Simulation of Local Climate and Crop Productivity in Andong after Multi-Purpose Dam Construction (임하 다목적댐 건설 후 주변지역 기후 및 작물생산력 변화)

  • A simulation study was carried out to delineate potential effects of the lake-induced climate change on crop productivity around Lake Imha which was formed after a multi-purpose dam construction in Andong, Korea. Twenty seven cropping zones were identified within the 30 km by 25 km study area. Five automated weather stations were installed within the study area and operated for five years after the lake formation. A geostatistical method was used to calculate the monthly climatological normals of daily maximum and minimum temperature, solar radiation and precipitation for each cropping zone before and after the dam construction. Daily weather data sets for 30 years were generated for each cropping zone from the monthly normals data representing "No lake" and "After lake" climatic scenarios, respectively. They were fed into crop models (ORYZA1 for rice, SOYGRO for soybean, CERES-maize for corn) to simulate the yield potential of each cropping zone. Calculated daily maximum temperature was higher after the dam construction for the period of October through March and lower for the remaining months except June and July. Decrease in daily minimum temperature was predicted for the period of April through August. Monthly total radiation was predicted to decrease after the lake formation in all the months except February, June, and September and the largest drop was found in winter. But there was no consistent pattern in precipitation change. According to the model calculation, the number of cropping zones which showed a decreased yield potential was 2 for soybean and 6 for corn out of 27 zones with a 10 to 17% yield drop. Little change in yield potential was found at most cropping zones in the case of paddy rice, but interannual variation was predicted to increase after the lake formation. the lake formation.

The Effect of Periodical and Individualized Educational Program for Long-term Hemodialysis Patient (장기혈액투석 환자를 위한 주기적 개별교육 프로그램 적용 효과)

  • Purpose: This study was conducted to provide an effective nursing intervention using an individualized educational program composed with knowledge, compliance, and physiologic parameters for long-term hemodialysis patients. Methods: A quasi-experimental study using a non-equivalent control group and pre- and post-test design was conducted with 40 hemodialysis patients at G university hospital in the J city from June to August, 2015. A data was analyzed using frequency, percentage, ${\chi}^2$ test, Shapiro-Wilk test, independent-samples t-test and repeated measures ANOVA using SPSS 21.0 program. Results: knowledge about hemodialysis and patient role behaviors were not significantly different between the two measures. In the biological index, there was significant difference between the groups by points in time and group in blood potassium, albumin, and Kt/v. However, there was no difference in gaining weight between hemodialysis, hemoglobin, and blood phosphorus. Conclusion: The individual training program in this study had an effect on changing some physiological indicators of long-term hemodialysis patients. Future research is warranted for developing various kinds of education program incorporating the findings of the study for the given population.

  • Thyroid radiology practice is a medical practice in which thyroid diseases are diagnosed using imaging modality and treated by imaging-based interventional techniques, and the primary care target is thyroid nodular disease. Diagnosis of thyroid nodules is primarily done by ultrasound imaging and biopsy; thyroid nodules can be treated by non-surgical interventional treatment and thyroidectomy. Ethanol ablation is the first-line treatment for cystic benign nodules, and radiofrequency ablation is used for the treatment of benign solid nodules and recurrent thyroid cancers. Thyroid radiology practice has an essential clinical role in diagnosis and nonsurgical treatment of thyroid nodular diseases, and treatment should be performed based on standard care guidelines for proper patient care. In order to provide the best care to patients with thyroid nodular disease, it is desirable to treat patients in the radiology outpatient clinic. Thyroid radiology practice centered on outpatient clinic practice needs to be expanded.

The Effect of Social Support on Compliance with Sick Role Behavior in Hypertensive Clients and Duration-of the Effect for up to 6 months (사회적 지지가 고혈압환자의 역할행위 이행에 미치는 영향과 지지요법 효과의 지속에 관한 연구 I)

  • This study was done using a Quasi-experimental research design to determine the effects of social support on compliance with sick role behaviors in hypertensive patients and to know if the effect of the social support on compliance lasted for at least 6 months. The subjects consisted of 81 hypertensive patients who were registered in the Cardio-Vascular OPD at Chonnam National University Hospital. They were divided by random sampling into 42 people for the experimental group and 39 for the control group. Data were gathered from June 3, 1996 to June 10, 1997 through individual interviews using a structured questionnaire. The results of the study were summarized as follows : 1. Compliance with sick role behaviors in hypertensive clients was significantly increased in the experimental group who received social support from the nurse as compared to the control group who did not receive social support(t=15.99. p<.001). 2. The effect of social support on compliance with sick role behaviors in hypertensive clients lasted for 6 months(t=7.99, p<.001). 3. Four of six people stopped smoking in experimental group after the intervention of social support, but none of the five in control group were able to stop smoking. Fisher's Exact test showed a significant difference between the experimental and control group(x²=4.385. p< .05). Mantel Haenszel test showed that the effect of social support on stopping smoking in the experimental group lasted for six months because there were no significant differences between one month after the social support and six months after, in the number of subjects who stopped smoking(x²=1.154, P>.05). Finally, social support was effective on compliance with sick role behaviors and stopping smoking in the hypertensive clients, and the effect of social support on compliance lasted for 6 months.

Effect of Periodic Video Education on Knowledge about Hemodialysis, Patient Role Behavior and the Physiologic Index in Patients with Hemodialysis (주기적 영상교육이 혈액투석환자의 투석 관련 지식, 환자역할행위, 생리적 지표에 미치는 효과)

  • Purpose: The purpose of this study was to evaluate the effect of periodic video education on knowledge about hemodialysis, patient role behavior and the physiologic index in patients with hemodialysis. Methods: One-group pretest-posttest design was applied. A total of 50 patients with hemodialysis at a university hospital hemodialysis unit in Daegu participated. Periodic video education about kidney function, diet, arteriovenous fistula management, medication, and exercise was administered to them for 12 weeks at intervals of 2 weeks. Before and after the education, the participants completed the self-administered questionnaires of knowledge about hemodialysis and patient role behavior and the measures of serum potassium and phosphorus concentration, weight gain between hemodialysis sessions, and adequacy of dialysis (Kt/V). Statistical analysis was performed using paired t-test. Results: Knowledge about hemodialysis and patient role behavior were significantly different between before and after education (p<.01). For the physiologic index, serum potassium concentration and weight gain between hemodialysis sessions were not significantly different but the serum phosphorus concentration and Kt/V were significantly different between before and after education (p<.05). Conclusion: The results suggest that the periodic video education for patients with hemodialysis contributed to the change and improvement in knowledge about hemodialysis, patient role behavior, and the physiologic index.

A Study on the Cooperation between Medical Care and Law - Focusing on the discussion of the role of clinical practice guideline in Japan - (의료와 사법(司法)의 협력 -일본에서의 진료가이드라인의 역할에 대한 논의를 중심으로-)

  • There are two aspects of clinical practice guidelines that act as non-legal control before medical practice and as legal control standards after medical practice. The essential purpose of clinical practice guidelines is the former, but the latter action cannot be excluded. The clinical practice guidelines are a means of linking law and medical care. The negative perception of clinical practice guidelines that medical professionals' autonomy can be violated by the enactment of clinical practice guidelines is an excessive negative evaluation of clinical practice guidelines. Rather, judicial judgment based on clinical practice guidelines plays a role in respecting the autonomy of medical professionals. In other words, the clinical practice guidelines suppress legal regulations on medical care as much as possible and are based on doctors' professional ethics and self-discipline, and patient awareness and cooperation. In order to establish an ideal relationship of cooperation between doctors and patients, 'medical ethics' must be incorporated as a legal means. Clinical practice guidelines are the most appropriate means for incorporating such medical ethics into legal procedures. The lawyer solves the case with a legal syllogism that establishes a norm and applies facts to it to conclude. For the resolution of medical disputes, Clinical practice guidelines are used to establish norms that doctors should perform for specific diseases, and conclusions are drawn by applying the established norms to specific medical practices. When it is not easy to apply the established norms to specific medical practices, medical judgments by experts, such as emotions, expert testimony, and explanations by expert members, are used. As such, the Law respects the autonomy of medical care even in the establishment of norms and the application of norms. In particular, Clinical practice guidelines prepared independently by the medical community are referred to in establishing norms, which are the prerequisites for legal syllogism. This shows that doctors participate in the formation of precedents and contribute to the formation of norms. The use of clinical practice guidelines in trials is respect and consideration for the autonomy of medical care. Although there may be an aspect in which the autonomy of individual doctors is limited by clinical practice guidelines, it should be considered that the autonomy of doctors as a group is respected. In this way, the clinical practice guidelines play a role in protecting the autonomy of the "medical" group from the logic of the "law."

The Effect of Discharge Education Program on Compliance with Sick-Role Behavior in Cerebrovascular Surgery Patients (퇴원교육 프로그램이 뇌혈관 수술환자의 역할행위 이행에 미치는 영향)

  • Purpose: The purpose of this study was to examine the effect of the discharge education program on compliance with the sick role behavior for patients having undergone cerebrovascular surgery. Method: Research was done using a posttest only design. The subjects were 60 patients who were admitted to the neurosurgery unit at C.N.U. Hospital in G. City and were divided into the experimental and control groups. The discharge education program were intervened two times in the experimental group by the researcher; the first one was at the time of discharge using a booklet about knowledge related to disease and compliance, and the other one was a telephone education session after a week from discharge. Data were collected two times by interview and telephone using questionnaires from January 19, to June 10, 2000. The first one was at hospital before discharge, and the other one was one month later from discharge. Data were analysed by $\chi^{2}$, t-test, ANOVA, and Pearson's correlation. Results: The experimental group showed a higher score of compliance(t=2.772, p=.008) than those of the control group, but knowledge about CVD was not significant between the two groups(p>.05). Conclusion: The discharge education program was effective on the compliance of the patients having undergone cerebrovascular surgery.

Effects of Individualized Education on Knowledge, Compliance, and Physiologic Parameters in Hemodialysis Patients (개별교육이 혈액투석환자의 투석관련 지식, 환자역할행위 이행 및 생리적 지수에 미치는 효과)

  • Purpose: This study was designed to explore the effects of providing individualized education for hemodialysis patients on their knowledge of hemodialysis, compliance of patient role behavior, and physiologic parameters. Method: A quasi experimental design with a non-equivalent control group and a non-synchronized design was used. The experiment was conducted with a total of 40 hemodialysis patients (20 in the experimental group and 20 in the control group) at the artificial kidney center, C University Hospital. The experimental group was provided with individualized education, 30 minutes per session, three times per week, for two weeks. Results: The experimental group had significantly higher knowledge of hemodialysis than the control group after the education. The compliance of patient role behavior was more enhanced in the experimental group than the control group. The experimental group showed significantly higher values of blood urea nitrogen, but not the values of blood creatinine, albumin, kalium, and phosphorus. Conclusion: The individualized education was found to be an effective intervention for improving patients' knowledge of hemodialysis and compliance of patient role behavior.

Development and Evaluation of Integrated Management Program for Hemodialysis Patients (혈액투석 환자를 위한 통합적 관리 프로그램의 개발 및 효과)

  • Purpose: This study aimed to develop and evaluate an integrated management program to enhance self-efficacy, compliance with sick-role behaviors, symptom management, and biomarker indication in hemodialysis patients. Methods: The integrated management program was developed through a systematic review of literature, analysis of relevant online data, and expert validation. It comprised 480 min of video-based education delivered eight times over four weeks, supplemented by weekly phone consultations and text message support from a nurse. To evaluate the program's effectiveness, it was administered to 44 patients with hemodialysis in a single group in a pre-post test experimental study. Changes in self-efficacy, sick-role behavior compliance, dialysis symptom index, and biomarkers were assessed. Results: The program yielded statistically significant improvements in self-efficacy (t=-7.13, p<.001), sick-role behavioral compliance (t=-7.35, p<.001), dialysis symptom index (t=4.32, p<.001), and blood urea nitrogen levels (t=2.55, p=.014) among the participants. Conclusion: The integrated management program is an effective intervention for improving hemodialysis patients' self-efficacy, compliance with sick-role behaviors, and experience of symptoms. Additionally, it is considered an intervention with high clinical applicability and efficiency through video reproducibility.

Semiotic approach to Resort's Organizational Culture : Applying Greimas Actant Model (리조트 기업의 조직문화에 대한 기호학적 분석 : 그레마스 행위소 모형 적용)

  • This research aimed to verify organizational culture of resort underlying bottom of organization and shared by employees. Healing-seeking and life style oriented experiences are in prevailing recently, which resort industry insomuch as presumably held from its origin. Unfortunately, it is also widely recognized that resort lost its entity, blurring business domains with hotel industry or yielding integrated resort by imitating theme park. In order to probe organizational culture, in-depth interview with 16 resort experts conducted in resort setting, prepared by Gremeis actant model which is effective in finding myth(story) structure of interviewees. Firstly, interviewer matched six actants with resort business involving-group and asked interviewee to metaphor freely based on 3 night resort experiences. Results showed that sender, receiver, subject, object, helper and opponent were matched with resort, visitor, company, product & service, supporters, and opponent group in orderly manners and also interviewees made each actant into metaphor as healing, patient, medical doctor, medicine, helper and charlatan, respectively. Significance and limitation of research were included in the end of article.
