Analysis of Effects of Chemotherapy using Failure Mode and Effect Analysis (FMEA) on Patient Safety and Safe Nursing (고장유형영향분석을 활용한 항암화학요법의 환자안전간호 효과분석)

  • Purpose: This study was done to apply failure mode & effect analysis (FMEA) to chemotherapy in order to reduce prescribing, dispensing and administering errors related to treatment and provide patients with a safe medical environment. Methods: A one group pre-post test design was used to verify the effects using the tool for FMEA in chemotherapy. Results: There was a statistically significant decrease in prescribing errors from 11.47% to 3.18%; administering errors decreased but they were not statistically significant. In a addition, there was no change in dispensing errors. Conclusion: The results show that FMEA removed risk factors that might occur during the process of chemotherapy and that it was an effective tool for prevention of negligent accident occurring in actual patients.

  • Evaluated are errors of the conformal mapping method in the analysis of a coplanar waveguides and asmmetric coplanar strips. The errors are mainly due to the negligence of fringe field effect after electrode structures are transformed to parallel electrodes. For the purpose of comparison, results of the conformal mapping(CM) method are compared with those of the successive over relaxation(SOR) method.

  • Memory errors can cause not only program malfunctions but also even unexpected system halt. Though a programmer checks memory errors, some memory errors with low occurrence frequency are missed to detect. In this paper, we propose a method for effectively detecting such memory errors using instruction transition diagrams through analyzing assembly codes obtained by disassembling an executable file. Out of various memory errors, local memory return errors, null pointer access errors and uninitialized pointer access errors are targeted for detection. When applying the proposed method to various programs including well-known open source programs such as Apache web server and PHP script interpreter, some potential memory errors are detected.

  • In clinical dental research, errors occur in spite of careful study design and conduct. Data cleaning procedures intend to identify and correct these errors or at least to minimize their influence on study. Outlier is the one of these errors. Outlier detection is the first step in data analysis process which has a serious effect in the field of dental research. Hence, this paper aims to introduce the methods to detect the outliers and to examine their influences in statistical data analysis.

Analysis of Chemotherapy Prescribing Errors Detected by Pharmacists (약사에 의해 탐지된 항암제 처방오류 분석)

  • Objective: The purpose of this study was to identify the type and frequency of chemotherapy-related prescribing errors and assess the pharmacist intervention in preventing potential harm. Methods: This study was performed in satellite pharmacy of oncology/hematology unit in tertiary teaching hospital from April to September, 2009. All chemotherapy prescribing errors detected by pharmacists were recorded. Frequency and characteristics of prescribing errors were analyzed. Pharmacists reviewed 28, 495 chemotherapy orders from 12,719 patients during 6-month periods. Results: A total of 835 prescription errors (2.93%) in 734 patients (5.77%) were detected by pharmacists. Alkylating agents (37.6%) followed by antimetabolite (23.35%), and mitotic inhibitors (21.44%) were the most prevalent classes in which errors occurs. The most common types of error detected were incorrect dose (34%), incorrect solution (33%), incorrect route (9%) and omission errors (8%). Changes in chemotherapy order due to pharmacists' intervention occurred in all error cases. Conclusion: Pharmacists' intervention in reviewing chemotherapy and drug orders intercepted potential harm due to prescribing errors. The current study provided strategies for reduction of medication errors.

  • The purpose of this study is to look for the plan of reconstructing a textbook through error analysis and the process of its correction in numbers and operations of the first grade. The Research materials are collected and analyzed through the journal about every lessons, the recording sheets of students' activity, the recording videotapes during lessons, the individual interview and observation. This study investigated 4 errors which are useful for reconstructing textbook, the errors of understanding relation between numerical expression and number line, the errors of drawing-strategy, the errors of understanding relation between additive expression and subtractive expression, the errors of subtraction has to be regrouped. The errors are classified into some types and analyzed focusing on content of each error. Reinstructing are carried out based on material analyzed for correcting errors.

Types of Errors on the Mathematical Ability Picture Test for Young Children (유아그림수학능력검사 과정에서의 오류 유형에 관한 연구)

  • This study examined types of errors made by young children on the Mathematical Ability Picture Test for Young Children(Hwang & Choi, 2007). The subjects were 30 5-year-old children. The process of taking the Mathematical Ability Picture Test was videotaped and the videotape was transcribed for analysis of errors. Findings were that in most of the sub-areas, strategy selection errors were dominant, followed by comprehension errors. Second, there was a tendency for boys and girls to make similar types of errors in every sub-area of the Mathematical Ability Picture Test for Young Children.

Measurement of the Volumetric Thermal Errors for CNC Machining Center Using the Star-type-styluses Tough Probe

  • One of the major limitations of productivity and quality in metal cutting is the machining accuracy of machine tools. The machining accuracy is affected by geometric errors, thermally-induced errors, and the deterioration of the machine tools. Geometric and thermal errors of machine tools should be measured and compensated to manufacture high quality products. In metal cutting, the machining accuracy is more affected by thermal errors than by geometric errors. This paper models the thermal errors for error analysis and develops an on-the-machine measurement system by which the volumetric errors are measured and compensated. The thermal error is modeled by means of angularity errors of a column and thermal drift error of the spindle unit which are measured by the touch probe unit with a star type styluses and a designed spherical ball artifact (SBA). Experiments show that the developed system provides a high measuring accuracy, with repeatability of $\pm$2$\mu\textrm{m}$ in X, Y and Z directions. It is believed that the developed measurement system can be also applied to the machine tools with CNC controller. In addition, machining accuracy and product quality can be also improved by using the developed measurement system when the spherical ball artifact is mounted on a modular fixture.

THE PRE-SERVICE SECONDARY TEACHERS' PRESCRIPTION FOR THE MIDDLE SCHOOL STUDENTS' ERRORS IN LINEAR FUNCTIONS

  • This study was subjected to 9th graders after making a conformity analysis about errors in function from a selected linear function domain learned in 8th grade, and using this we analyzed some errors learners have in the linear function domain. Learners showed the most deficiency in mastery of prerequisite facts concepts out of errors in linear functions and lack of skill in interpreting the content of the questions and technical errors occurred often as well. How the pre-service secondary school teachers prescribed these errors of linear function was analyzed from the point of problem solving strategies, accessing methods and whether or not the learner's error was used. Looking into the pre-service secondary teachers' prescription of the learners' errors in 3 fields, for the problem solving strategy a procedural strategy was used more than a conceptual strategy, and as for the accessing methods over 90% gave teacher led type explanations to the students. Also over 90% of pre-service secondary teachers did not use the learner's errors that turned up in problems.
