도라지 뿌리 에탄올 추출물이 streptozotocin으로 유발된 흰쥐의 혈당지질, 당대사에 미치는 영향 (The Effect of Platycodon grandiflorum Root Ethanol Extract on Blood Glucose, Lipid, Activities of Carbohydrate Metabolism Related Enzyme in Streptozotocin-Induced Diabetic Rats)

  • Streptozotocin(STZ)을 45mg/kg.b.w의 용량으로 흰쥐의 미정맥에 투여 한 후 유발된 당뇨 흰쥐에게 1일 1회 7일간 1,000mg/kg의 용량으로 도라지 뿌리 에탄올 추출물을 투여 후 glucose 함량과 당대사에 관여하는 효소인 glucose-6-phosphatase(G-6-Pase) glucose-6-phosphate dehydrogenase(G-6-PDH), glucokinase(GK)활성과 glycogen 함량, triglyceride(TG), total cholesterol등의 지질대사에 관여하는 물질들을 측정하였다. 그 결과 도라지 뿌리 에탄올 추출물 투여군이 glucose, TG, total cholesterol등의 함량과 G-6-Pase 활성의 유의적인 감소(p<0.05)를 나타내었으며 glycogen 함량과 HDL-cholesterol, G-6-PDH, GK의 활성이 유의적인 증가(p<0.05)를 나타내었다. 이와 같이 도라지 뿌리 에탄올 추출물이 항당뇨 개선효과를 갖는 유효성분을 함유하고 있음을 알 수 있었다.

선택약가제도가 건강보험 약품비 지출에 미치는 영향: 경구용 당뇨병 치료제를 대상으로 한 시나리오 분석 (The Effect of Tiered Copayment System on Pharmacy Benefit Expenditure of National Health Insurance in Korea: Scenario Analysis based on the Use of Oral Antidiabetic Medications)

  • Purpose: This study was aimed to examine the financial effect of 2-tiered copayment system on annual expenditure of pharmacy benefit in the National Health Insurance (NHI) of Korea, focusing on oral antidiabetic medications. Methods: 284 oral antidiabetic products with 14 different active single ingredients listed in the National Formulary of NHI (August 2009) were assigned to tier 1 or tier 2 according to the selected criteria. 10 different combinations of coinsurance rates were selected to estimate the changes in drug expenditure cost of NHI. Results: The annual drug cost was estimated based on the drug price per unit listed in the National Formulary and the used amount of products in 2009 from the IMS Health data of Korea. In the combinations of coinsurance rate of 20% for tier 1 and 40% for tier 2, the total annual drug cost was estimated to be reduced by 1.3% in the case of no change of generic and original drug consumptions, and to be reduced by 4.3% in the case of 10% increased generic drug consumptions. Conclusion: The tiered copayment system with optimal coinsurance rates appears to be a potential strategy to reduce the financial burden of NHI in Korea by promoting the use of generic products.

기능성 식품 복합처방의 항산화 및 항당뇨 효과 (The Antioxidant and The Antidiabetic Effects of Ethanol Extract from Biofuntional Foods Prescriptions)

  • To investigate the antioxidant activitys of Biofuntional Foods Prescriptions (BFP), we were measured radical scavenging activity with 1,1-diphenyl-2-picryl-hydrazyl (DPPH) method and anti-lipid peroxidative efficacy on human LDL with thiobarbituric acid reactive substances (TBARS) assay. The DPPH and TBARS assay were revealed that Two BFP had significantly activity when compared to that of contrast drug. The antidiabetic effect of Dicollow-B (Rx B) were investigated in streptozotocin-induced diabetic Spraque-Dawley (SD) rats. When each 100,200 kg/mg fraction of Rx B was administrated. the blood glucose showed 155.7 mg/dl, 124.7 mg/dl and inhibitive activity of the serum total cholesterol showed 67.8 mg/dl, 61.5 mg/dl and the serum triglyce of hyperglycemic rats revealed 66.3 mg/dl, 62.3mg/dl. Those were decreased do dependently when compared with control group. The effect of Rx B on The body weight was not dose-dependently decreased dosedependently when by the administration of 100 mg/kg or 200 mg/kg fractions. But it was dose-dependantly decreased from $22.2{\times}4.9$ g to $18.9{\times}4.4$ g by the administration of 400 mg/kg fraction.

황칠, 닥나무, 꾸지뽕 혼합 추출물의 항당뇨 효과 (Antidiabetic Effects of Mixed Extract from Dendropanax morbiferus, Broussonetia kazinoki, and Cudrania tricuspidata)

  • Dengropanax morfiferus (D), Broussonitia kazinoki (B), and Cudriania tricuspidata (E), a widely cultivated species in South Korea, has been used as traditional medicine to treat numerous diseases. In this study, we evaluated the antidiabetic effects in a various signaling mechanisms using mixed extract and major component contents were analyzed by HPLC in the combined extracts from Dengropanax morfiferus, Broussonitia kazinoki, and Cudriania tricuspidata (DBCE). DBCE inhibited ${\alpha}$-glucosidase and ${\alpha}$-amylase activation and showed potent antioxidant effects, which are evaluated using DPPH, ABTS, and SOD assay. Cytokines, which are released by inflammatory cells in pancreatic islets, are involved in the pathogenesis of type 1 diabetes mellitus. DBCE showed the protective effects in RINm5F cells against cytokines-induced damage by suppressing inducible nitric oxide (NO) synthase and COX-2 expression and NO production. Insulin resistance is the primary characteristic of type 2 diabetes. Therefore, the regulatory effect of DBCE on glucose uptake and production are investigated in insulin-responsive human HepG2 cells. DBCE stimulated glucose uptake, prevented Glut2 and phosphor-IRS1 downregulation induced by high glucose (HG, 30 mM). Moreover, DBCE pretreatment diminished glucose levels, PEPCK and G6Pase overexpression provoked by HG. These findings suggest that DBCE might be used for diabetes treatment through alpha-glucosidase or alpha-amylase activity regulation, pancreatic beta cell protection, hepatic glucose sensitivity improvement. Cytokines, which are released by inflammatory cells' infiltrations around the pancreatic islets, are involved in the pathogenesis of type 1 diabetes mellitus.

오가나무 잎, 줄기의 항산화 및 항당뇨 효능 분석 (Antioxidant and antidiabetic effects of leaves and stems of Acanthopanax sieboldianum (Makino) Koidz)

  • The aim of this study was to investigate the potential of Acanthopanax sieboldianum (Makino) Koidz (ACS) as a potent antioxidant and antidiabetic agent. The antioxidative and alpha-glucosidase inhibitory activities were examined using the methanol extracts and solvent fractions from ACS-leaf and ACS-stem. Antioxidative activities were measured by in vitro methods such as DPPH and ABTS radical scavenging activity and superoxide dismutase (SOD) activity. When the chloroform and ethyl acetate fractions of ACS-leaf and ethyl acetate fractions of ACS-stem were compared with the control, the SOD-like activity was impaired even at the low treatment concentrations. In addition, the ethyl acetate fractions of ACS-leaf and ACS-stem showed alpha-glucosidase inhibition activities at low treatment concentrations. Analysis of the major components in the fractions of ACS-leaf and ACS-stem was also performed using HPLC. Finally, astragalin, isoqurecetin, chlorogenic acid and caffeic acid contents were measured. Based on this work, we propose that ACS-leaf and ACS-stem have great potential as natural antioxidant and antidiabetic materials related to health benefits.

당뇨병 치료제 후보약물 정보를 이용한 기계 학습 모델과 주요 분자표현자 도출 (A machine learning model for the derivation of major molecular descriptor using candidate drug information of diabetes treatment)

  • 본 연구는 당뇨병 치료제 후보약물 정보를 이용하여 항당뇨에 영향을 미치는 물질구조를 발견하는데 목적이 있다. 정량적구조 활성관계를 이용한 기계 학습 모델을 만들고 부분최소자승 알고리즘을 통해 실험데이터 별로 결정계수를 파악한 후 변수중요도척도를 활용하여 주요 분자표현자를 도출하였다. 연구 결과, 후보약물 구조정보를 반영한 molecular access system fingerprint 데이터로 분석한 결과가 in vitro 데이터를 이용한 분석 결과보다 설명력이 높았으며, 항당뇨에 영향을 미치는 주요 분자표현자 역시 다양하게 도출할 수 있었다. 제안된 항당뇨 예측 및 주요인자 분석 방법을 활용한다면 유사한 과정을 반복 실험하는 기존 신약개발 방식과는 달리, 많은 비용과 시간이 소요되는 후보물질 스크리닝 (screening) 기간을 최소화하고, 신약개발 탐색기간도 단축하는 계기가 될 수 있을 것으로 기대한다.

대사증후군 개선을 위한 뽕잎, 오디, 누에 분말의 혼합 비율 최적화 (Optimization of Mixing Ratio of Mulberry Leaf, Mulberry Fruit, and Silkworm for Amelioration of Metabolic Syndrome)

  • Objectives: The aim of this study was optimized mixing ratio of mulberry leaf, mulberry fruit, and silkworm for amelioration of the metabolic syndrome by using response surface method (RSM). Methods: Antioxidant, antidiabetic and antihypertensive activities of fifteen mixed powder of mulberry leaf, mulberry fruit, and silkworm by RSM were measured as indicators of metabolic syndrome. Results: The optimal mixing ratio of mulberry leaves, mulberry fruits, and silkworm with the greatest antioxidant, antidiabetic and antihypertensive activities was as follows: 2.5890 of mulberry leaf (A), 0.1222 of mulberry fruit (B), 2.9999 of silkworm (C). At this time, it was predicted that the total polyphenol content was estimated to be 185.51 tannic acid equivalent mg/g, 1, 1-diphenyl-2-picrylhy drazyl radical scavenging activity 84.77%, 1-deoxynojirimycin content 415.66 mg/100 g, ${\alpha}-glucosidase$ inhibitory activity 64.31%, ${\gamma}-aminobutyric$ acid content 267.77 mg/100 g, potassium content 1,899.11 mg/100 g, and angiotensin-converting-enzyme inhibitory activity was estimated to be 73.78%. Conclusions: It was concluded that the significant effect of the mulberry leaf, mulberry fruit, and silkworm on the metabolic syndrome-related biological activity indices (antioxidant activity, antidiabetic activity and antihypertensive activity) was as follows: 2.5890 of mulberry leaf (A), 0.1222 of mulberry fruit (B), 2.9999 of silkworm (C).

Psidium guajava L. leaf extract inhibits adipocyte differentiation and improves insulin sensitivity in 3T3-L1 cells

  • BACKGROUND/OBJECTIVES: Psidium guajava L. (guava) leaves have been shown to exhibit hypoglycemic and antidiabetic effects in rodents. This study investigated the effects of guava leaf extract on adipogenesis, glucose uptake, and lipolysis of adipocytes to examine whether the antidiabetic properties are mediated through direct effects on adipocytes. MATERIALS/METHODS: 3T3-L1 cells were treated with 25, 50, 100 ㎍/mL of methanol extract from guava leaf extract (GLE) or 0.1% dimethyl sulfoxide as a control. Lipid accumulation was evaluated with Oil Red O Staining and AdipoRed assay. Immunoblotting was performed to measure the expression of adipogenic transcription factors, fatty acid synthase (FAS), and AMP-activated protein kinase (AMPK). Glucose uptake under basal or insulin-stimulated condition was measured using a glucose analog 2-[N-(7-nitrobenz-2-oxa-1,3-diazol-4-yl) amino]-2-deoxy-D-glucose. Lipolysis from fully differentiated adipocytes was measured by free fatty acids release into the culture medium in the presence or absence of epinephrine. RESULTS: Oil Red O staining and AdipoRed assay have shown that GLE treatment reduced lipid accumulation during adipocyte differentiation. Mitotic clonal expansion, an early essential event for adipocyte differentiation, was inhibited by GLE treatment. GLE inhibited the expression of transcription factors involved in adipocyte differentiation, such as peroxisome proliferator-activated receptor 𝛄 (PPAR𝛄), CCAAT/enhancer-binding protein α (C/EBPα), and sterol regulatory element-binding protein-1c (SREBP-1c). FAS expression was also decreased while the phosphorylation of AMPK was increased by GLE treatment. In addition, GLE increased insulin-induced glucose uptake into adipocytes. In lipid-filled mature adipocytes, GLE enhanced epinephrine-induced lipolysis but reduced basal lipolysis dose-dependently. CONCLUSIONS: The results show that GLE inhibits adipogenesis and improves adipocyte function by reducing basal lipolysis and increasing insulin-stimulated glucose uptake in adipocytes, which can be partly associated with antidiabetic effects of guava leaves.

합병증을 동반하지 않은 2형 당뇨병에 대한 갈근황금황련탕의 혈당개선 효과와 안전성 연구 : 체계적 문헌고찰과 메타분석 (A Study on the Blood Sugar Improvement Effect and Safety of Galgunhwangryunhwanggum-tang for Type 2 Diabetes without Complications: A Systemic Review and Meta-Analysis)

  • Objectives: The purpose of this study is to assess the antidiabetic effect and safety of Galgunhwangryunhwanggum-tang for type 2 diabetes without complications by analyzing related research. Methods: For a systematic review and meta-analysis, we searched for the antidiabetic effect and safety of Galgunhwangryunhwanggum-tang for type 2 diabetes without complications in 10 databases up to September 2021. Only randomized controlled trials were chosen. Results: In the treatment effectiveness analysis and meta-analysis, Galgunhwangryunhwanggum-tang had significant improvement effects on fasting plasma glucose level, 2-hour postprandial glucose level, glycated hemoglobin, fasting insulin, and homeostasis model assessment for insulin resistance compared to the control group when treated in parallel with oral glycemic drugs. Conclusion: Galgunhwangryunhwanggum-tang is effective in improving blood sugar and insulin resistance in type 2 diabetes patients without complications and can especially be considered in parallel treatment with oral hypoglycemic drugs. A large-scale randomized controlled clinical trial is required to complement the limitations presented in this study in the future.

Antidiabetic Effect of Standardized Chrysanthemum rubellum Hydroethanolic Extract by Targeting α-Glucosidase and the PTP-1B Signaling Pathway for Alleviating Diabetes in Experimental Model

  • Objectives: The study's goal was to find out whether Chrysanthemum rubellum extract has anti-diabetic properties by concentrating on α-glucosidase and the PTP-1B signaling pathway. C. rubellum flowers were used for extraction using Methanol/water (80/20) as solvent. Methods: LC-MS techniques was used to check the presence of phytoconstituents present in C. rubellum extract. In vitro antidiabetic activity was evaluated using α-glucosidase inhibitory activity and PTP-1B signaling pathway. On Streptozotocin (STZ)-induced rats with diabetes, the in vivo antidiabetic efficacy was assessed using a test for oral glucose tolerance. Results: The phytoconstituents identified in the extract of C. rubellum were apigenin, diosmin, myricetin, luteolin, luteolin-7-glucoside, and Quercitrin as compound 1-6, respectively. Results showed that diosmin exhibited highest α-glucosidase inhibitory activity i.e. 90.39%. The protein level of PTP-1B was lowered and the insulin signalling activity was directly increased by compounds 1-6. The maximum blood glucose levels were seen in all groups' OGTT findings at 30 minutes following glucose delivery, followed by gradual drops. In comparison to the control group, the extract's glucose levels were 141 mg/dL at 30 minutes before falling to 104 mg/dL after 120 minutes. The current study has demonstrated, in summary, that extract with phytoconstituents reduce blood sugar levels in rats. Conclusion: This finding suggests that extract may reduce the chance of insulin resistance and shield against disorders like hyperglycemia.
