A Case Study on User Evaluation of Patients' Room for User-Oriented Design - Focused on H General Hospital in Busan - (사용자 중심 디자인을 위한 다인병실에 대한 사용자 평가연구 - 부산 H 종합병원 사례를 중심으로 -)

  • This study was carried out to focus on the user-oriented design of patients' room in general hospital. The purpose of the study was to examine the users' evaluation on their patients' rooms in H General Hospital and to suggest how to improve the patients' room design. The users were inpatients, care givers, and nurses, 300 subjects of each 100 users in H hospital. They evaluated 26 items related to the patients' room design using by 5-points Likert scale. Also, they answered three elements which bothered them, what needed improvement, and what they feel ill at ease. The results were as followings; 1) The users evaluated positively the location and size of window. This implies that the current one is relatively ideal and deserve to be maintained. 2) The users responded negatively to the thermostat system and the space for care-givers. 3) The lack of space for care-givers and visitors, common use of refrigerator and bathroom, and the lack of space bothered the users. 4) Most users mentioned the lack of space in patients' room. This implies that it needs to consider the change of the person number per patient's room, from 5 persons to 4 ones. 5) The lacks of sleeping space, dining space, and bathroom space bothered the care-givers. This implies that the space for care-givers such as sleeping, dinging, and bathing should be considered in patients' room.

A Study on the Needs and Estimation of Users in the Playground of Child Care Facilities (보육시설의 실외놀이 환경에 대한 사용자 평가 및 요구조사)

  • The purpose of this study is to clarify playground types by characteristics of playground environment in child care facilities, to analyze the needs and estimation of users according to playground types. The subjects of this study were care-givers, who assist outdoor play activities of children and manage safety in playground, to estimate adequateness of playground's environment to children's outdoor plays and to report needs. The data were collected by field measurement survey for clarification playground environments in 21 child care facilities and structured-questionnaire for estimation and needs of 181 care-givers in them. The major results showed the following. 1) The playgrounds were clarified to five type according to number of child and size of playground. The five types include A-type as large-scale facility/small-size playground, B-type as small-scale facility/large-size playground, C-type as small-scale facility/small-size playground, D-type as middle-scale facility/large-size playground, and E-type as large-scale facility/large-size playground. 2) The adequateness of playground environment of D-type were estimate higher than others. C-type were estimated lower than other types in size and outdoor play areas organization of playground. 3) The care-givers in D-type and E-type wanted to install various play equipments, but the care-giver in C-type didn't wanted to install play equipment. 4) The various outdoor play areas were needed in D-type.

Effects of Long Term Care Hospital Care-givers' Fall Prevention Self Efficacy and Fall Prevention Health Belief on Fall Prevention Awareness (요양병원 간병사의 낙상예방효능감과 낙상예방건강신념이 낙상예방인지도에 미치는 영향)

  • The purpose of this study is to investigate the factors that affect the fall prevention awareness of care-givers working at long-term care hospitals. A convenience sampling method was used to select care-givers who worked at 7 different long term care hospitals which were located in D city and N city. Data were collected from July 2014 to August 2014. 200 data were used for analysis out of 215 data which were collected. The hierarchical regression analysis reveals the following results: Firstly, the effects of care-givers' education level, certificate status, period of work experience and fall-related learning hours on their fall prevention awareness level were statistically significant. Secondly, among fall prevention health beliefs, perceived benefit, perceived disability, perceived sensitivity and self-efficacy were positively related to the level of fall prevention awareness. Thirdly, while such factors as perceived benefit, perceived sensitivity and self-efficacy showed positive effects on the fall prevention awareness, the period of work experience had negative effects. The regression model shows the power of explanation of 31.7 percents. In conclusion, the study suggests a fine-tuned program to improve care-givers' fall prevention awareness in a way of promoting fall prevention self-efficacy, perceived benefits and perceived sensitivity while considering the care-givers' period of work experience.

A Study on the Family Problem of the Hospitalized Patients (입원환자가족의 가정문제에 관한 연구)

  • This study was conducted to identify the family problems of the in-patients and to analize factors Influencing to the family problems. The subjects for this study were 277 family members those who were giving care for the adult patients during hospitalized in general wards at Seoul National University Hospital in Seoul. Data were collected through interviews with the questionnaire from September second to September twentieth in 1989. The instrument used for this study was the family problems scale which was developed by the researcher. Analysis of data was done by frequency, percent, mean, t-test, ANOVA, Pearson-Correlation Coefficients, and Stepwise Multiple Regression Analysis. The results of this study are summarized as follows: 1. General characteristics of the care-giver in family. The average age of care-givers was 37.9 years, and the $26.4\%$ of monthly Income of family was 310,000-500,000 won group. The $93.5\%$ of family had taken the responsibility of caring for the patients instead of hiring the care-givers, and the $12.3\%$of the care-givers complained weakning of health status during care giving for the patients. The spouse took the largest part of responsibility of the care-giving services to the patient among the family members. 2. General characteristics of the patients. The average age of patient was 47 years, and the $80.9\%$ of patient was married status. The $39\%$ of patient was father in the position of family, and the $41.5\%$ had the responsibility to support their family before hospitalization. The average hospitalization period of patient was 24.3 day and the $50.9\%$ had admission experience. 3. The factors of family problems which were faced by the family were classified into six problems. The factors of family problems were ranked as follows; the first rank problem was related to care-giving for the patients. the second problem was resulted from the patients diseases, the theirds problem was related with adaptation to the hospital enviroments, the fourth problem was related to the arisen conflicts with medical team. the fifth problem was related to the change of family function. and the sixth problem was the financial problem. 4. The relationship between the family problems and the general charateristics of the care-givers showed that the nuclear type family was higher the family problems, that the admission period of patients became longer, and that the family who had the worse condition of health status of the care givers during care giving for the patients. From the above results, it was confirmed that the family care giving for patients was faced with some problems resulted from patient's illness, relation to the medical team, adaptation to the hospital enviroment, financial problem. change of family function, and care-giving for patients.

  • This study aims for strengthening the ability of home visiting care giver by figuring out interpersonal stress of bearing hardships which affect turnover intentions of home visiting care givers and by focusing on mediating effect of burnout. First, in summary, the analysis result was that the age of care givers and the presence of spouse are in relational factors correlated with burnout. Also age and experience of turnover have some connection with turnover intentions. Second, interpersonal stress of burnout and turnover intentions have static correlation. Third, the interpersonal stress represents increasing level of stress of burnout and the intervention of burnout shows static effects on turnover intentions of care givers. Suggestions of this study's results are, first, solution of interpersonal stress which home visiting care givers are facing should be improved by families' support or encouragement. In addition, the more professional and organized job training should be advanced to improve the understanding of various situations and coping skills. Second, the more interest should be need on business characteristics and environmental limitation. Also many centers that support care givers should reinforce their role. Third, to decrease the turnover intention which comes from interpersonal stress, doing some burnout research could be helpful and especially the legal regime which supports the research of burnout is essential.

The Effect of Sex Education about Old People for Care givers (요양보호사를 위한 노인의 성 교육 프로그램의 효과)

  • The aim of this study was to examined the effect of knowledge and attitudes about old people in care givers. This study was a quasi experimental designs of a nonequivalent control group comprised of a pre & post-test. The data was collected from a total of 54 (26 in the experimental group, 28 in the comparison group) subjects, who consented to the purpose and participation among care givers in the care facilities for old people throughout S-si. The data was analyzed using the SPSS/WIN 19.0 program with a Chi-square test, t-test, and ANOVA. The education program belonging to the experimental group showed a significantly increase in knowledge, and it took effect on making change into positive attitude of old people's sex. Given results of this study, it is suggested that a sex education program be provided as a training material for care givers to develop knowledge and to change the positive attitude about old people's sex in care facilities.

The Perceived Social Support of the Family with Pneumoconiosis Patient (진폐가족의 사회적지지 특성)

  • Purpose: The study has planned to find out the perceived social support of the families with pneumoconiosis patients. Method: The subjects of the study were the 300 family care givers of the pneumoconiosis patients who were hospitalized in Taeback, Donghae and Jeongsun Occupational Medical Center. The Social Support Survey Instrument developed by Park(1985) was adopted. Results: The Direct Perceived Supports showed statistically differences by the age(F=1.70 p=0.01) and the state of the disease(F=3.09 p=0.027) of the patients. The Health Situation Centered Support was different by the marietal situation(F=2.29 p=0.48) of the pneumoconiosis patients. The Indirect Perceived Supports were statistically different by sex(t=3.76 p=0.043) and relation with the patient (F=2.49 p=0.048), group joining(t=3.79 p=0.042) of the family care givers. The DPSs were statistically different by family income(F=2.25 p=0.025), family authority(F=2.81 p=0.031) and health insurance status(F=2.13 p=0.026). Recommendation: It is recommended to develop an active social support program at the pneumoconiosis care centers for the middle aged female family care givers of the pneumoconiosis patients with the support of Ministry of Labor, Ministry of Health and Welfare and other NGOs of pneumoconiosis.

Magnitude and its effected factors of non-covered services expenditures among long-term care facilities benefits user in Long-term Care Insurance (노인장기요양보험 시설서비스 이용자의 비급여 본인부담 크기 및 영향요인)

  • The purpose of this study is to understand magnitude and its related factors of user's cost-sharing for non-covered services in long-term care facilities. We corrected data for 1,016 subjects, based on the long-term care benefits cost specification. Eighteen subjects were excluded from the data analysis due to missing data on family care-givers characteristics. Finally, 998 subjects were included in the study. The average cost of non-covered services per month was 209,093 won and distributed from 0 to 1,011,490 won. There was a significant difference by the characteristics of family care-givers and long-term care facilities. The monthly average cost for meal materials per person was 199,181 won(0~558,000), average cost of additional charge caused by using private bed was 232,992 won (50,000~600,000), and costs for haircut and cosmetics were 8,599 won. For the rest, there were various programs costs(93,328 won), diaper and its disposal cost(109,628 won), purchase cost for daily necessaries(24,435 won) and etc. The related factors for the magnitude of non-covered services expenditures were education level of family care-givers, occupancy rate and location of LTC facilities, and the costs of using private bed, haircut and cosmetics, and various programs among non-covered services. These findings suggest that present level range of LTC facilities users' cost-sharing is wide and it is urgent to prepare the standard guideline for cost and level in non-covered services.

The Effect of Self-Efficacy and Job Stress on Quality of Service of Care Givers (요양보호사의 자기효능감과 직무스트레스가 서비스 질에 미치는 영향)

  • The purpose of this study was to investigate the effects of Self-Efficacy and Job Stress on Quality of Service. Data were collected through self reported structured questionnaire from Care givers using convenient sampling methods from May 1 to August 30, 2019. The data were analyzed with SPSS/WIN 25.0 using independent t-test, one way ANOVA, Pearson's correlational coefficient and multiple regression. These factors together explained 42.0% of the Quality of service in Care givers(F=13.86, p=<.001). Factor influence the Quality of Service was Self-Efficacy(β=0.56, p=<.001). The findings of this study indicate that education programs should be developed to improve the self-efficiency of caregivers and that further studies should be conducted on their effects.

Influence of Attachment Relationship Between Group-home Adolescents and Surrogate Care-givers on Psycho-social Adjustment - The Mediating Pathway of Optimism - (그룹홈 청소년이 맺는 대리양육자와의 애착관계가 심리사회적응에 미치는 영향 -낙관성의 매개효과를 중심으로-)

  • This study designed a mediating pathways models to examine the theory, 'the influence of attachment relationship between adolescents and their parents affects on adolescents' psycho-social adjustment through the mediating variable of optimism'. Authors tried to examine if this theory is applied to group-home adolescents who live with surrogate care-givers instead of their parents. For this study, two models, 'Psychological Adjustment Model' and 'Social Adjustment Model' were constructed. Each model has path, 'Attachment with Care-givers → Psychological Adjustment' and ''Attachment with Care-givers → Social Adjustment' respectively. 'Optimism' is mediating variable in between these two variables. The results were very clear. In two models, no direct effects from 'Attachment' to 'Psychological Adjustment' and from 'Attachment' to 'Social Adjustment' were found. However, indirect effects from 'Attachment' to 'Psychological Adjustment' and from 'Attachment' to 'Social Adjustment' were statistically significant through the mediating variable of 'Optimism'. This study suggests that more research on 'Optimism' which has not been focused among social welfare researchers needs to be followed.
