A CAD/CAM System for Steam Paths of Turbine Generators (터빈발전기의 Steam Path 전용 CAD/CAM시스템)

  • The purpose of this research is to develop a method for cutting non-circular holes on a bent thick plate. Generally in order to cut the holes on the large plates, a special-purpose 5-axis machine is needed. However, such a machine is unavailable in most of the machine shops. This paper provides a description of such a method that utilizes a general-purpose 5-axis water-jet machine in place of the special-purpose machine: First, the bent piece is transformed into a flat plate, where the shape of the holes is reconstructed by considering deformation during bending. Then, after a 5-axis NC data is generated, the holes on the flat plate are cut using the 5-axis water-jet machine. The final step is to return to its, original shape by bending the plate with its newly-cut holes. The proposed methodology is implemented as a dedicated system by customizing a commercial CAD/CAM system. Some illustrations are provided throughout the paper in order to show the validity of the proposed methods and the developed system.

  • In this study we propose a novel approach with which design engineers can define and design free-from surface more easily. Free-from surface is defined as a parameterics shape which consists of a set of contr ol points. Parametric shape which not only has the advantage of allowing users to perform design changes efficiently, but also provides designers with a natural design environment in which they can do their work more naturally and creatively. The objective of this study is to develop a PC level freeform surface modelling system which explicitly represents information of part geometry. In this study, freeform surface modeling system consist of three modules : freeform surface modeler, input/output modules and interface with Database. In recent year there are increasing demands for the 5-axis machining of a complicated shape. This study is the development of a CAM software system (or NC module) for the 5-axis machining of a general shape with a flat-end mill, a round-end mill, a ball-end mill.

Mucosal Changes in the Small Intestines in Portal Hypertension: First Study Using the Pillcam SB3 Capsule Endoscopy System

  • Background/Aims: To evaluate patients with portal hypertension (PH) of varied etiologies for portal hypertensive enteropathy (PHE) using the PillCam SB3 capsule endoscopy (CE) system. Methods: Consecutive patients with PH presenting with unexplained anemia and/or occult gastrointestinal bleeding were evaluated using the PillCam SB3 CE system. Abnormal findings were categorized as vascular or non-vascular. The patients with ongoing bleeding caused by PHE were treated. The correlation of the CE scores of PHE with the clinical, laboratory, and endoscopic features was determined. Results: Of the 43 patients included in the study, 41 (95.3%) showed PHE findings. These included varices (67.4%), red spots (60.5%), erythema (44.2%), villous edema (46.5%), telangiectasia (16.3%), and polyps (16.3%). The CE scores varied from 0 to 8 ($mean{\pm}standard$ deviation, $4.09{\pm}1.8$). Five patients (11.6%) showed evidence of ongoing or recent bleeding due to PHE. Three of these five patients underwent endotherapy, and one patient underwent radiological coil placement. Conclusions: The PillCam SB3 CE system revealed a high prevalence of PHE in the patients with PH. Using this system, evidence of bleeding due to PHE was found in a small but definite proportion of the patients.

  • Nowadays, it is surely the quack which stands as one of the most controversial, problematic. the quack has been a consistent target of contested public protection strategies in the past few centuries in many countries. Recently, complementary and alternative medicine (CAM) is increasingly utilized and accepted by patients and providers throughout the health care system in the world, most accounts attribute this growing acceptability to the shortcomings of conventional medicine, the appeal of CAM's core beliefs, and the growing body of research indicating that CAM actually works. However, the governments of western countries have called for measures to ensure that the public are protected from incompetent and dangerous practitioners. Common to these controversies has been a suggestion to ban, exclude or limit the medical practice of those deemed to be damaging rather than improving the health of individuals as a measure of public protection. This article describes the experiences of western counties' health care system which is moving in a more pluralistic direction. By examining the ways in which regulatory efforts in the countries have come to address what is invariably described as a growing interest in CAM, this study show how the problem of CAM/quackery today is increasingly located in an ethical field of practitioner competency, qualifications, conduct, responsibility and personal professional development, regardless of the form of therapy in question. Many countries developed a series of measures and strategies to contain the acceptance of CAM groups, such as insisting on scientific evidence of safety and efficacy, resisting integration of CAM with conventional medicine and opposing government support for research and education. In a sense, those countries' movements serve to protect not only patients, but the dominant position of medicine and its allied professions, and to maintain existing jurisdictional boundaries within the healthcare system. The popular support for CAM will require that health professional stakeholders continue to address the challenges this poses, and at the same time protect their position at healthcare system. To cope with the quack, professional body, public sector and health authorities should consider the safety of consumers of healthcare and responding to the demands of the community for CAM therapies as well as the claims of the established healthcare professions. Finally, some implications for future health care were suggested.

Evaluation of internal fit of interim crown fabricated with CAD/CAM milling and 3D printing system

  • PURPOSE. This study is to evaluate the internal fit of the crown manufactured by CAD/CAM milling method and 3D printing method. MATERIALS AND METHODS. The master model was fabricated with stainless steel by using CNC machine and the work model was created from the vinyl-polysiloxane impression. After scanning the working model, the design software is used to design the crown. The saved STL file is used on the CAD/CAM milling method and two types of 3D printing method to produce 10 interim crowns per group. Internal discrepancy measurement uses the silicon replica method and the measured data are analyzed with One-way ANOVA to verify the statistic significance. RESULTS. The discrepancy means (standard deviation) of the 3 groups are $171.6\;(97.4){\mu}m$ for the crown manufactured by the milling system and 149.1 (65.9) and $91.1\;(36.4){\mu}m$, respectively, for the crowns manufactured with the two types of 3D printing system. There was a statistically significant difference and the 3D printing system group showed more outstanding value than the milling system group. CONCLUSION. The marginal and internal fit of the interim restoration has more outstanding 3D printing method than the CAD/CAM milling method. Therefore, the 3D printing method is considered as applicable for not only the interim restoration production, but also in the dental prosthesis production with a higher level of completion.

Development of Measurement System for Tappet Rotation in the Valve Train System (밸브 트레인 시스템의 태핏 회전 측정 장치의 개발)

  • The purpose of this paper is to measure the rotational speed of tappet in OHC valve train system. Tappet has eccentricity about cam center, which induces the tappet rotation and prevents from wear. In this paper, the experimental test rig which composes of one cam system is developed to measure the tappet rotation by using the laser generating system, rotary encoder, optical fiber, and photo transistor. The direction of tappet rotation is judged from the oder of optical signal. As results of experiment, average and instant rotational speed and average rotation angle per one cam revolution are presented. Measured results show that eccentricity ratio is dominant factor for the tappet rotation, and tappet is rotated at the base circle.

  • VCT(Variable Cam Timing) system is one of very useful engine components to improve fuel economy and overcome emission regulation etc. In order to adapt the VCT to a base engine, many design mod ifications in the mechanical and lubrication fields are required. Especially, because the VCT performance itself depends on supplied oil flow rate and pressure, it is very important to evaluate the engine lubrication system in a viewpoint of supplied oil flow rate and pressure. In this paper, unsteady transient flow network analysis on the engine oil circuit system was carried out to do this.

Research Trend Analysis of the Prevalence of Complementary and Alternative Medicine in Korea (국내 보완대체의학 사용 실태조사 연구의 동향 분석)

  • Objectives: This study reviewed research trends concerning the prevalence of complementary and alternative medicine (CAM) use and to suggest future research directions appropriate to medical circumstances in Korea. Methods: We searched for surveys of CAM use in 8 databases including 6 Korean databases, Ovid MEDLINE, and the CINAHL electronic database. Three independent reviewers working in pairs screened titles and abstracts of articles for eligibility. Full text was retrieved in case of disagreement on the eligibility. The main analysis targets included survey researcher's affiliation, terminology used in the title, study subject, definition of CAM, classifications of CAM modalities, and the area assortment of CAM and traditional Korean medicine (TKM). Results: 92 articles were included for analysis. The major constituent of affiliation was doctors (53%). According to years, study subjects were diversified to a large range of diseases. Since 2003, terminology is absorbed to use CAM. But actually, the most commonly used definition in the research was comprehensive such as "not generally considered part of major medicine" (55.4%) and the most used classification of CAM was self-criteria (61.9%). As for area assortment of CAM and TKM, many therapies exist in a gray zone between CAM and TKM. Conclusions: Standardized definition and classification criteria about CAM fit to the Korean healthcare system have not yet developed. For traditional Korean medicine academia, more concern should be paid to establishing appropriate development of definitions and classification criteria.

Use of Complementary and Alternative Medicine in Patients with Gynecologic Cancer: a Systematic Review

  • Purpose: Research carried out with gynecologic cancer patients using CAM was reviewed to provide a source for discussing which CAM method is used for which purpose, patients' perceptions on the effects/side effects occurred during/after using CAM and their sources of information regarding CAM. Materials and Methods: This literature review was carried out for the period between January 2000 and March 2015 using Scopus, Dynamed, Med-Line, Science Dırect, Ulakbim, Research Starters, Ebscohost, Cinahl Complete, Academic Onefile, Directory of Open Access Journals, BMJ Online Journals (2007-2009), Ovid, Oxford Journal, Proquest Hospital Collection, Springer-Kluwer Link, Taylor & Francis, Up To Date, Web Of Science (Citation Index), Wiley Cochrane-Evidence Base, Wiley Online Library, and Pub-Med search databases with "complementary and alternative medicine, gynecologic cancer" as keywords. After searching through these results, a total of 12 full length papers in English were included. Results: CAM use in gynecologic cancer patients was discussed in 8 studies and CAM use in breast and gynecologic cancer patients in 4. It was determined that the frequency of CAM use varies between 40.3% and 94.7%. As the CAM method, herbal medicines, vitamins/minerals were used most frequently in 8 of the studies. When the reasons why gynecologic cancer patients use CAM are examined, it is determined that they generally use to strengthen the immune system, reduce the side effects of cancer treatment and for physical and psychological relaxation. In this review, most of the gynecologic cancer patients perceived use of CAM as beneficial. Conclusions: In order that the patients obtain adequate reliable information about CAM and avoid practices which may harm the efficiency of medical treatment, it is recommended that "Healthcare Professionals" develop a common language.

Suitable scanning procedures for various prosthodontic treatments and the utilization of intraoral scanner (임상가를 위한 특집 3 - 여러 보철 치료 술식에 따른 바른 스캐닝 과정과 구강스캐너의 활용)

  • With the development of digital dentistry, various intra-oral scanners which acquire intraoral image without conventional impression taking and stone pouring steps have been introduced. Fixed dental prostheses such as inlay, onlay, crown, and bridge fabricated by CAD/CAM technique combined with digital impressions is getting popular due to the recent rapid progress of digital impression taking system. In comparison with traditional prosthetic procedure, the advantages of intraoral image acquiring and CAD/CAM technique are as follows; the omission of conventional impression materials, reduced workflow step, and increased efficiency by online communication with clinic and laboratory. This review article covers some opinions about the suitable scanning procedures for the various prosthodontic treatments and the utilization of digital intraoral scanner and CAD/CAM system.
