Clinicopathological Factors and Gastric Cancer Prognosis in the Iranian Population: a Meta-analysis

  • Background: Gastric cancer is the most common cancer in the Iranian population. The aim of this study was to determine the effect of clinicopathological factors on prognosis by meta-analysis. Materials and Methods: A literature search was conducted using MEDLINE, EMBASE and Cochrane library and extensive literature search using the Persian databases until February 2011. Prospective follow up studies with multivariate analysis of overall survival of the patients with gastric cancer were included in this review. The data were analyzed by CMA.2. Publication bias are checked by funnel plot and data are shown as Forest plots. Results: From a total of 63 articles, 14 retrospective studies which examined 5 prognostic factors and involving 10,500 patients were included. Tumor size (>35mm) was the main significant factor predicting an unfavorable prognosis for the patients with gastric cancer (RR=1.829, p<0.001) followed by presence of distant metastases (RR=1.607, p<0.001), poor differentiation (RR=1.408, p<0.001) and male sex (RR=1.194, p<0.001). Lymph node metastases (RR=1.058, p=0.698) and moderate differentiation (RR=0.836, p=0.043) were not statistically significant as prognostic factors. Conclusions: This meta-analysis suggests that tumor size>35mm, poor differentiation, presence of distant metastasis and male gender are strongly associated with a poor prognosis in Iranian patients with gastric cancer.

A Meta-Analysis of the Ventilator Circuit Change Period on Ventilator-Associated Pneumonia (인공호흡기 회로 교환주기가 인공호흡기 관련 폐렴에 미치는 영향에 관한 메타 분석)

  • Purpose: This meta-analysis was aimed to investigate the evidence of proper period of the ventilator circuit change using existing research. Methods: For this study, 14 published studies between 1995 and 2010 were tested by Macaskill, Funnel Plot, the Odds Ratio of DerSimonian and Laird, Fisher and Liptak analysis. Results: There were no publication bias found in the subjects. The results of the meta-analysis demonstrated no statistically significant differences were observed in neither the Odds Ratio (OR=1.18, 95% CI=0.94-1.47) of the frequency of ventilator-associated pneumonia and the mortality based on the period of the ventilator circuit change (Fisher p=.332, Liptak p=.498), nor the ventilation duration of ventilator (Fisher p=.843, Liptak p=.506), and the hospital length of stay (Fisher p=.254, Liptak p=.480). Conclusion: In order to present more concrete guidelines on the period of the ventilator circuit change, further research is warranted to thoroughly control confounding variables which related to the periods of the ventilator circuit change.

Cytochrome P450 monooxygenase analysis in free-living and symbiotic microalgae Coccomyxa sp. C-169 and Chlorella sp. NC64A

  • Microalgae research is gaining momentum because of their potential biotechnological applications, including the generation of biofuels. Genome sequencing analysis of two model microalgal species, polar free-living Coccomyxa sp. C-169 and symbiotic Chlorella sp. NC64A, revealed insights into the factors responsible for their lifestyle and unravelled biotechnologically valuable proteins. However, genome sequence analysis under-explored cytochrome P450 monooxygenases (P450s), heme-thiolate proteins ubiquitously present in species belonging to different biological kingdoms. In this study we performed genome data-mining, annotation and comparative analysis of P450s in these two model algal species. Sixty-nine P450s were found in two algal species. Coccomyxa sp. showed 40 P450s and Chlorella sp. showed 29 P450s in their genome. Sixty-eight P450s (>100 amino acid in length) were grouped into 32 P450 families and 46 P450 subfamilies. Among the P450 families, 27 P450 families were novel and not found in other biological kingdoms. The new P450 families are CYP745-CYP747, CYP845-CYP863, and CYP904-CYP908. Five P450 families, CYP51, CYP97, CYP710, CYP745, and CYP746, were commonly found between two algal species and 16 and 11 P450 families were unique to Coccomyxa sp. and Chlorella sp. Synteny analysis and gene-structure analysis revealed P450 duplications in both species. Functional analysis based on homolog P450s suggested that CYP51 and CYP710 family members are involved in membrane ergosterol biosynthesis. CYP55 and CYP97 family members are involved in nitric oxide reduction and biosynthesis of carotenoids. This is the first report on comparative analysis of P450s in the microalgal species Coccomyxa sp. C-169 and Chlorella sp. NC64A.

Quantitative Determination of Triterpenoids from the Fruits of Zizyphus jujuba

  • Eleven triterpenoids, colubrinic acid (1), alphitolic acid (2), 3-O-cis-p-coumaroyl alphitolic acid (3), 3-O-trans-p-coumaroyl alphitolic acid (4), 3-O-cis-p-coumaroyl maslinic acid (5), 3-O-trans-p-coumaroyl maslinic acid (6), betulinic acid (7), oleanolic acid (8), betulonic acid (9), oleanonic acid (10), and zizyberenalic acid (11), were isolated from the fruits of Zizyphus jujuba Mill. A simple and rapid HPLC method, using a $C_{18}$ column, has been developed for the quantitative analysis of these compounds 1 (0.74%),2 (0.09%), 3 (0.19%), 4 (0.19%), 5 (0.08%),6 (0.08%), 7 (0.41%), 8 (0.05%), 9 (0.50%),10 (0.59%), and 11 (0.19%).

Effects of the Electroless Ni-P Thickness and Assembly Process on Solder Ball Joint Reliability (무전해 Ni-P 두께와 Assembly Process가 Solder Ball Joint의 신뢰성에 미치는 영향)

  • The ability of electronic packages and assemblies to resist solder joint failure is becoming a growing concern. This paper reports on a study of high speed shear energy of Sn-4.0wt%Ag-0.5wt%Cu (SAC405) solder with different electroless Ni-P thickness, with $HNO_3$ vapor's status, and with various pre-conditions. A high speed shear testing of solder joints was conducted to find a relationship between the thickness of Ni-P deposit and the brittle fracture in electroless Ni-P deposit/SAC405 solder interconnection. A focused ion beam (FIB) was used to polish the cross sections to reveal details of the microstructure of the fractured pad surface with and without $HNO_3$ vapor treatment. A scanning electron microscopy (SEM) and an energy dispersive x-ray analysis (EDS) confirmed that there were three intermetallic compound (IMC) layers at the SAC405 solder joint interface: $(Ni,Cu)_3Sn_4$ layer, $(Ni,Cu)_2SnP$ layer, and $(Ni,Sn)_3P$ layer. The high speed shear energy of SAC405 solder joint with $3{\mu}m$ Ni-P deposit was found to be lower in pre-condition level#2, compared to that of $6{\mu}m$ Ni-P deposit. Results of focused ion beam and energy dispersive x-ray analysis of the fractured pad surfaces support the suggestion that the brittle fracture of $3{\mu}m$ Ni-P deposit is the result of Ni corrosion in the pre-condition level#2 and the $HNO_3$ vapor treatment.

NEW COMPLEXITY ANALYSIS OF PRIMAL-DUAL IMPS FOR P* LAPS BASED ON LARGE UPDATES

  • In this paper we present new large-update primal-dual interior point algorithms for $P_*$ linear complementarity problems(LAPS) based on a class of kernel functions, ${\psi}(t)={\frac{t^{p+1}-1}{p+1}}+{\frac{1}{\sigma}}(e^{{\sigma}(1-t)}-1)$, p $\in$ [0, 1], ${\sigma}{\geq}1$. It is the first to use this class of kernel functions in the complexity analysis of interior point method(IPM) for $P_*$ LAPS. We showed that if a strictly feasible starting point is available, then new large-update primal-dual interior point algorithms for $P_*$ LAPS have $O((1+2+\kappa)n^{{\frac{1}{p+1}}}lognlog{\frac{n}{\varepsilon}})$ complexity bound. When p = 1, we have $O((1+2\kappa)\sqrt{n}lognlog\frac{n}{\varepsilon})$ complexity which is so far the best known complexity for large-update methods.

Comparative analysis of cardiopulmonary resuscitation accuracy and fatigue by posture in hospitals (병원 내 심폐소생술의 자세별 피로도와 가슴압박 정확도 비교 분석)

  • Purpose: This study aimed to identify effective cardiopulmonary resuscitation methods by comparing the fatigue of rescuers according to various positions in cardiopulmonary resuscitation situations conducted on beds in hospitals. Methods: An experimental study of students in the department of emergency medical service in H University, G Metropolitan City was conducted in four positions for applying chest pressure on mannequins on beds. Results: As a result of measuring the muscle fatigue of four muscle attachments according to the four positions conducted on the bed, the average was 3.4%, the P was significant at 0.001, and the fatigue difference was confirmed to occur depending on the attachment. An analysis of pressure depth by pose revealed that P1, P2, P3, and P4 have a depth of 58.3, 55.1, 56.4, and 56.3 mm, respectively, with P4 having the deepest depth. Conclusion: Among the various postures of the rescuer during cardiopulmonary resuscitation performed on the bed in the hospital, P1 is thought to be the most tiring, although its associated CPR quality is good.

Response Surface Analysis of Dietary n-3/n-6 and P/S Ratio on Reduction of Plasma Lipids in Rats (흰쥐현장지질 감소에 관한 n-3/n-6 와 P/S 섭취비율의 반응표면분석)

  • Response surface analysis was used to study dietary ratios of n-3/n-6 fatty acid and P/S to minimize plasma triglycerides, total cholesterol and LDL ${\cdot}$ VLDL-C levels and maximize plasma HDL ${\cdot}$ C levels of rats. Because the dietary components were not statistically independent, they were studied in combinations of two variables. The two-variable combinations were the most useful in locating the desired maximum or minimum plasma triglycerides, total cholesterol and LDL ${\cdot}$ VLDL-C response in terms of the proportions of the dietary components. Response surface contours and three dimensional plots were developed for each plasma lipid response. The contours and three dimensional plots were used to help determine those combinations of the dietary fatty acid ratios that would produce the desired minimum or maximum lpid responses. The statistical analyses indicated that the minimized plasma cholesterol response levels could be attained with a diet consisting of 2.26 n-3/n-6 fatty acid and 2.15 P/S ratios.

A methodology for the identification of the postulated initiating events of the Molten Salt Fast Reactor

  • The Molten Salt Fast Reactor (MSFR) with its liquid circulating fuel and its fast neutron spectrum calls for a new safety approach including technological neutral methodologies and analysis tools adapted to early design phases. In the frame of the Horizon2020 program SAMOFAR (Safety Assessment of the Molten Salt Fast Reactor) a safety approach suitable for Molten Salt Reactors is being developed and applied to the MSFR. After a description of the MSFR reference design, this paper focuses on the identification of the Postulated Initiating Events (PIEs), which is a core part of the global assessment methodology. To fulfil this task, the Functional Failure Mode and Effect Analysis (FFMEA) and the Master Logic Diagram (MLD) are selected and employed separately in order to be as exhaustive as possible in the identification of the initiating events of the system. Finally, an extract of the list of PIEs, selected as the most representative events resulting from the implementation of both methods, is presented to illustrate the methodology and some of the outcomes of the methods are compared in order to highlight symbioses and differences between the MLD and the FFMEA.

The Effects of distilled Cervi Pantotrichum Cornu Pharmacopuncture and Zizyphi Spinosi Semen Pharmacopuncture on the Heart Rate Variability (녹용 약침과 산조인 약침이 정상인의 심박변이도(HRV)에 미치는 영향)

  • Objective : This study was to investigate the effects of distilled Cervi Pantotrichum Cornu Pharmacopuncture and Zizyphi Spinosi Semen Pharmacopuncture on autonomic nervous system with Heart Rate Variability(HRV). Purpose of the trial was to observe what influence distilled Cervi Pantotrichum Cornu Pharmacopuncture and Zizyphi Spinosi Semen Pharmacopuncture have on the autonomic nervous system. Methods : 60 healthy male volunteers were divided into three groups which consist of two experimental groups such as Cervi Pantotrichum Cornu Pharmacopuncture and Zizyphi Spinosi Semen Pharmacopuncture, and a control group. Study design was a randomized, placebo-controlled, double-blind clinical trial. 20 subjects in experimental group were injected distilled Cervi Pantotrichum Cornu Pharmacopuncture at $GB_{21}$(Kyonjong), 20 subjects in other experimental group were injected distilled Zizyphi Spinosi Semen Pharmacopuncture and 20 subjects in control group were injected Normal Saline at $GB_{21}$(Kyonjong). One volunteer of each groups were excluded from analysis because of error during measuring HRV. At the end of the study 57 volunteers completed HRV analysis. HRV was measured by QECG-3:LXC3203(LAXTHA Inc. Korea) at P0, P1, P2, P3, P4, P5 and P6 and its time-course dependent change in each group was analyzed using paired t-test, and the difference of HRV fluctuation among two experimental group and a control was evaluated by one way ANOVA(p<0.05). Results : A. Time Domain Analysis 1. Analysis of Mean HRV, SDNN and Complexity After injection of distilled Cervi Pantotrichum Cornu Pharmacopuncture, Mean HRV was significantly decreased only at 20 minutes after injection. SDNN was significantly increased from 15 minute after injection. Complexity was significantly decreased only at 15 minute after injection. After injection of distilled Zizyphi Spinosi Semen Pharmacopuncture, Complexity was significantly decreased at 10, 15 and 30 minutes after injection. 2. Analysis of HRV index, pNN50 After injection of distilled Cervi Pantotrichum Cornu Pharmacopuncture, pNN50 was significantly decreased at 15, 20 and 25 minutes after injection. Compared with Normal Saline injection, distilled Cervi Pantotrichum Cornu Pharmacopuncture and Zizyphi Spinosi Semen Pharmacopuncture showed significant difference on HRV index and pNN50 at 5 minutes after injection. B. Frequency Domain Analysis 1. Analysis of Ln(TP), Ln(VLF) After injection of distilled Cervi Pantotrichum Cornu Pharmacopuncture, Ln(TP) was significantly increased at 15 and 30 minutes after injection. Ln(VLF) was significantly increased at 10, 15 and 30 minutes after injection. After injection of distilled Zizyphi Spinosi Semen Pharmacopuncture, Ln(VLF) was significantly increased only at 30 minutes after injection. Compared with Normal Saline injection, distilled Cervi Pantotrichum Cornu Pharmacopuncture showed significant difference on Ln(TP) after 5 minutes of injection. Conclusions : Our results suggest that Cervi Pantotrichum Cornu Pharmacopuncture and Zizyphi Spinosi Semen Pharmacopuncture in healthy adult man tend to activate the autonomic nervous system and sympathetic nervous system compared to Normal Saline within normal range.
