남용 약물에 대한 갈망은 중독적인 행동을 지속하는 데 있어 중요하다. 갈망은 강화적이거나 보상적인 약물을 복용하고자 하는 강한 욕구와 관련된 다차원적인 현상으로 여겨진다. 본 연구에서는 알코올 사용 장애자와 정상인이 알코올 관련 자극(소량의 술을 마시기, 알코올 관련 시각 자극)에 노출되었을 때 갈망에 대한 심리적인 반응의 차이를 밝히고자 하였다. 알코올 관련 시각 자극을 개발하기 위하여 Normative Appetitive Picture System(NAPS)과 1차 집단 심리 실험을 통하여 알코올 자극을 선정하고 자극의 적합성과 효과성을 검증하여 자극을 최종 개발하였다. 본 실험을 위해 알코올 사용 장애를 가진 9명의 남녀 대학생(남:8명)과 9명의 비의존 남녀 대학생(남:8명)을 선정하였다. 실험참여자는 5cc 정도의 알코올을 복용하고, 알코올 자극, 음료 자극, 통제 자극(흐릿하게 변형된 알코올 자극)과 고정점 자극(십자가 기호)에 무선적으로 노출되었으며, 각 종류의 자극이 제시되고 나면 마우스를 이용하여 심리적 갈망 수준을 보고하였다. 심리반응 결과, 강박음주갈망척도, 음주 전, 음주 후 알코올 갈망에서 두 집단 간 유의한 차이가 나타났다. 알코올 사용 장애 집단은 알코올 자극을 포함한 모든 자극에서 정상인 집단보다 높은 알코올 갈망 수준을 나타내었다. 결론적으로 알코올 사용 장애자는 알코올 자극에 의해 유발된 심리적 갈망이 정상인과 다른 것으로 나타났다.
Objectives: The purpose of this study is to reveal the phenomenological meaning and experience of alcohol craving experienced by alcoholics through qualitative research. Methods: From November 8, 2020 to October 10, 2022, in-depth interviews were conducted with four recovering alcoholics within two years of a single week who were diagnosed with alcoholism in the past and had been hospitalized for alcoholism problems. The data were analyzed using phenomenological qualitative research methods. Results: The alcohol craving experienced by alcoholics worked in connection with the life of alcoholics. Alcohol intensity was not entirely determined by external factors, but varied by the individual's internal state and circumstances surrounding the individual, and cravings were found to be classified into when control and intervention were possible. Also, the power to neutralize alcohol cravings was a warm 'self-compassion' mind. Conclusion: This study presented a new perspective on alcohol craving, and based on this perspective, the necessity of developing an alcohol addiction prevention program and a recurrence prevention program was presented. In addition, the basis for emphasizing the necessity of complete abstinence in health education services for preventing alcohol and preventing recurrence in the community was presented.
Objective : This study was designed to test the anti-craving effect of Gigaltang(extracts of Oriental medicinal herbs ; Hovenia dulcis and Puerariae flos) in C57BL/6 mice. Anti-craving effect was substituted by alcohol intake amount of C57BL/6 mice. Method : 60 C57BL/6 mice was randomized into vehicle(water) group and Gigaltang group. Only 39(vehicle group(21) and Gigaltang group(18)) was eligible for this study, and 21 C57BL/6 mice was dropped out. The procedure was composed of 5 days of adjustment period and 32 days of alcohol exposure and withdrawal period with limited access paradigm and 10 days of treatment period. Alcohol was offered only for 2 hours a day from 2 to 4PM and water was offered for 22 hours left. Food was offered for 24 hours a day. 1140mg/Kg/day of Gigaltang was administrated to 18 mice of Gigaltang group for 10 days whereas water to 21 mice of vehicle group. The amount of alcohol intake, water intake, food intake, body weight were measured every other day. Result : There weren't significant differences in 2 hours of alcohol intake, 22 hours of water intake, 24 hours of food intake and body weight for ten days between Gigaltang and vehicle group. Conclusion : Anti-craving effect of Gigaltang on C57BL/6 mice wasn't verified. To verify the anti-craving effect, further study with advanced animal model, various dose of Gigaltang and diverse period of drug administration should be taken.
Objectives : The Alcohol Urge Questionnaire(AUQ) has been used in alcohol dependence treatment and research. The goal of this study is to develop of the Korean Alcohol Urge Questionnaire(AUQ-K). Methods : To examine the AUQ-K's psychometric properties, responses from 104 patients admitted in alcohol dependence treatment facility were investigated. Results : The internal consistency of the 8-item AUQ-K, measured by coefficient ${\alpha}$, was high(Cronbach's ${\alpha}$=0.78). AUQ-K scores showed significant correlation when the retest interval was 1 day(p<0.01). The AUQ-K's validity was investigated using correlational analyses with two other craving scales[the Obsessive Compulsive Drinking Scale(OCDS) and the Visual Analogue Scale(VAS)]. The high correlations were obtained between total AUQ-K scores and total OCDS scores, and between total AUQ-K scores and the VAS scores(p<0.01, respectively). Conclusion : The AUQ-K is a reliable and valid short scale for measurement of self-reported alcohol craving. This scale may offer significant advantages over existing single-item measures of alcohol craving in the fields of alcohol dependence treatment and research.
During abstinence from alcohol, craving is elicited by the cues and contexts previously associated with alcohol, and contributes to relapse. To prevent the craving and relapse experienced by alcoholics, cue-exposure therapy (CET) has been used to extinguish the association between alcohol and alcohol-related cues and contexts. This study applied CET, using a virtual reality (VR) system, to eight members of an Alcoholics Anonymous group, in eight sessions. Cues and contexts most likely to elicit an urge to drink were selected through a preliminary survey in order to compose VR-CET scenarios: a glass, bottle, food, and a bar were judged to be the most tempting for people in alcohol dependence and abstinence. By these cues and contexts, a Japanese style pub and a western bar were composed. Each session was administered for 30 minutes by a psychiatrist and included an introduction, immersion, VR navigation, interviews about feelings, and self-report questionnaires about cravings. The eight sessions consisted of initial and closing sessions, and six cue- and context-focused sessions. As a result, a reduction in cue-elicited craving after VR-CET was reported. A mean score of 15.75 (SD = 10.91) on the Alcohol Urge Questionnaire in the first session decreased to 11.57 (SD = 6.88) in the final session.
본 연구는 음주거절훈련을 위한 전 단계로 가상현실을 통한 자극 제시가 알코올 중독 환자의 갈망감에 어떠한 변화를 주는 지를 알아보고자 하였다. 본 연구결과를 통해 향후 음주 거절 훈련 프로그램의 개발 뿐만 아니라 알코올 중독 환자들을 위한 새로운 치료 및 훈련 도구로서의 가상현실기법의 활용 가능성을 알아보기 위한 기초적인 자료 제공을 목적으로 한다. 가상현실 자극은 알코올 중독자들의 음주유발 상황으로 알려진 음주권유(사회적 압력)상황과 알코올 노출 여부 그리고 긍정적 상황 부정적 상황으로 구성하였다. 대상자는 2006년 12월부터 2007년 9월까지 입원중인 알코올 중독 환자 12명이었으며 수집된 자료는 SPSS/WlN 11.5 프로그램으로 통계처리 하였으며, Wilcoxon Matched-pairs Signed-Ranks Test로 분석하였다. 분석 결과 배경에서 아바타의 음주권유 여부가 일반적인 배경만 제시되는 경우보다 더 높은 갈망감을, 알코올이 노출된 장면이 그렇지 않은 장면보다 더 높은 갈망감을 나타내었고 긍정적 부정적이냐는 상황은 유의미한 결과를 나타내지 못하였다. 이러한 결과를 토대로 가장현실을 활용한 훈련이 실제의 사회적 압력과 유사한 음주 압력 상형과 환경을 느끼도록 하며, 가상현실 기법이 기존의 알코올 치료 기법들과 연합되어 효과적인 치료 도구로써의 활용가능성을 많이 가지고 있다는 것을 확인 할 수 있었다.
목 적: 본 연구에서는 알코올 의존의 재발 예측 인자로 중요하다고 알려진 사회 인구학적 변인, 음주력, 음주갈망, 치료 동기, 병식과 같은 요인들 중에 어떤 요인들이 한국인 알코올 의존의 재발을 예측하는지를 전향적으로 조사하였다. 방 법: 알코올 의존 환자 48명을 대상으로 사회 인구학적 특성, 음주력, Timeline Follow-Back(TLFB), Obsessive-Compulsive Drinking Scale(OCDS), Alcohol Urge Questionnaire(AUQ), Pennsylvania Alcohol Craving Scale(PACS), University of Rhode Island Change Assessment(URICA), Hanil Alcohol Insight Scale(HAIS)등을 조사한 후, 24주간 총 11회에 걸쳐 외래를 방문하도록 하였다. 24주 동안 조사한 TLFB자료에 근거하여 대상자 중에서 과음(일일 5standard drinking 이상)일이 나타난 경우 혹은 더 이상 추적 방문이 되지 않은 경우 모두 재발한 것으로 하여 재발군으로 분류하였으며, 그렇지 않은 경우를 비재발군으로 분류하였다. 알코올 의존의 재발을 예측할 수 있는 요인을 찾는 방법으로 SPSS PC+11.5 프로그램의 로지스틱 회귀분석을 이용하였다. 결 과: 음주를 시작한 연령, 음주로 인한 문제가 발생된 연령, 최근 1년간 1일 음주량, 최근 1년간 월 음주일수, PACS, AUQ, OCDS, URICA, HAIS와 같은 요인들 중 최근 1년간 1일 음주량과 HAIS 점수가 알코올 의존의 재발에 영향을 주는 것으로 나타났다(p<.05). 결 론: 알코올 의존 환자의 치료에서 1일 음주량이 많은 경우와 병식이 낮은 경우가 재발 가능성이 높으므로 앞으로의 치료에서 이러한 요인을 고려해야 할 것이다.
Purpose: The purpose of this study was to identify the subjective relapse experiences of patients with alcohol use disorder in one's life context. Methods: A Q methodology was used to analyze the subjectivity of relapse experiences among 55 participants with alcohol use disorder. Fifty-five Q-statement were derived from interviews and literature review. Q-statements were classified into normally distributed shapes using a 9-point scale. Data were analyzed using the QUANL program. Results: Four types of relapse experiences were identified: failure to self-regulation drinking cravings, fear of relapse and awareness of the need for treatment, drinking as a coping mechanism and defensive coping, and lack of motivation to change drinking behavior. Conclusion: The results of this study suggest that patients with alcohol use disorder need a differential approach based on four types of relapse experiences in the recovery process.
Objectives Many studies have suggested different neurobiological findings and clinical courses in alcoholism. Recently, subtyping in alcohol dependence has become essential to overcome the heterogeneity of patients. Among several criteria of subtypes, Lesch's typology is proposed to integrate biological, social, and psychological factors. This review provides neurobiological findings and treatment-responses of alcohol dependence according to Lesch's typology. Method We searched the international published medical literature using the search terms 'Lesch's typology' and 'alcohol dependence' and using the limits 'human'. Results We identified 17 studies with subjects of alcohol dependence according to Lesch's typology. Conclusion They indicated that each subtype of Lesch's typology can have specific neurobiological factors and different clinical responses as follows. Lesch's subtype 1 is characterized by severe withdrawal symptoms and associated with elevated glutamate and homocysteine. Lesch's subtype 2 is defined by individuals who drink alcohol as self-medication for anxiety. Their craving has significant positive correlations with prolactin, leptin level, or intake-volume (vasopressin). Lesch's subtype 4 is related to cerebral dysfunction and associated with increased glutamate and left-handedness. Clinical trials showed that naltrexone was effective in Lesch's subtype 3 and 4 patients, while acamprosate was effective in the subtypes 1 and 2.
Objectives It is well known that problem drinking is associated with alterations of brain structures and functions. Brain functions related to alcohol consumption can be determined by the resting state functional connectivity in various resting state networks (RSNs). This study aims to ascertain the alcohol effect on the structures forming predetermined RSNs by assessing their cortical thickness. Methods Twenty-six abstinent male patients with alcohol dependence and the same number of age-matched healthy control were recruited from an inpatient mental hospital and community. All participants underwent a 3T MRI scan. Averaged cortical thickness of areas constituting 7 RSNs were determined by using FreeSurfer with Yeo atlas derived from cortical parcellation estimated by intrinsic functional connectivity. Results There were significant group differences of mean cortical thicknesses (Cohen's d, corrected p) in ventral attention (1.01, < 0.01), dorsal attention (0.93, 0.01), somatomotor (0.90, 0.01), and visual (0.88, 0.02) networks. We could not find significant group differences in the default mode network. There were also significant group differences of gray matter volumes corrected by head size across the all networks. However, there were no group differences of surface area in each network. Conclusions There are differences in degree and pattern of structural recovery after abstinence across areas forming RSNs. Considering the previous observation that group differences of functional connectivity were significant only in networks related to task-positive networks such as dorsal attention and cognitive control networks, we can explain recovery pattern of cognition and emotion related to the default mode network and the mechanisms for craving and relapse associated with task-positive networks.
