ON THE PROPERTIES OF POSITIVE BOOLEAN DEPENDENCIES BY GROUPS IN THE DATABASE MODEL OF BLOCK FORM

  • The article proposed a new type of data relationship: Positive Boolean dependencies by groups on block and slice in the database model of block form, where instead of considering value pairs, we consider a group of p values (p ≥ 2). From this new concept, the article stated and demonstrated the equivalence of the three types of deduction, namely: deduction by logic, deduction by block with groups, deduction by block has no more than p elements with groups. Operations on blocks or slices performed for index attributes on blocks, the properties related to this new concept as theorem the equivalen of the three types of deduction, closure of set positive Boolean dependencies by groups and representation and tight representation set of positive Boolean dependencies by groups when the block degenerated into relation are true in the relational database model and also stated and proven in this paper.

These block designs are constructed by using balanced block designs obtained by cyclically developing a single initial block. Also, the efficiencies of block designs are tabulated for number of lines 26 or less.

Effectiveness of Tramadol and Butorphanol as an Adjuvant to Local Anesthetic in Mandibular Nerve Block in Beagle Dogs

  • To evaluate butorphanol and tramadol as adjuvants to lidocaine in dogs undergoing mandibular nerve block. Fifteen beagles were allocated to groups based on the following treatments: lidocaine alone (L group), lidocaine + butorphanol (LB group), or lidocaine + tramadol (LT group). After mandibular nerve block with opioids as an adjunct to local anesthetics, the onset time, duration of action, and depth of anesthesia were evaluated using a quantitative method through neuromuscular blockades (NMBs) monitoring. The onset time of nerve block was 4.60 ± 2.06 min, 2.00 ± 0.00 min, and 2.60 ± 1.62 min in the L, LB, and LT groups, respectively; however, there was no statistically significant difference. The duration of nerve block was 111.88 ± 34.78 min, 302.00 ± 76.72 min, and 260.40 ± 49.88 min in the L, LB, and LT groups, respectively, with a significant difference between L and LB groups. The LB group demonstrated a more profound depth of anesthesia compared to the L and LT groups. In this study, using a quantitative method through NMBs monitoring, it was demonstrated that lidocaine and butorphanol in combination can increase the duration of nerve block and more profound the depth of anesthesia rather than lidocaine alone. Additionally, the combined use of lidocaine and opioids presented an objective indicator that could provide a more clinically stable nerve block.

The Antinociceptive Effects of Epidural Tramadol with Bupivacaine in Beagle Dogs

  • This study investigated the antinociceptive effect of epidural tramadol with bupivacaine in 36 healthy Beagle dogs. The dogs were divided into 6 groups; 1) C (control), 2) B (0.5% bupivacaine 0.1 mL/kg), 3) BT0.5 (0.5% bupivacaine 0.1 mL/kg + tramadol 0.5 mg/kg), 4) BT1 (0.5% bupivacaine 0.1 mL/kg + tramadol 1 mg/kg), 5) BT2 (0.5% bupivacaine 0.1 mL/kg + tramadol 2 mg/kg), 6) BT3 (0.5% bupivacaine 0.1 mL/kg + tramadol 3 mg/kg). The epidural injection was performed under isoflurane inhalation, after then, nociceptive block and motor block scores were assessed with physiologic parameters (HR, RR, RT, MAP). BT groups showed significantly longer antinociceptive time than C and B, while motor block time of BT groups were not different from B except BT3. Durations of total nociceptive block of BT2 ($60.83{\pm}19.08min$) and BT3 ($74.17{\pm}8.61min$) were significantly longer than those of BT0.5 ($33.33{\pm}8.76min$) and BT1 ($37.50{\pm}19.43min$), but there was no significant difference between BT2 and BT3. Durations of total motor block in all groups were less than 20 minutes although that of BT3 was significantly longer than B. There were no significant differences in HR, RR, RT, MAP among groups. Consequently, epidural administration of tramadol (2 mg/kg) with 0.5% bupivacaine (0.1 mL/kg) can be used safely and effectively in dogs.

Ischiorectal Block with Bupivacaine for Post Hemorrhoidectomy Pain

  • Background: Hemorrhoid is one of the most common surgical diseases occurring in the anorectal region. In this study, we evaluated the effect of ischiorectal fossa block on alleviating post hemorrhoidectomy pain. Methods: In this study, 90 patients suffering from hemorrhoids were evaluated. They were randomly divided into 3 groups. The first group had no block, the second group an ischiorectal block with placebo (normal saline), and the third group a preemptive ischiorectal block with bupivacaine. Postoperative variables such as pain intensity, pethidine consumption, nausea, and vomiting were compared between the groups. Results: The postoperative pain score in group 1 was $8.5{\pm}1.3$ and $8.1{\pm}0.9$ (P=NS) in group 2. The post operative analgesic demand was $3.1{\pm}1.5$ and $3.3{\pm}1.8$ hours in groups 1 and 2, respectively (P=NS). The post operative pain score and analgesic demand were $4.2{\pm}2.1$ and $9.3{\pm}2.7$ hours, respectively, in group 3 (P < 0.0001). Conclusions: Preemptive ischiorectal block reduces the posthemorrhoidectomy pain and opioid demand.

개흉술 후 흉부동통억제를 위한 늑간신경냉동 요법과 약물주입요법에 관한 비교연구 (Cryoanalgesia vs Bupivacain Intercostal Nerve Block for the Post -thoracotomy Pain)

  • Post-thoracotomy pain is so severe that lead to postoperative pulmonary complications due to the patient`s inability to cough, deep breathing and chest wall motion. Many authors have been trying to reduce the post-thoracotomy pain, but there is no method of complete satisfaction. In 1990, we reported the result that the cryoanalgesia reduce the immediate postoperative pain significantly. We try to compare the effect of cyroanalgesia with the effect of Bupivacain intercostal nerve block for the post-thoracotomy pain control. Ninety patients, who undergoing thoracotomy in Korea University Medical Center Guro Hospital between the January 1993 and September 1993, were evaluated. The patients were divided into three groups : Group A, control, the patients without pain control procedure [N=30], Group B, the patients with Bupivacain intercostal nerve block [N=30], Group C, the patients with cryoanalgesia [N=30]. Postoperative analgesic effects were evaluated by the scoring system which made arbiturary by author. The results were that the Bupivacain intercostal nerve block markedly reduced the immediate postoperative pain compare with two other groups, the cryoanalgesia reduced the immediate postoperative pain significantly compare with control group, the pain reduction effect of the two groups- cryoanalgesia and Bupivacain intercostal nerve block- were not different at postoperative seventh day, probably due to the action time of Bupivacain and the result that there were no significant complications of the procedures.

Comparative Study of the Effects of the Retrocrural Celiac Plexus Block Versus Splanchnic Nerve Block, C-arm Guided, for Upper Gastrointestinal Tract Tumors on Pain Relief and the Quality of Life at a Six-month Follow Up

  • Background: The celiac plexus and splanchnic nerves are targets for neurolytic blocks for pain relief from pain caused by upper gastrointestinal tumors. Therefore, we investigated the analgesic effect of a celiac plexus block versus a splanchnic nerve block and the effects of these blocks on the quality of life six months post-intervention for patients with upper GIT tumors. Methods: Seventy-nine patients with inoperable upper GIT tumors and with severe uncontrolled visceral pain were randomized into two groups. These were Group I, for whom a celiac plexus block was used with a bilateral needle retrocrural technique, and Group II, for whom a splanchnic nerve block with a bilateral needle technique was used. The visual analogue scale for pain (0 to 100), the quality of life via the QLQ-C30 questionnaire, and survival rates were assessed. Results: Pain scores were comparable in both groups in the first week after the block. Significantly more patients retained good analgesia with tramadol in the splanchnic group from 16 weeks onwards (P = 0.005, 0.001, 0.005, 0.001, 0.01). Social and cognitive scales improved significantly from the second week onwards in the splanchnic group. Survival of both groups was comparable. Conclusions: The results of this study demonstrate that the efficacy of the splanchnic nerve block technique appears to be clinically comparable to a celiac block. All statistically significant differences are of little clinical value.

Synthesis and Characterization of Alternating Block Copolycarbonates Containing Constituent Groups of Polysulfone

  • A series of alternating and alternating block copolycarbonates containing the constituent groups of polysulfone was synthesized through a multi-step solution condensation method. For the regulation of block length, monodisperse oligomers were prepared by using a large excess of the bisphenols and were subsequently incorporated into the copolymer chains. Separating the unreacted bisphenols from the oligomers by dissolution/precipitation steps took advantage of solubility differences. The structures of the monomers, oligomers, and copolymers were characterized and confirmed by GPC, NMR spectroscopy, mass spectrometry, and elemental analysis. Monodispersity of the oligomers, which is critical for control over the block length in the copolymers, was confirmed by GPC and mass spectrometry. Of the two constituent groups of the polysulfone, the sulfone linkage stiffens the polycarbonate copolymer chain, while the ether linkage softens it.

개흉술후 벽측흉막외 신경차단법에 의한 진통효과 (Post-thoracotomy pain control with paravertebral intercostal nerve block)

  • Effrctive analgesia after elective thoracotomy can be provided by continuous extrapleural intercostal nerve block.This study was designed to prove the effectiveness of continuous extrapleural intercostal nerve block. Twenty patients undergoing elective thoracotomy were randomized into two groups. Group I received lumbar epidural block[N=10] and group II received continuous extrapleural intercostal nerve block[N=10]. Postoperative pain relief was assessed on Numeric Rating Scale[NRS] and recovery of pulmonary function was assessed by coparison of preoperatrive and postoperative FVC[Forced Vital Capacity], FEV1[Forced expiratory Volume in 1 second], VC[Vital Capacity]. Arterial blood gas analysis[ABGA], vital signs and amount of additive analgesics were compared also. No significant difference was observed between the groups concerning these parameters mentioned above. Systemic complications, such as urinary retention[2/10] and weakness of lower extremity[2/10], occurred in group I but no complication occurred in group II. We conclude that continuous extrapleural intercostal nerve block is as effective as epidural block in pain relief and restoration of pulmonary mechanics with fewer comlications. Also because of it`s ease and safetiness, this must be considered as a substitute of epidural block in routine use for thoracotomy pain relief.
