DA-9701: A New Multi-Acting Drug for the Treatment of Functional Dyspepsia

  • Motilitone$^{(R)}$ (DA-9701) is a new herbal drug that was launched for the treatment of functional dyspepsia in December 2011 in Korea. The heterogeneous symptom pattern and multiple causes of functional dyspepsia have resulted in multiple drug target strategies for its treatment. DA-9701, a compound consisting of a combination of Corydalis Tuber and Pharbitidis Semen, has being developed for treatment of functional dyspepsia. It has multiple mechanisms of action such as fundus relaxation, visceral analgesia, and prokinetic effects. Furthermore, it was found to significantly enhance meal-induced gastric accommodation and increase gastric compliance in dogs. DA-9701 also showed an analgesic effect in rats with colorectal distension induced visceral hypersensitivity and an antinociceptive effect in beagle dogs with gastric distension-induced nociception. The pharmacological effects of DA-9701 also include conventional effects, such as enhanced gastric emptying and gastrointestinal transit. The safety profile of DA-9701 is also preferable to that of other treatments.

ROV Manipulation from Observation and Exploration using Deep Reinforcement Learning

  • The paper presents dual arm ROV manipulation using deep reinforcement learning. The purpose of this underwater manipulator is to investigate and excavate natural resources in ocean, finding lost aircraft blackboxes and for performing other extremely dangerous tasks without endangering humans. This research work emphasizes on a self-learning approach using Deep Reinforcement Learning (DRL). DRL technique allows ROV to learn the policy of performing manipulation task directly, from raw image data. Our proposed architecture maps the visual inputs (images) to control actions (output) and get reward after each action, which allows an agent to learn manipulation skill through trial and error method. We have trained our network in simulation. The raw images and rewards are directly provided by our simple Lua simulator. Our simulator achieve accuracy by considering underwater dynamic environmental conditions. Major goal of this research is to provide a smart self-learning way to achieve manipulation in highly dynamic underwater environment. The results showed that a dual robotic arm trained for a 3DOF movement successfully achieved target reaching task in a 2D space by considering real environmental factor.

엽록체내 Protoporphyrin IX(PPIX) 생합성계에서 S-23142의 작용기구 (Action Mechanism of S-23142 on the PPIX Biosynthesis System of Spinacia oleracea L. Chloroplast)

  • In vitro 조건에서 protoporphyrin IX(PPIX) 생합성에 미치는 제초제 S-23142와 acifluorfen의 영향을 알아보고자 시금치잎에서 엽록체내의 stroma와 membrane을 분리, 분획하고 형 광검출기가 장착된 역상HPLC를 이용하여 생합성된 PPIX 함량용 분석한 결과는 다음과 같다. 1. PPIX의 생합성이 이루어지는 부위는 엽록체 내의 stroma분획이었고 이는 모두 ALA(${\delta}$-aminolevulinic acid)에 의한 반응산물이었다. 2. PPIX의 생합성에 관련된 in vitro실험은 stroma분획을 이용하는 것이 가장 효율적이었다. 3. In vitro계에서 S-23142와 acifluorfen은 동일하게 PPIX의 생합성을 억제하였으며, 그 작용부위는 엽록체내의 stroma일 것으로 판단되었다.

Condurango (Gonolobus condurango) Extract Activates Fas Receptor and Depolarizes Mitochondrial Membrane Potential to Induce ROS-dependent Apoptosis in Cancer Cells in vitro -CE-treatment on HeLa: a ROS-dependent mechanism-

  • Objectives: Condurango (Gonolobus condurango) extract is used by complementary and alternative medicine (CAM) practitioners as a traditional medicine, including homeopathy, mainly for the treatment of syphilis. Condurango bark extract is also known to reduce tumor volume, but the underlying molecular mechanisms still remain unclear. Methods: Using a cervical cancer cell line (HeLa) as our model, the molecular events behind condurango extract's (CE's) anticancer effect were investigated by using flow cytometry, immunoblotting and reverse transcriptase-polymerase chain reaction (RT-PCR). Other included cell types were prostate cancer cells (PC3), transformed liver cells (WRL-68), and peripheral blood mononuclear cells (PBMCs). Results: Condurango extract (CE) was found to be cytotoxic against target cells, and this was significantly deactivated in the presence of N-acetyl cysteine (NAC), a scavenger of reactive oxygen species (ROS), suggesting that its action could be mediated through ROS generation. CE caused an increase in the HeLa cell population containing deoxyribonucleic acid (DNA) damage at the G zero/Growth 1 (G0/G1) stage. Further, CE increased the tumor necrosis factor alpha ($TNF-{\alpha}$) and the fas receptor (FasR) levels both at the ribonucleic acid (RNA) and the protein levels, indicating that CE might have a cytotoxic mechanism of action. CE also triggered a sharp decrease in the expression of nuclear factor kappa-light-chain-enhancer of activated B cells ($NF-{\kappa}B$) both at the RNA and the protein levels, a possible route to attenuation of B-cell lymphoma 2 (Bcl-2), and caused an opening of the mitochondrial membrane's permeability transition (MPT) pores, thus enhancing caspase activities. Conclusion: Overall, our results suggest possible pathways for CE mediated cytotoxicity in model cancer cells.

Identification of Alkylation-Sensitive Target Chaperone Proteins and Their Reactivity with Natural Products Containing Michael Acceptor

  • Molecular chaperones have a crucial role in the folding of nascent polypeptides in endoplasmic reticulum. Some of them are known to be sensitive to the modification by electrophilic metabolites of organic pro-toxicants. In order to identify chaperone proteins sensitive to alkyators, ER extract was subjected to alkylation by 4-acetamido-4 -maleimidyl-stilbene-2,2 -disulfonate (AMS), and subsequent SDS-PAGE analyses. Protein spots, with molecular mass of 160, 100, 57 and 36 kDa, were found to be sensitive to AMS alkylation, and one abundant chaperon protein was identified to be protein disulfide isomerase (PDI) in comparison with the purified PDI. To see the reactivity of PDI with cysteine alkylators, the reduced form ($PDI_{red}$) of PDI was incubated with various alkylators containing Michael acceptor structure for 30 min at $38^{\circ}C$ at pH 6.3, and the remaining activity was determined by the insulin reduction assay. Iodoacetamide or N-ethylmaleimide at 0.1 mM remarkably inactivated $PDI_{red}$ with N-ethylmaleimide being more potent than iodoacetamide. A partial inactivation of $PDI_{oxid}$ was expressed by iodoacetamide, but not N-ethylmaleimide (NEM) at pH 6.3. Of Michael acceptor compounds tested, 1,4-benzoquinone ($IC_{50}, 15 \mu$ M) was the most potent, followed by 4-hydroxy-2-nonenal and 1,4-naphthoquinone. In contrast, 1,2-naphthoquinone, devoid of a remarkable inactivation action, was effective to cause the oxidative conversion of $PDI_{red}$ to $PDI_{oxid}$. Thus, the action of Michael acceptor compounds differed greatly depending on their structure. Based on these, it is proposed that POI, one of chaperone proteins in ER, could be susceptible to endogenous or xenobiotic Michael acceptor compounds in vivo system.

지역거점 공공병원의 폐렴 적정성 평가 지표 개선을 위한 융복합 실행연구 (An Action Convergence Research to Improve Evaluation Index of Pneumonia Adequacy in Regional Public Hospital)

  • 본 연구는 지역거점 공공병원의 폐렴 적정성 평가 지표를 개선하기 위한 전략을 개발하고 그 효과를 확인하기 위한 실행연구이다. 이 연구는 N시에 소재한 지역거점 공공병원에 근무하는 내과의사 1명, 간호사 8명, 전산실 직원 1명, 간호학 교수 1명이 참여하였으며 2021년 3월부터 10월까지 이루어졌다. Zuber-Skerritt & Fletcher의 개념모형을 바탕으로 실행연구를 수행하였다. 계획, 실행, 평가, 성찰의 단계를 2 cycle 로 운영하면서 2분기와 3분기에 효과평가를 하였으며 폐렴 적정성 평가지표를 효과평가의 도구로 사용하였다. 2019년 폐렴 적정성 평가지표와 비교하여 2021년에는 모든 지표가 기준목표에 100% 도달하였다. 따라서 본 연구결과를 바탕으로 향후 보다 질높은 의료서비스를 제공하기 위해 다학제간 참여와 협조 아래 실행연구를 통한 의료 질 개선 활동을 적용하고 임상현장의 적용가능성을 검토해볼 필요가 있다.

Total Scalp의 방사선 치료 시 Helmet Bolus 제작방법에 관한 연구 (Comparison of Three Different Helmet Bolus Device for Total Scalp Irradiation)

  • 목 적: Total Scalp의 방사선 치료 시 표면선량(surface dose)을 증가시키기 위해 조직보상체(bolus)를 사용한다. 이에 본 저자는 두피(scalp) 전체를 덮는 조직보상체로서 Bolx-II, paraffin wax, solid thermoplastic물질을 각각 적용한 Helmet bolus를 제작하여 유용성을 비교 평가해보고자 한다. 대상 및 방법: Rando 팬톰을 이용하여 두피 고정용구(mask)를 만든 후 전체 두피에 Bolx-II (Action Products, USA), paraffin wax (Densply, USA), solid thermoplastic 물질(Med-Tec, USA)을 0.5 cm 두께로 동일하게 적용한 Helmet bolus를 각각 제작 하였다. 각 Helmet bolus에 대한 물리적 특성을 평가하기 위해 전산화 단층촬영(computed tomography) (GE, Ultra Light Speed16)을 시행 후 방사선치료계획시스템(radiation therapy planning system) (Philips, Pinnacle)을 이용하여 최적의 치료 계획(radiationtherapy plan)을 세웠으며, 각 조직 보상체(bolus)에 대한 밀도 변화(density variation)화를 분석하였다. 선량 분포(dose distribution)평가(evaluation)를 위해 선량체적히스토그램(Dose-Volume Histogram) (DVH)을 이용해 임상표적체적(Clinical Target Volume) (CTV)에 대한 Dose-homogeneity index (DHI, $D_{90}/D_{10}$), Conformity index (CI, $V_{95}/TV$)를, 정상 뇌 조직(Normal brain tissue)에 있어$V_{20}$, $V_{30}$을 각각 구하여 비교 하였다. 또한 총 제작시간을 측정하여 제작과정에서의 효율성을 평가하였고, 열형광선량계 (Thermoluminescent Dosimeter)를 사용하여 실제 치료 시 각 두피의 표면선량을 확인 하였다. 결 과: Bolx-II, paraffin wax, solid thermoplastic 물질에 대한 밀도측정(density measurement) 결과 $0.952{\pm}0.13g/cm^3$, $0.842{\pm}0.17g/cm^3$, $0.908{\pm}0.24g/cm^3$로 Bolx-II의 밀도변화(density variation)가 가장 작았다. Bolx-II, paraffin wax, solid thermoplastic 물질을 적용한 각 Helmet bolus DHI는 0.89, 0.85, 0.77, CI는 0.86, 0.78, 0.74로 Bolx-II가 가장 우수 하였으며, 정상조직에 있어 뇌(braintissue)의 $V_{20}$은 11.50%, 10.80%, 10.07%, $V_{30}$은 7.62%, 7.40%, 7.31%로 나타났다. 제작 시 총 소요 시간은 30분, 120분, 90분 이였으며, Bolx-II를 적용한 Helmet bolus 제작과정이 가장 간단하였다. 열형광선량계(Thermoluminescent Dosimeter) 측정 결과세 Helmet bolus 모두 ${\pm}7%$ 이내의 선량오차를 보였다. 결 론: Total Scalp의 방사선치료 시 Bolx-II를 사용한 Helmet bolus의 적용은 CTV (Clinical Target Volume)에 대한 Homogeneity와 Conformity를 높여 줄 뿐만 아니라 제작시간 및 제작방법이 간단하여 임상 적용 시 유용하리라 생각된다.

사회취약 독거노인 주택의 맞춤형 개조 현장상황 기술 연구 (A Descriptive Research on Field Situation of Customized Modification for Vulnerable Single Elderly Home)

  • Elderly population has increased rapidly and adjusting the environment is in demand to support their health. To improve the living environment of elderly, an appropriate plan for customized home modification is needed upon the personal contexts. The purpose of this study is to explain the sequence and to build an insight on customized home modification for vulnerable single elderly. It is a case study based on single target that includes 6 months of execution period deeply seeking the appropriate plan for implementation. The target is a male elderly aged 74 living in a single story detached house. The various research methods are applied in this study including exploratory, participatory, action and trade-off preference researches. The result shows that plan for home modification is complex outcome of interlocking uniqueness between residence environment and resident situation. If customized modification is targeted for residentially vulnerable social group, more sensitive approach aiming for the highest result is required due to financial constraint. To conclude, comprehensive understanding of overall circumstances of a dwelling, a resident and various stakeholder is essential throughout the process. Since the aging-in-place of elderly cannot be achieved without self-sustaining living environment, customized home modification should be a tool to meet housing suitability. It is important to strengthen the experts and future workforce with networks of local residents to share opinions and appropriate plans.

Anti-diabetic Effect and Mechanism of Korean Red Ginseng in C57BL/KsJ db/db Mice

  • The present study was designed to investigate the anti-diabetic effect and mechanism of Korean red ginseng in C57BL/KsJ db/db mice. The db/db mice were divided into three groups: diabetic control group (DC), Korean red ginseng group (KRG, 100 mg/kg) and metformin group (MET, 300 mg/kg), and treated with drugs once per day for 10 weeks. Compared to the DC group, fasting blood glucose levels were decreased by 19.8% in KRG-, 67.7% in MET-treated group. With decreased plasma glucose and insulin levels, the insulin resistance index of the KRG-treated group was reduced by 27.6% compared to the DC group. The HbA1c levels in KRG and MET-treated groups were also decreased by 11.0% and 18.9% compared to that of DC group, respectively. Plasma triglyceride and non-esterified fatty acid levels were decreased by 18.8% and 16.8%, respectively, and plasma adiponectin and leptin levels were increased by 20.6% and 12.1%, respectively, in the KRG-treated group compared to those in DC group. Histological analyses of the liver and fat tissue of mice treated with KRG revealed significantly decreased number of lipid droplets and decreased size of adipocytes compared to the DC group. From the pancreatic islet double-immunofluorescence staining, we observed KRG has increased insulin contents, but decreased glucagon production. To elucidate action mechanism of KRG, effects on AMP-activated protein kinase (AMPK) and its downstream target proteins responsible for fatty acid oxidation and gluconeogenesis were explored in the liver. KRG activated AMPK and acetyl-coA carboxylase (ACC) phosphorylations, resulting in stimulation of fatty acid oxidation. KRG also caused to down regulation of SREBP1a and its target gene expressions such as FAS, SCD1 and GPAT. In summary, our results suggest that KRG exerted the anti-diabetic effect through AMPK activation in the liver of db/db mice.

물리적인 힘과 스코어 캐릭터를 반영한 립모션 체험형 가상현실 게임개발 (The Development of Realistic Virtual Reality Game with Leap Motion Reflected Physical strength and Score Characters)

  • 게임기술의 발전에 따라 콘텐츠 분야에서 현실적인 그래픽 기술과 자유로운 인터페이스와 실감형 콘텐츠 서비스가 요구되고 있다. 인터페이스 방식은 CLI에서 GUI로 GUI을 지나 NUI로 발전되고 있다. NUI는 기존의 인터페이스와는 달리 인간의 자연스런 행동을 직관적이고 실감적으로 이용할 수 있는 인터페이스이다. 본 논문에서는 NUI 인터페이스 가운데 립 모션을 이용한 권투 시뮬레이션 게임을 제안하고자 한다. 개발된 VR 헤드셋 게임을 통하여 실감형 3D 실험환경을 제공하여 사용자가 제어하는 인터페이스 (주먹)가 내부의 목표물 (샌드백)을 타격하게 되면, 물리적 특성인 목표물의 충격 각도의 변화에 따라 점수를 계산하는 방법을 적용하였다.
