Purpose: Cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) have been shown to improve survival in select patients with gastric cancer and peritoneal metastases. It remains unclear, however, whether this multimodal treatment protocol is also beneficial for signet-ring cell gastric cancer (SRC) patients with peritoneal metastases. Materials and Methods: Clinical data of patients scheduled for upfront systemic chemotherapy consisting of 5-FU (2,600 $mg/m^2$), folinic acid (200 $mg/m^2$), docetaxel (50 $mg/m^2$), and oxaliplatin (85 $mg/m^2$) followed by CRS and HIPEC using cisplatin (50 $mg/m^2$) at the Comprehensive Cancer Center, University Hospital T$\ddot{u}$bingen, Germany were retrospectively analyzed. Results: Eighteen consecutive patients for whom irresectability has been ruled out by a computed tomography scan were enrolled. However, complete cytoreduction could only be achieved in 72% of patients. When categorizing patients with respect to the completeness of cytoreduction, we found no difference between both groups considering tumor- or patient-related factors. The overall complication rate following complete cytoreduction and HIPEC was 46%. Within a median follow-up of 6.6 (0.5~31) months, the median survival for CRS and HIPEC patients was 8.9 months as opposed to 1.1 months for patients where complete cytoreduction could not be achieved. Following complete cytoreduction and HIPEC, progression-free survival was 6.2 months. Conclusions: In SRC with peritoneal metastases, the prognosis appears to remain poor irrespective of complete CRS and HIPEC. Moreover, complete cytoreduction could not be achieved in a considerable percentage of patients. In SRC, CRS and HIPEC should be restricted to highly selective patients in order to avoid exploratory laparotomy.
As the volume of internet and web contents have shown an explosive growth in recent years, lately contents recommendation system (CRS) has emerged as an important issue. Consequently, researches on contents recommendation method (CRM) for CRS have been conducted consistently. However, traditional CRMs have the limitations in that they are incapable of utilizing in web 2.0 environments where positions of content creators are important. In this paper, we suggest a novel way to recommend web contents of high quality using both degree of centrality and TF-IDF. For this purpose, we analyze TF-IDF and degree of centrality after collecting RSS and FOAF. Then we recommend contents using these two analyzed values. For the verification of the suggested method, we have developed the CRS and showed the results of contents recommendation. With the suggested idea we can analyze relations between users and contents on the entered query, and can consequently provide the appropriate contents to the user. Moreover, the implemented system we suggested in this paper can provide more reliable contents than traditional CRS because the importance of the role of content creators is reflected in the new system.

To prevent the overflow of various harmful chemicals, it is necessary to modify the chemical management system with an expansion to institutionally regulated substances. This modification should be preceded by selection of the priority chemicals, with a diverse chemical ranking system (CRS) applied to select the chemicals in developed countries. In Korea, a systematic CRS was used in a project related to soil and groundwater, however, it is inadequate to compare soil and groundwater CRS to that of surfacewater. In this study, a priority chemical ranking system for surfacewater was proposed through the analysis of international and domestic CRS cases. This was then applied to 161 chemicals to derive the priority list of harmful chemicals. As a result, Chemical RAnking of surFacewater polluTants (CRAFT) is presented for the protection of human health and the aquatic ecosystem from surfacewater pollutants. The components of CRAFT are the human health toxicity, aquatic ecosystem toxicity and reliability assessment factors. Three lists were derived from the 161 priority harmful chemicals for the protection of human health, aquatic ecosystem or both. It is expected that this result can be useful to prioritize harmful chemicals for the protection of human health and the aquatic ecosystem from Korean surfacewater.

Objectives: This study was performed to check whether the CRS (Chemical Ranking and Scoring) system is appropriate as a method to determine substances as candidates for substances subject to permission and to apply this system to the selection of candidates for substances subject to permission. Methods: A risk score was obtained by multiplying the hazard score and the exposure score and then ranking them. The hazard sub-indicators are carcinogenicity, germ cell mutagenicity, reproductive toxicity, specific target organ toxicity-repeated exposure, respiratory sensitization and endocrine disrupting chemicals. Exposure sub-indicators are persistence, bioaccumulation and emission volume. Sensitivity analysis was performed for missing values. Correlation analysis and multivariable linear regression analysis were performed among hazard, exposure and risk in order to confirm that CRS was an appropriate method. Results: As a result of the sensitivity analysis on missing values, it was confirmed that the effect on the risk ranking was not sensitive. Correlation and regression analysis confirmed that exposure had a greater effect on risk than hazard. Conclusions: The CRS system, which derives a risk score using a hazard and exposure score, is judged to be appropriate as a method for the selection of preliminary of candidates for substances subject to permission. Benzene, cadmium, nickel, and cobalt were selected as priority candidates for substances subject to permission.
In this study paper, we examine pre-trained language models used in Multi-Goal Conversational Recommender Systems (MG-CRS), comparing and analyzing their performances of various pre-trained language models. Specifically, we investigates the impact of the sizes of language models on the performance of MG-CRS. The study targets three types of language models - of BERT, GPT2, and BART, and measures and compares their accuracy in two tasks of 'type prediction' and 'topic prediction' on the MG-CRS dataset, DuRecDial 2.0. Experimental results show that all models demonstrated excellent performance in the type prediction task, but there were notable provide significant performance differences in performance depending on among the models or based on their sizes in the topic prediction task. Based on these findings, the study provides directions for improving the performance of MG-CRS.
In this paper, we propose the opportunistic non-orthogonal multiple access (NOMA)-based cooperative relaying system (CRS) with channel state information (CSI) available at the source, where CSI for the source-to-destination and source-to-relay links is used for opportunistic transmission. Using the CSI, for opportunistic transmission, the source instantaneously chooses between the direct transmission and the cooperative NOMA transmission. We provide an asymptotic expression for the average achievable rate of the opportunistic NOMA-based CRS under Rayleigh fading channels. We verify the asymptotic analysis through Monte Carlo simulations, and compare the average achievable rates of the opportunistic NOMA-based CRS and the conventional one for various channel powers and power allocation coefficients used for NOMA.

The purpose of this research attempted to developed the Community Reintegration support Program(H-CRsP) on patients with Spinal Cord Injury(SCI) and identity the effect. To assess the effectiveness of a Hospital based H-CRsP with a comprehensive team approach for treating inpatients with Spinal Cord Injury(SCI), offered by the Korea National Rehabilitation Hospital. Participants with SCI were recruited from the Korean National Rehabilitation Hospital. Twenty-five participants who met inclusion criteria were provided with a H-CRsP. A H-CRsP with modules related to the training of daily living skills, driving, vocations and school support activities, leisure or recreational activities, peer counseling, sexual rehabilitation, and others support activities. The 25 enrolled patients had been treated with average of 6.1 programs 2 times a week for over 2 or 3months. COPM, HADS, WHOQOL-BREF, AD-R were administered before the treatment(pre-test) and also at the time of discharge(post-test) to compare the treatment outcomes in the 25 program participants. Measured level of community integration by CIQ at discharge. The data were analyzed by such statistical methods as frequency and paired t-test analysis. Significant improvements were found on the Occupational Performance and Satisfaction, Acceptance of Disability, Quality of Life score in the COPM, AD-R, WHOQOL-BREF (p<0.05). In addition, 10 of the 25 patients returned to home, work or school. These results demonstrated the effectiveness of the H-CRsP in helping patients with SCI return their communities to which they belong. It considered development of the variety of programs and prepare systematically related system.
Background: The impairment of the appearance is a major problem for patients with carcinomas of the oral cavity. These patients want to recover their preoperative facial appearance. Some do not realize that this is not always possible and hence develop a desire for further cosmetic and reconstructive surgery (CRS) which often causes psychological problems. Method: The desire of patients for CRS (N = 410; 26%) has been acquired in this $D{\ddot{O}}SAK$ rehab study including multiple reasons such as medical, functional, aesthetic and psychosocial aspects. They relate to the parameters of diagnosis, treatment and postoperative rehabilitation. Patients without the wish for CRS (N = 1155; 74%) served as control group. For the surgeons, knowledge of the patient's views is relevant in the wish for CRS. Nevertheless, it has hardly been investigated for patients postoperatively to complete resection of oral cancer. In this retrospective cross-sectional study, questionnaires with 147 variables were completed during control appointments. Thirty-eight departments of Oral and Maxillofacial Surgery took part, and 1652 German patients at least 6 months after complete cancer resection answered the questions. Additionally, a physician's questionnaire (N = 1489) was available. Statistical analysis was performed with SPSS vers. 22. Results: The patient's assessment of their appearance and scarring are the most important criteria resulting in wishes for CRS. Furthermore, functional limitations such as eating/swallowing, pain of the facial muscles, numb regions in the operating field, dealing with the social environment, return to work, tumour size and location, removal and reconstruction are closely related. Conclusion: The wish for CRS depends on diverse functional psychosocial and psychological parameters. Hence, it has to be issued during conversation to improve rehabilitation. A decision on the medical treatment can be of greater satisfaction if the surgeon knows the patients' needs and is able to compare them with the medical capabilities. The informed consent between doctor and patient in regard to these findings is necessary.
Cosmological shocks form as an inevitable consequence of gravitational collapse during the large scale structure formation and cosmic-rays (CRs) are known to be accelerated at collisionless shocks via diffusive shock acceleration (DSA). We have calculated the evolution of CR modified shocks for a wide range of shock Mach numbers and shock speeds through numerical simulations of DSA in 1D quasi-parallel plane shocks. The simulations include thermal leakage injection of seed CRs, as well as pre-existing, upstream CR populations. Bohm-like diffusion is assumed. We show that CR modified shocks evolve to time-asymptotic states by the time injected particles are accelerated to moderately relativistic energies (p/mc $\ge$ 1), and that two shocks with the same Mach number, but with different shock speeds, evolve qualitatively similarly when the results are presented in terms of a characteristic diffusion length and diffusion time. We find that $10^{-4} - 10^{-3}$ of the particles passed through the shock are accelerated to form the CR population, and the injection rate is higher for shocks with higher Mach number. The CR acceleration efficiency increases with shock Mach number, but it asymptotes to ${\~}50\%$ in high Mach number shocks, regardless of the injection rate and upstream CR pressure. On the other hand, in moderate strength shocks ($M_s {\le} 5$), the pre-existing CRs increase the overall CR energy. We conclude that the CR acceleration at cosmological shocks is efficient enough to lead to significant nonlinear modifications to the shock structures.
Objectives : We aimed to identify the antidepressant effect of liver tonification acupuncture treatment (ACU (LT); KI10, LR8, LU8, LR4) and four gate acupuncture treatment (ACU (FG); LI4, LR3) and its brain neural activity in the normal and chronic restraint stress (CRS)-induced mouse model. Methods : Firstly, normal mice were given ACU (LT) or ACU (FG) and the c-Fos expressions in each brain region were analyzed to examine brain neural activity. Secondly, CRS was administered to mice for 4 weeks, then ACU (LT) or ACU (FG) was performed for 2 weeks. The depression-like behavior was evaluated using open field test (OFT) before and after acupuncture treatment. Then, the c-Fos expressions in each brain region were analyzed to examine brain neural activity. Results : In normal mice, ACU (FG) regulated brain neural activities in the hypothalamus, hippocampus, and periaqueductal gray. ACU (LT) changed more brain regions in the prefrontal cortex, insular cortex, striatum, and hippocampus, including those altered by ACU (FG). In CRS-induced model, ACU (LT) alleviated depression-like behavior more than ACU (FG). Also, brain neural activities in the motor cortex area 2 (M2), agranular ventral part and piriform of insular cortex (AIV and Pir), and cornu ammonis (CA) 1 and CA3 of hippocampus were changed by ACU (LT), and those of AIV and CA3 were also changed by ACU (FG). As in normal mice, ACU (LT) resulted in changes in more brain regions, including those altered by ACU (FG) in CRS model. M2, Pir, and CA1 were only changed by ACU (LT) in depression model, suggesting that these brain regions reflect the specific effect of ACU (LT). Conclusions : ACU (LT) relieved depression-like behavior more than ACU (FG), and this acupuncture effect was associated with modulation of brain neural activities in the motor cortex, insular cortex, and hippocampus.
