Determinants of Amount of Service Use in Community-Based Long-term Care for Elders (노인장기요양보험 재가서비스 이용량 결정요인)

  • Purpose: This study was done to explore factors related to amount of service use for elders with long-term care needs. Methods: A descriptive-correlation design was used. The sample included 259 elders and their primary caregivers who had cared for the elders for at least 6 months. Data on long-term care need assessment, service use and interviews with primary caregivers were analyzed. Results: There was no significant relationship between the sociodemographic characteristics and the amount of services use. Amount of service use differed significantly by Long-term care classification. The mean scores for class 1, 2 and 3 were 22.68, 21.47 and 17.87 days respectively. Primary caregiver relationship with the elders and the number of family-friend helpers were also significant. Multivariate regression analysis showed that gender, marital status, activities of daily living, cognitive impairment, and secondary caregiver support explained 17% of the total variance of service use among these elders (F=3.50, p<.001). Conclusion: The results of this study indicate that critical factors including secondary caregiver support and individual background, and other functional dependencies except for physical function should be considered in accurately predicting the amount of service use for community dwelling elders with long-term care needs.

Safeguard system using distribution line and telecommunication network for controlling aimless behavior in senile elderly patients

  • This is a successive study of the development and application of an electronic safeguard system for elderly men (senile dementia patients) who wander without purpose because of declined mental capability, while retaining their physical ability. The new safeguard system is designed with some additional functions on the basis of the previously developed system. Firstly, alarms are designed not to disturb other patients at night, so that informations about doors from which the patients go out may be transmitted to helpers individually by radio paging system. Secondly, the system hardware can be set up anywhere without laying particular signal transmission cables, provided that there exist AC power distribution lines for the utilization as a transmission line of signals to alarm indicators. Thirdly, it is possible to have a grasp of the whole states of the safeguard systems at the center of operation by monitoring the operational state of each system with a necessary data acquisition according to its instruction through telecommunication network. Thus, each safeguard system can be economically supplied to the special nursing homes and the helpers are ensured more released from physical and psychological burdens so that they can devote themselves to the care of senile elderly men, thereby improving their patients' comfort and human dignity.

  • This study is to propose the dwelling planning of collective housing for the senior citizen who lives alone in rural and fishing villages. and this paper analyzed a change element regarding the field of care service for the elderly. The results of research are as follows: First, the range a village unit of "Home for the Elderly" was investigated most primarily. Second, on the lifestyle of a bedroom, proper using person per a room was investigated to two people to use with one room and life style of using bed preferred more in life style of sitting on the floor(ondol). Third, most necessary facilities were examined into bathroom and steam-room, and the separation of man and woman space, toilet was investigated to 1 ranking. Fourth, to be most difficult care service was investigated by long-distance circulation. This is the item which can be reduced naturally in case of service activity caring for a visit in a collective housing.

Measuring Workload of Home Visit Care Activities Using Relative Values (방문요양 행위 업무량의 상대적 가치 측정)

  • Objectives : The purpose of this study was to measure the workload of home visit care activities and their relative values. This study examined also factors that affect the workload of home visit care activities. Methods : The participants of this study were 126 home-helpers of 50 home visit care agencies at the 2nd Long-term Care Insurance Demonstration Project. The workload of home visit care activities was divided into total work and four dimensions ; physical efforts, mental efforts, stress and time. Home visit care activities consisted of four categories with 24 items. We used magnitude estimation method to measure their relative values of the four dimensions. The participants answered the relative values of each activities based on the reference service. We used the activity for supporting their elderly's evacuation as the reference service. Results : Most of the respondents were over 40 years old female. They consumed most their time supporting elderly's going out. They consumed their highest physical, mental efforts, and stress for activities of coping with emergency situation. The Pearson correlation coefficients showed significant relationships between workload and each dimensions. This study showed that all four dimensions are statistically significant predictors of workload of home visit care activities. Also, we found that the home-helper's career affects the workload of home visit care activities. Conclusions : The workload of home visit care activities could be explained by physical efforts, mental efforts, stress and time.

Nutritional Assessment and Management in Long-term Care Insurance's Home Visit Care Service (경기도 일부 노인의 노인장기요양보험 방문요양서비스의 영양상태 평가 및 영양관리 현황)

  • The purpose of the study was to examine nutritional management and nutritional care demands of home-visit care service and the nutritional status of serviced elderly. The survey was carried out from $5^{th}$ to $21^{st}$ of January, 2011 among 37 In-Home Service institutions. The number of elderly surveyed by 143 care helpers was 281. Analysis was performed for 203 subjects (male: 44, female: 159). The age distribution was from 50 to 99 years. The grading by long-term care insuranceshowed 18,2% on level 1 and 2, and 81.8% on level 3. For the disease treatment, the proportion of implementing diet treatment turned out to be very low. The proportion of subjects living with their children was 45.3%, and living alone was 29.1%. The proportion of home-visit care among types of In-Home Service was 70%. Subjects who needed additional necessary diet management service turned out to be 40.9%. Diet management was the most necessary services from all levels. MNA (specifiy MNA) scores were significantly (p < 0.001) associated with BMI, mid-arm circumference (MAC), calf circumference (CC), intake problems, and weight loss during the last 3 months. For studied elderly, 45.3% were malnourished, and 46.8% were at the risk of malnutrition by MNA score. Based on the result of this survey, the nutritional care was not systematic on the In-Home Service. Prevention and management of the disease was much better than the treatment to improve the quality of life. We conclude that nutrition management is necessary for the elderly. To improve nutritional status of elderly in home care services, systematic nutrition management should be implemented.

Studies on the Demands of Development Program of Silver Industrial Professionals in the Northern Gyeonggi-do Area (경기 북부 실버관리인력 양성 과정에 관한 수요도 조사)

  • The principal objective of this study was to assess the demands of the development program of silver health care professionals for elderly individuals residing in the Northern Gyeonggi-do area. To this end, a survey was conducted to investigate college students and employees related with elderly individuals in the area regarding their recognition, interest, and involvement in the labor training program. In the case of college students, the health education they had received was only 1 to 2 hours, from a school lecture(35%). The content of health education they desired was exercise(34%) and stress management skills(28%). The sources of health information they received included mass media(77%) and the internet(12%), and they trusted the information they received from health professionals(45%), and the mass media(34%). In the case of health professionals who were working at silver care facilities, the sources of health information to which they had access were mass media(51%), internet(14%), reliable health professionals(56%), mass media(22%), and books related to health (18%). The principal issues they reported as being relevant to the elderly were dementia(39%), hypertension(14%), arthritis (11%), and they reported that the most important personnel for elderly in the future would be care managers(44%), and care helpers(21%). 88% of subjects believed that there was a need for a silver welfare integrated information system. 43% of subjects used the internet, 77% of them required in-service training programs for the welfare of the elderly. Via this developmental program of silver health care professionals, a variety of new job opportunities can be provided in the future, and a program related to the silver service industry must be established as soon as possible.

A Study on Double-Care and Multiplicity of Caring Experiences among Women Aged 30s to 40s in Korea (한국 30대~40대 여성의 이중돌봄 현실과 돌봄경험의 다중성에 관한 연구)

  • The study is to show the prevalence of double-care burdened households who care simultaneously for children(under 6) and provide invalid elderly care for aging parents among women aged 30s to 40s in Korea. The study aims to place care as ordinary needs for our whole lifetime, not as a special happening to a certain age group at specific period. Also, study attempts to reveal that care experiences include both burden and pleasure, and that care relationship, such as care giver-receiver-assistants(second-helpers), strongly affects the diversity of caring experience. Results are as follows. 38.1% of the survey respondents report that they are/have ever been the double-care burdened households who carry both child care under 6 and elderly care suffering from health problems in the past or the present. If including those who expecting the double burden of care in the near future, the prevalence rate goes up to 54.9%. As hypothesized, caring includes both pleasure and burden. The order of pleasant feelings is child care> parents care>in-law parents care, and that of burden is in reverse. However, caring relationship has a strong influence on pleasure or agony of the caring experience. The association between the relationship and the experience, which is expressed by pleasure or pain, occurs to both daughter-parents and in-laws. Interestingly, the caring experiences by daughter to her parents tend to go more burdensome because their husbands do not commit their identity as carer aid, with no additional caring effort for their in-laws. In conclusion, some policy suggestions for double care problems are provided.

A Study of Stress on the Family Caregivers for the Hospitalized Elderly (입원 노인을 돌보는 가족원의 스트레스에 관한 연구)

  • This study explored the variables to influence the stress of the family caregivers providing care for the hospitalized elderly The subjects for this study were 104 family caregivers selected from the caregivers in the in patient departments of two major medical centers in Jeonju city. Data were collected from December 18th to 27th, 1995 using a Liker Scale Questionaire and interview. Data analyses included percentage, mean, standard deviation, t and F-test. Results were following : 1. The mean of this sample is 21.48, and the standard deviation is ${\pm}5.64$ in the perceived stress. The major variables to influence on the stress of caregivere for the hospitalized elderly were the stay (16-24hrs/day) in hospital(F=7.99, p=0.006), no other helpers(t=-2.56, P=.012), percieving severely on illness(F=3.90, P=.0232) perceiving moderately on physical health status(F=6.20, P=.0029). 2. Regarding the perceived social support of the family caregivers, the higher support the group perceived the lower stress(F=3.54, P=.0326).

Factors affecting family Caregivers' Preference for Utilization of Community Eldercare Services (가족부양자의 재가복지서비스 이용의사에 영향을 미치는 요인에 관한 연구)

  • This study examined the factors affecting family caregiver's preference for utilization of community care services among those who are caring for 65+ elderly parents, and aimed to show how social eldercare services would be settled in Korea. Help-seeking behavior model developed by Anderson and Newman(1973) was used to analyze the factors affecting their preference for utilizing the community care service among 283 family caregivers. Frequency, Chi-square, and Multinominal logistic analysis on SAS 6.12 was used. According to the results, about 90% of the family caregivers have preference for community and institute care services. In community care service, about a half comprise the preference with charge while the other without charge. However, about 90% of those for institute care service show their willingness to pay for the service. Also, a majority of caregivers like to rely on social eldercare service, rather than family as exclusively responsible, against long-term care for their elderly parents. Multinominal analysis demonstrates that use versus nonuse of community care services is primarily affected by predisposing factors(including age, carer-caree closeness, and familism) and need factors (including economic or psychological burden of eldercare, and additional role for family care). Enabling factors, such as family income level, economic support from other family members and siblings, and supportive care-helpers, are mainly associated with the preferences of free versus charge in service use. These findings provide some implications and suggestions for the development of social eldercare services in our aging society.

Research Trends of Young Children's Parent Education Programs Children in South Korea from 2001 to 2020 (영유아 부모교육 프로그램 연구의 2001-2020년간의 국내 연구 동향)

  • Objective: The purpose of the current research is to collect basic data to diagnose the current status and to foresee the future research trends of young children's parent education programs published in South Korea from 2001 to 2020. Methods: Data collection was made out of RISS of Korea. Excel 2016 was used to categorize 210 finally filtered and collected data which included academic journal articles, MA/PhD dissertations, and funded research reports according to analysis criteria by publication year/5-year-term, research theme, research method, and researchers' academic area. Results: Annual/5-year-term analysis shows increasing trends of parent education programs for young children. Quantitative research was the most frequently implemented method, followed by literature reviews, qualitative research, and mixed research method in order. In research theme, parent-competency reinforcement program was the most frequently implemented theme followed by parent education, socio-emotional issue, special education for young children with special needs, family support, early childhood sex education, and program analysis in order. Education area showed the most active participation in parent education program for young children in comparing with other academic areas in research field. Conclusion/Implications: Research trends of young children's parent education programs showed steady increase in their amount, frequency, and diversity as well. Minority parents need more attentions for providing the next young generation's educational equality. Parent programs during COVID-19 need to gain more research attentions as well as care-giving grandparents, social workers, and public health care helpers in child caring service areas in order to alleviate low-birth rate.
