Effects of Foot Reflexology on Essential Hypertension Patients (본태성 고혈압 환자에게 미치는 발반사마사지 효과)

  • Purpose: This study was to evaluate the effects of foot reflexology on blood pressure, serum lipids level and life satisfaction in essential hypertension patients. Method: The research design used was a nonequivalent control group pretest-posttest design. Foot Reflexology was used as the experimental treatment from June 23rd, 2003 until August 31st, 2003. Thirty-four subjects were assigned to an experimental group(18) and control group(16). Foot Reflexology was administered twice a week for 6 weeks and self foot Reflexology was administered twice a week for 4 weeks on the experimental group. Result: There was a significant decrease in systolic blood pressure but no significant decrease indiastolic pressure in the experimental group compared to the control group. The total cholesterol level in the experimental group compared to the control group was not significantly decreased after foot reflexology. However, the triglyceride level in theexperimental group compared to the control group was significantly decreased after foot reflexology. On the other hand, high density lipoprotein and low density lipoprotein levels in the experimental group compared to the control group was not significantly decreased after foot reflexology. Life satisfaction in the experimental group compared to the control group was significantly improved after foot reflexology. Conclusion: The results proved that foot reflexology was an effective nursing intervention to decrease systolic pressure, and triglyceride but not for the blood cholesterol and to improve life satisfaction. Therefore, blood cholesterol should be further evaluated in a larger group of subjects and for a longer period. Further research is regarded as necessary to evaluate and to compareeffects of self-foot reflexology and foot reflexology.

Physician Factors Associated with the Blood Pressure Control among Hypertensive Patients (진료를 받는 고혈압 환자의 혈압 조절과 관련된 의사 요인)

  • Objectives : Little is known about the physician-related factors that are associated with the management of Hypertension. The purpose of this study was to determine the physician-related factors associated with blood pressure control in hypertensive patients. Methods : We surveyed 154 physicians at 117 public health (subhealth) centers in Gyeonggi-do, Forty-one physicians completed the survey (response rates : 26.6%) and 31 physicians were finally included as the study subjects. Using the information obtained from the self-reported survey, we measured the physician-related factors associated with hypertension control, including their perception of hypertension, prescription patterns (combination prescription rates, specific antihypertensives prescription rates among patients with diabetes mellitus), and sociodemographic factors. We then collected data on blood pressure and medication use in patients seen by these physicians from the health center#s information system. We compared the physicians# perceived hypertension control rates with the actual rates, and then evaluated the rate of high overestimation (overestimation by more than 25% of the median degree of hypertension control rate overestimation) among the physicians. The physicians# antihypertensive prescription patterns were also evaluated. Multiple logistic regression analysis was used to evaluate the independent association between hypertension control and physician-related factors. Results : The physicians tended to overestimate the proportion of their patients with controlled blood pressure (79.5% perceived vs. 57.8% actual). The percentage of physicians with high overestimation was 35.5% (11 physicians). The physicians with lower control rates were more likely to highly overestimate their patients# control rates. Physicians with below-median actual control rates tended to prescribe fewer combination treatments for patients with uncontrolled blood pressure and angiotensin-converting enzyme inhibitors or fewer angiotensin receptor blockers for patients with diabetes mellitus. The rate of high overestimation by physicians was 1.31 times higher in patients with uncontrolled blood pressure than in patients with other conditions (OR=1.31, 95% CI : 1.17-1.48). Conclusions : Physicians have a tendency to overestimate the rates of hypertension control in their patients. Because physicians have a direct role in treatment outcomes, physicians# overestimation about hypertension management contributes to inadequate blood pressure control. Thus, interventions for improving physician# awareness regarding the management of patients with hypertension are needed.

Effect of Chin Gan Sik Pung Tang on Blood Pressure and induced Hypercholesteremic Rabbit (진간식풍탕(鎭肝熄風湯)이 가토(家兎)의 혈압(血壓) 및 혈청(血淸) Total Cholesterol에 미치는 영향(影響))

  • In order to investigate the therapeutic effects on blood pressure and hyper cholesteremia, aqueous extract of Chin Gan Sik Pung Tang were studied. The result of the total cholesterol contents in serum and blood pressure of each group were as follows, 1. The aqueous extract of Chin Gan Sik Pung Tang inhibited increased Total cholesterol in serum of rabbits administrated with cholesterol rich diet. 2. Blood pressure manifested gradual response by the fall of 4, 3, 9.2, 19.9 percent in proportion to the administration of 10, 30, 100 mg/kg of Chin Gan Sik pung Tang, respectively 3. Administration of Chin Gan Sik Pung Tang to the rabbit pretreated with Vagotomy, Atropine and Regitine did not show any significant difference in the blood pressure, compare with that of the control group. 4. Administration of Chin Gan Sik Pung Tang to the rabbit pretreated with propranolol show significant difference in the blood pressure, compare with that of the control group. From the above results, it is suggested that Chin Gam Sik Pung Tang has the action on adrenergic ${\beta}-receptor$ and can he used therapeutic effect on the hypertension, and inhibit the increase of Total Cholesterol contents in serum.

Effects of Casein Hydrolysate on the Systolic Blood Pressure and Serum Lipid Profiles in Spontaneously Hypertensive Rats

  • The effects of casein hydrolysate on blood pressure (BP) and serum lipid profiles were investigated in spontaneously hypertensive rats (SHR). Twenty-four 6-wk old male SHRs were assigned to 3 groups in a completely randomized design. Experimental groups were as follow: control, market milk group (MLG) and casein hydrolysate group (CHG). Reference blood pressure (RBP) showed average $198.94{\pm}1.46mmHg$. Blood pressure (BP) of CHG reduced 24 mmHg after 2 wk of treatment, but these increased after 3 wk. BP of CHG was significantly lower than BP of Control in experimental time (p<0.05). Serum lipid profiles were not differ significantly among groups (p<0.05). These data suggest that casein hydrolysate may beneficially improvement of blood pressure level in SHR.

Effects of Aqua-Acupuncture of Jibaikjihwangtang on the Blood Pressure in Hypertensive Rats (지백지황탕(知柏地黃湯) 약침(藥鍼)이 혈압(血壓)에 미치는 영향(影響))

  • Objective : This study was performed to investigate the effects of aqua-acupuncture of Jibaikjihwangtang in two-kidney one clip Goldblatt hypertensive rats and spontaneously hypertensive rats. Methods : we injected aqua-acupuncture solution into Shin-Soo ($BL_{23}$) which corresponds to human acupuncture point in two-kidney one clip Goldblatt hypertensive rats and spontaneously hypertensive rats. Systolic blood pressure, renin activity, aldosterone and atrial natriuretic peptide (ANP) plasma levels were tested. Results : Systolic blood pressure decreased significantly after aqua-acupuncture of jibaikjihwangtang. Acupuncture group in two-kidney one clip Goldblatt hyper-tensive rats had deference with control group. In plasma levels of atrial natriuretic peptide, acupuncture group of spontaneously hypertensive rats increased meaningfully but to two-kidney one clip Goldblatt hypertensive rats it was decreased meaningfully. In Serum Aldosterone density, the acupuncture group of spontaneously hypertensive rats had significant alteration than control group, but the acupuncture group of two-kidney one clip Goldblatt hypertensive rats had decreased alteration than control group. Conclusion : According to these results, after Aqua-Acupuncture of Jibaikjihwangtang blood pressure decreased significantly and data suggest that blood pressure reduction activity connected with renin activity reduction in renal hypertensive rat.

The digital wrist band tye blood pressure device for home use are popular already in the market. It is useful for checking blood pressure level at home and control of hypertension. Especially. It is very essential home device to check the health condition of blood circulation disease. Nowadays many product types are available. But the measurement accuracy of blood pressure is not enough compared to the mechanical type. It needs to be upgraded to assure the precise health data enough to use in the hospital. The structure, feature and output signal of capacitor type pressure sensors are analyzed. An improved design fa capacitor sensor is suggested. It shows more precise health data after use on a wrist band type health unit. They can be applied for remote u-health medical service.

Effects of Capsici fructus on Blood Figures and Blood Pressure in Spontaneously Hypertensive Rats (자발성(自發性) 고혈압백서(高血壓白鼠)에서 고추(Capsici fructus)의 장기투여(長期投與)가 혈액상(血液像) 및 혈압(血壓)에 미치는 영향(影響))

  • The present study was undertaken to investigate effect of feeding hot pepper on the blood pressure and blood figures in SHR. Also studied were effect of hot pepper on plasma electrolyte concentration as well as blood sugar level. Twentytwo SHR at age of 15 weeks were divided into control and CF administered group. And in experimental group hot pepper (1 gm/kg/day) was administered into stomach of each animal by gastric intubation for 8 weeks. The results obtained are as follows: 1) On a day before starting experiment blood pressure(systolic) of the control and CF group were respectively $196.7{\pm}4.7\;mmHg$ and $197.4{\pm}3.9\;mmHg$ while it changed to $207.4{\pm}4.8\;mmHg$ and $227.5{\pm}4.2\;mmHg$ respectively on the last day of experiment. Comparing blood pressures of two groups a significant, though not remarkable, difference was observed. 2) No significant differences in blood figures(RBC, WBC, Hct, and Hb level), concentration of plasma electrolytes($Na^+$ and $K^+$)and blood sugar level were found between control and experimental group.

The Association between Blood Pressure and Obstructive Sleep Apnea-Hypopnea Syndrome

  • Obstructive sleep apnea-hypopnea (OSAH) is known to be related to nocturnal blood pressure (BP) and hypertension. The aim of this study was to evaluate the prevalence of hypertension according to the apnea-hypopnea grading. A total of 2,210 adults with snoring and obstructive sleep apnea were referred to our sleep center from July 2009 to May 2013. Clinical blood pressure (BP) was measured before sleeping (bedtime BP) and immediately after waking up in the next morning (morning BP). Subjects were classified into four groups based on the apnea-hypopnea index (AHI) from PSG as follows: control group (n=470) simple snoring and with AHI<5; mild group (n=577) with $AHI{\geq}5$ and <15; moderate group (n=508) $AHI{\geq}15$ and <30; and severe group (n=655) with $AHI{\geq}30$. The differences and correlations between BP and PSG parameters according to the AHI groups were analyzed. Patient's were classified as nomentensive (blood pressure <120/90 mmHg, n=700), prehypertensive (blood pressure < $140-120{\leq}mmHg$, n=1297) hypertensive (blood pressure ${\geq}140/90mmHg$, n=214) according to the office blood pressure measurements. The comparison of sleep parameters showed that OSA groups had a significantly higher stage N1 (control group vs. moderate OSA, severe OSA; $66.4{\pm}30.7$ vs. $85.5{\pm}36.6$, $128.4{\pm}57.3$, p<0.001) and total arousal number (control vs. moderate OSA, severe OSA; $110.7{\pm}47.7$ vs. $150.8{\pm}56.6$, $236.6{\pm}95.8$, p<0.001) compared to control group. The comparison of sleep parameters showed that OSA groups had a significantly lower stage N2 (control group vs. moderate OSA, severe OSA; $172.6{\pm}47.2$ vs. $150.7{\pm}50.5$, $120.3{\pm}57.4$, p<0.001), stage N3 (control group vs. moderate OSA, severe OSA; $38.4{\pm}33.4$ vs. $27.4{\pm}26.0$, $56.1{\pm}27.5$, p<0.001), REM (control group vs. moderate OSA, severe OSA; $64.3{\pm}25.5$ vs. $56.1{\pm}27.5$, $47.3{\pm}25.9$, p<0.001) and mean SaO2% (control group vs. moderate OSA, severe OSA; $90.0{\pm}3.5$ vs. $82.5{\pm}5.5$, $70.0{\pm}8.8$, p<0.001) compared to control group. The Apnea-hypopnea index was significantly higher in OSA groups, increased systolic and diastolic blood pressure than in the nomentensive group (bed time systolic pressure vs. AHI; <120 vs. 120-139, 140-159, >159; $17.5{\pm}18.6$ vs. $24.9{\pm}21.0$, $31.0{\pm}25.7$, $42.3{\pm}31.7$, p<0.001), (bed time diastolic pressure vs. AHI; 60-79 vs. 80-89, 90-99, >99; $19.3{\pm}19.7$ vs. $22.4{\pm}20.3$, $29.8{\pm}23.3$, $38.8{\pm}28.5$, p<0.001). AHI was positively correlated with morning systolic pressure, diastolic pressure, bed time systolic pressure and diastolic pressure (r=0.314, 0.279, 0.233 and 0.200, respectively, p<0.001). We conclude that BMI, Age, neck circumference and AHI increase with the blood pressure.

Effects of 6-week Foot Reflexology on the Blood Pressure and Fatigue in Elderly Patients with Hypertension (6주간의 발반사마사지가 노인 고혈압환자의 혈압과 피로에 미치는 효과)

The thirty-four participants were assigned to either an experimental group (18) or a control group (16), Foot reflexology was administered twice a week for 6 weeks to participants in the experimental group. Results: There was a significant decrease in systolic blood pressure and diastolic pressure in the experimental group compared to the control group. After the foot reflexology, total cholesterol and triglyceride levels for the experimental group did not decrease significantly compared to the control group. High density lipoprotein find low density lipoprotein levels also did not decrease significantly after foot reflexology Fatigue in the experimental group decreased significantly after foot reflexology. Self-efficacy in the experimental group did not decrease significantly after foot reflexology. Conclusion: The results show that foot reflexology is an effective nursing: intervention to decrease systolic blood pressure, diastolic blood pressure and to treat fatigue but not serum lipids. Therefore, blood cholesterol should be further evaluated with a larger group of participants and for a longer period. Further research is necessary to evaluate and to compare effects of self-foot reflexology and foot reflexology.

Effects of Bamboo Stepping Exercise on Blood Pressure in the Elderly (대나무 밟기 운동이 노인의 혈압에 미치는 영향)

  • Purpose : This study investigated the effect of bamboo stepping exercises on the blood pressure of the elderly. This study was performed for 8 weeks. Methods : The study participants included 25 people aged 65 and over. Of the participants, 16 were in the hypertensive elderly group and 9 were in the normal blood pressure elderly group. The participants stepped on semicircular bamboo for 20 minutes 3 times a week for 8 weeks. The participants' blood pressure was measured 3 times before and after 8 weeks of exercise. The mean values of the 3 before and after measurements were compared and analyzed statistically. Results : The systolic blood pressure of the hypertensive elderly group decreased significantly from $134.6{\pm}21.1mmHg$ to $119.9{\pm}18.1mmHg$ (p<.05). The diastolic blood pressure of the hypertensive elderly group decreased from $70.2{\pm}10.5mmHg$ to $66.1{\pm}9.8mmHg$, but the difference was not significant. The systolic blood pressure of the normal blood pressure elderly group decreased significantly from $127.2{\pm}18.7mmHg$ to $115.5{\pm}19mmHg$ (p<.05). The diastolic blood pressure of the normal blood pressure elderly group decreased from $72.6{\pm}11.3mmHg$ to $68.2{\pm}12mmHg$, but the decrease was not significant. Conclusion : After completing 8 weeks of the bamboo stepping exercises, the elderly participants' systolic blood pressure was effectively reduced. Today, lifestyle modifications, such as daily exercise, are necessary to control the blood pressure of the elderly. We hope that the bamboo stepping exercises, which are easy to perform and are not restricted by place and time, will become part of the elderly health policy.
