The Effect of Bojungikgi-tang on Stress Urinary Incontinence: A Systematic Review and Meta-Analysis (복압성 요실금에 대한 보중익기탕의 효과 : 체계적 문헌고찰과 메타분석)

  • Objectives: This study investigates the effects and safety of Bojungikgi-tang for stress urinary incontinence by systemic review and meta-analysis of randomized controlled trials (RCTs). Methods: RCTs were selected from articles published until December 2019 in seven domestic and foreign databases. The quality of the literature was evaluated using Cochrane's risk of bias (RoB) tool, and RevMan 5.3 was used to synthesize the results. Results: A total of 694 patients with stress urinary incontinence participated in eight RCTs. Meta-analysis showed that the total effective rate of treatment that combines pelvic floor muscle training (PFMT) and Bojungikgi-tang was significantly higher than that of PFMT alone. The volume of urine leakage per hour after the combined treatment was significantly lower than that of PFMT alone. The International Consultation on Insurance Questionnaire-Short Form (ICIQ-SF) scores from combining PFMT and Bojungikgi-tang were significantly lower than those for PFMT alone. Conclusion: This study suggests that Western medical treatment combined with Bojungikgi-tang for urinary incontinence from stress might be more effective in improving symptoms than conventional Western medical treatment alone. However, the number of studies included in the meta-analysis was insufficient, and the quality of the selected literature was generally low. Therefore, high-quality clinical studies on herbal medicine treatment for urinary incontinence would be required in the future.

Outcome of pregnant mothers with systemic lupus erythematosus (focusing on congenital heart block) (전신성 홍반성 루푸스 산모의 출산아의 임상적 고찰 (완전 방실 블록을 중심으로))

  • Purpose : Neonatal lupus is characterized by congenital complete heart block(CCHB), cutaneous rash, and laboratory abnormalities in infants born to mothers with systemic lupus erythematosus(SLE). This study aims to examine the incidence of CCHB and clinical outcome in neonates born to mothers with SLE. Methods : The study group consisted of 49 neonates, born from 57 pregnancies of 55 women with SLE, diagnosed at Hanyang University Hospital for the period between January 1997 and January 2005. Clinical and laboratory data were retrospectively identified from medical record. Results : There were 5(8.8 percent) spontaneous abortions and one(1.8 percent) still births among 57 pregnancies of 55 mothers. Of 49 live births, 15(26.3 percent) were premature and eight(12.3 percent) were small for their gestational age. There was one(1.8 percent) CCHB suspected during pregnancy on fetal echocardiograpy in a fetus of mother with systemic lupus erythematosus and the fetus was not born by artificial abortion because of mother. There was no CCHB among EKG findings of 49 newborns. Laboratory testing showed hematologic abnormalities among 25.6 percent(10/39) of the babies. 5.1 percent(2/39) and 7.7 percent(3/39) of them were diagnosed as neutropenia, and thrombocytopenia was seen respectively. Anti-SSA(Ro) and antiphospholipid antibodies were predictive factors for prematurity(P=0.003, P=0.049). Anticardiolipin antibodies were predictive factors for ventilatory care(P=0.018). Conclusion : The incidence of CCHB among neonates born to mothers with SLE, which was measured in this study, was lower than that in earlier studies. A high incidence of hematologic abnormalities was found in our study. It is suggested that careful examination should be made of skin for the diagnosis of neonatal lupus.

  • Recently, when software architecture is designed, the efficiency of reusability is emphasized. The reusability of the design can raise the quality of GIS software, and reduce the cost of maintenance. Because the object oriented GoF design pattern provides the class hierarchy that can represent repetitively, the importance is emphasized more. This method that designs the GIS software can be applied from various application systems. A multiple distributed sensor network system is composed of the complex structure that each node of the sensor network nodes has different functions and sensor nodes and server are designed by the combination of many classes. Furthermore, this sensor network system may be changed into more complex systems according to a particular purpose of software designer. This paper will design the CATL model by applying Facade pattern that can enhance the efficiency of reuse according to attributes and behaviors in classes in order to implement the complicated structure of the multiple distributed sensor network system based on TinyOS. Therefore, our object oriented GIS design pattern model will be expected to utilize efficiently for design, update, or maintenance, etc. of new systems by packing up attributes and behaviors of classes at complex sensor network systems.

A City Path Travel Time Estimation Method Using ATMS Travel Time and Pattern Data (ATMS 교통정보와 패턴데이터를 이용한 도시부도로 통행시간 추정방안 연구)

  • ATMS calculates section travel time using two-way communication system called DSRC(Dedicated Short Range Communications) which collects data of RSE (Road Side Equipment) and Hi-pass OBU (On-board Unit). Travel time estimation in urban area involves uncertainty due to the interrupted flow. This study not only analyzed real-time data but also considered pattern data. Baek-Je-Ro street in Jeon-Ju city was selected as a test site. Existing algorithm was utilized for data filtering and pattern data building. Analysis results repoted that travel time estimation with 20% of real-time data and 80% of pattern data mixture gave minimum average difference of 37.5 seconds compare to the real travel time at the 5% significant level. Results of this study recommend usage of intermixture between real time data and pattern data to minimize error for travel time estimation in urban area.

Analysis of Growth Response and Gene Expression by Waterlogging Stress on B73 Maize (침수 처리에 따른 B73 옥수수의 생육 반응 및 유전자 발현 분석)

  • Maize is thought to be an alternative crop to rice in paddy fields for efficient field management and maintenance of rice production at appropriate levels in Korea. Thus efforts to breed waterlogging-tolerant maize cultivars have been ongoing. However, molecular studies related to waterlogging tolerance are limited for developing molecular markers to select waterlogging tolerant maize varieties. In this study, we examined molecular biological changes of B73 in the V3 stage after immersion treatment for 7 days. Overall growth of maize was lower in treated samples compared to non-immersed control samples. The length of leaf and root decreased by 21.3% and 50.6%, respectively and the weight of root reduced by 21.6%. Soil plant analysis development (SPAD) value and chlorophyll content of leaf also decreased by 55.7% and 35.3%, respectively. Reactive oxygen species (ROS) content of root increased by 46.5% at 2 hours in immersion treatment. In addition, immersed roots were 2.5-fold thickened with additional aerenchyma formation in the cortex. In order to identify the causes of these changes, we performed a microarray and found increased expression of genes, such as WIP1, PMIP2, EXPA1, TPS1, and MAS1, in immersed samples. These differentially expressed genes and expression of previously reported genes, including ALDH2, Wusl1032, UP-1, UP-2, and CAT2 were further confirmed with qRT-PCR. Here, we report 7 differentially expressed genes after immersing treatment, which may be utilized as useful resources for breeding waterlogging- tolerant maize.

Aquatic Toxicity Assessment of Phosphate Compounds

  • Objectives Tricalcium phosphate and calcium hydrogenorthophosphate are high production volume chemicals, mainly used as foodstuff additives, pharmaceuticals, lubricants, synthetic resin, and disinfectants. Phosphate has the potential to cause increased algal growth leading to eutrophication in the aquatic environment. However, there is no adequate information available on risk assessment or acute and chronic toxicity. The aim of this research is to evaluate the toxic potential of phosphate compounds in the aquatic environment. Methods An aquatic toxicity test of phosphate was conducted, and its physico-chemical properties were obtained from a database recommended in the Organization for Economic Cooperation and Development (OECD) guidance manual. An ecotoxicity test using fish, Daphnia, and algae was conducted by the good laboratory practice facility according to the OECD TG guidelines for testing of chemicals, to secure reliable data. Results The results of the ecotoxicity tests of tricalcium phosphate and calcium hydrogenorthophosphate are as follows: In an acute toxicity test with Oryzias latipes, 96 hr 50% lethal concentration ($LC_{50}$) was >100 (measured:>2.14) mg/L and >100 (measured: >13.5) mg/L, respectively. In the Daphnia test, 48 hr 50% effective concentration ($EC_{50}$) was >100 (measured: >5.35) mg/L and >100 (measured: >2.9) mg/L, respectively. In a growth inhibition test with Pseudokirchneriella subcapitata, 72 hr $EC_{50}$ was >100 (measured: >1.56) mg/L and >100 (measured: >4.4) mg/L, respectively. Conclusions Based on the results of the ecotoxicity test of phosphate using fish, Daphnia, and algae, $L(E)C_{50}$ was above 100 mg/L (nominal), indicating no toxicity. In general, the total phosphorus concentration including phosphate in rivers and lakes reaches levels of several ppm, suggesting that phosphate has no toxic effects. However, excessive inflow of phosphate into aquatic ecosystems has the potential to cause eutrophication due to algal growth.

  • Colorless and transparent cubic zirconia($Zr_{0.73}Y_{0.27}O_{1.87}$) crystal has been synthesized by the Bridgman-Stock-bager method(also called Skull melting method). $Y_2O_3$ is used as stabilizer. The crystal shows a vitreous luster with a slight oily appearance. Under a polarizing microscope, it shows isotropic nature with no appreciable anisotropism. Mohs hardness value is measured to be $8{\sim}8\frac{1}{2}$ and specific gravity 5.85. Under ultraviolet light it shows a faint white glow. The crystal structure of yttria stabilized zirconia was determined, using single crystal X-ray diffraction techniques to be a cubic symmetry, space group $Fm\overline{3}m({O^5}_h)$ with $a=5.1552(5){\AA}$, $V=136.99(5){\AA}^3$, Z=4, and R=0.0488 for 29 unique reflections. Each zirconium atom is at the center of eight oxygen atoms situated at the corners of a surrounding cube and each oxygen atom is at the center of a tetrahedron of zirconium atoms. So a coordination of 8:4 holds in the structure.

Numerical Modeling of Circulation Characteristics in the Kwangyang Estuarine System (광양만 권역의 해수순환 수치모델 실험)

  • The ECOM3D is used to study the circulation characteristics and density current from the Sumjin River runoff in the Kwangyang Estuarine System, South Sea, Korea. Annual mean value of $120m^3/s$ was imposed from the Sumjin River. The numerical model results in terms of tidal height, current and salinity field show satisfactory with skill scores over 90%. The current velocity showed the range of 1~2.5 m/s during flood and ebb phases. In particular, very strong flow occur in the narrow Channels of Noryang, Daebang and Changson exceeding over 2.0 m/s. The tidal residual currents in the various locations in the Kwangyang Estuary showed the range of 1~21 cm/s, The density-driven current through the Yeosu and Noryang Channels are about 12 cm/s and 4 cm/s, respectively. The current path through the Yeosu Channel is deflected toward west Bank. Based on budget analysis of the volume flux, the volume flux through the Yeosu Channel and the Noryang Channel were estimated to be 97.4 and $22.1m^3/s$ accounting for the 81.5% and 18.5% of total flux, respectively.

Protective Effect of Corni Fructus Extracts on MIA-induced Animal Model of Osteoarthritis: Effect of Corni Fructus Extracts on OA (MIA로 유발된 골관절염 동물모델에서 산수유 추출물의 골관절염 개선 효과)

  • Objectives: Osteoarthritis (OA) is a chronic and degenerative joint disease characterized by progressive degeneration of articular cartilage. Inflammation is a recognized and important factor of OA progression. The present study was designed to investigate the protective effect of Corni Fructus water extract (CFW) on a monosodium iodoacetate (MIA)-induced rat model of OA. Methods: Osteoarthritis was induced by injection of MIA (50 µL; 80 mg/mL) into the knee joint cavity of rats. After an adaptation period for seven days, the rats were divided into 4 groups (n=8/group): normal, control, indomethacin-treated (5 mg/kg), and CFW-treated (200 mg/kg) groups. The rats were treated orally for 14 days. Pain was evaluated by determining hind paw weight distribution. For biochemical analyses, we measured the changes in reactive oxygen species (ROS) and peroxynitrite (ONOO-) in the knee joint. The presence of anti-oxidant proteins and inflammatory proteins was determined by western blotting. Results: The administration of CFW significantly improved the hind paw weight distribution. The ROS and ONOO- levels of knee joint were significantly decreased in the CFW group. CFW inhibited the production of inflammatory mediators, such as COX-2, and inflammatory cytokines, including IL-6 and IL-1β, via the NF-κB signaling pathway. The expression of anti-oxidant enzymes, such as catalase and GPx-1/2 also increased significantly. Conclusions: The findings indicate that CFW has a therapeutic and protective effect on OA by suppression of inflammation. Therefore, CFW could represent a potential and effective candidate for OA treatment.

Hospital Marketing Condition and Strategy -Of a General Hospital- (병원마케팅 실태와 전략방안 -지역사회의 일 종합병원을 대상으로-)

  • The purpose of this study was to investigate the real condition of hospital marketing and concrete strategy on medical consumers need for enhance the effectiveness of hospital management. The data were collected from January 27 to February 3, 2000 at a general hospital located in M city to 205 in-patients and out-patients by questionnaires. The research tool was based on literatures. The data was analysed by the use of percentage, mean, t-test and ANOVA by SAS(Statistical Analysis System) Package Program. The results of this study are summerized as follows: 1. Mean score of the marketing mix (4P 's) was 3.1 in total and each mean score was 3.6 in product, 2.5 in prices, 3.3 in place and 3.1 in promotion. The following are the items which received the highest scores in each: 'The hospital is clean' in service category; 'the prices of meals at the restaurant the patients' families use are reasonable ' in price sphere. In distribution, 'it is easy to locate the hospital' and in promotion,'we'll use this hospital again' and 'we'll recommend this hospital to others'. 2. Marketing mix factors(4P's) was significantly different on the general characteristics in terms of marriage status, occupations and the period of hospitalization. 3. The most important reason for choosing this hospital was 'This is a general hospital with good facilities' and the next were 'The hospital staffs are kind and the hospital is clean' and 'The traffic is convenient to come'. The most important factors contributing to a good image formation of the hospital were 'this is a clean hospital', 'This is a hospital with kind people' and 'this hospital is equipped with good medical facilities'. 4. The factors for making a good image concerning the nursing service were professional knowledge, good nursing skills and kindness. After grasping the marketing conditions of the hospital aided by the above-mentioned research results, the researchers concludes that it is necessary to develop an institutionalized service strategy to increase the satisfaction the patients feel about the hospital facilities and kindness of the staff and as a result, to differentiate it from other medical institutions.
