보건계열과 비보건계열 학생의 신체이형장애와 자가우울에 대한 비교분석 (Comparative Analysis of Body Dysmorphic Disorder and Depressive Disorder between Health and Non-Health related Majors)

  • 신체이형장애는 정상적인 용모를 가진 사람이 용모에 대해 상상으로 변형이나 결손 등의 문제가 있다는 생각 또는 사소한 결손을 과장되게 변형된 것으로 보는 생각 등에 집착해 있는 상태이다. 본 연구에서는 대학생들을 보건계열 학생과 비보건계열 학생을 대상으로 신체 이형장애와 우울증에 대한 조사를 통해 이 두 군을 비교하고 우울과의 관련성에 대해서도 알아 보고자 하였다. 본연구의 대상자는 강원도에 소재한 K대학교 400명을 대상으로 하였다. 그 중 200명 비보건계열, 200명은 보건계열을 대상으로 자기기입식 설문을 총 319부를 자료분석에 사용하였다. 그 결과보건계열 학생과 비보건계열 학생들은 신체변형장애의 차이가 없음을 알 수 있다. 또한 신체변형장애 성향에 다양한 요인들이 있겠지만 특히 보건계열 비보건계열 모두 신체 변형 장애가 심할수록 자가우울이 높으며 통계적으로 유의미한 영향을 미쳤다. 따라서 신체변형 장애에 대한 관리가 필요하며 특히 보건계열학생들에 대한 연구가 이루어질 필요가 있으며 신체 변형 장애와 자가 우울에 대한 임상적으로 구체적인 연구가 더 이루어 져야 한다고 생각된다.

한국판 신체 이형장애 자가 평가척도의 표준화 연구 : 대학생들을 대상으로 (Korean Version of Body Image Dysmorphic Disorder Examination Self Report(BDDE-SR) with College Students)

  • Objective : Recently, there is an increasing trend of attachment to physical appearance of individuals in many persons with spread of social concern through mass communication. Domestically, several studies has been performed, but they were limited to obesity mainly for juveniles or female college students. Therefore, purpose of this study were to standardize Body image dysmorphic disorder examination self report with male and female college students in Cheonnan area. Methods : This study were total 825 college students in Cheonan area. Subject group are consist of 393 males (47.6%) and 427 females (51.8%), and the mean age was $22.28{\pm}3.24$ years old. Results : The BDDE-SR had adequate internal consistency (Cronbach's alpha=.93) and test-retest (r=.82) and interator reliability. Four factors were extracted by factor analysis with the Varimax rotation. They were 'the camouflage and comparison factor', 'the self-consciousness and negative self-evaluation factor', 'the avoidance factor', and 'the distressing and embarassment factor'. The total score of BDDE-SR was $50.8{\pm}27.1$, female's score ($59.2{\pm}25.5$) was statistically higher than male's score ($41.85{\pm}25.84$). Conclusion : It is considered that BDDE-SR is reliable instrument for body image dissatisfaction, provided correct information in predicting clinical status of Body dysmorphic disorder that is most largely influenced by individual psychologic factor and family system.

Associations between body dysmorphic disorder (BDD) with the dental health component of the index of orthodontic treatment need (IOTN-DHC) and other BDD risk factors in orthodontic patients: A preliminary study

  • Objective: Body dysmorphic disorder (BDD) is a form of obsessive-compulsive disorder that may be negatively associated with the self-image. It might be associated with orthodontic treatment demand and outcome, and therefore is important. Thus, this study was conducted. Methods: The Yale-Brown Obsessive-Compulsive Scale modified for Body Dysmorphic Disorder (BDD-YBOCS) questionnaire was used in 699 orthodontic patients above 12 years of age (222 males, 477 females), at seven clinics in two cities (2020-2021). BDD diagnosis and severity were calculated based on the first 3 items and all 12 items of the questionnaire. The dental health component of the index of orthodontic treatment need (IOTN-DHC) was assessed by orthodontists. Multivariable and bivariable statistical analyses were performed on ordinal and dichotomized BDD diagnoses to assess potentially associated factors (IOTN-DHC, age, sex, marital status, education level, and previous orthodontic consultation) (α = 0.05). Results: IOTN-DHC scores 1-5 were seen in 13.0%, 39.9%, 29.8%, 12.4%, and 4.9% of patients. Age/sex/marital status/education were not associated with IOTN-DHC (p > 0.05). Based on 3-item questionnaire, 17.02% of patients had BDD (14.02% mild). Based on 12-item questionnaire, 2.86% had BDD. BDD was more prevalent or severer in females, married patients, patients with a previous history of orthodontic consultation, and patients with milder IOTN-DHCs (p < 0.05). Conclusions: IOTNDHC was negatively/slightly associated with BDD in orthodontic patients. Being female and married may increase BDD risk.

The Reliability and Validity of the Korean version of the Body Shape Questionnaire

  • Objective : There is an increasing number of people are not satisfied with their appearance and shape. Therefore, assessment tools are needed to evaluate the appearance or body shape. This study investigated the reliability and validity of a Korean version of the Body Shape Questionnaire (BSQ). Methods : Participants completed the BSQ, Body Dysmorphic Disorder Examination-Self Report, Eating Disorder Inventory-2, Beck Depression Inventory, Self-Esteem Scale, State-Trait Anger Expression Inventory, and State-Trait Anxiety Inventory. Results : The Korean version of BSQ presented satisfactory test-retest reliability, internal consistency and validity. Exploratory factor analysis yielded four factors as follow: feeling fat, shame and inferiority about one's body shape, attitudes concerning body image perception, and purging behavior. Conclusion : These results show the Korean BSQ exhibits good psychometric properties and can accurately evaluate the body shape concerns among Korean adults.

강원지역 대학생들의 BMI에 따른 신체이형장애 조사 (Relationship of BMI to Body Dysmorphic Disorder among College Students in Gangwon Province)

  • 우리나라는 날씬하고 마른 체형을 선호하며 정상이나 저체중에도 체중조절이 일반화 되어 일상에 만연된 외모중시 풍조가 젊은 여성들 사이에 유행처럼 퍼져나가는 사회적분위기로 번지면서 지나친 다이어트로 인하여 건강에 문제를 초래하고 있다. 이 연구는 보건계열과 비보건계열의 대학생들을 대상으로 체질량지수(BMI)에 따라 3군으로 분류하고 각 군들의 신체 이형장애와의 관련성 그리고 우울과의 영향을 미치는 여러 요인들이 무엇인지 알아보고자 하였다. 본 연구는 2012년 9월10일부터 21일 까지 강원 소재한 K대학교 보건계열 200명 비보건계열 200명을 대상으로 자기기입식 설문을 하여 총 319부를 자료분석에 사용하였다. 본 연구의 수집된 자료는 SPSS(Statistical Package for the Social Science) WIN 18.0 프로그램을 이용하여 분석하였다. 여학생들의 BMI에 따라 신체 불만 부위는 하체 전부에 대해서는 과체중인 여학생일수록 불만족하였으며(${\chi}^2$=6.97, p<.05), 허리-배에 대해서는 정상체중인 여학생이 가장 불만족하였고, 저체중인 여학생은 다른 여학생보다 덜 불만족하였으며, 통계적으로도 유의미한 차이를 보였다(${\chi}^2$=8.12, p<.05). 가슴(${\chi}^2$=8.45, p<.05)과 유방(${\chi}^2$=6.44, p<.05)에 대해서는 저체중인 여학생일수록 불만족하였으며, BMI에 따라 유의미한 차이를 보였다. 여학생들의 BMI에 따라 신체 불만 부위에 대해 살펴본 결과, 과체중인 여학생일수록 하체 전부와 몸 전체에 대해 불만족하였으며, 정상체중인 여학생은 다른 여학생보다 허리-배에 대해 불만족하였고, 저체중인 여학생일수록 가슴과 유방에 대해 불만족하였다. 그러나, BMI에 따라 신체 불만 부위 변화를 위해 시도해보고 싶어 하는 방법의 차이가 없음을 알 수 있었다. 이런 신체이형장애는 우울증과 연관되기 쉽고 사회 문화적인 문제를 야기시킬 수 있기 때문에 상담 프로그램의 개발과 추가 연구를 진행하는 것이 필요하다고 생각된다.

젊은 성인에서 대인관계문제 및 신체불만족이 자살사고에 미치는 영향 (Impact of Interpersonal Problems and Body Dissatisfaction on Suicidal Idea of Young Adults)

  • Objective : The aim of this study was to investigate the relationship between interpersonal problems, body dissatisfaction, and suicidal idea. Methods : Two hundred ninety-four participants filled out the self-reporting questionnaires : Body dissatisfaction was measured with Body Dysmorphic Disorder Examination-Self Report (BDDE-SR) ; Interpersonal problems was measured with Korean Inventory of Interpersonal Problem (KIIP) ; Suicidal Ideation Questionnaire (SIQ) was used to assess suicidal idea. A path analysis was conducted to identify the impact of interpersonal problems and body dissatisfaction on the suicidal ideas. Results : 'Socially inhibited (KIIP_C4)', 'overly-accommodating (KIIP_C6)', age, and gender had a direct effect on their body dissatisfaction, and those factors had a indirect effect on the suicidal ideas. Also, 'cold (KIIP_C3)' and body dissatisfaction had a direct effect on the suicidal ideas. Conclusions : These findings can help to identify how interpersonal problems and body dissatisfaction influence the suicidal ideas of people. It would be useful to consider and assess suicidal ideas in the patients with 'cold (KIIP_C3)', 'socially inhibited (KIIP_C4)', 'overly-accommodating (KIIP_C6)'.

인천지역 대학생의 식이태도 및 체형만족도와 비만도와의 비교 연구 (A Comparative Study of Body Image, Eating Attitudes and Obesity Degree in Incheon University Students)

  • Objective : This study was aimed to find out correlation body image satisfaction, eating attitudes and body composition. Method : 78 subjects who are university student were analyzed obesity degree by using Inbody 720, and were surveyed by using Korean eating attitude test-26, Body image satisfaction questionnaire. Results : Many people who was normal BMI and normal waist-hip ratio was dissatisfied with their waist circumference(66.7%). There were high positive correlations between BMI and perceived body image satisfaction(r=-.515) and between perceived BMI and eating attitude (r=.390) in women Conclusion : We have to examine the self-body image of patients and set a objective goal before proceeding obesity treat program and correct the attitude to beauty and obesity in order to set both physical and psychological treatment.

Development and Validation of the Expectations of Aesthetic Rhinoplasty Scale

  • Background There is a growing concern in the field of aesthetic surgery about the need to measure patients' expectations preoperatively. The present study was designed to develop and validate the Expectations of Aesthetic Rhinoplasty Scale (EARS), and to compare expectations between rhinoplasty patients with and without body dysmorphic disorder (BDD). Methods In total, 162 college students and 20 rhinoplasty candidates were recruited. The measures included the newly developed EARS, a measure of psychopathology, and demographics. The DSM-IV structured clinical interview for BDD was used to confirm the diagnosis in rhinoplasty patients. Results The EARS was constructed of six items based on their significant content validity. In the scale development phase, Cronbach's alpha was 0.87. The test-retest reliability coefficient of the scale was satisfactory (intraclass correlation coefficient, 0.94; 95% confidence interval, 0.82-0.98) over a four-week period. Scores on the EARS were significantly positively correlated with psychopathological symptoms (r=0.16; P<0.05). Moreover, comparison of EARS scores between BDD (M=25.90, standard deviation [SD]=6.91) and non-BDD rhinoplastic patients (M=15.70, SD=5.27) suggested that BDD patients held significantly higher expectations (P<0.01). Conclusions The expectations of aesthetic rhinoplasty patients toward surgery may play a crucial role in their postoperative satisfaction. While the value of patients' expectations is clinically recognized, no empirical study has measured these expectations in a psychometrically sound manner. The current study developed and validated the EARS. It may be easily used as a valid and reliable instrument in clinical and research settings.

The Psychosomatic Disorders Pertaining to Dental Practice with Revised Working Type Classification

  • Psychosomatic disorders are defined as disorders characterized by physiological changes that originate partially from emotional factors. This article aims to discuss the psychosomatic disorders of the oral cavity with a revised working type classification. The author has added one more subset to the existing classification, i.e., disorders caused by altered perception of dentofacial form and function, which include body dysmorphic disorder. The author has also inserted delusional halitosis under the miscellaneous disorders classification of psychosomatic disorders and revised the already existing classification proposed for the psychosomatic disorders pertaining to dental practice. After the inclusion of the subset (disorders caused by altered perception of dentofacial form and function), the terminology "psychosomatic disorders of the oral cavity" is modified to "psychosomatic disorders pertaining to dental practice".

젊은 성인에서 신체 불만족과 성격 특성 및 스트레스와의 관계 (The Relationship of Body Dissatisfaction, Personality Characteristic, and Stress in Young Adults)

  • Objective : The aims of this study was to evaluate relationship between body dissatisfaction, personality characteristic, and stress in young adults. Methods : One hundred and ninety five young, healthy, and normal participants filled out all 3 types of self-reported questionnaire, the Body Dysmorphic Disorder Examination-Self Report (Korean version of BDDE-SR ; K-BDDE-SR), Neuroticism, Extraversion, Openness-Five Factor Inventory (NEO-FFI) and Stress Scale of Korean version of the Depression Anxiety Stress Scale (K-DASS-21). Results : Out of the 195 participants, 30% (n=59) was male and 70% (n=136) was female. There was no statistical difference between high K-BDDE-SR group (K-BDDE-SR score 75 percentile or higher, n=50) and low K-BDDE-SR group (K-BDDE-SR score below than 75 percentile, n=145) in terms of weight, height, and BMI. In high K-BDDE-SR group, neuroticism was significantly higher than low K-BDDE-SR group, and extraversion, agreeableness, conscientiousness were significantly lower than low K-BDDE-SR group. Also, level of stress in high K-BDDE-SR group was significantly higher than low K-BDDE-SR group. Conclusion : Our study revealed that individual's with more body dissatisfaction were more neurotic and stressful. Thus, body dissatisfaction and diverse psychiatric diseases should be considered in treating neurotic and stressful young adults.
