Efficacy of intraosseous saline injection for pain management during surgical removal of impacted mandibular third molars: a randomized double-blinded clinical trial

  • Background: Surgical extraction of impacted mandibular third molars is the most common procedure performed by oral surgeons. The procedure cannot be performed effectively without achieving profound anesthesia. During this procedure, patients may feel pain during surgical bone removal (at the cancellous level) or during splitting and luxation of the tooth, despite administration of routine nerve blocks. Administration of intraosseous (IO) lignocaine injections during third molar surgeries to provide effective anesthesia for pain alleviation has been documented. However, whether the anesthetic effect of lignocaine is the only reason for pain alleviation when administered intraosseously remains unclear. This conundrum motivated us to assess the efficacy of IO normal saline versus lignocaine injections during surgical removal of impacted mandibular third molars. The aim of this study was to assess the efficacy of IO normal saline as a viable alternative or adjunct to lignocaine for alleviation of intraoperative pain during surgical removal of impacted mandibular third molars. Methods: This randomized, double-blind, interventional study included 160 patients who underwent surgical extraction of impacted mandibular third molars and experienced pain during surgical removal of the buccal bone or sectioning and luxation of the tooth. The participants were divided into two groups: the study group, which included patients who would receive IO saline injections, and the control group, which included patients who would receive IO lignocaine injections. Patients were asked to complete a visual analog pain scale (VAPS) at baseline and after receiving the IO injections. Results: Of the 160 patients included in this study, 80 received IO lignocaine (control group), whereas 80 received IO saline (study group) following randomization. The baseline VAPS score of the patients and controls was 5.71 ± 1.33 and 5.68 ± 1.21, respectively. The difference between the baseline VAPS scores of the two groups was not statistically significant (P > 0.05). The difference between the numbers of patients who experienced pain relief following administration of IO lignocaine (n=74) versus saline (n=69) was not statistically significant (P > 0.05). The difference between VAPS scores measured after IO injection in both groups was not statistically significant (P >0.05) (1.05 ± 1.20 for the control group vs. 1.72 ± 1.56 for the study group) Conclusion: The study demonstrates that IO injection of normal saline is as effective as lignocaine in alleviating pain during surgical removal of impacted mandibular third molars and can be used as an effective adjunct to conventional lignocaine injection.

The Relationship Between Foreign Student Academic Stress and Depression Tendency: The Mediating Effect of Self-Esteem (외국인 유학생의 학업스트레스와 우울과의 관계에서 자아존중감의 매개효과)

  • Objectives: The purpose of this study was to examine the mediating effect of self-esteem on the relationship between academic stress and depression in foreign students in Korea. Methods: The subjects of this study were 343 foreign students enrolled in universities and graduate schools in Daegu and Gyeongbuk area, and data were collected by conducting a questionnaire survey using the internet. The collected data was analyzed using SPSS 23.0 and PROCESS macro Model No 4. Results: The results were as follows. First, academic stress of foreign students increased the level of depression statistically significantly. Second, academic stress significantly reduced the self-esteem of foreign students. Third, the self-esteem of foreign students partially mediated the effect of academic stress on depression (p<0.05). In other words, it suggests that academic stress lowered self-esteem, and that the lowered self-esteem had an effect on the increase in depression. Conclusions: These results suggest that an intervention program to reduce academic stress is needed as a countermeasure for the alleviation and prevention of depression in foreign students, and that strategies for improving self-esteem should be considered as important.

The Effects of Exercise Therapy and Transcutaneous Electrical Nerve Stimulation for the Alleviation of Low Back Pain After Coronary Angiography (관동맥 조영술 후 요통완화를 위한 운동요법과 경피적 전기 신경자극의 효과)

  • Background and Purpose: After the coronary angiography procedure, patients are required to remain on bed rest to reduce the risk of bleeding and hematoma formation at the puncture site. This prolonged bed rest in the supine position is difficult for many patients, who frequently complain of low back pain. The purpose of the study was to determine whether a specially designed exercise therapy and transcutaneous electrical nerve stimulation (TENS) had an effect on the alleviation of low back pain. Method: Sixty-two patients were assigned to one of three groups : specially designed exercise therapy plus TENS plus general nursing care (exercise group N=21), general nursing care plus TENS (TENS group, N=23) or general nursing care (control group, N=18). The exercise therapy consisted of five movements including stretching, pelvic tilting, knee to chest, modified situps and trunk rotation with minimizing the motion of the puncture site. The severity of low back pain was assessed by a visual analogue scale(VAS) every two hours. The use of analgesic and any development of bleeding or other complications were monitored as well. The level of serum ${\beta}$-endorphin was determined before and after the three interventions. Result: The pain score of the exercise group was significantly lowered compared to that of the other groups. There was no difference in the serum ${\beta}$-endorphin level among three groups. Analgesic were less frequently taken by the exercise group. However the incidence of bleeding complications was not significantly different among the three groups. Conclusion: Exercise therapy is more effective than general care or TENS in alleviating low back pain of the patients with coronary angiography.

Effect of Bacillus aryabhattai H26-2 and B. siamensis H30-3 on Growth Promotion and Alleviation of Heat and Drought Stresses in Chinese Cabbage

  • Plants are exposed to biotic stresses caused by pathogen attack and complex abiotic stresses including heat and drought by dynamic climate changes. To alleviate these stresses, we investigated two bacterial stains, H26-2 and H30-3 in two cultivars ('Ryeokkwang' and 'Buram-3-ho') of Chinese cabbage in plastic pots in a greenhouse. We evaluated effects of bacterial strains on plant growth-promotion and mitigation of heat and drought stresses; the role of exopolysaccharides as one of bacterial determinants on alleviating stresses; biocontrol activity against soft rot caused by Pectobacterium carotovorum subsp. carotovorum PCC21. Strains H26-2 and H30-3 significantly increased fresh weights compared to a $MgSO_4$ solution; reduced leaf wilting and promoted recovery after re-watering under heat and drought stresses. Chinese cabbages treated with H26-2 and H30-3 increased leaf abscisic acid (ABA) content and reduced stomatal opening after stresses treatments, in addition, these strains stably colonized and maintained their populations in rhizosphere during heat and drought stresses. As well as tested bacterial cells, exopolysaccharides (EPS) of H30-3 could be one of bacterial determinants for alleviation of tested stresses in Chinese cabbages, however, the effects were different to cultivars of Chinese cabbages. In addition to bacterial activity to abiotic stresses, H30-3 could suppress incidence (%) of soft rot in 'Buram-3-ho'. The tested strains were identified as Bacillus aryabhattai H26-2 and B. siamensis H30-3 based on 16S rRNA gene sequence analysis. Taken together, H26-2 and H30-3 could be candidates for both plant growth promotion and mitigation of heat and drought stresses in Chinese cabbage.

The Effects of Alpiniae Oxyphyllae Fructus on Osteoporosis and Muscle Dystrophy of Male Mice (수컷 생쥐의 골다공증과 근위축에 대한 익지인(益智仁)의 효과)

  • Objective: To investigate the effect of Alpiniae oxyphyllae fructus (AOF) on the alleviation of musculoskeletal disorders caused by aging, we conducted experiments on osteoporosis and muscle atrophy. Methods: The experimental group was classified into a control group, aging-elicited (AE) group and AOF group. The control group comprised 8-week-old Institute of Cancer Research (ICR) mice. The AE and AOF groups were ICR mice at 50 weeks of age. For the AE group, 10 mL of distilled water was administered once a day for 180 days without any treatment. An AOF extract (0.54 g/kg) was dissolved in distilled water and administered to the mice in the AOF group once a day for 180 days. Results: In the experiment on the alleviation of osteoporosis, the distribution of glucosaminoglycan in the bone matrix of the femoral bone was increased in the AOF group; moreover, the osteocalcin (OCN) positive reaction was increased and 8-OHdG positivity was decreased. In addition, AOF positively decreased RANKL, positively increased OPG, and positively decreased MMP-3. Muscle fiber loss in the endomysium following muscle degeneration of the quadriceps was reduced more in the AOF group compared with the AE group, and caspase-3 positive responses were also decreased. In addition, the 8-OHdG and p-lkB positivity in the AOF group decreased compared with the AE group, and the Myo-D positivity increased. Conclusion: We found that increasing bone formation alleviates osteoporosis, and that reducing bone loss alleviates muscle atrophy by reducing muscle loss and increasing muscle development.

Hepatoprotective Activity of Salidroside Isolated from Acer Termentosum Max on D-galactosamine Induced Hepatotoxicity in Rats (산겨릅나무에서 분리한 salidroside의 간보호 효과)

  • Acer tegmentosum Max which is one of the specialized wildness medicinal herbs in gangwon province, has been widely used for hepatitis, liver cirrhosis, hepatic cancer, leukemia, diabetes mellitus, renal necrosis and edema, etc. In this study, the antioxidative and hepatoprotective effects of in vitro and in vivo were investigated in order to evaluate the possibility as hepatoprotective agents. Oral administration of methanol and butanol extact of Acer tegmentosum Max to d-galactosamine (D-GaIN) induced experimental liver injured rats was significantly reduced activities of marker enzymes(AST, ALT) and LDH activity in serum. Salidroside(Sal) isolated from the BuOH extract of Acer termentosum Max potenty showed the scavenzing effect on DPPH and inhibitory effect on lipid peroxidation. And significantly decrcease of MDA level in liver and activities of SOD GSH-Px and catalase were significantly improved by the treatment of Sal. Results of this study revealed that Sal could afford a significant protection in the alleviation of D-GaIN-induced hepatocellular injury.

Hypoglycemic Effect of Padina arborescens Extract in Streptozotocin-induced Diabetic Mice

  • This study investigated the hypoglycemic effect of the Padina arborescens extract in STZ-induced diabetic mice. Freeze-dried Padina arborescens were extracted with 80% methanol and concentrated for use in this study. The hypoglycemic effect was determined by inhibitory activities against ${\alpha}$-glucosidase and ${\alpha}$-amylase as well as the alleviation of postprandial blood glucose level. Padina arborescens extracts showed higher inhibitory activities than acarbose, a positive control against ${\alpha}$-glucosidase and ${\alpha}$-amylase. The $IC_{50}$ values of Padina arborescens extracts against ${\alpha}$-glucosidase and ${\alpha}$-amylase were 0.26 and 0.23 mg/mL, respectively, which evidenced as more effective than observed with acarbose. The increase of postprandial blood glucose levels were significantly suppressed in the Padina arborescens extract administered group than the control group in the streptozotocin induced diabetic mice. Furthermore, the area under the curve (AUC) was significantly lowered via Padina arborescens extract administration in diabetic mice (p 0.05). These results indicated that the Padina arborescens extract might be used as an inhibitor of ${\alpha}$-glucosidase and ${\alpha}$-amylase and delay absorption of dietary carbohydrates.

Inhibitory effect of epigallocatechin from Camellia sinensis leaves against pro-inflammatory mediator release in macrophages

  • To investigate the anti-inflammatory activity of natural products, we determined the anti-inflammatory activity of purified epigallocatechin (EGC) from Camellia sinensis leaves. In the present study, we found that EGC inhibited the production of proinflammatory mediators (IL-6, TNF-${\alpha}$, NO, and $PGE_2$) in lipopolysaccharide (LPS)-stimulated Raw 264.7 cells. Suppression of IL-6 seems to be at least partly attributable to the inhibitory effect of EGC. TNF-${\alpha}$ is a major cytokine produced by LPS-induced macrophages, and they have a wide variety of biological functions including regulation of inflammation. The inhibition of IL-6 and TNF-${\alpha}$ production by EGC may downregulate the acute-phase response to LPS, thereby reducing LPS-induced inflammation. In addition to IL-6 and TNF-${\alpha}$, EGC effectively reduced the production of other key inflammatory mediators, including NO and $PGE_2$. The inhibitory effect of EGC on NO and $PGE_2$ production was supported by the suppression of inducible nitric oxide synthase and COX-2 at protein levels. These results support the traditional use of EGC in the alleviation of various inflammation-associated diseases and suggest that EGC might be useful in the development of new functional foods for inflammatory diseases.

Protective Effect of Albizzia julibrissin Leaf Extract on the Cytotoxicity Induced by Cupric Acetate Metallic Mordant (금속매염제인 초산구리의 세포독성에 대한 자귀나무잎 추출물의 보호 효과)

  • Objectives: This study assessed the cytotoxicity of the metallic mordant cupric acetate (CA) and the protective effect of Albizzia julibrissin (AJ) leaf extract on CA-induced cytotoxicity in NIH3T3 fibroblasts. Methods: For this study, cell viability and antioxidative effects such as the inhibitory activity of lipid peroxidation (LP) and superoxide anion-radical (SAR) scavenging activity were assessed. Results: CA significantly decreased cell viability in a dose-dependent manner, and the $XTT_{50}$ value was measured as $55.0{\mu}M$ of CA. The cytotoxicity of CA was determined as highly toxic by Borenfreund and Puerner's toxic criteria. The catalase antioxidant significantly increased cell viability diminished by CA-induced cytotoxicity in these cultures. Regarding the protective effect of AJ leaf extract on CA-induced cytotoxicity, AJ leaf extract remarkably increased the SAR scavenging ability and the inhibitory ability of LP. From these findings, it is suggested that oxidative stress is involved in the cytotoxicity of CA, and AJ leaf extract effectively protected CA-induced cytotoxicity via antioxidative effects. Conclusions: Natural resources like AJ leaf extract may be a putative therapeutic agent for treatment or alleviation of the toxicity induced by CA metallic mordant.

A clinical study based on Sasang Consitutional Medicine on the treatment of atopic dermatitis (아토피 피부염 치료에 관한 사상의학적 임상연구)

  • Purpose: Atopic dermatitis, which is one the prevalent allergic diseases in toddlers and adolescent population, has not been treated with Sasang Constitutional Medicine due to the limitation of constitution differentiation in children. This study was aimed to examine the effect of treatment for atopic dermatitis with Sasang Constitutional Medicine. Method: Forty-three patients over age 3 who were diagnosed with atopic dermatitis by the criteria of Hanifin and Rajka were participated in this study from August, 2001 thru May, 2002 at the Pohang Oriental Medicine Hospital, Kyungsan University. Result: 1. Soyangin showed the highest proportion of 79% of the sample. 2. The symptom of the disease by patients' constitution according to the degree of affected skin and itching was severe in Soyangin, and mild in Soeumin 3. For treatment 9 kinds of prescription regimen were given; Yangkyuksanhwatang was mostly used for Soyangin followed by Hyungbangsabaksan. Hayngsayangwetang and Galgunhaegitang were prescribed for Soeumin and Taeumin, respectively. 4. The treatment effect was found in 84% (n=36) of the patients markedly. Six out of seven who showed no progress were Soyangin. Conclusion: The treatment based on the constitution differentiation and prescription of the Sasang Constitutional Medicine was effective for the symptom alleviation in a short term period in atopic dermatitis. Further clinical study is needed to prove the effect of Sasang Constitutional Medicine in a larger sample and various diseases.
