Frailty and its related Factors in Vulnerable Elderly Population by Age Groups (취약계층 노인의 연령별 허약정도와 관련 요인)

  • Purpose: This study aimed to investigate factors affecting frailty by age groups among vulnerable elders in Korea. Methods: In this secondary analysis, data were collected from records for 22,868 elders registered in the Visiting Health Management program of Public Health Centers in 2012. Health behaviors, clinically diagnosed disease, frailty, depression and cognitive condition were assessed. Data were analyzed using stepwise regression to determine the associated factors of frailty by age group. Results: Alcohol consumption, physical activity, number of diseases, DM, CVA, arthritis, urinary incontinence, depression and cognitive condition were found to be factors significantly associated with frailty among the elders aged 65~74 (F=135.66, p <.001). Alcohol consumption, physical activity, CVA, arthritis, urinary incontinence, depression and cognitive condition were found to be factors associated with frailty in the elders aged 75~84 (F=245.40, p <.001). Physical activity, CVA, arthritis, depression and cognitive condition were factors associated with frailty in the elders over 85 years of age (F=96.48, p <.001). Conclusion: The findings show that frailty of elders and associated factors were different by age group, and common factors affecting frailty were physical activity, CVA, arthritis, depression and cognitive condition. Thus, these factors should be considered in the development of intervention program for care and prevention of frailty and program should be modified according to age group.

Suppressive effect of Spirulina fusiformis in relation to lysosomal acid hydrolases, lipid peroxidation, antioxidant status, and inflammatory mediator TNF-alpha on experimental gouty arthritis in mice

  • The anti-inflammatory effect of Spirulina fusiformis on monosodium urate crystal-induced inflammation in mice has been investigated and compared with the non-steroidal anti-inflammatory drug Indomethacin. The paw volume, lysosomal enzyme activities, lipid peroxidation, anti-oxidant status and inflammatory mediator tumour necrosis factor-$\alpha$ were studied in control and monosodium urate crystal-induced mice after oral administration of Spirulina platensis in an experimental model for gouty arthritis. In the induced mice, the levels of lysosomal enzymes, inflammatory mediator tumour necrosis factor-$\alpha$, lipid peroxidation and the paw volume increased significantly, whereas the antioxidant status decreased when compared to control mice. $\beta$-glucuronidase and lactate dehydrogenase level were also found to be increased in untreated monosodium urate crystal-incubated polymorphonuclear leucocytes. After the oral administration of Spirulina fusiformis, the physical and biochemical changes observed in monosodium urate crystal-induced animals were significantly restored to near normal levels. The results clearly indicated the anti-inflammatory role of Spirulina fusiformis, a promising drug for gouty arthritis.

Effects of Tai Chi Exercise and Auricular Acupressure in Patients with Rheumatoid Arthritis (류마티스 관절염환자를 위한 타이치운동과 이압요법의 효과 - 영국 옥크셔지역을 중심으로 -)

  • Purpose: The purpose of this study was to evaluate the effects of a program of Tai Chi and auricular acupressure in patients with rheumatoid arthritis. Methods: The study design was a one-group pretest-posttest design. The subjects were 14 outpatients with a diagnosis of rheumatoid arthritis based on the American College of Rheumatology Standards. The setting was a Academic Section of Musculoskeletal Disease in the Leeds Institute of Molecular Medicine in United Kingdom. The program was Tai Chi exercise with auricular acupressure per two times a week, for twelve weeks. Results: A program of Tai Chi with auricular acupressure showed significant improvements in amount of pain of RADAI, 28 Joint count swollen, fatigue, physical, affect and symptom of AIMS2, function and symptom of ASES (p<0.05). However, no differences were found in the score of self-efficacy relate to the pain, and quality of healthy related life to social interaction. Conclusion: In order to improve the quality of healthy related life and self-efficacy relate to the pain, it requires long-term intervention. Also different research designs are needed to confirm the effect of this study.

Inhibitory Effects of Soyeum Pharmacopuncture (SPP) on Rheumatoid Arthritis in Collagen II-induced Arthritis (CIA) Mice (소염약침액이 Collagen II 유발 관절염 mouse의 TNF-α, IFN-γ 생성 및 비장세포 증식에 미치는 영향)

  • Objective : The aim is to examine the effect of Soyeum Pharmacopuncture (SPP) on collagen-induced arthritis (CIA) in DBA/1OlaHsd mice. Methods : To determine the effect of SPP on chronic IFNlammatory joint disease, we induced CIA in DBA/1OlaHsd mice by immunization with bovine type II collagen. Animals were treated with intraperitoneal injection doses of 2 mg/kg of SPP, beginning 3 days before the expected onset of disease symptoms. Inhibitory Effects of SPP were observed by serum levels of TNF-${\alpha}$, and IFN-${\gamma}$,,, or cell proliferation in the spleen cell culture and histological examination of knee joint. Results : In the CIA Mice, serum levels of TNF-${\alpha}$, and IFN-${\gamma}$,, production in the spleen cell culture were reduced. At the histopathological examination of knee joint, chondropathy of cartilage in the synovial joint in the SP group was repaired while compared with control group. Conclusion : These results suggest that the SPP may be effective for the prevention and treatment of rheumatoid arthritis disease.

Preparation and Evaluation of Chondroitin Sulfate/Gelatin Microspheres Containing Dexamethasone 21-Acetate (덱사메타손 21-아세테이트를 함유한 콘드로이틴 설페이트/젤라틴 마이크로스피어의 제조 및 평가)

  • Chondroitin sulfate/gelatin microspheres containing dexamethasone 21-acetate were prepared by complex coacervation method and their release patterns were examined in vitro. Microspheres prepared with a small amount of crosslinking agent had smooth surface and few pores, but those with a large amount of crosslinking agent were more porous and less spherical. In vitro release patterns were varied by changing polymer/drug weight ratio and amount of crosslinking agent. The release rate of dexamethasone 21-acetate in the presence of collagenase was faster than that in the absence of collagenase. Anti-inflammatory effect of dexamethasone 21-acetate microspheres was more efficient than that of dexamethasone 21-acetate solution in carrageenan-induced arthritis in the rat. On the basis of the above results, we might expect the degradation and drug release rate of these microspheres to be regulated by the degree of crosslinking and the level of enzymes. In patients with severe rheumatoid arthritis who have high concentration of collagenase, more drug would be released from the microspheres. An intra-articular injection therapy of rheumatoid arthritis with desired release kinetics could be developed to enhance patient compliance and therapeutic index.

Effects of Far Infrared on Nitric Oxide level at a rat model of Autoimmune Arthritis Induced (원적외선이 자가면역성 관절염 흰쥐모델의 산화질소정량에 미치는 영향)

  • OBJECTIVE: This study was Far infrared(FIR) irradiating to a rat collagen-induced autoimmune arthritis and observed nitric oxide level of spine. METHODS: Type II collagen(CII) in adjuvant was injected into rats on day 0. The results showed that rats injected with CII plus adjuvant had signs of autoimmune arthritis on day 14. Thereafter, Far infrared(FIR) irradiation was begun on day 15. The rats were irradiated FIR twice per day for 20 min each time. RESULTS: Seven days later(Day 22), the level of NO at FIR irradiated group(FRG; Arth+FIR) was decreased more than FIR-non irradiated group(FNRG; Arth). Twenty-one days later(Day 36), the level of NO at FRG was more closed to the control group's NO level than the FNRG. CONCLUSION: These results indicated that Far infrared irradiating appears to reduce the nitric oxide level at a rat model of autoimmune arthritis induced.

Study on the Effect of Geopungjeseup-tang in Rheumatoid Arthritis (거풍제습탕(祛風除濕湯)의 류마토이드 관절염 치료효과에 대한 실험적 연구)

  • This study was carried out to know the effect of Geopungjeseup-tang(GPJST) to on Rheumatoid Arthritis by using human fibroblast-like synoviocytes(hFLS). GPJST consists of Coisis Semen, Coccllus Trilobus DC., Lycopi Herba, Arisaematis Rhizoma. The author peformed several experimetal items : that is cytotoxicity of GPJST, mRNA expression of TNF-${\alpha}$, IL-1${\beta}$, ICAM-1, production of NO. The results were obtained as follows : The mRNA expression of pro-inflammatory cytokines TNF-${\alpha}$, IL-1${\beta}$, ICAM-1 were dose-dependently reduced in hFLS. The production of NO was reduced. Talking all these observations into account, GPJST is considered to be effective in treating rheumatoid arthritis. Therefore we have to survey continuously in looking for the effective substance and mechanism in the future.

Study on the Effect of Gwanjulbang-5 in Rheumatoid Arthritis (관절 5호방의 류마토이드 관절염 치료효과에 대한 실험적 연구)

  • This study was carried out to know the effect of Gwanjulbang-5(hereinafter refer to GJB-5) to on Rheumatoid Arthritis by using human fibroblast-like synoviocytes(hFLS). We performed several experimetal items : that is cytotoxicity of GJB-5, mRNA expression of pro-imflammatory cytokines in hFLS and production of NO, ROS. The results were obtained as follows : GJB-5 reduced the production of pro-inflammatory cytokines TNF-${\alpha}$, IL-1${\beta}$, IL-6, IL-8 in hFLS, increased the production of TIMP-1. As well as GJB-5 reduced the production of ICAM-1, MMP-3, NOS-II, the production of NO and ROS, and the proliferation of hFLS in proportion to the concentration of GJB-5. In conclusion, these results shows that GJB-5 had immunomodulatory effects in treating rheumatoid arthritis.

A Case Report of Clinical Effects of Complex Korean Medicine Treatment in Patients with Scleroderma Caused by Juvenile Idiopathic Arthritis (소아 류마티스 관절염으로 인한 경피증에 대한 한방복합치료 증례보고 1례)

  • Objectives This study is to report a case on a patient with scleroderma caused by juvenile idiopathic arthritis. The patient's symptoms were improved by the complex Korean medicine treatment. Methods The patient was treated for 20 days with acupuncture, moxa, physical therapy, herbal medicine (Ganghwalbinsosan, 羌活檳蘇散), and herbal dressing therapy(Dohongsamultang, 桃紅四物湯). Results NRS and mRSS were used evaluate patient's symptoms daily to assess the changes. After 20 days of the treatments, the patient showed improvement of the symptoms including pain, pruritus and scleroderma of the left leg. Conclusions This case study showed that symptoms in the patient with scleroderma caused by juvenile idiopathic arthritis were dramatically improved by the Korean medicine treatment which includes acupuncture, moxa, herbal medicine, and herbal dressing therapy.

Case Report of Sjögren's Syndrome with Arthritis diagnosed as Taeeumin's Dry-febrile Symptom (태음인(太陰人) 조열증(燥熱證)으로 진단한 관절염 동반 쇼그렌 증후군 치험례)

  • Objectives This study was designed to evaluate the effects of Korean medicine therapy(Cheongsimyeonja-tang) on Taeeumin patients diagnosed with Sj$\ddot{o}$gren's syndrome with arthritis. Methods The Sj$\ddot{o}$gren's syndrome with arthritis patient was treated with Cheongsimyeonja-tang. The progress was evaluated with the visual analogue scale and erythrocyte sedimentation rate on hematologic examination. Results There was improvement in the patient's Sj$\ddot{o}$gren's syndrome symptoms. The patient's symptoms of the alimentary system, dried fever and arthralgia evaluated with the visual analogue scale improved. Erythrocyte sedimentation rate was decreased on hematologic examination. Conclusions This study suggests that Korean medicine(Cheongsimyeonja-tang) is effective in the treatment of Sj$\ddot{o}$gren's syndrome on Taeumin patients.
