Objective : This study assessed the effect of a combination of Korean medicine on a thyroid papillary cancer patient who was diagnosed with local lymph node metastasis after thyroidectomy and lymph node dissection but did not want surgery. Methods : Gami-Palmultang administration and moxibustion(large Bmoxa cautery) were performed for six years. Treatment outcomes were evaluated with Brief Fatigue Inventory (BFI), Numerical Rating Scale (NRS), Insomnia Severity Index (ISI), Functional Assessment of Cancer Therapy-General (FACT-G), blood test/ CT imaging results, and patient's statements. Results : After the treatment, all symptoms have been alleviated, the quality of life has increased, and it has been maintained without further metastasis of tumors for six years. Conclusion : Korean medicine treatment along with active observation can be an alternative to patients who do not want surgical treatment after recurrence of local lymph nodes in thyroid papillary cancer, and can have positive results in improving the quality of life.

Radiopharmaceuticals targeting prostate-specific membrane antigens (PSMA) are essential for the diagnosis, evaluation, and treatment of prostate cancer (PCa), particularly metastatic castration-resistant PCa, for which conventional treatment is ineffective. These molecular probes include [68Ga]PSMA, [18F]PSMA, [Al18F]PSMA, [99mTc]PSMA, and [89Zr]PSMA, which are widely used for diagnosis, and [177Lu]PSMA and [225Ac]PSMA, which are used for treatment. There are also new types of radiopharmaceuticals. Due to the differentiation and heterogeneity of tumor cells, a subtype of PCa with an extremely poor prognosis, referred to as neuroendocrine prostate cancer (NEPC), has emerged, and its diagnosis and treatment present great challenges. To improve the detection rate of NEPC and prolong patient survival, many researchers have investigated the use of relevant radiopharmaceuticals as targeted molecular probes for the detection and treatment of NEPC lesions, including DOTA-TOC and DOTA-TATE for somatostatin receptors, 4A06 for CUB domain-containing protein 1, and FDG. This review focused on the specific molecular targets and various radionuclides that have been developed for PCa in recent years, including those mentioned above and several others, and aimed to provide valuable up-to-date information and research ideas for future studies.

Objective: Proper management of lymph nodes (LNs) with ultrasonographic (US) indeterminate features in thyroid cancer patients remains elusive. We aimed to evaluate the malignancy risk and US findings predictive of malignancy for US indeterminate LNs in preoperative thyroid cancer patients through node-by-node correlation. Materials and Methods: A total of 348 LNs in 284 thyroid cancer patients, who underwent fine-needle aspiration or core-needle biopsy between December 2006 and June 2015, were included. We determined the malignancy risks for US probably benign, indeterminate, and suspicious categories. For US indeterminate LNs, which had neither echogenic hilum nor hilar vascularity in the absence of any suspicious finding, US findings were compared between benign and metastatic LNs using Mann-Whitney U test and Fisher's exact test. Results: US imaging diagnoses were probably benign in 20.7% (n = 72) cases, indeterminate in 23.6% (n = 82), and suspicious in 55.7% (n = 194). Malignancy risk of US indeterminate LNs (19.5% [16/82]) differed from those of the US probably benign (2.8% [2/72]) (p = 0.002) and US suspicious LNs (78.4% [152/194]) (p < 0.001). Among US indeterminate LNs, there were no significant differences in short, long, and long-to-short diameter (L/S) ratios between benign and metastatic LNs (3.9 vs. 3.8 mm, p = 0.619; 7.3 vs. 7.3 mm, p = 0.590; 1.9 vs. 1.9, p = 0.652). Conclusion: US indeterminate LNs were frequently encountered during preoperative evaluation and had intermediate malignancy risk. Given the lack of discriminative power of size criteria and L/S ratio, clinical factors such as surgical strategy and node size should be considered for proper triage of US indeterminate LNs in thyroid cancer.
최근 의료영상기술에서 유방암 엑스선 진단기술의 주요 이슈는 정확한 조기암 진단과 환자의 피폭선량의 감소에 있다. 엑스선 영상대비도를 높이고 피폭선량을 줄이는 기술 중 하나로써 다층박막거울을 이용한 단색 엑스선을 획득하는 연구가 선행되어 왔다. 그러나 기존의 Uniform 다층박막거울은 거울면의 일부 반사영역에서만 원하는 파장대역의 단색 엑스선을 얻을 수 있어서 엑스선 영상기술 응용에 한계가 있다. 본 연구에서는 다층박막거울의 전 영역에 걸쳐 동일한 단색엑스선을 얻기 위해 거울에 입사하는 백색 엑스선의 입사각에 상응하는 선형적 기울기의 박막두께를 갖는 Graded 다층박막거울을 설계하였고, 기존 이온빔 스퍼터링 장치에 마스크 제어 장치를 추가 제작하여 $100{\times}100mm^2$ 크기로 제작하였다. 제작된 Graded 다층박막거울은 17.5keV의 단색 엑스선을 획득할 수 있도록 설계하였으며 박막두께주기는 2.88nm~4.62nm(Center 3.87nm)이다. 엑스선 반사율은 60% 이상이며, 단색 엑스선의 FWHM은 1.4keV 이하이고 엑스선 빔 폭은 3mm정도이다. 유방촬영에 적합한 몰리브덴 특성엑스선에 해당하는 17.5keV의 단색 엑스선을 얻음으로써 저선량 고감도 유방암 진단장치 개발에 응용할 수 있을 것으로 기대된다.
목적 : 전산화단층촬영(CT)상 폐암과 유사하게 보인 기관지 탄분 섬유화증 환자에서 자기공명영상(MRI) 소견을 알아보고자 하였다. 대상 및 방법 : 기관지내시경검사상 기관지 탄분 섬유화증으로 입증된 50명의 환자에서 CT상 중 페암과 유사하게 보인 10명의 환자(남 : 여 = 2 : 8, 평균 연령 68세, 연령 분포: 58-79세)를 대상으로 하였다. 전 예에서 CT(n=10)검사와 MRI(n=10) 검사를 시행하였고 경피적 조직생검(n=1)은 1예에서 시행하였다. 조영증강 없이 축상 T1 강조영상(600/30/3, repetition time/echo time/excitation)과 T2 강조영상(3500/99/4)에서 종괴, 무기폐와 림프절의 신호강도를 2명의 방사선과 전문의가 합의에 의해 후향적으로 분석하였다. 결과 : CT 소견은 4명에서는 종괴로, 4명에서는 무기폐로, 2명에서는 기관지벽 비후였다. 전 예에서 기관지 탄분 섬유화증의 특징인 종격동 림프절은 커져 있었다. 종괴는 T1 강조영상과 T2 강조영상에서 모두 저 신호강도로 보였다. 무기페는 T1 강조영상에서 중등도 신호 강도로, T2 강조영상에서 저 신호강도로 보였다. 림프절은 9명의 환자에서 T1 과 T2 강조영상에서 저 신호 강도로 보였다. 한명의 환자에서 T2 강조영상에서 림프절이 중심부는 고 신호강도로, 주변부는 저 신호강도로 보였다. 결론 : 폐암과 유사하게 보인 기관지 탄분 섬유화증환자에서 종괴, 무기폐와 림프절은 T2 강조영상에서 주로 저 신호강도로 보여 폐암과의 감별에 자기공명영상이 도움이 될 것으로 생각한다.
The cocktails of two $^{131}I$ labeled Monoclonal antibody (MCAB) (Anti CA 19-9 F$(ab')_2$ + Anti CEA $F(ab')_2$ fragment), which react specially, with human gastrointestinal cancers, were administered to 10 patients with colorectal (7), stomach(2) and pancreas(1) cancer for scintigraphic detection. All patients were known or postoperatively recurrent cases, and serum tumor markers, CA 19-9 and CEA, were measured with immunoradiometric assay, just before immunoscintigraphy (ISG). The tumor marker's level in serum is not correlated with positive tumor uptake in ISG. The sensitivity and specificity of ISG in detection of 21 tumor sites, based on surgery, CT, ultrasonography and pathology, were 90.5% and 100% One case of colon cancer showed gall bladder metastasis, which was neglected on CT study. Tumor/non tumor uptake ratio of radiolabelled antibody were progressively increased from day 3 to day 7 during study. We summerized as follows 1) The use of cocktails of CEA and CA 19-9 MCAB $F(at')_2$ increased sensitivity and specificity in ISG. 2) Delayed imaging (later than 5 days) increases sensitivitv and specificity due to exclusion of nonspecific iodine accumulation in stomach and lung. 3) Second tracer technique is essential for anatomical landmark by use of a double isotope scan, but subtraction technique, a possible source of artifacts, is no longer necessory when delayed imaging is performed. 4) It may be possible to use two MCAB cocktails of CA 19-9 and CEA in Radioimmunodetection of stomach and pancreas cancer. In conclusion, ISG using MCAB cocktails, $F(ab')_2$ fragment of anti CA 19-9 and Anti CEA, provide additional opportunity for tumor localization and detection of colorectal and other G-I cancer, such as stomach and pancreas.
Purpose : The aim of this study is to conform the possibility of the liquid type EPID as a QC tools to clinical indication and of replacement of the film dosimetry. Aditional aim is to describe a procedure for the use of a EPID as a physics calibration tool in the measurements of radiation beam parameters which are typically carried out with film. Method & Materials : In this study we used the Clinac 2100c/d with EPID. This system contains 65536 liquid-filled ion chambers arranged in a $256{\times}256$ matrix and the imaging area is $32.5{\times}32.5cm$ with liquid layer thickness of 1mm. The EPID was tested for different field sizes under typical clinical conditions and pixel values were calibrated against dose by producing images using various thickness of lead attenuators(lead step wedge) using 6 & 10MV x-ray. We placed various thickness of lead on the table of linear accelerator and set the portal vision an SDD of 100cm. To acquire portal image we change the field size and energy, and we recorded the average pixel value in a $3{\times}3$ pixel region of interest(ROI) at field center was recorded. The pixel values were also measured for different field sizes in order to evaluate the dependence of pixel value on x-ray energy spectrum and various scatter components. Result : The EPID, as a whole, was useful as a QA tool and dosimetry device. In mechanical check, cross-hair centering was well matched and the error was less than ?2mm and light/radiation field coincidence was less than 1mm also. In portal dosimetry the wider the field size the the higher the pixel value and as the lead thickness increase, the pixel value was exponentially decreased. Conclusions : The EPID was very suitable for QA tools and it can be used to measure exit dose during patients treatment with reasonable accuracy. Background: Pancreatic cancer has a high mortality rate because it is usually diagnosed late. Since little is known about this cancer in Iran, with the aim of improving this knowledge deficiency, we evaluated clinical, laboratory biomarkers, imaging findings and treatment modalities in Iranian patients with pancreatic adenocarcinoma. Materials and Methods: 131 cases of pancreatic adenocarcinoma in 2010-2013 were obtained from the Taleghani Hospital Record Department. Cases confirmed by histopathology from CT-guided biopsy, EUS-FNA and surgery examination were included. We excluded those with incomplete medical records. Results: The study included 131 subjects between 24 and 97 years of age and a mean age of $63{\pm}13.4$ years. Eighty (61.1%) were male and 51 (38.9%) female. Previous history included diabetes mellitus in 36 (27.5%), alcohol drinking in 5 (3.9%), smoker in 28 (21.4%) and opium addiction in 13 (10%). The common presenting history included weight loss in 79 (60.3%), abdominal pain in 77 (58.8%), fever in 11 (8.4%), nausea in 30 (22.9%), jaundice in 72 (55%), pruritus in 52 (39.7) and anemia in 33 (25.2%). CA19-9 levels with cut offs of 50, 100 and 200 U/ml were increased in 81%, 72% and 66% of patients, respectively. Tumor staging was: stage I, 3 (2.3%); stage II, 10 (7.6%); stage III, 58 (44.3%); and stage IV, 60 (45.8%). From 45 patients, 17 received ERCP inserted metallic stents and 22 plastic stents, the remaining 6 failed that PTC was done. Whipple surgery and chemotherapy were conducted for 10 and 29 patients, respectively. Conclusions: This disease affected older people and there was a male preponderance. The commonest risk factors were diabetes mellitus, smoking and cholelithiasis. The majority of patients presented with loss of appetite, loss of weight, jaundice, abdominal pain and discomfort. Almost all presented at late stages of the disease so that curative surgery was impossible. Also chemotherapy was only performed in a few patients as a neoadjuant treatment.
Background: The objective of this research is the computed axial tomography (CT) imaging grading of radiation induced pneumonitis (RP) and its correlation with clinical and radiotherapeutic parameters. Materials and Methods: The chest CT films of 20 patients with non-small cell lung cancer who have undergone threedimensional conformal radiation therapy were reviewed. The proposed CT grading of RP is supported on solely radiological diagnosis criteria and distinguishes five grades. The manifestation of RP was also correlated with any positive pre-existing chronic obstructive pulmonary disease (COPD) history, smoking history, the FEV1 value, and the dosimetric variable V20. Results: The CT grading of RP was as follows: 3 patients (15%) presented with ground glass opacity (grade 1), 9 patients (45%) were classified as grade 2, 7 patients (35%) presented with focal consolidation, with or without elements of fibrosis (grade 3), and only one patient (5%) presented with opacity with accompanying atelectasis and loss of pulmonary volume (grade 4). Both univariate and multivariate analysis revealed as prognostic factors for the radiological grading of RP the reduction of FEV1 and the V20 (P=0.026 and P=0.003, respectively). There was also a significant (P<0.001) correlation of radiological grading of RP with FEV1 and V20 (spearman rho 0.92 and 0.93, respectively). Conclusions: The high correlation of the proposed radiological grading with the FEV1 and the V20 is giving a satisfactory clinical validity. Although the proposed grading scale seems relevant to clinical practice, further studies are needed for the confirmation of its validity and reliability.
목 적: 전자포탈영상장치 기반의 환자특이적 정도관리를 위한 portal dose image prediction (PDIP)와 anisotropic analytical algorithm (AAA)을 비교하여 성능을 분석하고, AAA를 사용한 portal dosimetry의 임상적 사용 가능성을 평가하고자 한다. 대상 및 방법: 폐암 환자 15명과 간암 환자 17명, 총 32명의 환자를 후향적으로 선정하였다. PDIP와 AAA를 사용하여 검증용 치료계획을 생성하였다. 계산된 분포와 측정된 분포를 비교한 감마통과율(Gamma passing rate, GPR)과 다엽콜리메이터(Multileaf collimator, MLC) 위치 차이를 얻었다. 결 과: 폐암 환자군의 GPR 평균값은 PDIP 사용시 3%/3 mm에 대해 99.5% ± 1.1%, 1%/1 mm에 대해 90.6% ± 5.8%였다. AAA 사용시 3%/3 mm에 대해 98.9% ± 1.7%, 1%/1 mm에 대해 87.8% ± 5.2%였다. 간암 환자군의 GPR 평균값은 PDIP 사용시 3%/3 mm에 대해 99.9% ± 0.3%, 1%/1 mm에 대해 96.6% ± 4.6%였다. AAA 사용시 3%/3 mm에 대해 99.6% ± 0.5%, 1%/1 mm에 대해 89.5% ± 6.4%였다. MLC 위치 차이는 0.013 mm ± 0.002 mm로 적었으며, 감마통과율과 상관관계를 보이지 않았다. 결 론: 전자포탈영상장치 기반 환자특이적 정도관리를 수행할 때 AAA를 임상적으로 portal dosimetry 계산 알고리즘으로써 사용할 수 있다.

제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
