Recovery of NK(CD56+CD3-) Cells after One Year of Tenofovir Therapy for Chronic Hepatitis B Infection

  • Natural killer (NK) cells have been reported to be dysfunctional in chronic hepatitis B (CHB) infection. However, the functional recovery of NK cells under antiviral therapeutic agents in CHB was not clearly understood. In this study, we investigated the phenotypic changes of NK(CD56+CD3-) cells in terms of their functional markers (CD16, NKG2A, NKG2D) during tenofovir therapy in CHB. The frequency of NK(CD56+CD3-) cells in CHB patients was significantly increased after 12 months of tenofovir therapy when compared with baseline. The expression levels of CD16+/CD56+CD3- and NKG2A+/CD56+CD3- cells were also affected by tenofovir treatment. In addition, there was a positive correlation between the proportion of NK(CD56+CD3-) cells and HBV DNA (log copies/ml) in CHB patients.

소음인 A형 인플루엔자 감염 치험례 (Treatment of Influenza A Virus Infection in a Soeumin Patient; a Single Case)

  • Objective This study aimed to report an improvement of influenza virus infection managed with Sasang Constitutional Medicine in a Soeumin patient with thrombocytopenia and histories of acute myeloid leukemia and interstitial pulmonary disease. Methods The medical records of patient including vital signs, laboratory test, simple x-ray, and symptoms were reviewed retrospectively. Patient was treated with Gunggwihyangso-san gamibang without any antiviral agents under the diagnosis of Congestive Hyperpsychotic symptomatology of Kidney Heat-based Exterior Heat disease. Results At admission, symptoms of shivering, fever, cough and myalgia were presented and her skin temperature was over $39^{\circ}C$. Influenza A virus was detected and inflammation biomarkers were elevated. After administration of herb medication, her vital signs and symptoms were improved and elevated inflammation biomarkers were decreased. There was no secondary complication during the treatment period. Conclusions This case treated with Gunggwihyangso-san gamibang showed a significant improvement from viral infection without any complication.

Progress on Understanding the Anticancer Mechanisms of Medicinal Mushroom: Inonotus Obliquus

  • Cancer is a leading cause of death worldwide. Recently, the demand for more effective and safer therapeutic agents for the chemoprevention of human cancer has increased. As a white rot fungus, Inonotus obliquus is valued as an edible and medicinal resource. Chemical investigations have shown that I. obliquus produces a diverse range of secondary metabolites, including phenolic compounds, melanins, and lanostane-type triterpenoids. Among these are active components for antioxidant, antitumoral, and antiviral activities and for improving human immunity against infection of pathogenic microbes. Importantly, their anticancer activities have become a hot recently, but with relatively little knowledge of their modes of action. Some compounds extracted from I. obliquus arrest cancer cells in the G0/G1 phase and then induce cell apoptosis or differentiation, whereas some examples directly participate in the cell apoptosis pathway. In other cases, polysaccharides from I. obliquus can indirectly be involved in anticancer processes mainly via stimulating the immune system. Furthermore, the antioxidative ability of I. obliquus extracts can prevent generation of cancer cells. In this review, we highlight recent findings regarding mechanisms underlying the anticancer influence of I. obliquus, to provide a comprehensive landscape view of the actions of this mushroom in preventing cancer.

Inosiplex가 세포성(細胞性) 및 체액성면역반응(體液性免疫反應)에 미치는 영향(影響) (Effect Inosiplex on Cellular and Humoral Immune Response)

  • The clinical need for agents to modify immune response in the treatment of viral infection has lead to an increased interest in cellular and biochemical mechanisms regulating the immune response and to the development of a variety of biological and chemical substance with immunomodulatory activity. Inosiplex has shown antiviral activity in tissue culture, animal models and huamn studies through augmentation of immune response. However, the effect of inosiplex on immune response in animal has not been extensively analyzed, and the effect of inosiplex on immune response has been paradoxical depending on the time of administration of inosiplex in relation to that of antigen. Therefore, this study was undertaken to assess the effect of inosiplex on the immune response to sheep red blood cells(SRBC) in normal and viral infected mice. Inosiplex increased cellular immune response and plaque forming lymphocyte response to SRBC, decreased the recovery of S. typhimurium from infected mice spleen, and restored the depressed cellular immune response by measle and newcastle disease virus infections. All of the above results were observed only when inosiplex was given after immunization but did not when given before immunization. These results indicate that inosiplex stimulate the efferent are of immune response and may even block the afferent are, and suggest that inosiplex is a very promising drug in therapy of many viral infections.

  • 미만성 폐포출혈은 흔히 교원성질환에 동반되어 발생하나 거대세포바이러스 폐렴과 동반되어 나타날 수 있으며 병인은 아직 밝혀지지 않았다. 따라서 이 두 질환의 연관관계에 대한 연구가 필요하다. 저자들은 뇌출혈로 입원 후 장기간 치료 중이던 환자에서 거대세포바이러스 폐렴과 이에 동반된 미만성 폐포출혈 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

Optimization of Culture Conditions for D-Ribose Production by Transketolase-Deficient Bacillus subtilis JY1

  • D-Ribose is a five-carbon sugar used for the commercial synthesis of riboflavin, antiviral agents, and flavor enhancers. Batch fermentations with transketolase-deficient B. subtilis JY1 were carried out to optimize the production of D-ribose from xylose. The best results for the fermentation were obtained with a temperature of $37^{\circ}C$ and an initial pH of 7.0. Among various sugars and sugar alcohols tested, glucose and sucrose were found to be the most effective for both cell growth and D-ribose production. The addition of 15 g/l xylose and 15 g/l glucose improved the fermentation performance, presumably due to the adequate supply of ATP in the xylose metabolism from D-xylulose to D-xylulose-5-phosphate. A batch culture in a 3.7-1 jar fermentor with 14.9 g/l xylose and 13.1 g/l glucose resulted in 10.1 g/l D-ribose concentration with a yield of 0.62 g D-ribose/g sugar consumed, and 0.25 g/l-h of productivity. Furthermore, the sugar utilization profile, indicating the simultaneous consumption of xylose and glucose, and respiratory parameters for the glucose and sucrose media suggested that the transketolase-deficient B. subtilis JY1 lost the glucose-specific enzyme II of the phosphoenolpyruvate transferase system.

Antibacterial Activity of (2S)-7,4'-dihydroxy-5-methoxy-8-(${\gamma}$, ${\gamma}$-dimethylally)-flavanone against Methicillin-Resistant Staphylococcus aureus

  • The emergence of methicillin-resistant of Staphylococcus aureus (MRSA) and vancomycin-resistant enterococci (VRE) has led to an urgent need for the discovery and development of new antibacterial agents. As part of an ongoing investigation into the antibacterial properties of the natural products, (2S)-7,4'-dihydroxy-5-methoxy-8-(${\gamma}$, ${\gamma}$-dimethylally)-f1avanone (2S-DMDF), isolated from the roots of Sophora flavescens, was found to be antibacterial active MRSA and VRE. Sophora flavescens has been used as antibacterial, antiviral, antiprotozoal, anti-inflammatory. Therefore, this study investigated the antibacterial activity of 2S-DMDF against all the bacterial strains tested. In this result, at the end point of an optically clear well, the minimum inhibitory concentrations (MICs) ranged from 0.97 to 15.6 mg/ml for 2S-DMDF, from 125 to 256 mg/ml for ampicillin, and from 64 to 512 mg/ml for gentamicin with MRSA, also, 7.8 to 15.6 mg/ml for 2S-DMDF, from 125 to 256 mg/ml for ampicillin, and from 512 to 1024< mg/ml for vacomicin with VRE. These findings indicated that the application of the tested 2S-DMDF alone might prove useful in the control and treatment of MRSA and VRE infections.

Identification of the active components inhibiting the expression of matrix metallopeptidase-9 by TNFα in ethyl acetate extract of Euphorbia humifusa Willd

  • Euphorbia humifusa Willd (EuH), called Ttang-Bin-Dae in Korea, is a traditional medicinal plant widely used for its anti-inflammatory and antiviral activity. Ethyl acetate (EA) extracts of EuH (EA/EuH) inhibit invasion and metastasis by inhibiting tumor necrosis factor TNFá-induced matrix metalloproteinases (MMP)-9 expression in human breast cancer cells. However, the bioactive components of EA/EuH mediating the inhibition of MMP-9 expression have not been identified. In the present study, three bioactive constituents of EA/EuH were isolated using high-performance liquid chromatography. Nuclear magnetic resonance spectroscopy revealed isoquercetin, avicularin, and astragalin as the bioactive compounds responsible for preventing TNFα-induced MMP-9 mRNA expression in breast cancer cells. These findings suggest that isoquercetin, avicularin, and astragalin could be used as valuable anti-metastatic agents against metastatic cancers.

Generation of Full-Length Infectious cDNA Clones of Middle East Respiratory Syndrome Coronavirus

  • Middle East respiratory syndrome coronavirus (MERS-CoV) was first identified in Saudi Arabia in 2012 and related infection cases have been reported in over 20 countries. Roughly 10,000 human cases have so far been reported in total with fatality rates at up to 40%. The majority of cases have occurred in Saudi Arabia with mostly sporadic outbreaks outside the country except for the one in South Korea in 2015. The Korean MERS-CoV strain was isolated from the second Korean patient and its genome was fully sequenced and deposited. To develop virus-specific protective and therapeutic agents against the Korean isolate and to investigate molecular determinants of virus-host interactions, it is of paramount importance to generate its full-length cDNA. Here we report that two full-length cDNAs from a Korean patient-isolated MERS-CoV strain were generated by a combination of conventional cloning techniques and efficient Gibson assembly reactions. The full-length cDNAs were validated by restriction analysis and their sequence was verified by Sanger method. The resulting cDNA was efficiently transcribed in vitro and the T7 promoter-driven expression was robust. The resulting reverse genetic system will add to the published list of MERS-CoV cDNAs and facilitate the development of Korean isolate-specific antiviral measures.

Herpes Zoster Accompanying Odontogenic Inflammation: A Case Report with Literature Review

  • Herpes zoster is caused by reactivation and multiplication of a latent varicella-zoster virus infection. Reactivation can frequently occur in older adults and immunosuppressed individuals. It is hypothesized that this is related to an aging society and a corresponding increase in the number of people with underlying chronic diseases, such as cancer and diabetes, that lower immunity. Clinically, the patient complains of pain, and a vesicular rash presents on one side of the face up to the midline in the dermatomes associated with the affected ganglion. Herpes zoster of the oral mucosa is rare. When oral lesions do occur, they are most often concurrent with pathognomonic unilateral linear vesicular skin lesions, facilitating both clinical diagnosis and management of the condition. Cases limited to the oral mucous membrane alone are most unusual. Treatment includes antiviral agents and analgesics for pain control. Antivirals should be administered within 72 hours of onset. Early diagnosis and treatment are important to avoid complications, such as postherpetic neuralgia. The present case report describes the adequate management of a patient diagnosed with shingles which affected the right side of the face and oral cavity. In addition, a literature review is presented.
