This study was intended to evaluate a post-operative relapse tendency in mandibular prognathism patients with open-bite. 18 patients with or without open-bite have undergone sagittal split ramus osteotomy and were investigated radiologically with cephalogram. The results were as following 1. The preoperative anterior facial height, mandibular plane angle and mandibular gonial angle were larger in open-bite patients than in nonopen-bite patients.(p>0.05) 2. There was significant correlation between surgical change of SNA, mandibular plane length and long-term relapse.(p<0.01). 3. There was relatively stable postoperative anterior facial height in open bite patients. 4. There was no significant correlation between the surgical change of mandibular plane angle, amount of mandibular setback and long-term relapse.(p>0.01) 5. This study showed that horizontal relapse was more significant than vertical relapse.
Cardiac arrhythmia is irregular heart rate. It's one of the reason of unpredictable sudden death. Accurate diagnosis and management of cardiac arrhythmia are the most important factors for the life of patient. To obtain a good prognosis, Dentist should be know and manage the multi-types of cardiac arrhythmia during dental treatment with the cooperation of medical doctor majored in cardiac circulation medicine. We casually found the cardiac arrhythmia in mandible prognathism patient during preparation for orthognathic surgery. Orthognathic surgery for cardiac arrhythmia patient was done successfully under general anesthesia with the temporary cardiac pace-maker.
Severe cases of mandibular prognathism can be treated with orthodontic therapy or surgical correction, but there are situations where only prosthetic improvement is possible. An understanding of class III patients is needed, including a vertical chewing pattern and the absence of anterior guidance. Additionally, it is relatively easy to increase occlusal vertical dimension to correct the anterior crossbite, but this approach can sometimes lead to unfavorable results, necessitating careful diagnosis and treatment planning. In this case report, oral rehabilitation was conducted in a patient with mandibular prognathism and asymmetry, utilizing implants and fixed dental prosthesis. Through a step-by-step treatment approach, the existing occlusal vertical dimension was maintained, and the final fixed dental prosthesis restoration was completed. Accordingly, it shows functional and aesthetically appropriate results, and reports on the patient's diagnosis and treatment process.
This study was intended to perform cephalometric analysis of the facial soft tissue profile after surgical correction of skeletal Class III malocclusion after SSRO in 29 patients (Males 12, females 17). Lateral cephalograms were taken in centric occlusion before and immediate, long term after surgeries. 1. Counter-clockwise rotation of mandible was observed after the surgery, average relapses of mandibular set back were 1.23-1.28mm. The net effects of the mandibular set-back after surgeries were 81.7-82.2%. Because these relapse tendencies may reduce the effects of the surgical outcomes, surgeon must consider these net before the surgical treatment planning. 2. The ratio of horizontal changes of hard tissue to soft tissue at lower lip, mentolabial sulcus, pogonion were 72.7-93.7%, 100.3%, 99.1-102.1% respectively. There were little changes at upper lip position anteroposteriorly. 3. The relationship of upper and lower lips were improved after surgery. Lower lip was posteriorly repostioned and upper lip was flattend and elongated in conjunction with deepening of inferior lobial sulcus. But profile of chin was still prominent after surgery. 4. Hard tissue horizontal changes and tissue vertical changes were significantly correlated with each other and there were reverse correlations with hard tissue vertical changes and soft tissue horizontal changes.
The patient, 8 years female, complained of crossbite of anterior teeth and prognathism of mandible. There was lingual tipping of upper lateral incisor in teeth lining. Cephalometric analysis revealed normal in maxilla, but forward relation to standard, so daignosised as skeletal class III case. Crossbite of anterior teeth was corrected by means of CIII elastics under multibanded system. Space for eruption of canine was regained by means of E.O.A. After 2 years and 7 months, she gained good interdigitation of buccal segments of attractive facial profile.
Obwegeser II method and Rigid fixation conserving the condylar position has been performed on 5 patients with severe mandibular prognathism, and we obtained the result after the follow up study follows. 1. Obwegeser II method is considered to move the distal segment passively when surgical correction of severe open bite correction and large amount of set back above 15mm is needed. 2. In one case that has more change of condylar position after operation, documented immediate post-operative relapse have been occurred 3. In the others that have adequate control of condylar position, passive set back and firm skeletal fixation, more functional and esthetic improvement and more post-operative stability has been achieved
Purpose: In this study, we analyzed and compared the anatomical position of the mandibular canal in normal occlusion and mandibular prognathism patients. Patients and Methods: Computed tomography image from 58 patients were divided into normal occlusion group and mandibular prognathism group, and each measurement were taken in the each measuring points(2nd premolar, 1st molar, 2nd molar, 3rd molar, ramus). Measurements were statistically analyzed by student's t-test. Results: BC (Thickness of the buccal cortex) value was 2.3~2.7 mm, CB (Distance from the canal to the lingual aspect of the buccal cortex) value was 1.3~4.3 mm, MC (Diameter of the canal) value was 3.2~3.8 mm, LI (Distance from the canal to the lingual aspect of the lingual cortex) value was 2.0~3.7 mm, TM (Thickness of the total mandible) value was 9.5~12.9 mm and CM (Distance from the canal to the inferior border of the mandible) value was 6.9~17.5 mm. Conclusion: In the comparison between two groups, there was statistically significant difference in CB value of 2nd, 3rd molar between normal occlusion and mandibular prognathism, and other value in the rest of the measuring points didn't show statistically significant difference.

So many maxillofacial plastc surgeons has made efforts to overcome these troubles and bring out more positive life. The proper proportion and shape decide esthetic quality. Lower third of face was consist with lip, cheek, mandibular lower border and mandibular angle. Widening lower third of face give a impression with muscular and recklessness. And lower and wide mandibular angle makes face square shape. Unilateral involvement cause asymmestric face. These face is considered unfavorable, especially in Korea or Japan. We prevent a number of with mandibular angle Bulging which was corrected with mandibular osteotomy or masseter myotomy.

This is a report of 2-cases of mandibular prognathism corrected by Intraoral oblique splitting osteotomy of mandibular ramus. The Intraoral oblique splitting osteotomy is a modification of sagittal split osteotomy of ramus and it is documented by Yoshida, on 1985. By this method. authors obtained the following results. 1. The patients' esthetic, psychological and functional problems were dissolved by setback of mandibular prognathism. 2. The postoperative infection, splitted bone segments fracture, paresthesia of the face and T.M.J. dysfunction were not appeared. 3. Postoperative intermaxillary fixation was maintained for 8 weeks. The patients could open their mouths in normal range after a week of intermaxillary fixation removal. 4. The soft tissue changes of lower lip and chin were about 1:1 to the hard tissue changes. 5. During intermaxillary fixation period and postoperative orthodontic treatment, slight relapse was observed. Now, the patients are under postoperative orthodontic treatment.

Purpose: The purpose of the study is to evaluate the effectiveness of anterior segmental osteotomy (ASO) in bimaxillary protrusion (BP) patients by comparison between the mandibular soft and hard tissue changes from orthodontic treatment and ASO. Methods: All 44 patients were diagnosed with BP in Kyung-Hee Medical Center. Orthodontic treatment with teeth extractions were underwent by 23 patients (Group A) and 21 patients underwent ASO (Group B). Mandibular soft and hard tissue changes were measured and evaluated, which were based on the vertical and horizontal reference line in lateral cephalometric radiographs. Statistical significance between the changes and correlation between each measurement were analyzed. Results: The amount of B point movement was lesser than that of the lower incisal tip (LIT) retraction, and LIT was tilted lingually in group A. The posterior movement discrepancy between LIT and B point was insignificant, and the inclination of lower incisor was not changed in group B. The soft tissues, including the lower lip, showed a posterior movement and reduction in the depth of mento-labial groove. According to the correlation analysis, the movement of the lower incisor was significantly related to the movement of the lower lip in group A, and the movement of the lower incisor was significantly related to that of the movement of lower lip, B point and Pog in group B. Conclusion: The orthodontic treatment in BP patients results in posterior tilting movement of the lower incisor, but ASO results in the bodily movement of the lower incisor. Consequently, ASO is more effective in BP cases because it ensures the controlled movement of the lower incisors.
