A Preliminary Study on the Korean Version of Quality of Life-Alzheimer's Disease (QOL-AD) Scale in Community-dwelling Elderly with Dementia (지역사회 거주 치매환자에서 한국판 삶의 질 -알쯔하이머병 척도 개발을 위한 예비연구)

  • Objectives: The Quality of Life-Alzheimer's Disease (QOL-AD) scale is a reliable and valid tool for assessing the quality of life (QOL) in the elderly with dementia. This study aimed to develop the Korean version of Quality of Life-Alzheimer's Disease (KQOL-AD) scale for the demented elderly living in the community. Methods: KQOL-AD was administered to two groups: 24 demented elderly and 72 cognitively impaired elderly with no dementia (CIND) who were living in the community Each elderly person and their caregiver rated the elderly QOL. The Korean version of mini-mental state examination (MMSE-K), the clinical dementia rating (CDR), the activities of daily living (ADL), and the neuropsychiatric inventory (NPI) were also assessed. The reliability and validity of the KQOL-AD were examined. Results: In the dementia group, the internal consistency (Cronbach's $\alpha$), the split half and the test-retest reliabilities of the KQOL-AD were excellent. Scores on the KQOL-AD were significantly correlated with the scores of the NPI, but they were not significantly correlated with scores of the MMSE-K, CDR and ADL. In addition, the CIND group showed similar results to the dementia group. Conclusions: KQOL-AD might be a reliable and valid instrument for assessing QOL in the elderly with dementia It could be used as an important outcome measure for research on the demented elderly.

On the Early Diagnosis of Dementia by Nonlinear Analysis of the EEG in Alzheimer's Disease (알츠하이머 환자 뇌파의 비선형 분석을 통한 치매증의 조기진단에 관한 연구)

  • The early diagnosis has an very important role in curing dementia. But there was not the effective method to diagnose it until now. In this paper we analyzed the EEG of Alzheimer's disease patients and normal groups by nonlinear methods. In the analysis we calculated the correlation dimensions $D_2$ and the largest Lyapunov exponent $L_1$. We found that patients with Alzheimer's disease have significantly lower $D_2$ and TEX>$L_1$ than normal groups. It means that brains injured by Alzheimer's disease have electrophysiological inactive elements and have decreased chaotic behaviour. We propose the nonlinear analysis of the EEG as a useful tool for the early diagnosis of Alzheimer's disease.

Analysis of Clinical Korean Medicine Studies on Dementia : Focused on Korean Literatures for 10 Years (치매(痴呆)에 대한 임상 한의학 연구 경향 분석 : 지난 10년간의 국내 논문을 중심으로)

  • Objective : In this article, we overviewed clinical Korean Medicine studies on dementia by analyzing papers published in Korea from 2010 to 2019. Methods : We explored research articles on dementia from 5 Korean research databases using the keyword 'dementia' and 'Korean Medicine'. We included original articles and reviews of clinical Korean Medicine and excluded single case study and experiment article of animal model. Then we analyzed them according to the published year, journal type, and research topics. Results & Conclusions : Research topics were classified into 'research trend', 'treatment', 'prevention', 'evaluation', 'medical expenses', 'collaborative practice' and 'others'. The largest number of articles were published in 2011(24.4%). 'Journal of Oriental Neuropsychiatry' was the journal with the most published articles(46.3%). In research topics, the analyses of research trend accounted for the largest portion(39.0%) and the articles about treatment for dementia or evaluation index in Korean medicine had the second largest proportion(14.6%). We hope that various researches for therapeutic effects, mechanisms and preventive effects of Korean Medicine in dementia would be conducted for future study.

Effects of Added Chongmyung-tang on Behavior and Molecular Factors in the Alzheimer's Disease Model (ACM의 알츠하이머 생쥐 모델의 행동과 생체인자에 미치는 영향)

  • This experiment was designed to investigate the effect of Added Chongmyung-tang (ACM) on Alzheimer's disease mouse model. Effects of ACM on learning behavior were investigated using the Morris water maze method. Expression levels of molecular factors related to Alzheimer's disease such as glial fibrillary acidic protein (GFAP), cluster of differentiation antigen 68 (CD68), and tau protein in the hippocampus of APP-SWE Tg2576 mice were analyzed by immunofluorescence staining method. ACM reduced escape latency in the Morris water maze test. ACM decreased the expression level of GFAP and tau protein in the hippocampus. These results suggest that ACM may be involved in regulating molecules that are known to play an important role in the pathogenesis of Alzheimer's disease.

Dementia Classification by Distance Analysis from the Central Coronal Plane of the Brain Hippocampus (뇌 해마의 관상면 중심점으로부터 거리분석에 따른 치매분류)

  • Alzheimer's disease has the significant factors for the both specific and characteristic features according to the disease progressing that are the volumetry and surface area by the brain hippocampus shrinking and thinning. However, we have suggested a shape analysis to calculate the variance by the roughness, coarseness or uneven surface on 3D MR images. For the reasons we have presented two methods: the first method is the distance calculation from major axis to edge points and the second method is the distance calculation from centroidal point to edge points on a coronal plane. Then we selected the shortest distance and the longest distance in each slice and analyzed the ANOVA and average distances. Consequently we obtained the available and great results by the longest distance of the axial and centroidal point. The results of average distances were 44.85(AD), 45.04(MCI) and 49.31(NC) from the axial points and 39.30(AD), 39.58(MCI) and 44.78(NC) from centroidal points respectively. Finally the distance variations for the easily recognized visualization were shown by the color mapping. This research could be provided an indicator of biomarkers that make diagnosis and prognosis the Alzheimer's diseases in the future.

Evaluation of Pharmacotherapy for the Patients with Depression in Alzheimer's Disease, Parkinson's Disease or Vascular Dementia (알츠하이머병, 파킨슨병 및 혈관성치매 환자들의 우울증에 대한 약물 치료 평가)

  • Background: Prevalence of depression comorbid with neurologic disorders such as Alzheimer' disease (AD), Parkinson's disease (PD) and vascular dementia (VD) is higher than that of primary depression. Antidepressant medications, suggested by many researches for depression comorbid with neurologic disorders such as AD, PD and VD, are mainly selective serotonin reuptake inhibitors (SSRI). Objective: The primary objective of this study is the evaluation of antidepressant drug therapy for AD, PD and VD. Method: This study was a retrospective study based on medical records, carried out for 3 years and 6 months (Jan. 2007~Jul. 2010). Patients, diagnosed as comorbid depression through Beck Depression Inventory (BDI), Cornell Depression Scale (CDS), Geriatric Depression Scale (GDS) among neurologic out-patients of Chungnam National University Hospital because of AD, PD and VD, were selected. The results were evaluated by efficacy and safety of antidepressant drug therapy. Results: In result, the prescribing rates of antidepressants were 30%, 55% and 40% for each AD, PD and VD. Depression cure rates of patients receiving antidepressants vs patients not receiving antidepressants were 40% vs 39%, 33% vs 23% and 38% vs 30% for AD, PD and VD. The frequencies of prescriptoin of SSRI were 21%, 11% and 27% for each AD, PD and VD. The frequencies of prescriptoin of benzodiazepine (BZD) was 61%, 82% and 61% for each AD, PD and VD. The ratio of single BZD prescription was more than that of combination prescription of antidepressants. Tricyclic antidepressants (TCA) were rarely prescribed. The rate of patients with BZD-related side effects was 54%. The most frequent side effects of BZD were dizziness (30%), drowsiness (21%) and headache (16%). Side effects of SSRI were rare. Conclusion: In conclusion, the frequencies of prescription of antidepressants were not common for AD, PD and VD. There was little difference in depression cure rate between patient receiving antidepressants and not receiving. Even though SSRI has to be the highest priority of usage, the frequencies of prescription of SSRI were lower than those of BZD. Additional researches and efforts are required to improve antidepressant drug therapy for neurologic disorders such as AD, PD and VD.

Inhibitory Effects of Extract of Rosae Rugosae Flos on the Acetylcholinesterase, β-secretase, and β-amyloid Aggregation (매괴화(玫瑰花) 추출물의 아세틸콜린에스테라제, 베타 시크리타제 및 베타 아밀로이드 응집에 대한 저해효과)

  • Alzheimer's disease(AD), one of the most common forms of dementia, is characterized pathologically by the presence of intracellular neurofibrillary tangles and deposition of β-amyloid(Aβ) peptides of 40-42 residues. Aβ has been believed to be neurotoxic and now is also considered to have a role on the mechanism of memory dysfunction. Only a few acetylcholinesterase(AChE) inhibitors have been developed for treatment of AD, although the numbers of patients are rapidly increasing within aging society. Here, we show that ethanol extract of Rosae Rugosae Flos(RR) or its butanol fraction reduce the enzyme activity of AChE and BACE1(β-site APP cleaving enzyme 1). Furthermore, We found that RR inhibits Aβ aggregation and removes Aβ aggregates by Transmission electron microscopy(TEM). In addition, RR reduces the free radical of 2, 2-diphenyl-1-picrylhydrazyl(DPPH). We suggest that Rosae Rugosae Flos may be useful as a herbal medicine to treat AD.

Review on Wandering Behavior in Persons with Dementia (치매 노인의 배회행동에 관한 문헌 조사)

  • In Korea, about 8.3% over aged 65 are suffering from Alzheimer's disease or other type of dementia. Among dementia-related behaviors, wandering is the one of strongest factors on burden and stress of caregivers. On Lee and Kwon's report with community living persons with dementia, upto 85% of caregivers reported wandering as a problem. Wandering is a frequently encountered problem in communities and long-term care facilities, but it is among least understood dementia. related behavior. Despite the prevalence of wandering, its significant adverse outcomes, and the increase in persons with dementia in Korea, no systematic research has been conducted in Korea. The purpose of this study is to review on literature related to wandering behavior among persons with dementia. The specific topics related to wandering are included: definition, the prevalence of dementia and wandering behavior, the influencing factor on wandering, outcomes of wandering behavior, and the measurement method. Wandering is defined as "aimless walking" and "Meandering, aimless or repetitive locomotion that exposes one to harm and is incongruent with boundaries, limits, or obstacles". Wandering is viewed as a problematic behavior, however, it has to be understood as need-driven compromised behavior. For example, wandering may be an expression of searching for familiar person and/or place. Recently, in Korea, there is an effort for establishing the therapeutic environment for elders who are wanderers in long-term-care facilities. Cognitive impairment of persons with dementia is found to be a consistent factor on wandering behavior through many national and international studies. The adverse outcomes of wandering are serious problem in persons with dementia as well as their caregivers. The adverse outcomes include falls, fractures, getting lost, use of restraints, or even death. In fact, wandering is one of the major reasons for a patient to be institutionalized. For measurement of wandering behavior, two methods are broadly used: observation using stop watch, and survey form. A revised instrument of the Korean version of Algase wandering scale (K_RAWS) is established the psychometric properties (Son, Song, & Lim, 2006) demonstrating valid and reliable instrument in measuring wandering behavior among persons with dementia who are residing in communities. K_RAWS has a 39 items with six subscales including persistent walking, repetitive walking, spatial disorientation, eloping behavior, negative outcome, and mealtime impulsivity. In conclusion, studies including the prevalence of wandering behavior and predictive factors on wandering should be conducted to understand wandering clearly before developing any types of intervention.

Investigation of the effect of Erythrosine B on a β-amyloid (1-40) peptide using molecular modeling method

  • Alzheimer's disease is one of the most common types of degenerative dementia. As a considerable cause of Alzheimer's disease, neurotoxic plaques composed of 39 to 42 residue-long amyloid beta($A{\beta}$) fibrils have been found in the patient's brain in large quantity. A previous study found that erythrosine B (ER), a red color food dye approved by FDA, inhibits the formation of amyloid beta fibril structures. Here, in an attempt to elucidate the inhibition mechanism, we performed molecular dynamics simulations to demonstrate the conformational change of $A{\beta}40$ induced by 2 ERs in atomistic detail. During the simulation, the ERs bound to the surfaces of both N-terminus and C-terminus regions of $A{\beta}40$ rapidly. The observed stacking of the ERs and the aromatic side chains near the N-terminus region suggests a possible inhibition mechanism in which disturbing the inter-chain stacking of PHEs destabilizes beta-sheet enriched in amyloid beta fibrils. The bound ERs block water molecules and thereby help stabilizing alpha helical structure at the main chain of C-terminus and interrupt the formation of the salt-bridge ASP23-LYS28 at the same time. Our findings can help better understanding of the current and upcoming treatment studies for Alzheimer's disease by suggesting inhibition mechanism of ER on the conformational transition of $A{\beta}40$ at the molecular level.

A Case Study on Drug Prescription for Vascular Dementia in Western and Oriental Medicine (혈관성치매에 대한 한.양방 치료약물의 처방 사례연구)

  • Objectives : Vascular dementia is the second common cause of dementia after Alzheimer's disease. It assumed that the ratio of prescription drugs on vascular dementia are quite different from each hospital in Western medicine or Oriental medicine, respectively. The aim of this research is to collect and analyze the ratio of prescription drugs on vascular dementia in Western medicine or Oriental medicine in university hospitals. Methods : We collected and analyzed the data related to prescriptions on vascular dementia in the department of neurology in three university hospitals(A, B, C) and in the department of internal medicine in two Oriental medicine hospitals(Daegu Haany Oriental Hospital, Dong Eui Oriental Hospital). Results : In the department of neurology in A university hospital, donepezil(69.1%), memantine(14.0%), rivastigmine(12.3%), galantamine(4.5%) were prescribed in order. In B university hospital, galantamine(57.8%) donepezil(33.3%), rivastigmine(6.7%), donepezil with memantine(4.4%) were prescribed. In C university hospital, donepezil(62.0%), rivastigmine(25.0%), galantamine(7.0%) memantine(6.0%) were prescribed. The average frequencies of prescribed medication in the department of neurology in A, B, C university hospitals were donepezil(54.8%), galantamine(23.1%), rivastigmine(14.7%), memantine(7.4%). In Oriental medicine hospitals, various prescriptions have been used for vascular dementia. Among them, Ansincheongnoetang (安神淸腦湯) and Gamijihangeumja(加味地黃飮子) were often prescribed in Daegu Haany Oriental Hospital, and Bojungikgitang(補中益氣湯) in Dong Eui Oriental Hospital, too. Conclusions : Cholinesterase inhibitors such as donepezil, galantamine, rivastigmine and NMDA receptor inhibitor like memantine have been used as a drug of choice for vascular dementia in all surveyed university hospitals. In oriental hospitals, various prescriptions have been used for vascular dementia.
