  • This study was conducted to find the differences in food consumption frequency of the middle aged(male 20, female 50) and the elderly(male 15, female 15) living in Chonju in December, 1998 according to food habits, smoking, alcohol drinking and exercising habit, health status, and the levels of nutrition knowledge and attitude store. The foods frequently consumed among the subjects were kimchi(15.4/week), mixed rice(11.5/week), rice(7.6/week), vegetables in soup, jjigae and jorim(5.0/week), mandarins and oranges(5.e/week), and seasoned laver(4.3/week). There were several factors influencing food consumption patterns. These were age, regularity of meal times, the status of smoking, alcohol drinking and exercising, and the level of nutrition attitude. The elderly ate cooked rice more frequently, while the middle aged ate fish, especially blue fishes more frequently. The middle aged who had breakfast regularly ate milk and milk products, legumes and fruits frequently. Those who smoked seemed to eat less cereals and starches and fats, while those who didn't smoke ate more sugars. The alcohol drinking group also ate less fats and the exorcising group ate almost all of food groups frequently. The status of health showed to be related with food consumption patterns. The normal group in hemoglobin ate eggs more frequently than the anemia group and the high risk group in blood pressure ate almost all of food groups more frequently. The high level group for nutrition attitude score chose vegetables, fruits, and milk and milk products more frequently than the other groups. On the other hand, low level group for nutrition attitude score was apt to eat ramyun, ham-sausage, and carbonated beverages more frequently. Therefore, nutrition education to improve the food habits find to change nutrition attitude is necessary to promote health status anti mole attention should be taken to the high risk group in blood pressure to guide proper food and nutrition intakes.

Chronic Alcohol Consumption Results in Greater Damage to the Pancreas Than to the Liver in the Rats

  • Alcohol consumption increases the risk of type 2 diabetes. However, its effects on prediabetes or early diabetes have not been studied. We investigated endoplasmic reticulum (ER) stress in the pancreas and liver resulting from chronic alcohol consumption in the prediabetes and early stages of diabetes. We separated Otsuka Long-Evans Tokushima Fatty (OLETF) rats, a type-2 diabetic animal model, into two groups based on diabetic stage: prediabetes and early diabetes were defined as occurrence between the ages of 11 to 16 weeks and 17 to 22 weeks, respectively. The experimental group received an ethanol-containing liquid diet for 6 weeks. An intraperitoneal glucose tolerance test was conducted after 16 and 22 weeks for the prediabetic and early diabetes groups, respectively. There were no significant differences in body weight between the control and ethanol groups. Fasting and 120-min glucose levels were lower and higher, respectively, in the ethanol group than in the control group. In prediabetes rats, alcohol induced significant expression of ER stress markers in the pancreas; however, alcohol did not affect the liver. In early diabetes rats, alcohol significantly increased most ER stress-marker levels in both the pancreas and liver. These results indicate that chronic alcohol consumption increased the risk of diabetes in prediabetic and early diabetic OLETF rats; the pancreas was more susceptible to damage than was the liver in the early diabetic stages, and the adaptive and proapoptotic pathway of ER stress may play key roles in the development and progression of diabetes affected by chronic alcohol ingestion.

Sex-specific differences in the association of a common aldehyde dehydrogenase 2 gene polymorphism and alcohol consumption with stroke risk in a Korean population: a prospective cohort study

  • BACKGROUND/OBJECTIVES: It is well-known that alcohol consumption is associated with stroke risk as well as with aldehyde dehydrogenase 2 gene (ALDH2) polymorphisms. However, it is unclear whether ALDH2 polymorphisms are associated with stroke risk independent of alcohol consumption and whether such association is modified by sex. We evaluated sex-specific associations of a common ALDH2 polymorphism and alcohol consumption with stroke risk in a Korean population. SUBJECTS/METHODS: We conducted a prospective cohort study involving 8,465 men and women, aged 40-69 years and free of stroke between June, 2001 and January, 2003, and followed for the development of stroke. We identified new cases of stroke, which were self-reported or ascertained from vital registration data. Based on genome-wide association data, we selected a single-nucleotide polymorphism (rs2074356), which shows high linkage disequilibrium with the functional polymorphism of ALDH2. We conducted Cox proportional hazards regression analysis considering potential risk factors collected from a baseline questionnaire. RESULTS: Over the median follow-up of 8 years, 121 cases of stroke were identified. Carrying the wild-type allele of the ALDH2 polymorphism increased stroke risk among men. The multivariate hazard ratio [95% confidence interval] of stroke was 2.02 [1.03-3.99] for the wild-type allele compared with the mutant alleles, but the association was attenuated after controlling for alcohol consumption. Combinations of the wild-type allele and other risk factors of stroke, such as old age, diabetes mellitus, and habitual snoring, synergistically increased the risk among men. Among women, however, the ALDH2 polymorphism was not associated with stroke risk. CONCLUSIONS: The prospective cohort study showed a significant association between a common ALDH2 polymorphism and stroke risk in Korean men, but not in Korean women, and also demonstrated that men with genetic disadvantages gain more risk when having risk factors of stroke. Thus, these men may need to make more concerted efforts to control modifiable risk factors of stroke.

Dietary Problems among Middle-Aged Japanese Men

  • Balanced intake of appropriate nutrients is the key to sustaining and promoting health as well as preventing and treating diseases. It is not always easy, however, to take balanced nutrition and various related factors must be taken into consideration. This report provides a summary of studies that have examined the nutrient intakes of Japanese middle-aged men and the relationship of this intake to drinking, job-related separation from families, and health practices. The alcohol consumption of Japanese middle-aged men has more than tripled in the last forty years. When nutrient intake was examined in relation to alcohol consumption, it was discovered that the maximum acceptable alcohol consumption was approximately 23 grams (about two drinks) of pure alcohol, provided the level of nutrient intake for drinkers was equal to that of non-drinkers. The alcohol energy ratio was approximately 5%. It was also discovered that middle-aged men's eating habits deteriorate when they relocate to new posts without their families and live by themselves. Compared to those living with their families, a higher proportion of those living alone have unfavorable eating habits including skipping breakfast or lunch, having a late lunch, and eating and drinking after dinner until bedtime. When Breslow's seven health practices, nutrient intake, and consumption weight by food group were examined, it was discovered that the group that had many beneficial eating and living habits consumed plenty of legume, pulses, fruit, green yellow vegetables and milk products. Their intake of vitamins and minerals was high and the results of a physical examination proved to be excellent. According to nutrition surveys conducted in Japan, China, the United Kingdom and the United States using a 24-hour recall method with common protocols and strict controls to ensure high levels of accuracy and cross-study validity, the Japanese had the highest cholesterol intake and the lowest dietary fiber intake among the four countries. Also, the alcohol energy ratio of the Japanese exceeded 8%, the highest among the four countries, while their intake of magnesium and iron was the lowest These results indicate that it is necessary to enhance nutritional education for middle-aged men and to reinforce the social environments in which they live and work in order to promote proper diet and nutrition in Japan. Culinary Cinnamon and Clove Powder Ameliorate Fatty Liver Formation Induced by Ethanol Supplementation in Zebrafish

  • Culinary herbs and spices have received much attention since they contain high concentrations of bioactive ingredients for antioxidant and anti-inflammatory activities. Protection effect of the herb and spice against acute alcohol consumption has been investigated using zebrafish as a vertebrate model. During 30 days bathed in water containing 1% Et-OH and the designated herb or spice, the survival rate of the Et-OH group was decreased sharply (up to 20% at 10 days). The cinnamon-fed group showed the highest and longer survival rate up to 80% up to for 30 days under the presence of Et-OH, while clove-fed group showed 40% survival rate for 25 days. Et-OH group serum exhibited the weakest antioxidant ability from ferric ion removal ability (FRA) assay; FRA ability was increased in the cinnamon-fed group up to 414%, while the clove and laurel group increased 256% and 309%, respectively. Histologic observation and Oil-red O staining showed hepatic tissue damage was severe in the Et-OH group. The cinnamon- or clove-fed group showed much ameliorated hepatic tissue morphology with minimized steatosis. The cinnamon- or clove-fed group showed lower serum GOT and GPT levels than the Et-OH group. Among hepatic tissue extract, the clove-fed group exhibited the lowest level of GOT and GPT. These results suggest that consumption of cinnamon and clove might be beneficial to attenuate progress of acute fatty liver change by alcohol consumption.

Aqueous extract of Laurus nobilis leaf accelerates the alcohol metabolism and prevents liver damage in single-ethanol binge rats

  • BACKGROUND/OBJECTIVES: Excessive alcohol consumption has harmful health effects, including alcohol hangovers and alcohol-related liver disease. Therefore, methods to accelerate the alcohol metabolism are needed. Laurus nobilis is a spice, flavoring agent, and traditional herbal medicine against various diseases. This study examined whether the standardized aqueous extract of L. nobilis leaves (LN) accelerates the alcohol metabolism and protects against liver damage in single-ethanol binge Sprague-Dawley (SD) rats. MATERIALS/METHODS: LN was administered orally to SD rats 1 h before ethanol administration (3 g/kg body weight [BW]) at 100 and 300 mg/kg BW. Blood samples were collected 0.5, 1, 2, and 4 h after ethanol administration. The livers were excised 1 h after ethanol administration to determine the hepatic enzyme activity. The alcohol dehydrogenase (ADH), aldehyde dehydrogenase (ALDH), superoxide dismutase (SOD), and glutathione peroxidase (GPx) activities in the liver tissue were measured. RESULTS: LN decreased the serum ethanol and acetaldehyde levels in ethanol-administered rats. LN increased the hepatic ADH and ALDH activities but decreased the alanine aminotransferase, aspartate aminotransferase, and gamma-glutamyl transferase activities in the ethanol-administered rats. In addition, LN inhibited lipid peroxidation and increased the activities of SOD and GPx. CONCLUSIONS: LN modulates the mediators of various etiological effects of excessive alcohol consumption and enhances the alcohol metabolism and antioxidant activity, making it a potential candidate for hangover treatments.

  • Objective : The present study investigates patterns of consumption of alcohol, cigarettes, and over the counter drugs (OTC) in Korean patients with PTSD and psychiatric patient controls and healthy controls from multicenter hospital samples. Method : Data were collected from 18 hospitals nationwide. Patients with PTSD were compared with nonpsychotic psychiatric disorders control and healthy control subjects on psychometric measures of consumption of alcohol, cigarettes, and OTC drugs. Result : Ninety-three patients with PTSD, 73 patients with psychiatric disorders control, and 88 healthy control subjects were enrolled. There were significant differences in the frequency of alcohol drinking among the three groups ($X^2$=12.93, df=6, p=0.044). Only 3.2% of healthy control subjects drank alcohol more than 4 times a week, but 43.9% of PTSD group and 46.5% of mental disorder controls drank alcohol more than 4 times a week, respectively. Regarding cigarette smoking, there were no differences among the three groups. There were significant differences in uses of OTC drugs among the three groups; 16.7 % in the PTSD group, 14.9% in the psychiatric control group and 1.3% in the heath control group, respectively ($X^2$=11.31, df=2, p=0.004). Conclusion : In this study, patients with PTSD showed significant differences in the frequency of consumption of alcohol and OTC drugs, as compared with healthy controls. However, there were no differences between the PTSD and psychiatric patient control groups. Since all subjects were enrolled from hospital based sample, results may differ in the community samples. Further studies will be needed for the evaluation of substance uses in these groups.

뇌졸중 환자의 식품 기호도 변화와 섭취빈도에 관한 연구 (Study on Altered Food Preference and Food Frequency in Stroke Patients)

  • The purpose of this study was to investigate the dietary habits and altered food preferences of stroke patients. One hundred and forty-six outpatients, who had experienced their first-ever stroke and were admitted to Asan Medical Center between July and December 2000, were studied. Using interviews, we assessed the altered food preferences, food consumption frequency, and other factors influencing the food preferences and food consumption frequency of the subjects. These results were analyzed with 2 t-tests, and multiple regression analysis, using the SPSS package program. Preferences for pork, red fish, coffee, bread and stews were higher in male stroke patients than in females. The frequency of consumption of beef, pork, white fish, red fish, egg, garlic, onion, coffee, instant noodles, bread, and culinary vegetables increased in the male stroke patients more than in the females. Food preferences were influenced by income, risk factors, subjective tastes and location of brain ischemic lesions. Food consumption frequency was affected by food preference, income, drugs, alcohol, marital status, sex, and dysgeusia. As a result of multiple regression analysis, the frequency of consumption of white fish, red fish, eggs, soy milk, milk, garlic, onions, coffee, noodles, bread, bean-paste stew, kimchi, culinary vegetables, and greasy foods were the most affected by each food preference. Our results suggest that food consumption frequency may vary with food preference, income, drugs, alcohol, marital status, sex, and dysgeusia, and nutrition education should be formulated to prevent stroke recurrence based on the food preferences, subjective tastes, and risk factors of individual stroke patients.

WTO/TBT협정하에서 알코올 라벨링에 대한 국제적 조화 가능성에 관한 연구 (A study for the possibility of international harmonization on alcohol labelling under WTO/TBT)

  • This paper determines if mandated alcohol health information labelling could be an obstacle to international trade by discussing international trade rules affecting member nations. WTO members could use alcohol labelling rules to protect human health, reduce the consumption of alcohol and provide product information. When a member uses alcohol labelling as a technical regulation or standard, it should comply with TBT Articles 2.2 and 2.4. If a member uses some specific warning messages for their objectives, it should be considered the contribution of the measure, trade restrictiveness, and alternatives. If WTO members want to employ their own alcohol labelling schemes for addressing carcinogenicity, intoxicating effects, dependence potential, effects on children, adolescents and pregnancy, it could be a technical barrier and also create adverse effects on international trade. Therefore, the harms of alcohol are supported by scientific evidence, and for this reason, the international harmonization for alcohol labelling should be discussed openly to prevent negative effects on trade.

음주와 순환기계질환 사망 및 전체사망과의 관련성 (Association between Alcohol Drinking and Cardiovascular disease Mortality and All-cause Mortality - Kangwha Cohort Study -)

  • Objectives : This study sought to examine relationships between alcohol drinking and cardiovascular disease mortality and all-cause mortality. Methods : From March 1985 through December 1999, 2,696 males and 3,595 females aged 55 or over as of 1985 were followed up for their mortality until 31 December 1999. We calculated the mortality risk ratios by level of alcohol consumption. Among the drinker, the level of alcohol consumption was calculated by the frequency of alcohol comsumption and the type of alcohol. Cox proportional hazard model was used to adjust for confounding factors. Results : Among males, compared to abstainer, heavy drinker had significantly higher mortality in all cause(Risk ratio=1.35), cardiovascular disease(Risk ratio=1.52) and cerebrovascular disease(Risk ratio =1.66). Although not significant, moderate drinker had lower ischemic heart disease mortality(Risk ratio =0.38). Among females, there was no statistically significant association between alcohol comsumption and mortality. Conclusion : The results of this study suggest that alcohol drinking has harmful effect on all-cause mortality, cardiovascular disease mortality and cerebrovascular disease mortality among males, especially in heavy drinker among males. Minimal evidence on protective effect for cardiovascular disease mortality in low or moderate drinker is observed.
