A Study on Life Style and Dietary Factors in Workers with High-risk of Vascular Disease (혈관계 질환 위험군으로 진단된 근로자들의 생활습관 및 식생활 요인과의 관련성 연구)

  • This study was conducted to investigate the life style and dietary factors in high-risk of vascular diseases among workers residing in Kyungbook province. The survey was carried out on March and April, 1997 The subjects of this study consisted of fifty men aged over 30 years who were diagnosed as hypercholesterolemia(>240mg/$m\ell$) or hypertension(>160/95mmHg). In contrast, fifty healthy men were participated in this study as a control group. General environmental factors, nutrition knowledge and food behavior were investigated by questionaires through direct interviews with the subjects. Alcohol intake and smoking level of the subjects tended to be higher than those of the control, but not significantly different. Nutrition knowledge and food behavior scores of high-risk group were significantly lower than those of the control group. The smoking amount of subjects was negatively correlated with blood glucose and BMI. Alcohol consumption of subjects appeared to have a positive correlation with systolic blood pressure and BMI. The relationship of food behavior score with BMI and blood glucose were negatively correlated. Nutrition knowledge score showed a negative correlation with total cholesterol, blood glucose level, BMI and systolic blood pressure. These results suggest that efficient nutrition education programs need to be developed to prevent the cardiovascular diseases of workers.

The Effect of Regular Exercise on the Level of Blood Lactate and LDH Production in College Women (운동습관이 혈액중 젖산농도 및 LDH 생성에 미치는 영향)

  • The purpose of this study was to compare the level of blood glucose and lactate and also plasma LDH production of college women in relation to exercise. College female student which majored athletics (E, exercise group, n=43) were recruited and compared with college women (Control group, n=60). Anthropometric measurements, triceps skinfold thickness were measured of two groups. And body fat mass and waist and hip circumferences were measured and the concentrations of plasma glucose, lactic acid and lactic acid dehydrogenase (LDH) were also assayed. Average height and weight of E group were slightly higher than that of control group. but there is no difference in body mass index (BMI) and waist hip ratio (WHR) between two groups. The percentage of body fat and body fat mass(kg) in E group was slightly lower than that of control group. Plasma glucose and LDH levels of E group were higher than those of control group, and plasma lactic acid concentration was significantly increased.

The activation of α2-adrenergic receptor in the spinal cord lowers sepsis-induced mortality

  • The effect of clonidine administered intrathecally (i.t.) on the mortality and the blood glucose level induced by sepsis was examined in mice. To produce sepsis, the mixture of D-galactosamine (GaLN; 0.6 g/10 ml)/lipopolysaccharide (LPS; $27{\mu}g/27{\mu}l$) was treated intraperitoneally (i.p.). The i.t. pretreatment with clonidine ($5{\mu}g/5{\mu}l$) increased the blood glucose level and attenuated mortality induced by sepsis in a dose-dependent manner. The i.t. post-treatment with clonidine up to 3 h caused an elevation of the blood glucose level and protected sepsis-induced mortality, whereas clonidine post-treated at 6, 9, or 12 h did not affect. The pre-treatment with oral D-glucose for 30 min prior to i.t. post-treatment (6 h) with clonidine did not rescue sepsis-induced mortality. In addition, i.t. pretreatment with pertussis toxin (PTX) reduced clonidine-induced protection against mortality and clonidine-induced hyperglycemia, suggesting that protective effect against sepsis-induced mortality seems to be mediated via activating PTX-sensitive G-proteins in the spinal cord. Moreover, pretreatment with clonidine attenuated the plasma tumor necrosis factor ${\alpha}$ ($TNF-{\alpha}$) induced by sepsis. Clonidine administered i.t. or i.p. increased $p-AMPK{\alpha}1$ and $p-AMPK{\alpha}2$, but decreased p-Tyk2 and p-mTOR levels in both control and sepsis groups, suggesting that the up-regulations of $p-AMPK{\alpha}1$ and $p-AMPK{\alpha}2$, or down-regulations of p-mTOR and p-Tyk2 may play critical roles for the protective effect of clonidine against sepsis-induced mortality.

A Case Report of Integrative Treatment in a Type 2 Diabetes Patient with Multiple Symptoms and Evaluation Using Continuous Glucose Monitoring (소갈 증상을 호소하는 제2형 당뇨 환자에서의 한양방 병행치료 치험 1례와 연속 혈당 측정기를 이용한 평가)

  • Introduction: We describe the effects of Sosiho-tang decoction and insulin combination therapy, as well as the effect of integrated traditional Korean medicine therapy, using continuous glucose monitoring (CGM) in a diabetic patient complaining of a range of diabetic symptoms. Case report: A 24-year-old female presented with symptoms of diabetes, including weight loss, thirst, and polyuria, and was diagnosed with type 2 diabetes through blood tests, endocrine tests, and autoantibody tests. During hospitalization, the patient received insulin therapy and Sosiho-tang decoction concurrently, achieving normal blood glucose levels. After discharge, adhering to the Sosiho-tang decoction and CGM enabled the insulin dosage to be gradually reduced while maintaining normal blood glucose levels. Conclusion: Combination therapy with insulin and Sosiho-tang rapidly reduced hyperglycemia in the short term. CGM post-discharge allowed for observation of the patient's blood glucose levels. Ultimately, Sosiho-tang medication lowered blood glucose levels, reduced insulin requirements, and facilitated a reduction in the insulin dosage.

Anti-Diabetic Effects of an Ethanol Extract of Cassia Abbreviata Stem Bark on Diabetic Rats and Possible Mechanism of Its Action - Anti-diabetic Properties of Cassia abbreviata -

  • Objectives: This study aimed to evaluate the hypoglycemic effects of an ethanol extract of Cassia abbreviata (ECA) bark and the possible mechanisms of its action in diabetic albino rats. Methods: ECA was prepared by soaking the powdered plant material in 70% ethanol. It was filtered and made solvent-free by evaporation on a rotary evaporator. Type 2 diabetes was induced in albino rats by injecting 35 mg/kg body weight (bw) of streptozotocin after having fed the rats a high-fat diet for 2 weeks. Diabetic rats were divided into ECA-150, ECA-300 and Metformin (MET)-180 groups, where the numbers are the doses in mg.kg.bw administered to the groups. Normal (NC) and diabetic (DC) controls were given distilled water. The animals had their fasting blood glucose levels and body weights determined every 7 days for 21 days. Oral glucose tolerance tests (OGTTs) were carried out in all animals at the beginning and the end of the experiment. Liver and kidney samples were harvested for glucose 6 phosphatase (G6Pase) and hexokinase activity analyses. Small intestines and diaphragms from normal rats were used for ${\alpha}-glucosidase$ and glucose uptake studies against the extract. Results: Two doses, 150 and 300 mg/kg bw, significantly reduced the fasting blood glucose levels in diabetic rats and helped them maintain normal body weights. The glucose level in DC rats significantly increased while their body weights decreased. The 150 mg/kg bw dose significantly increased hexokinase and decreased G6Pase activities in the liver and the kidneys. ECA inhibited ${\alpha}-glucosidase$ activity and promoted glucose uptake in the rats' hemi-diaphragms. Conclusion: This study revealed that ECA normalized blood glucose levels and body weights in type 2 diabetic rats. The normalization of the glucose levels may possibly be due to inhibition of ${\alpha}-glucosidase$, decreased G6Pase activity, increased hexokinase activity and improved glucose uptake by muscle tissues.

Changes in Blood Pressures, Blood Profiles and Physical Conditions among Adults in the Daejeon Area (대전지역 일부 직장인의 혈압, 혈액성분 및 체위의 변화)

  • The body mass index, blood pressures and blood profiles of adults, who worked at three colleges in the Daejeon area, were evaluated for use in developing well-balanced menus at their workplace cafeterias and/or in creating nutrition education programs tailored to their health statuses. The data analyzed were from physical examinations performed in 2002 and in 2004 by the National Health Insurance Corporation. Overall, levels of systolic and diastolic blood pressure and serum cholesterol were lower in the 2004 samples than in those of 2002, but most samples in 2004 showed higher levels of SGOT. In 2002, statistical differences occured in the levels of blood pressure and serum cholesterol among age groups, but not in 2004. And in 2004, the older the age group, the higher the BMI. Among subjects in their twenties, three variables (systolic and diastolic blood pressure, and serum cholesterol) showed statistical differences; this increased to four variables (systolic and diastolic blood pressure, blood glucose and serum cholesterol) among subjects in their thirties and to six variables (Hb, SGOT, systolic and diastolic blood pressure, blood glucose and serum cholesterol) among those in their forties, followed by a decline to four variables (systolic and diastolic blood pressure, Hb and serum cholesterol) among subjects in their fifties. In general, the results of physical examinations showed improvements in blood pressure and blood profiles in 2004 in comparison with results in 2002. The level of SGOT was the exception. Despite these improvements, of those employees who had physical examinations in 2004, $5.2\%$ had high blood glucose and $18.1\%$ hypertension, $27.9\%$ hypercholesterolemia, $8.3\%$ registered abnormal liver functions, and $31.3\%$ were classified as overweight and obese. Moreover, because some vulnerable blood profiles were found in the older age groups, various programs, including nutrition education, and/or development of healthful diets as well as excercise activities, may be needed within their workplaces.

Measurement Accuracy of Blood Glucose Test on the Forearm (전완 부위 혈당검사의 정확도 분석)

  • The present study aimed to evaluate the accuracy of the blood glucose test with almost painless vacuum assisted auto-lancing technique. The blood glucose concentration measurements in the capillary blood sampled from the alternative (forearm) site were compared with the standard venous or finger sampling techniques. Capillary blood was sampled from both the left index finger and the forearm, followed by the venous blood sampling in 531 patients who visited the C University Hospital. Blood sugar test was performed for each blood samples for comparative analysis. Mean blood glucose concentration measured on the forearm was closer to the venous glucose level than that of the finger, but the difference was approximately 10 mg/dL small enough to ignore from the clinical point of view. The correlation coefficients of the measured blood glucose levels between finger and vein and between forearm and vein were 0.94 and 0.92, respectively (p<0.001). The blood glucose measurement on the forearm also well correlated with the finger, showing a correlation coefficient of 0.94, and both data were very much close to each other. Therefore, the blood sugar test on the alternative capillary blood sampling site such as forearm was accurate enough for clinical practice. The vacuum assisted auto-lancing technique on the alternative site would be of particular interest for the disease management of the chronic diabetes, since it induces almost no pain when sampling blood from the capillaries.

A Report of Health Status of University Staffs According to the Work Classification

  • This study was conducted to evaluate the health status of staff by medical examination data according to the work classification as professional, office worker and laborer in K University in Seoul, Korea. Two thousand four hundred and eighty-four staff (men : 1154, female: 1330) from the university were studied for this report. The anthropometric (height, weight and BMI) and blood pressure (systolic, diastolic) and biochemical parameters (hemoglobin, glucose, cholesterol, AST, ALT) were measured. All groups were calculated using GLM multivariate analysis for three groups after adjustment for age. The average BMI was significantly higher in laborers than professionals and officers after adjustment for age. In blood pressure, especially in SBP, the significant difference was found in females according to the job classification. Blood glucose levels of female laborers were significantly higher than those of officers and professionals. The level of blood total cholesterol of male professionals was significantly higher than those of laborers. The level of blood total cholesterol of female laborers was significantly higher than officers or professionals. Importantly, significant differences were found in BMI, SBP, blood glucose level and cholesterol level of female staff after adjustment for age. These results showed that there were differences in health subjects of staff according to the work classification. This study would provide basic data to prepare the program of health promotion for the college staff according to work classification. Further research is required to discover factors influencing health promotion of staff in colleges.

Effects of seaweed supplementation on blood glucose concentration, lipid profile, and antioxidant enzyme activities in patients with type 2 diabetes mellitus

  • The present study was carried out to evaluate the physiological effects of seaweed supplementation on blood glucose levels, lipid profile, and antioxidant enzyme activities in subjects with type 2 diabetes mellitus. Subjects were randomized into either a control group or a seaweed supplementation group. Pills with equal parts of dry powdered sea tangle and sea mustard were provided to the seaweed supplementation group three times a day for 4 weeks. Total daily consumption of seaweed was 48 g. We found that total dietary fiber intake was 2.5 times higher in subjects receiving seaweed supplementation than in the control group. Accordingly, fasting blood glucose levels (p<0.01) and 2-hour postprandial blood glucose measurements (p<0.05) were decreased significantly in those ingesting seaweed. Furthermore, the serum concentrations of triglycerides were decreased and high-density lipoprotein cholesterol was increased significantly in seaweed supplement group (p<0.05). However, the concentrations of total cholesterol and low-density lipoprotein cholesterol were not affected by seaweed supplementation. The level of thiobarbituric acid reactive substances in erythrocytes was significantly lower with seaweed supplementation compared to controls (p<0.05). Catalase and glutathione peroxidase activities with seaweed supplementation were higher than the controls (p<0.05), but superoxide dismutase activity was not affected. We, therefore, conclude that ingestion of seaweed influences glycemic control, lowers blood lipids, and increases antioxidant enzyme activities.

The association between diabetes mellitus and community periodontal index: The 5th Korean National Health and Nutrition examination survey (당뇨와 지역사회치주지수의 관련성: 제5기 국민건강영양조사)

  • Objectives: The purpose of the study was to examine the association between diabetes mellitus and community periodontal index in Korean adults. Methods: The study populations were recruited by the Fifth Korea National Health and Nutrition Examination Survey. Study subjects were 10,411 who were examined oral examination, blood test, and aged over 19 years. Using multiple logistic regression analyses, the variables were adjusted for gender, age, household income, family history of diabetes, body mess index, smoking habit, and frequency of tooth brushing. Periodontal tissue examination of the subjects was performed and scored by Community periodontal index(CPI). Using probe, six teeth were examined for hemorrhage, plaque, and pocket depth and classified into $CPI_0$, $CPI_1$, $CPI_2$, $CPI_3$ and $CPI_4$. Healthy periodontal groups($CPI_{0-2}$) and periodontal disease groups($CPI_{3-4}$) were divided by the periodontal disease status. The definition of diabetes mellitus(DM) was decided by the diagnosis by the doctors and fasting blood sugar level. Those who were diagnosed as DM were included in DM group. The DM variables included normal blood sugar level, increased fasting blood sugar level, and DM blood sugar level. The DM variables were compared to periodontal disease blood sugar level and analyzed. Results: The periodontitis prevalence rate was 23.2%. Those who had diabetes mellitus accounted for 5.5% of the subjects. Those who had impaired fasting glucose accounted for 17.7% and 7.9% of subjects were diabetes mellitus by blood test. In the confirmed diabetes group by doctor, the periodontitis prevalence rate was significantly higher than the non-diabetic group. Diabetic group by blood test had the highest prevalence rate of periodontitis than those who had impaired fasting glucose group or normal group. After adjusting for gender, age, household income, family history of diabetes, body mass index, smoking habit, and frequency of tooth brushing, the risk of periodontitis in diagnosed diabetes mellitus was 1.57 times(95% CI; 1.27-1.94) higher than the normal group. In impaired fasting glucose group and diabetes mellitus group by blood test, the risk of periodontitis was 1.11 times(95% CI; 0.95-1.30) and 1.45 times(95% CI; 1.45-2.12) higher, respectively. Conclusions: There was a significant relationship between diabetes mellitus and periodontitis in Korean adults. These results suggest that diabetes mellitus is a risk factor for periodontitis.
