Nurse-Patient Interaction Patterns and Patient Satisfaction in the Emergency Department (군집분석을 이용한 응급실 간호사-환자 상호작용 유형 및 환자의 상호작용 만족도)

  • Purpose: The purpose of this study was to identify effective nurse interaction patterns with patients in the emergency department. Methods: For this study, video technology was used to record complete conversations between the nurse and patient. The participants were 28 nurses and 63 patients in the emergency department at one university hospital located in Seoul. The data were collected from November, 2002 to April, 2003. The video recordings were observed for 4 hr for each case and coded using an adapted version of Roter's Interaction Analysis System (RIAS). The data were analyzed using cluster analysis to identify the patterns of nurse-patient interaction. Results: Cluster analysis revealed 4 distinct nurse interaction patterns; 1) "closed" characterized by orientation and negative talk, 2) "positive" characterized by positive affective talk, 3) "informative and directing" characterized by task-focused behavior including data gathering, and giving information about medical condition and treatment, 4) "facilitative" characterized by balance of psychosocial and biomedical topics. Patient satisfaction was highest in the facilitative interaction pattern. Conclusion: The patient centered interaction pattern, balancing information exchange and psychosocial exchange are the most effective interactions in the emergency department, suggesting that effective interaction skill is a core clinical nursing intervention in acute care.

Demerol, Phenergan, and Atarax Premedication in Dentistry for Children (소아치과 환자 치료에 있어서 Demerol, Phenergan, Atarax 준비투약에 대한 연구)

  • 저자는 소아치과 환자의 일반적인 치료시 흔히 나타나는 과민성, 불협조, 공포감 등의 아동 취급상의 난점들을 심리적인 충격없이 해소함을 목적으로 Demerol, Phenergan과 Atarax들의 약제를 치료전에 준비투약 함으로써 다음과 같은 결과를 얻었다. 1) 일반 소아과에서 사용되는 약물용량보다 소아치과 외래환자에선 2배의 용량에서 만족스러운 효과를 얻을 수 있었다. 2) 용량결정에 있어서 최초 투약용량을 정확히 산출함으로써 준비투약의 성공을 가져올 수 있었다. 3) 준비투약의 성공은 일정한 기간후 환자의 recall appointment시 준비투약 없이도 보다 나은 협조을 얻을 수 있었다. 4) Demerol, Phenergan, Atarax사용에 있어서 그 부작용은 거의 무시할 수 있을 정도로 경한 것이였으며 환자의 심리적인 지원을 갖어 올 수 있어 아동 취급상에 큰 효과를 얻을 수 있있다.

Determinants of Patients Satisfaction and Intent to Revisit Oriental Medical Hospitals (한방병원 환자 만족도 및 재이용 의사 결정요인)

  • This study aimed to investigate the determinants of patients satisfaction and intent to revisit oriental medical hospitals. The level of overall patient satisfaction was used as an intervening variable and the level of intent to revisit was used as a dependent variable. The sample used in this study consisted of 578 patients from 3 oriental medical hospitals located in Chungnam Province. Data were collected from October 2014 to December 2014 with a structured and self-administrated questionnaire and analysed using path analysis. The results of the study indicate that oriental medical hospitals should make an effort to improve the overall satisfaction of patients, in the case of outpatient, especially focusing on the doctors' service and medical procedure, and in the case of inpatient, especially focusing on the facilities convenience, medical price and hospital ward life which will lead to high level of intent to revisit of patients.

  • This study aims to provide direction for improvement of the hospital's management environment for customer satisfaction management, which is ultimately consumer-oriented in practice. this study was analyzed the relationship between hospital use satisfaction relative to expected level and medical service provision process for inpatients. The study method collected data from September 10 to September 30, 2018 for patients with inpatient care at hospitals and general hospitals under 300 beds, and conducted a t-test, ANOVA, and logistic regression. According to the analysis, hospital brands were the most frequently chosen reason for hospitalization, followed by doctor brands. The satisfaction level of the inpatient medical service provision process showed no statistical differences depending on the general characteristics. It has been confirmed that the hospital's satisfaction level relative to the expected level of inpatients has a causal relationship that significantly affects the hospitalized procedure the course of the provision of medical services.

Perception and Satisfaction on Nutrition Counseling Service for Patients Consuming a Therapeutic Diet at Hospitals in Busan (부산지역 일부병원 치료식 섭취 환자의 영양상담에 대한 인식 및 만족도)

  • The purpose of this study was to investigate the perception and satisfaction on the nutrition counseling service for patients consuming a therapeutic diet at hospitals in Busan. The subjects were 153 inpatients at five hospitals with over 400 beds each. The research was performed through the interviewing process using questionnaires conducted from January to February, 2008. In a total of 88 patients, 57.5% had experienced nutrition counseling and were through the motives of counseling with doctors 64.2% and themselves 29.3% of the patients. In the method of nutrition counseling, 58% of the patients had an individual counseling. In the patients' perception on the nutrition counseling, 75.0% of the patients understood very well, 83.0% of them perceived the explanation as very important, 79.5% were very satisfied and 78.7% were helped in nutrition-management. On a scale of 5.00 for the nutrition counseling satisfaction, the average scores were 3.80 for 'explanation of knowledge', 3.71 for 'cognitive communication skills', 4.05 for 'effective communication skills' and 3.60 for 'facilitation skills'. The items of low scores in the nutrition counseling satisfaction were 'follow up diet-therapy after discharge', 'providing to personalized nutrition information', 'presentation of specific menu' and 'methods of menu planning'. There were significant (p<0.01) positive correlation between perception and satisfaction on the nutrition counseling. Therefore, it was suggested that dietitians increase the patients' satisfaction on the nutrition counseling with developing the model based on the patient-centered counseling.

Percutaneous K-wire Fixation of Supracondylar Fracture of Humerus in Children (소아 상완골 과상부 골절의 경피적 K-강선 고정술)

  • During the period from March 1990 to November 1993, 22 children with diplaced supracondylar fracture of humerus were treated by closed reduction and percutaneous K-wire fixation at department of Orthopaedic surgery in Yeungnam University. All fracture were treated with closed reduction under the general anesthesia and percutaneous pinning. After the fracture was internally fixed, intraoperative anteroposterior reontgenograms of each distal humerus were compared. Only anatomical reduction ensured good result, because the main cause of late cubitus varus was medial tilting of distal fragment. We could follow up 10 patients more than one year. By Flynn's criteria, satisfactory result were obtained in 9 of the 10. Unsatisfactory one had a limitation in flexion of elbow joint, but had no problem in cosmetic feature. In short term follow up of 7 patients, 5 patients had a satisfactory result, too. There were no neurovascular complication and hospitalization was shortened to 11.4 days. Closed reduction and percutaneous pinning provided stabillity, vascular safety, simplified management, reduced hospital stay. This is safe and reliable technique for obtaining and maintaining an excellent reduction.
