A Clinical Study of Postpartum Care in Oriental-Western Cooperative Medicine and Survey of Patients' Satisfaction (협진 의뢰 산모의 한방산후조리 현황 분석 및 만족도 조사)

  • Purpose: We analyzed the characteristics of women who gave birth at an obstetrician and those who received oriental medicine postpartum care, and investigated the level of their satisfaction to obtain fundamental data for Oriental-Western cooperative medicine treatment for postpartum care. Methods: The review was based on a 5-months' data, from August 1 through December 31, 2009. Information on age, labor record, level of education, and occupation of the patients who had received postpartum oriental-western cooperative medicine treatment was based on Electric Medical Record(EMR) of O O Oriental Medicine Hospital. The questionnaires were completed by the patients at the time of their discharge from the hospital. Results: The patients who were admitted to oriental medicine hospitals tended to be primiparae, and they were more likely to be younger women with higher education levels. The level of satisfaction on postpartum care program of oriental medicine hospitals was high. Conclusion: It is necessary to specialize the facilities, dietary formula, and staff training of oriental medicine hospitals through a survey of postpartum patients' demand and satisfaction level. In oriental-western cooperative medicine hospitals, it is needed to specialize the postpartum program for creating demand of premature delivery mothers.

A Clinical Study for Rota Virus Infection of Neonates in Oriental Medicine Hospital Postpartum Care Center (한방병원 산후조리센터 내 신생아의 Rota Virus 감염증에 대한 임상적 고찰)

  • Objective : The aim of this study is to investigate rota virus infection of neonates In the Postpartum care center. Methods : This clinical study was carried out with the 205 neonates who admitted to the Postpartum Care Center of ${\bigcirc}{\bigcirc}$ Oriental Medicine Hospital, from July, 2004 to July, 2005. Stools were examined daily with Rotazyme ELISA tests(Bio Line Rota Virus, SD Standard Diagnostics, INC.) for 3 days from the day of admission. Supplementary test was examined when change was found in stools. Results : Among 205 newborn babies, 58 cases turned out to be positive. The positive rate of rota virus was not significantly related to sex, birth weight, gestation age. But, it was significantly related to delivery method and it was higher in caesarean section group. The manifestation rates of symptoms were 51.7% in Rotazyme positive neonates. In the Rotazyme positive 58 cases, 30 cases(51.7%) had diarrhea, 8 cases had fever(13.8%), 1 case(1.7%) had vomiting. The positive rate of rota virus was higher during from october to march. There were 190 cases(92.2%) of mixed feeding and 15 cases(7.8%) of artificial feeding. There was significance between breast feeding and the positive rate of rota virus. There was no significance between hospital of birth and the rate of positive rotazyme test. Conclusions : A further study of neonates in postpartum care center is necessary as preference of postpartum care center was increased. And the oriental medicine treatment of rota virus IS expected to be effective, so a further study is necessary.

An Investigation into the State of Doctors' Linkage and Oriental Medical Treatment at All Postpartum Care Center in Seoul (서울시 소재 산후조리원의 의료인 연계 및 한방의료 현황 조사)

  • Objectives: The purpose of this study is to investigate all postpartum care center in Seoul about its state of doctors' linkage and oriental medical treatment. Methods: From May 1st 2011 to July 10th 2011, survey was done using a selfmade questionnaire by phone or visit. There were 12 categories including the state of doctors' linkage in postpartum care centers, treatment form of oriental medical doctors, utilization of herbal medicine and so on. The data collected was analyzed using Excel 2007 FOR WINDOWS. Results: 1. The doctors' linkage with postpartum centers included pediatricians(96.1%), oriental medicine doctors(61.2%), and gynecologists(34.0%). Of those postpartum centers, 66% had both pediatrician and oriental medical doctor which were the vast majority. 2. Out of 70 postpartum centers providing herbal medicine, 9 of them were supplied of herbal medicine by herb sellers(not doctor). Where as, at 63 oriental medical doctors-affiliated centers, 85.7% of them had oriental medical doctors' examinations. 3. Out of 70 postpartum centers providing herbal medicine, personalized prescription amounted to 35(50.0%), same prescriptions amounted to 34(48.6%), and 1(1.4%) refused to answer. 4. Out of all the patients that received explanation about the effects of herbal medicine, 62.9% had them explained by oriental medical doctors. Conclusions: These findings suggests that oriental medical doctors are actively involved with postpartum care centers but problems surfaced regarding individualized prescription insufficiency and the lack of administration. In the near future, if the expertise expand in the areas of pediatrics as well as obstetrics and gynecology, oriental medicine will have a more solid role in postpartum care part.

A Study on the Maternal Characteristics and Clinical Changes After Korean Medicine Postpartum Care - A Comparative Study with Pre-Study in 2010 - (한방 산후조리 이용 산모의 특성과 치료결과 보고 - 2010년 선행 연구와의 비교 고찰 -)

  • Objectives: This study aims to assess the maternal characteristics and changes of body among patients who received Korean medicine postpartum care compared to pre-study. Methods: From January 1, 2018 to February 28, 2019, we included 31 postpartum patients who had body composition analysis at admission and discharge among 34 patients who received postpartum care in ${\square}{\square}$ University Hospital. We used SPSS 21.0 for window to test for statistical significance. Results: The average age, weight, and caesarean section rate increased compared to previous study. The average period of postpartum care has decreased. After postpartum care, abdominal circumference, body weight and Body Mass Index (BMI) was significantly decreased, but not as much as pre-study. The coefficient of determination was derived to predict the effects of postpartum care and calculated as "$Abdominal\;circumference\;change=2.745+0.593{\times}admission\;period$", "$weight\;change=0.214+0.345{\times}admission\;period$" and "$BMI\;change=0.198+0.120{\times}admission\;period$". Conclusions: Compared to previous study, as the maternal age increased and admission period decreased, the weight loss after childbirth decreased. It is necessary to provide individual treatment through coefficient of determination.

A study on eating habits of the Buddhist Priesthood in Seoul and Kyongnam -II. Staple food and side dish- (서울, 경남지역 승가(僧家)의 식생활(食生活)에 관한 조사연구 -II. 주식(主食)과 부식(副食) 중심으로-)

  • The purpose of this study was to understand dietary culture of the Buddhist priesthood in Seoul and Kyongnam. This survey was carried out through questionnaries for menu, pretreatment, seasoning and recipe and the subjects were 26 temples and hermitages. Foods of the Buddhist priesthood were divided into staple food and side dish. The results of this study can be summarized as follow: 1. Staple foods of the Buddhist priesthood divided into cooked rice, gruel, noodle and Sujebi. Main materials of cooked rice and gruel were rice, noodle and Sujebi were wheat flour. Menues of staple foods were used white cooked rice, white gruel, wheat noodle and wheat Sujebi, respectively. Soups of noodle and Sujebi were used mushroom and seaweed. 2. Side dishes of the Buddhist priesthood divided into soup(국), stew(찌게), hard-boiled food(조림), steaming food(찜), raw vegetables(생채), cooked vegetables(나물), roast vegetables(볶음), pan fried food(전), baked(구이), wrapped rice(쌈), raw fish(회), Mook(묵). Vegetables divided into green-steam vegetables, edible roots, fruit vegetables, edible mountain herbs and seaweeds. A great difference of used for vegetables had been observed kinds of side dishes. Main materials of wrapped rice, raw fish and Mook was lettuce, raw mushroom and acorn, respectively. Seasonings were red pepper, garlic, green onion and sesame oil.

Isolation of 20(S)-Ginsenoside Rg3 and Rg5 from the Puffed Red Ginseng (팽화 홍삼으로부터 20(S)-Ginsenoside Rg3와 Rg5의 분리 및 구조동정)

  • Red ginseng tail roots (9.8 g water/100 g sample) were puffed at 7, 8, 9, and 10 $kg_{f}/cm^{2}$ using a rotational puffing gun. Puffed red ginseng was extracted with 70% ethanol, and the concentrated extract was successively partitioned with diethyl ether, n-butanol and $H_{2}O$. Two unknown ginsenosides from puffed red ginseng were found at 63 and 65 min of retention time in HPLC chromatogram suggesting that chemical structure of some ginsenosides might be altered during the puffing process. Identification of two unknown compounds was carried out using TLC, HPLC and NMR. Two major compounds were isolated from TLC. According to TLC result, compound I was expected to be the mixture of ginsenosides Rk1 and Rg5, and compound II was expected to be a 20(S)-ginsenoside $Rg_{3}$. Three compounds were isolated from n-butanol fraction through repeated silica gel and octadecyl silica gel column chromatographies. From the result of $^{1}H$- and $^{13}C$-NMR data, the chemical structures of unknown compounds were determined as ginsenoside $Rg_{5}$ and 20(S)-ginsenoside $Rg_{3}$. Unfortunately, ginsenoside $Rk_{1}$ could not be separated from ginsenoside-$Rg_{5}$ in the compound I. It was carefully reexamined using HPLC and confirmed that the last unknown compound was ginsenoside-$Rk_{1}$.

Influences of Roasting Conditions to Herbal Tea Containing Saururus chinensis, Artemisia capillarisin, Zizyphus vulgaris, Angelica gigas, Paeoniae radix and Cnidium officinale on Its Benzo[${\alpha}$]pyrene Changes (로스팅 조건이 삼백초, 인진쑥, 산조인, 당귀, 작약 및 천궁을 첨가한 한방차의 벤조피렌 변화에 미치는 영향)

  • The following is the study result of herbal tea roasted at different temperatures between $80{\sim}140^{\circ}C$. Depending on treatment temperature the water content decreased, some carbonization occurred and crude ash content relatively increased. Also crude protein and crude fat decreased little. Solid elution rate of herbal tea showed 0.15~0.32%(w/w) and the rate of solid elution decreased with higher roasting temperature. There was no big change in $80{\sim}110^{\circ}C$ treatment section but the solid elution decreased rapidly in $110{\sim}140^{\circ}C$ section. The reason for decreasing solid elution rate at higher treatment temperature is because the compact inner tissue makes elution difficult. Benzopyrene content (0.29~0.51ppb) showed a tendency to increase with higher treating temperature. From this result, the $B({\alpha})P$ content differed depending on the treatment temperature and raw materials. In case of roasting, the actual inside temperature is around $200^{\circ}C$ but since the surface temperature of the roaster reaches around $2000^{\circ}C$ some portion of $B({\alpha})P$ content was presumed to be produced from the area that came in contact with this surface. When the processing which is a main component of food carbohydrate, protein, fat reason despite serve heat treatment as a whole is to be detected even though the $B({\alpha})P$ in this way is considered to be.

Effects of Mokhyangsaenghwa-tang beverage on Postpartum Recovery and Lactation (목향생화탕(木香生化湯) 산후음료의 안전성과 산후조리에 관한 유효성 임상연구)

  • Purpose : This study was conducted to investigate the effect of Mokhyangsaenghwa-tang beverage on postpartum recovery and lactation. One theory of oriental medical treatment for postpartum care is focused on increasing vitality and building up blood. Saengwha-tang are representative remedies for these respective principle of treatment. This experiment was carried out to evaluate the clinical effects of Mokhyangsaenghwa-tang on blood components, Body weight, BMI and prolactin level in postpartum. Methods : 65 postpartum women applied for this study, and 30 of them were included in this study. They were divided two groups. The experimental group were treated by Mokhyangsaenghwa-tang beverage, the control group were treated by citron beverage during 40 days. Then we observed the changes of the body weight, BMI, complete blood cell count, liver function test, renal function test, level of serum prolactin, Urine analysis and general conditon Questionnaire among baseline(before treatment), 1st phase(after 20 days treatment) and 2nd phase(after 40 days treatment). Results : After taking Mokhyangsaenghwa-tang beverage, body weight, body water, edema index, prolactin ana general condtion became better, but they were not statistically significant. Conclusion : This study shows that Mokhyangsaenghwa-tang can improve postpartum recovery, but more research is needed.

  • The purpose of this study was carried out to investigate medicinal resources herb plants that using through the ages in Korea among the spontaneous and naturalized flavor or frangrant plants and to discuss some way of utilizations of their products to promote human health. Another goal of this study was to collect and systematically synthesize those materials connected with fragrant medicinal plants to serve useful informations on this problems for following researchers. The major findings of this study were as follows ; In Korea, many species of fragrant plants were used for medicinal purpose through the ages. And most of the everyday`s common drugs in Korea's traditional medicine were contained fragrant resources plants as major medicinal material. Therefore, We can find that mast of the fragrant resources plants are very excellent material for traditional medicinal use. At the same time, we should reveal to fragrant material's activity for human health and to investigate the effect of aromatherapy in the future.

The Study to Investigate the Recognition on Postpartum Symptom among Korean, Korean resident in America and American (한국인.재미교포.미국인의 산후풍과 산후조리에 대한 인식 조사)

  • Purpose: There has not been decisional consensus in the concepts and the ways of postpartum cares between Western and non-Western cultures though the importance of postpartum period has been emphasized by the multitude of health professionals. In this study, we investigated the differences in recognition concerning postpartum care among Koreans, Korean residents in America, and Americans (hereafter, K, KA, A). Methods: We used questionnaire of women who had given birth from last 6 weeks to less than 13 years. The women consisted of three groups;104 K, 44 KA, and 44 A. Results: In the perception survey concerning postpartum care, every three group thought that postpartum care was important. However, there were differences in understanding how it would affect women's health, and what they did in the period. K significantly agreed with the content and the importance of postpartum care. The investigation concerning seven categories of postpartum care showed that K group performed the 6 categories except consuming medication beneficial for the postpartum care. KA group performed in a low rate and A group performed two categories in a significantly low rate two categories; avoiding cold stimulation and keeping the body warm. Conclusion: Based on this study, it should be suggested that there were common points and differences among the recognition concerning postnatal care in three groups. Therefore, health professionals make efforts in understanding diversities on postnatal care and providing the adequate services reflecting each mother's needs and recognition.
