저혈당성 뇌병증에서 뇌백질 변화의 빠른 퇴행 (Rapid Regression of White Matter Changes in Hypoglycemic Encephalopathy)

  • 목적: 이전의 증례 보고에서는 저혈당성 뇌병증 환자의 뇌백질의 변화가 퇴행하기까지 수 일이 소요되었다. 본 저자들은 저혈당성 뇌병증에서 확산강조영상의 빠른 변화를 경험하였다. 이에 문헌 고찰과 함께 보고자 한다. 증례 보고: 58세 남자 환자가 반혼수 상태로 발견되었으나 혈액 검사 상 저혈당 (44 mg/dL)이 나타난 것 외 특이 소견이 없었다. 혈당 수치를 빠르게 교정한 후 곧바로 촬영한 뇌 확산강조영상에서 양쪽 피질하 백질의 병변이 관찰되었다. 5시간 후 재시행한 확산강조영상에서 피질하 백질이 회복된 대신 양쪽 전방측두두정엽 피질의 병변이 새로이 발견되었다. 결론: 저혈당성 뇌병증 환자에서 뇌백질과 피질이 모두 이상소견을 보이는 증례 보고는 이전에도 다수 있었지만 이처럼 뇌백질 변화가 빠른 퇴행을 보이는 증례는 거의 보고된 바 없다. 저자들은 저혈당성 뇌병증에서 자기공명 영상의 변화가 빠르게는 수 시간 내로도 나타날 수 있다는 걸 알리고자 한다.

  • Objectives We aimed to identify the neuroimaging marker for prediction of the use of atypical antipsychotics (AAP) in dementia patients. Methods From April 2010 to March 2013, 31 patients who were diagnosed as dementia at the psychiatric department of Soonchunhyang University Hospital, completed the brain magnetic resonance imaging scan and cognitive test for dementia. Ten patients were treated with AAP for the improvement of behavioral and psychological symptoms of dementia (BPSD) and the other 21patients were not. Using T1 weighted and Fluid Attenuated Inversion Recovery (FLAIR) images of brain, areas of white matter (WM), gray matter (GM), cerebrospinal fluid (CSF) and white matter hyperintensities (WMH) have been segmented and measured. Multivariate logistic regression models were applied for assessment of association between AAP use and the GM/WM ratio, the WMH/whole brain (GM + WM + CSF) ratio. Results There was a significant association between AAP use and the GM/WM ratio (odds ratio, OR = 1.18, 95% confidence interval, CI 1.01-1.38, p = 0.037), while there was no association between AAP use and the WMH/whole brain ratio (OR = 0.82, 95% CI 0.27-2.48, p = 0.73). Conclusions The GM/WM ratio could be a biological marker for the prediction of AAP use and BPSD in patients with dementia. It was more likely to increase as dementia progress since atrophy of WM was more prominent than that of GM over aging.

  • Metronidazole is an antimicrobial agent widely used for the treatment of anaerobic infection or antibiotics-associated diarrhea. It is generally thought to be safe, but can induce reversible toxic encephalopathy in the case of excessive or cumulative over-dose. Metronidazole-induced encephalopathy generally demonstrates the characteristic features of typical lesion location and bilaterality on magnetic resonance imaging (MRI). We report a case of metronidazole-induced encephalopathy with the involvement of asymmetric white matter. To our knowledge, only a few cases have been reported with respect to white matter lesion characteristics on MRI with diffusion-weighted images.

A case of acute necrotizing encephalopathy associated with parainfluenza virus infection

  • Acute necrotizing encephalopathy (ANE) may be suspected when a young child presents with abrupt onset of altered mental status, seizures, or both. Definitive clinical diagnosis is based on magnetic resonance imaging (MRI) results. ANE is associated with influenza virus infections. Preliminary data suggests that up to 25% of ANE patients die, and up to 25% of ANE survivors develop substantial neurologic sequelae. Here, we describe a case of a comatose 22-month-old girl who was admitted to our hospital because of febrile illness and seizures. On day 13 of her illness, she died from ANE associated with infection from parainfluenza virus. Brain MRI results indicated diffuse bilateral symmetric signal changes in both basal ganglia, thalami, periventricular white matter, pons, and cerebral white matter, as well as generalized swelling of the brain.

MRI Findings to Predict Neurodevelopmental Outcomes in Preterm Infants Near Term-Equivalent Age

  • Purpose: Preterm infants are at high risk for adverse neurodevelopmental outcomes. Magnetic resonance imaging (MRI) has been proposed as a means of predicting neurodevelopmental outcomes in this population. It is controversial whether diffuse excessive high signal intensity (DEHSI) represents damage to the white matter or delayed myelination in preterm infants. This study investigated MRI findings for predicting the severity of neurodevelopmental outcomes and assessing whether preterm infants with DEHSI near term-equivalent age have abnormal neurodevelopmental outcomes. Materials and Methods: Preterm infants (n = 64, gestational age at birth < 35 weeks) undergoing brain MRI near term-equivalent age and subsequent neurodevelopmental outcomes were evaluated between 18 and 24 months of age. The associations of MRI findings and the risk of severe cognitive delay, severe psychomotor delay, cerebral palsy (CP), and neurosensory impairment were analyzed. The associations of DEHSI with risks of severe cognitive delay, severe psychomotor delay, CP, and neurosensory impairment (hearing or visual impairment) were analyzed. Outcome data were evaluated by logistic regression and the Fisher's exact test. Results: There were significant associations between abnormal white matter findings and delayed mental development, delayed psychomotor development, neurosensory impairment, and presence of CP. The presence of DEHSI was not correlated with delayed neurodevelopmental outcomes or presence of CP. In multivariate logistic regression analyses, cystic encephalomalacia, punctate lesion, loss of white matter volume and ventricular dilation were significantly associated with CP. Conclusion: Abnormal MRI findings near term-equivalent age in preterm infants predict adverse neurodevelopmental outcomes. No significant association between DEHSI and adverse neurodevelopmental outcomes was demonstrated.

VGG-based BAPL Score Classification of 18F-Florbetaben Amyloid Brain PET

  • Amyloid brain positron emission tomography (PET) images are visually and subjectively analyzed by the physician with a lot of time and effort to determine the ${\beta}$-Amyloid ($A{\beta}$) deposition. We designed a convolutional neural network (CNN) model that predicts the $A{\beta}$-positive and $A{\beta}$-negative status. We performed 18F-florbetaben (FBB) brain PET on controls and patients (n=176) with mild cognitive impairment and Alzheimer's Disease (AD). We classified brain PET images visually as per the on the brain amyloid plaque load score. We designed the visual geometry group (VGG16) model for the visual assessment of slice-based samples. To evaluate only the gray matter and not the white matter, gray matter masking (GMM) was applied to the slice-based standard samples. All the performance metrics were higher with GMM than without GMM (accuracy 92.39 vs. 89.60, sensitivity 87.93 vs. 85.76, and specificity 98.94 vs. 95.32). For the patient-based standard, all the performance metrics were almost the same (accuracy 89.78 vs. 89.21), lower (sensitivity 93.97 vs. 99.14), and higher (specificity 81.67 vs. 70.00). The area under curve with the VGG16 model that observed the gray matter region only was slightly higher than the model that observed the whole brain for both slice-based and patient-based decision processes. Amyloid brain PET images can be appropriately analyzed using the CNN model for predicting the $A{\beta}$-positive and $A{\beta}$-negative status.

  • Abstract - In this paper, a segmentation method for brain Magnetic Resonance(MR) image using region clustering technique with statistical distribution of gradient image and fuzzy rules is described. The brain MRI consists of gray matter and white matter, cerebrospinal fluid. But due to noise, overlap, vagueness, and various parameters, segmentation of MR image is a very difficult task. We use gradient information rather than intensity directly from the MR images and find appropriate thresholds for region classification using gradient approximation, rayleigh distribution function, region clustering, and merging techniques. And then, we propose the adaptive fuzzy rules in order to extract anatomical structures and diseases from brain MR image data. The experimental results shows that the proposed segmentation algorithm given better performance than traditional segmentation techniques.

Chronic cerebral hypoperfusion and plasticity of the posterior cerebral artery following permanent bilateral common carotid artery occlusion

  • Vascular dementia (VaD) is a group of heterogeneous diseases with the common feature of cerebral hypoperfusion. To identify key factors contributing to VaD pathophysiology, we performed a detailed comparison of Wistar and Sprague-Dawley (SD) rats subjected to permanent bilateral common carotid artery occlusion (BCCAo). Eight-week old male Wistar and SD rats underwent BCCAo, followed by a reference memory test using a five-radial arm maze with tactile cues. Continuous monitoring of cerebral blood flow (CBF) was performed with a laser Doppler perfusion imaging (LDPI) system. A separate cohort of animals was sacrificed for evaluation of the brain vasculature and white matter damage after BCCAo. We found reference memory impairment in Wistar rats, but not in SD rats. Moreover, our LDPI system revealed that Wistar rats had significant hypoperfusion in the brain region supplied by the posterior cerebral artery (PCA). Furthermore, Wistar rats showed more profound CBF reduction in the forebrain region than did SD rats. Post-mortem analysis of brain vasculature demonstrated greater PCA plasticity at all time points after BCCAo in Wistar rats. Finally, we confirmed white matter rarefaction that was only observed in Wistar rats. Our studies show a comprehensive and dynamic CBF status after BCCAo in Wistar rats in addition to severe PCA dolichoectasia, which correlated well with white matter lesion and memory decline.

건강한 성인에서 대뇌 백질 변성과 호모시스테인 농도의 연관성 (Relationship between White Matter Changes and Homocysteine Concentration in Healthy Adults)

  • 본 연구는 뇌졸중, 치매 병력이 없는 50-75세 건강한 성인을 대상으로 뇌 CT를 통해 대뇌 백질 변성과 뇌혈관 위험인자들이 상관성이 있는지 분석하고자 하였다. 2016년과 2017년에 일개 병원에서 건강 검진을 받은 900명 중 722명을 대상으로 연구를 진행하였으며 의무기록을 토대로 후향적으로 조사하였고, SPPS를 이용하여 카이제곱검정, 독립표본 t검정, 단변량 로지스틱 회귀분석을 이용하여 분석하였다. 대뇌 백질 변성이 있는 집단과 없는 집단으로 나누어 연구 대상자 특성을 확인하였는데, 대뇌 백질 변성이 있는 군이 없는 군보다 평균 연령과 고혈압, 당뇨 유병률이 높았고, 교육 기간은 짧았으며, 호모시스테인 농도가 높았다. 대뇌 백질 변성이 있는 군에서는 호모시스테인 농도가 증가함에 따라 백질 변성군 내에서 차지하는 비율도 증가했다. 호모시스테인 농도를 4분위하여 가장 낮은 집단(Q1)을 기준으로 상대적 위험도를 비교하였을 때, 나이[p<0.001], 고혈압[p<0.001], 고호모시스테인혈증(Q4)[p=0.021]이 대뇌 백질 변성의 위험인자임을 확인하였다. 이는 건강한 사람에게서 고혈압, 고호모시스테인혈증과 같은 대뇌 백질 변성의 조절 가능한 위험 인자를 확인했다는데 임상적으로 의의가 있다. 하지만 지금까지 고호모시스테인혈증의 일으키는 원인별 위험성에 대한 보고가 없고, 고호모시스테인혈증과 대뇌 백질 변성에 대해 한국인을 대상으로 한 연구가 알려지지 않아 대규모 전향적 연구가 필요할 것으로 사료된다.

인삼사포닌 분획이 에탄올을 투여한 쥐의 뇌에서 분리한 신경세포와 Astrocyte의 Aldehyde Dehydrogenase 활성에 미치는 영향 (The Effect oi Saponin Fraction of Panax Ginsen C.A. Meyer on Aldehyde Dehydrogenase Activity in Neurons and Astrocytes Isolated from Ethanol Administered Rat Brain)

  • The changes in aldehyde dehydrogenase(ALDH, E.C. 1.2.1.3.) activity in neurons and astrocytes isolated from rat brains were investigated after administration of ethanol and Korean red ginseng(Panax ginseng C.A. Meyer) saponln. The cerebral ALDH activity with acetaldehyde and Propionaldehyde was higher in the white matter than in the gray matter. However, using indole-3-a-cetaldehyde and 3,4-dihydroxyphenylacetaldehyde as substrates, there was no significant difference in activity between two regions in cerebrum. In ethanol treated group, ALDH activity with all the substrates in the gray and white matter was lower than in normal group. In ethanol-saponin treated group, the enzyme activity in the white matter remarkably Increased. The ALDH activity in neurons isolated from cerebral cortex in ethanol-treated group was lower than in normal group. In ethanol-saponin treated group, neuronal ALDH activity with propionaldehyde was significantly recovered but not with Indole-3-acetaldehyde. In astrocytes, although the ALDH activity with propionaldehyde in the ethanol-treated group was not changed as compared with normal group, considerable increase in activity was found in ethanol-saponin treated group. These results suggest that Korean red ginseng saponin may protect the neuronal functions from the toxic effects of acetaldehyde derived from ethanol by stimulation of ALDH activity in astrocytes surrounding nerve cells.
