Analysis of occupational accidents for fisher's on gillnet fishing vessel using the written verdict (재결서를 이용한 자망어선 어선원의 어로작업 중 안전사고 분석)

  • We analyzed work safety risk factors, which are likely to occur during fishing in gillnet fishing vessels using the written verdict of the Korea Maritime Safety Tribunal from 2016 to 2020, and considered work safety management. Of the total of 37 fatal accidents, three cases in the East Sea, six cases in the South Sea, and 28 cases in the West Sea were very frequent. The accident rate per vessel by sea area (%) was 0.08% in East Sea, 0.12% in South Sea, and 0.40% in the West Sea. Based on the East Sea, the number of fatal accidents was 1.6 times higher in the South Sea and 5.4 times higher in the West Sea. Six cases (16.2%) occurred during departure and preparation for fishing in the fishing process, and all other 31 cases (83.8%) occurred during fishing operation. In the order of accident types, 21 cases (56.8%) of being struck by object, eight cases (21.6%) of contact with machinery and six cases (16.2%) of falls from height were found to be fatal accidents in gillnet fishery. Human factors, such as fishers' carelessness and negligent safety management by captain accounted for 27 cases (73.0%) of the main cause, and 35 cases (94.6%) of the secondary cause. In addition to human factors such as fisher's carelessness and negligent safety management by captain, mechanical factors, environmental factors and management factors must be improved together to reduce human casualties. These results are expected to be utilized as basic data for reducing safety accidents during the work of fishers.

Dose analysis of nearby residents and workers due to the emission accident of gaseous radioactive material at the spent resin mixture treatment facility

  • The dose from a possible accident at a microwave-based spent resin mixture treatment facility that was to be installed and operated at the Wolsong nuclear power plant was analyzed to evaluate the radiological safety prior to its installation and operation. The dose to which workers and nearby residents are likely to be exposed was calculated based on the atmospheric dispersion and deposition factors using the XOQDOQ code. The highest atmospheric dispersion factors were 1.349E-05 s/m3 (workers) and 1.534E-06 s/m3 (residents). The highest doses due to emissions from the mock-up tank before operation were 1.91E-06 mSv (workers) and 1.78E-07 mSv (residents). Even after 3 h of operation, emissions from the mock-up tank had the greatest impact ranging from 4.63E-08 to 1.24E-06 mSv (workers) and 2.74E-10 to 1.16E-07 mSv (residents), respectively. The doses were 7.09E-09-4.55E-07 mSv and 4.18E-11-4.25E-08 mSv at 4-5 h of operation, and the maximum doses after operation reached 5.69E-07 mSv and 5.31E-08 mSv for the workers and residents, respectively. Even at the exclusion area boundary (EAB), 4.76E-08-9.51E-07 mSv (annual dose:9.52E-05–1.90E-03 mSv/y) was below the dose limit of the EAB, and the safety of the facility installation inside the NPP was confirmed.

Assessment of Radiation Safety Incident Risk Factors in Radiation Oncology Department Using the P-mSHEL Factor Analysis Model (P-mSHEL 요인분석 모델을 이용한 방사선종양학과 방사선 안전사고 위험 요인 평가)

  • Radiation oncology departments are at high risk for potential radiation safety incidents. This study aimed to identify risk factors for these incidents using the P-mSHEL (Patient, Management, Software, Hardware, Environment, and Liveware) model and to evaluate potential accident types through Failure Mode and Effects Analysis (FMEA). FMEA identified seven accident types with high Risk Priority Number (RPN). A total of 56 detailed risk factors were classified using the P-mSHEL model, and measures to prevent radiation safety incidents were implemented. The effect of these preventive measures on workers' safety perception was confirmed through two indicators (FMEA and safety perception). After implementing the preventive measures, the FMEA analysis showed that the highest reduction in RPN was for A-6 (radiation exposure while other patients/guardians are present) with a reduction rate of 33.3%, followed by B-3 (radiation exposure while staff are present) with a reduction rate of 33.3%. Overall safety perception significantly improved after the preventive measures (4.17±0.35) compared to before (2.76±0.33) (p<0.05), with notable increases in both employee safety culture (3.93±0.51) and patient safety culture (3.73±0.62) (p<0.05). This study identified risk factors in radiation oncology departments. Continuous management, maintenance, and fostering a strong safety culture are crucial for preventing incidents. Regular problem identification and collaboration with relevant departments are essential for maintaining safety standards.

Impacts Analysis of the operation of DVR(Driving Video Recorder) on Driver's Behavior Change and Reduction of Traffic Accident (교통사고 영상기록장치(DVR : Driving Video Recorder)의 설치가 운전자의 운전태도 변화와 교통사고 저감에 미치는 효과 분석)

  • The aim of this study is to analyze the effects of DVR(Driving Video Recorder) operation on decreasing the number of traffic accidents, the cost of traffic accident claim, and the behavioral change in drivers' driving. The data for this research are obtained from taxi drivers in Busan. For this, Structural Equation Model and two-way ANOVA are employed for empirical analysis. Overall results of this study show that the number of traffic accidents of 4 taxi corporations in Busan has decreased by average 32.7 percent after using DVRs. In addition, as to the cost of taxi accident claims, it is expected that the DVR operation has a considerable effect on economic benefits of taxi corporations. Moreover, this study could make clear the difference in behaviors between DVR users and non-users, and discriminate the positive and negative impacts of the DVR operation on the drivers' driving behavior. The study quantitatively examined the indirect impact of 'attitude', 'subject norm' and 'behavioral control' factors on planned 'behavior', and the direct impact of 'behavioral control' factor on the planned 'behavior'. This study suggests that they should add the video recoding function of DVRs when operation recorder(blackbox for the car) is obligatorily set up on cars for business by traffic security law.

  • Police agency is indicated that the number of children's traffic accident is tending downwards, however about one child is dead every day due to traffic accident. Major parts of the accidents happen during walking; among the rest, jaywalking is the biggest reason. Many accidents take plate on the road to school or near the home so government legislate children safeguard zone at 1995. According to the legislation, drivers have to reduce speed and there are safety facilities for children at children safeguard zone. This study finds the problems of children safeguard zone and suggest more effective and quantitative method for children safeguard zone. Firstly this study grasps the characters of children's pattern movement and children's traffic accident at children safeguard zone and then divides specific danger factors of children's traffic accident at children safeguard zone. Secondly, each factor is given danger level depending on danger degree and suggests effective method for assignment standard of children safeguard zone using all of these things.

Temporal Change in Radiological Environments on Land after the Fukushima Daiichi Nuclear Power Plant Accident

  • Massive environmental monitoring has been conducted continuously since the Fukushima Daiichi Nuclear Power accident in March of 2011 by different monitoring methods that have different features together with migration studies of radiocesium in diverse environments. These results have clarified the characteristics of radiological environments and their temporal change around the Fukushima site. At three months after the accident, multiple radionuclides including radiostrontium and plutonium were detected in many locations; and it was confirmed that radiocesium was most important from the viewpoint of long-term exposure. Radiation levels around the Fukushima site have decreased greatly over time. The decreasing trend was found to change variously according to local conditions. The air dose rates in environments related to human living have decreased faster than expected from radioactive decay by a factor of 2-3 on average; those in pure forest have decreased more closely to physical decay. The main causes of air dose rate reduction were judged to be radioactive decay, movement of radiocesium in vertical and horizontal directions, and decontamination. Land-use categories and human activities have significantly affected the reduction tendency. Difference in the air dose rate reduction trends can be explained qualitatively according to the knowledge obtained in radiocesium migration studies; whereas, the quantitative explanation for individual sites is an important future challenge. The ecological half-lives of air dose rates have been evaluated by several researchers, and a short-term half-life within 1 year was commonly observed in the studies. An empirical model for predicting air dose rate distribution was developed based on statistical analysis of an extensive car-borne survey dataset, which enabled the prediction with confidence intervals. Different types of contamination maps were integrated to better quantify the spatial data. The obtained data were used for extended studies such as for identifying the main reactor that caused the contamination of arbitrary regions and developing standard procedures for environmental measurement and sampling. Annual external exposure doses for residents who intended to return to their homes were estimated as within a few millisieverts. Different forms of environmental data and knowledge have been provided for wide spectrum of people. Diverse aspects of lessons learned from the Fukushima accident, including practical ones, must be passed on to future generations.

Development of Safety Management Index for Contractor Assessment in Bidding Process (입찰단계 수급자 평가를 위한 안전경영지수 개발)

  • The construction industry is still the most risky industry and labor accident is a significant management factor on construction site. Safety management has been studied for improving the safety performance on construction sites. However, Almost all of them have been considered on the site safety management for the contractors and evaluation of accident rate. To resolve the limitation of current research, this study is focused on considering accident result and effort by performing the evaluation of safety level of the head office and site in construction companies. By studying the research about a safety management and interviewing with specialists in safety management, 7 items were selected and weighted based on the KOSHA18001 List. Consequently, The checklist was developed to evaluate safety performance management in construction company and Safety Management Index was suggested to coordinate 1) the score of results in accident rate and 2) the effort to evaluate a safety performance management. Safety Management Index is more helpful to evaluate the safety management results and effort. As a result of considering the score of the accident rate and the safety performance management, the outcome of prequalification was changed. Using this index, contractors will attempt to prevent accidents and owners will be able to discriminate individual differences in bidding process.

Analysis of Temporal and Spatial Distribution of Traffic Accidents in Jinju (진주시 교통사고의 시계열적 공간분포특성 분석)

  • Since changes in land use in urban space cause traffic volume and it is closely related to traffic accidents. Therefore, an analysis on the causes of traffic accidents is judged to be an essential factor to establish the measure to reduce traffic accidents. In this regard, the analysis was conducted on the clustering by using the nearest neighbor indexes with regard to the occurrence frequencies of commercial and residential zone based on traffic accident data of the past five years (2009-2013) with the target of local small-medium sized city, Jinju-si. The analysis results, obtained in this study, are as follows: the occurrence frequency of traffic accidents was the highest in spring and the lowest in winter respectively. The clustering of traffic accident occurrence at nighttime was stronger than at daytime. In addition, terms of the analysis on the clustering of traffic accident according to land use, changes according to the seasons was not significant in commercial areas, while clustering density in winter tended to become significantly lower in residential areas. The analysis results of traffic accident types showed that the side-right angle collision of cars was the highest in frequency occurrence, and widespread in both commercial areas and residential areas. These results can provide us with important information to identify the occurrence pattern of traffic accidents in the structure of urban space, and it is expected that they will be appropriately utilized to establish measures to reduce traffic accidents.

Implementation Techniques for the Seafarer's Human Error Assessment Model in a Merchant Ship: Practical Application to a Ship Management Company (상선 선원의 인적과실 평가 모델 구축기법: 선박관리회사 적용 실례)

  • In general, seafarer's human error is considered to be the preponderant muse for the majority of maritime transportation accidents in a merchant ship. The implementation techniques for Human Error Model (HEM) to assess possible accident risk by deck officers including captain, chief officer, second mate and third mate are described in this study. The scope of this work is focused to 642 deck officers in the ship management company with 130 vessels. At first, HEM can be constructed through the statistical analysis and expert's brainstorming process with human data to 642 deck officers. Then the variables $\upsilon$ for the human factors, the evaluation level EP($\upsilon$) for $\upsilon$, the weight $\alpha$ of $\upsilon$, and the title weight $\beta$ of each deck officers can be decided. In addition, through the analysis of ship's accident history, the accident causation ratios by human error ${\gamma}_H$ and by external error ${\gamma}_B$ can be found as 0.517(51.7%) and 0.483(48.3%), respectively. The correlation coefficients to $\upsilon$ are also shown significant for a 95% confidence interval (p < 0.05) for each coefficient. And the validity of HEM is also surveyed by the analysis of normal probability distribution of risk level RL to each deck officer.

Characteristics of Night Specialized Traffic Accident Hotspot Using Continuous Risk Profile (CRP) Analysis (CRP (Continuous Risk Profile) 분석을 이용한 야간 특화 교통사고 다발구간의 특성)

  • The purpose of this study is to develop a methodology to select a cluster of night traffic accidents using CRP (Continuous Risk Profile) analysis. the other purpose of this study is to present the characteristics of the traffic accident cluster section selected at night using CRP analysis. The CRP analysis was performed considering traffic volume of target routes through traffic accident data. In addition, variables were set according to the freeway sections. the method of subtracting the daytime CRP from the nighttime CRP was used to analyze the nighttime traffic accident. As a result, Using the CRP analysis, the sections of hotspot were identified and plotted based on traffic accidents. Also, the sections where traffic accidents are frequent were those where IC or Tunnels were installed, and there was a deviation from the general section. In conclusion, CRP analysis could be used to calculate the frequent section of specialized traffic accidents at night, and it was selected as a point in need of improvement due to the frequent occurrence of specialized traffic accidents at night in the section where IC or tunnel facilities are installed. In addition, it is inferred that the number of specialized traffic accidents at night in the section where IC or tunnel facilities are installed is a factor in the problem of night visibility due to lighting facilities.
