women who have given birth. The phenomenological perspective of qualitative research theory guided the approach to the study. The sample consisted of 20 women who had undergone normal labor and had delivered a healthy baby at term. Collection of data was by means of in-depth interviews conducted 1-2 days after delivery in the admission room from March to may 1998. On average, the interviews lasted for about 30 minutes. Interviews were taken with the consent of the subjects. Data were analyzed by means of Giorgi's method and categorized according to the similarities of their contents. The investigator read the data repeatedly to identify themes and categories. Four categories were : 1) a sense of security 2) self esteem 3) willingness to learn 4) a sense of comfort. Under these categories there were twelve themes. I. A sense of security : (a) presence (b) conversation (c) touching II. Self esteem : (a) praise (b) encouragement (c) treating the women with respect (d) caregiver with good character III. Willingness to learn : (a) teaching (b) information IV. A sense of comfort : (a) self controlled pain relief (b) artificially controlled pain relief (c) skillfulness of caregiver The findings should sensitize nurses to the various needs of women in labor who are under their care. Further research should focus on developing instruments to assess the nursing needs of parturients. Researchers also need to identify ways to assess women's satisfaction with nursing needs.

Effectiveness of a combined obesity prevention program in South Korea for children from low-income families that included primary caregiver participation

  • Purpose: Obesity among children from low-income families is becoming a social problem. The aim of this study was to evaluate the effectiveness of an obesity prevention program that included physical activity, nutrition education, behavioral modification, and primary caregiver participation components among children from low-income families. Methods: The study analyzed a nonequivalent control group using a pretest-posttest design. A total of 77 children were recruited from six community childcare centers using purposive sampling. For the intervention group (n=40), the pretest was administered before the combined intervention program involving the participants' primary caregivers was conducted for 8 weeks. The posttest was conducted immediately after the program and again four weeks after the program. Results: Flexibility (F=4.64, p=.020), muscular endurance (F=11.22, p<.001), nutritional knowledge scores (F=4.79, p=.010), body image satisfaction scores (F=4.74, p=.012), and self-esteem scores (F=3.81, p=.029) showed significant differences and interactions between group and time for the intervention and control groups. Conclusion: Strategies to actively engage the primary caregivers of low-income families in children's obesity programs are needed. Obesity prevention programs for children based on the program in this study should be routinely developed, and continuing attention should be given to children from low-income families.

뇌성마비아 부모의 부담감 및 가족기능에 관한 연구 (A Study on Family Functioning and Burden of Parents with Cerebral Palsy Children)

  • Today, more chronically ill and handicapped children are being cared for at home by a family member caregiver. The task of caring for a family member may feel burden that the caregiver has less time and money and more work. Family functioning and their burden have influence on coping and adaptation of families with chronically-ill children. This study attempted to identify the levels of burden and family functioning in families of children with cerebral palsy and to examine their relationships. The instruments were burden scale developed by Suh and Oh(1993), and a modified Feetham Family Functioning Survey based on Roberts and Feetham.(1982). The subjects were 98 parents of children, under 15 years, who have cerebral palsy and being treated and living in Pusan. Data were collected through a self-administered question- aire from April 26 to May 29. The collected data were tested using frequencies, percentiles, means, t-test, ANOVA, and Pearson correlation coefficient with SPSSWIN program. The results of this study were as follows ; 1. The mean rating scores of burden and family functioning were 2.79±.51 and 4.12±.69, respectively. 2. The relationships between general character- istics and burden were statistically significant difference : degree of children's handicap(F=6.333, P<.01). The relationships between general characteristics and family functioning were statistically significant differences : familial relation with the children(F=3.628, P<.05), caregiver's health status(F=4.359, P<.05), age of children (F=4.185, P<.05), and duration of treatment (F=6.802, P<.01). 3. In families of children with cerebral palsy, there was significantly negative correlation between the burden of parents and the performance of family functioning(r=-.230, P<.05). There were significantly negative correlations between the burden of parents and the satisfaction of family functioning (r=-.211, P<.05), and between the perceived importance and the satisfaction of family functioning(r=-.481, P<.01); however, there was a positive correlation between the performance and the perceived importance of family functioning(r=.425, P<.01). In conclusion, this study suggests that families of children with cerebral palsy need family-focused nursing interventions as supportive care for relieving their burden and for improving family functioning.

요양보호사의 소명의식과 조직성과의 관계에서 직무만족의 매개효과 (Mediating Effect of Job Satisfaction in the Relationship Between Caregivers' Calling and Organizational Performance)

  • 최근 장기요양보험제도에 도입된 시장화와 공급주체 다원화의 영향으로 조직성과에 대한 관심이 높아지고 있다. 본 연구는 장기요양시설의 조직성과에 요양보호사의 소명의식의 영향을 분석하고 그 경로에서 직무만족의 매개효과를 검증하기 위하여 서울시와 경기도의 요양보호사를 대상으로 자료를 수집하였다. 다중회귀분석으로 소명의식이 직무만족과 조직성과에 미치는 영향을 분석하였고 위계적 회귀분석으로 직무분석의 매개효과를 검증하였다. 분석 결과 소명의식은 조직성과와 직무만족에 정(+)의 영향을 미쳤고, 직무만족은 소명의식과 효과성 사이에서 완전매개효과를, 소명의식과 효율성 사이에서는 부분매개효과를 나타냈다. 이를 바탕으로 직무만족 관리를 통하여 효과성을 제고하고, 직무만족과 소명의식을 관리하여 효율성을 관리할 필요성을 논의하였다.

노인대상자를 돌보는 비전문 간호제공자의 대응기전과 건강반응 예측요인 (Coping Strategies Utilized in the Caregiving Situation and Predictors of Health Responses among Informal Caregivers of Older Adults)

  • The sample of this study consisted of 140 informal caregivers who provided care to the older adults(over 60 years of age) in Great Cleveland, USA. Self-rated questionnaires were utilized to collect information. The purpose of the study was to identify coping strategies most frequently utilized by informal caregivers of older adults and to examine predictors of the caregivers' health responses to the caregiving situation applying Lazarus and Folkman stress model(1984). Stepwise multiple regression was used to identify significant predictors among caregivers' demographic-socio-economic factors, older adult's dependency of activities of daily living(ADLs), caregiver's appraisal to the caregiving situation, and coping strategies. Informal caregivers (N=140) included in the study utilized help-seeking and problem-solving coping strategies more than self-blame and minimization of threat coping strategies. Caregivers' responses to the caregiving situation were observed by caregivers' perceived physical health, depression and life satisfaction. For perceived physical health, threat appraisal, older adult's dependency on ADLs, existential growth coping strategy, and monthly income accounted for 25% of the variance. Caregivers who appraised the caregiving situation as more threatening, reported higher dependency on ADLs, used more existential growth coping strategy, and had higher monthly income reported better physical health. For depression, threat appraisal, stress appraisal, existential growth coping strategy, self-blame coping strategy, and monthly income accounted for 48% of the variance. Caregivers who used more existential growth coping and less self-blame coping, appraised the situation as less threatening, less stressful, and had higher monthly income reported less depression. For life satisfaction, self-blame coping, existential growth coping, monthly income, stress appraisal accounted for 49% of the variance. Caregivers who used more existential growth coping, less self-blame coping, less stress appraisal, lower monthly income reported better life satisfaction. In conclusion, informal caregivers in this study utilized positive coping strategies such as problem-focused, existential growth, help-seeking, rather than negative coping strategies including self-blame. When they utilized positive coping strategies more often, caregivers experienced higher perceived physical health, higher life satisfaction and lower depression. Therefore, nursing intervention which utilized positive coping strategies is needed to enhance informal caregivers to have positive health responses to the caregiving demands.

노년기 수발 경험에 따른 건강 변화 : 사회적 관계망과 만족도의 매개효과 분석 (Elderly Caregiving and Health : Analysis of the mediate effect of social network and Satisfaction)

  • 본 연구는 수발 경험에 따른 수발자의 건강상태 변화를 살펴보고, 수발 경험이 수발자의 건강에 미치는 영향에 대해 사회적 관계망과 만족도가 매개변수로서 어떻게 작용하는지를 검증하는데 목적이 있다. 소기의 목적을 달성하기 위해 고령화연구패널자료를 활용하여 구조방정식모형을 활용하여 분석을 시도하였다. 분석 결과 수발 이후 남녀 모두 우울감이 증가하고 주관적 건강상태나 낮아졌다. 사회적 관계망과 만족도 변화 양상에는 성별에 따른 차이가 있는 것으로 나타났다. 여성의 경우 수발이 주관적 건강상태와 우울감에 영향을 미쳤으며, 매개효과는 발견되지 않았다. 남성의 경우 수발 경험이 우울감에 미치는 총 효과는 1.087(p<.05), 간접효과는 0.546(p<.05)이었으며, 직접효과는 유의미하지 않은 것으로 나타났다. 남성의 수발 경험은 기능적 가족망의 감소(0.42, p<.01)를 완전매개로 주관적 건강상태에 영향을 미쳤다. 이러한 연구 결과를 통해 수발 이후 수발자들의 건강 취약성이 확인되었다. 또한, 수발자의 건강과 이를 매개하는 요인을 탐색하는 데 있어 성별에 따라 그 양상이 따르게 나타나는 것으로 나타났다. 특히 남녀 모두 기능적 가족망이 유의미하게 감소하였음에도 남성에게서만 기능적 가족망의 매개효과가 확인되어 남성의 기능적 가족망 강화의 필요성을 제기하였다.

요양 보호사 교육에서 교육생의 참여동기가 교육만족도에 미치는 영향 (The Effect of Motive for Participation on the Satisfaction with Training for Caregivers)

  • 본 논문은 요양보호사의 교육에 참가한 교육생들의 교육 참여동기가 교육만족도에 미치는 영향을 알아봄으로서, 요양보호사 교육의 질 향상에 기여하고자 실시하였다. 본 연구는 B시에 소재하고 있는 요양보호사 교육원의 교육생 281명을 대상으로 교육 참여동기와 교육만족도에 대한 조사를 실시하였다. 통계처리는 SPSS 12.0을 사용하여 신뢰도와 타당도를 검증하였고 기술통계와 요인분석, 회귀분석을 실시하였다. 교육 참여동기 중 활동지향형이 가장 높게 나타났으며 교육만족도는 지도강사에 대한 만족도가 가장 높았다. 교육 참여동기가 교육만족도에 미치는 영향은 학습지향성과 활동지향성으로 나타났으나 목표지향성은 교육만족도에 영향력을 미치지 않는 것으로 나타났다. 따라서, 요양보호사 교육생들의 교육 만족도는 중간이상으로 높았으며, 교육 참여동기는 교육만족도에 영향을 미치는 것으로 나타났다.

장기요양노인을 돌보는 가족의 부양부담과 생활만족도의 관계에서 사회적 지지 자원의 조절효과 -장기요양재가서비스 이용자를 돌보는 가족부양자를 중심으로- (The Moderating Effect of Social Support Resources on Caregiving Burden and Life Satisfaction of Family Caregivers Performing Long-Term Care of the Elderly: A Focus on Using Home Care Services Under the Long-Term Care Insurance System)

  • This study aimed to explore the moderating effect of social support resources on the life satisfaction and caregiving burden of family caregivers taking care of the elderly in long-term care services. The study participants were selected from among the family caregivers utilizing 30 home care centers in Seoul, Korea. Of the candidates who agreed to participate in the study, 250 caregivers who had supported the elderly at home for at least 6 months were selected. The data collected were analyzed using SPSS ver. 21.0. The study findings were as follows. First, the whole family caregivers' caregiving burden of family caregivers was found to be slightly higher than the median value. More specifically, the physical burden was the heaviest, followed by the family relationship burden, the social activity burden, the emotional burden, and the financial burden. Social support resources and life satisfaction were found to be a little bit higher than the median value. Second, the variables that statistically significantly increased life satisfaction, starting with the strongest effect, were a lower psycho emotional burden, greater informational support resources, a lower economical burden, greater emotional support, a younger caregiver age, a higher monthly income, and a shorter care period. Third, informational support resources were found to have a moderating effect on the relationship between the support burden and life satisfaction. The more the participants used the informational support resource subcategory of the social support resources, the narrower the life satisfaction decrease, even though the caregivers felt a support burden. Informational support resources appear to have a statistically significant buffering effect.

간호사의 이직의도에 영향을 미치는 예측 요인 (The Risk Factors Influencing Turnover Intention of Nurses)

  • Purpose: This study was done to identify the risk factors influencing turnover intention of nurses. Method: The participants in this descriptive survey on causal relations were 756 nurses who were working at a tertiary university hospital in Seoul. The data were put in to multiple regression analysis to build a prediction model. Results: Turnover intention according to general features were shown as following.: Age, Clinical careers, Educational level, Marital status, Economic status(Yearly income). The relationship between turnover intention and job stress had positive correlation. But the relationship between turnover and other factors that job satisfaction, internal marketing, and organizational commitment had negative correlation. The causal factors of turnover intention were organizational commitment, the factors of organizational support and patient/caregiver relationships among subcategories of job stress and the factor of professional position among subcategories of job satisfaction. Conclusions: The findings of study suggest that board intervention program should be provided to prevent problems of turnover. It is also recommended that a program be developed that can help control the variables identified in this study along with follow up study to verify the model.

요양보호사의 직무환경이 조직몰입에 미치는 영향: 직무만족의 매개효과 검증 (The Effect of Care-workers Work Environment on Organizational Commitment: The Mediation Effects of Job Satisfaction)

  • 본 연구는 요양보호사의 직무환경이 조직몰입에 미치는 영향을 파악하고 이를 토대로 직무만족의 매개효과를 검증하는 것을 목적으로 한다. 자료수집은 경기지역 노인요양시설을 중심으로 요양보호사 300명을 대상으로 설문조사를 하였고, 회수되지 않은 설문지와 이상치를 제거하고 최종 267명을 연구대상으로 선정하였다. 자료분석은 SPSS WIN 23.0, PROCESS Macro V3.5를 통해 결과를 도출하였고, 직무만족의 매개효과 검증을 위해 Hayes(2013)의 Macro Model Number 4를 적용하였다. 분석결과, 요양보호사의 직무환경이 조직몰입에 미치는 영향에서 직무만족 변인의 부분매개효과를 확인하였다. 분석결과를 토대로 요양보호사의 직무환경 개선과 직무만족 증가를 위한 실천적·정책적 제언과 본 연구의 한계를 극복하기 위해 후속 연구를 제안했다.
