Detection of periodontal disease related bacteria from the implant-abutment interface in oral cavity (구강내 임플랜트-지대주 연결부에서 치주질환관련 세균의 검출)

  • Statement of the problem: Implant systems result in gaps and cavities between implant and abutment that can act as a trap for bacteria and thus possibly cause inflammatory reactions in the peri-implant soft tissues. Purpose: Porphyromonas gingivalis, Prevotella intermedia, Tannerella forsythia, Treponema denticola, and Aggregatibacter actinomycetemcomitans, related to implant-abutment interface microleakage. Material and methods: Samples were taken from 27 subjects with sterilized paper points and were transported in $1{\times}PBS$. The detection of periodontopathogens were performed by polymerase chain reaction with species-specific primers based on 16S rDNA. Results: Our data showed that the detection rate of P. gingivalis and P. intermedia in implant fixture was 59% and 82% in patients respectively. Detection rate of P. gingivalis and P. intermedia in implant crevice was 44% and 82% in patients. Detection rate of P. gingivalis and P. intermedias in tongue was 82% and 82% in patients. Conclusion: Current implant systems cannot safely prevent microbial leakage and bacterial colonization of the inner part of the implant.

The Effect of S. thermophilus Isolated from Saliva Treated with Phytoncide on P. gingivalis (피톤치드 처리 후 구강 내 잔존 S. thermophilus의 P. gingivalis에 대한 효과)

  • The antibacterial effect of phytoncide on Porphyromonas gingivalis, which is the main causative agent of periodontal disease and halitosis, has been reported. However, little is known about its effect on normal oral microflora. The present study was performed to observe the effect of phytoncide on oral normal microflora and the inhibitory effect of surviving resident oral bacteria on P. gingivalis. In this study, saliva from each of 20 healthy subjects was treated with 1% phytoncide from Japanese Hinoki (Chamaecyparis obtusa Sieb. et Zucc.). Surviving salivary bacteria were isolated on blood agar plates and identified by 16S rDNA sequencing. In order to select inhibitory isolates against P. gingivalis, the isolates from the phytoncide-treated saliva were cultured with P. gingivalis. The results were as follows: 1. In general, the number of bacteria in saliva from periodontally healthy subjects was decreased when the saliva was treated with 1% phytoncide. 2. The majority of the salivary bacteria surviving the treatment of phytoncide were S. thermophilus (53%). 3. Most of the surviving salivary bacteria (72.5%) inhibit the growth of P. gingivalis A7A1-28 and P. gingivalis W83 on blood agar plates. 4. Among the surviving S. thermophilus, 85.8% of them were observed to inhibit P. gingivalis strains and 75.8% of the surviving S. sanguinis were inhibitory. Taken together, oral resident bacteria surviving phytoncide, which has been shown to inhibit P. gingivalis, may exert an additional inhibitory activity against the periodontopathic bacterium. Therefore, phytoncide can be used for preventing and ceasing the progress of periodontal disease and halitosis, and thus is expect to promote oral health.

The 200 surviving resident oral bacterium were 109 Streptococcus salivarius(54.5%), 25 Streptococcus sanguinis(12.5%), 15 Streptococcus mitis(7.5%). 3. Among the 148 bacteria which inhibit Pr. intermedia, Streptococcus salivarius was 85.3%(93/109), Streptococcus sanguinis was 64.0%.(16/25), Streptococcus mitis was 54.3%(8/15), Streptococcus parasanguinis was 66.7%(6/9), and Streptococcus Alactolyticus was 100%(8/8). Taken together, among the surviving resident oral bacterium, Streptococcus salivarius, Streptococcus sanguinis, Streptococcus mitis were mainly observed to inhibit Pr. intermedia. and they may exert an additional inhibitory activity against the periodontopathic bacterium. Therefore, phytoncide can be used for preventing and ceasing the progress of periodontal disease and halitosis, and thus is expect to promote oral health.

Anticariogenic activity of acanthoic acid isolated from Acanthopanax koreanum $N_{AKAI}$ against oral pathogens (섬오가피로부터 분리된 acanthoic acid의 항치아우식효과)

  • This research was aimed to search for natural antimicrobial agents that are sefe for humans and specific for oral pathogens. Acanthoic was isolated from the chloroform fraction of methanol extract of Acanthopanax koreanum $N_{AKAI}$ and its structure were elucidated by 13-NMR, 1H-NNR and ESIMS. Antibacterial activity of acanthoic acid was investigated by the minimum inhibitory (MIC) and minimun bactericidal (MBC) concentration. MIC/MBC of acanthoic acid against Streptococcus mutans $N_{AKAI}$ causing dental caries was determined to be $2/4\;{\mu}g/mL$, which was much lower than these of other natural antimicrobial agents such as $8/16\;{\mu}g/mL$ of sangurinarine and $250/500\;{\mu}/mL$ of green tea extract, $500/600\;{\mu}g/mL$ of thymol and borneol. Acanthoic and significantly inhibited the growth of other cariogenic bacteria such as Streptococcus sobrinus $N_{AKAI}$ and Streptococcus sanguis $N_{AKAI}$, and Streptococcus gordonii $N_{AKAI}$ in the MIC range of $4{\sim}32\;{\mu}g/mL$. My finding suggests that acanthoic acid could be employed as a potential antibacterial agent for preventing dental caries.

Study on Detection of Oral Bacteria in the Saliva and Risk Factors of Adults (성인의 타액 내 구강세균 검출과 위험요인에 관한 연구)

  • As oral diseases are developed by mixed infections, not by any single element, an accurate analysis of the causative microorganisms related to dental caries and periodontal diseases is required. In this study, saliva was collected from selected adults to determine if the bacteria that are well known as the causative microorganisms of dental caries and periodontal diseases would be detected in their saliva. In addition, this study examined whether there would be any differences among adults according to age, smoking, drinking and presence or absence of diseases in the distribution of oral bacteria to determine the risk factors for oral bacteria. The study subjects were 120 adults ranging in age from 20 to 65 years. The experiment data was collected from March 15, to May 2014. The gDNA was collected from the saliva, and the distribution of bacteria for oral diseases was investigated by PCR. The findings of the study were as follows. S. mutans was detected from 72 adults, and P. intermedia was detected from 88 adults. Both bacteria were detected from 54 adults, and no oral bacteria was detected in 14 adults. An analysis of the risk factors of oral bacteria showed that smokers had a 2.8-fold higher risk of S. mutans than nonsmokers, and the former had a 3.5-fold higher risk of P. intermedia than the latter. Drinkers had a 3.3-fold higher risk of S. mutans than nondrinkers. Patients who suffered from systemic diseases had a 4.1-fold higher risk of P. intermedia than those with no diseases. Therefore, smoking, drinking and systemic diseases are factors that increase the likelihood of oral bacteria detection. More periodontal disease bacteria were detected from older adults, and more oral bacteria were found in adults who were in their 20s, as dental caries and periodontal diseases were more common in this age group. The adults in which oral bacteria were detected are more likely to have dental caries or periodontal diseases, and they should try to keep their mouth cavity clean and make regular visits to a dental clinic to prevent possible oral diseases.

Studies on the Oral Disease Improvement Effects of Probiotics : A Review (프로바이오틱스의 구강질병 개선에 관한 연구 고찰)

  • This study is a review of research articles on the characteristics of probiotics. Probiotics are effective at ameliorating immune disease, alleviating glucose intolerance, and improving constipation and diarrhea. Furthermore, they have anticancer and antitumor effects, preventive effects against cardiovascular disease, antidiabetic effects, antioxidant effects, antibacterial effects, and they produce useful metabolites. It has been demonstrated that oral probiotics are effective in eliminating halitosis and forming a favorable oral ecosystem, by creating an environment that is not readily inhabited by harmful bacteria, such as Streptococcus mutans, which produces lactic acid and causes tooth decay, or Porphyromonas gingivalis, which causes gum disease.As a result, oral probiotics are being considered not only as therapeutics against diseases, but also as preventive agents for the maintenance of a healthy oral balance. In spite of some limitations, clinical trials are currently underway, and this study can provide evidence to support the use of probiotics to improve oral health conditions, including dental caries, periodontal disease, and Candida colonization.

Antimicrobial Effect of Mouthwash against Streptococcus mutans by Visual Staining Method (시각적 염색 방법을 이용한 마우스워시의 구강균에 대한 항균효과 확인)

  • Dental caries are one of the most common oral diseases and Streptococcus mutans (S. mutans) plays an important role in the initiation and progression of dental caries. Oral malodor is primarily the result of microbial metabolism such as Porphyromonas gingivalis (P. gingivalis) that produce volatile sulfur compounds (VSCs), causing oral malodor. Prevotella intermedia (P. intermedia) is known as typical periodontopathic bacteria, and periodontal disease is chronic inflammatory disease that leads to damage of gingival connective tissue and alveolar bone, eventually loss of teeth. In this study, we investigated antimicrobial effect of mouthwash containing cetylpyridinium chloride (CPC), sodium fluoride (NaF), green tea water extract and pine needles water extract against cariogenic and periodontopathic bacteria sucn as S. mutans, P. gingivalis and P. intermedia. As a result, the reduction ratios of S. mutans and P. gingivalis were 4.00 Log and 4.68 Log reduction for 30 s, and P. intermedia were 2.40 Log reduction for 30 s and 2.70 Log reduction for 60 s. Dentocult SM Strip mutans (SM Strip) provides easy detection of visual data showing a significant inhibition on S. mutans. In conclusion, we expected that mouthwash containing CPC, NaF, green tea water extract and pine needles water extract could help preventing the dental disease like dental caries and oral malodor.

Antibacterial Effect on Oral Normal flora of Phytoncide from Chamaecyparis Obtusa (구강 상주균에 대한 편백 피톤치드의 항균효과)

In this study, saliva from each of 20 healthy subjects was treated with 1% phytoncide from Japanese Hinoki (Chamaecyparis obtusa Sieb. et Zucc.). The surviving salivary bacterium were isolated on blood agar plates and identified by 16S rDNA sequencing. In order to select inhibitory isolates against F. nucleatum, the isolates from the phytoncide-treated saliva were cultured with F. nucleatum. The results are as follows: 1. Among the 200 surviving resident oral bacterium, 70(35.0%) bacterium inhibit the growth of F. nucleatum on blood agar plates. 2. Among the 70 bacterium which inhibit F. nucleatum, Streptococcus salivarius was 41.3%(45/109), Streptococcus sanguinis was 28%.(7/25), Streptococcus mitis was 20%(3/15), Streptococcus parasanguinis was 33.3%(3/9), Streptococcus Alactolyticus was 100%(8/8), Streptococcus vestibularis was 28.6%(2/7) and Streptococcus sp. was 50%(2/4). Taken together, among the surviving resident oral bacterium, Streptococcus salivarius, Streptococcus sanguinis, Streptococcus mitis were mainly observed to inhibit F. nucleatum. and they may exert an additional inhibitory activity against the periodontopathic bacterium. Therefore, phytoncide can be used to prevent and cease the progress of periodontal disease, halitosis. Thus it is expected to promote oral health.

Effects of Magnoliae Cortex and Zizyphi Fructus Extract Mixtures on the Progression of Experimental Periodontitis in Beagle Dogs (후박 및 대조 추출혼합물이 치주질환유발 성견의 치주질환억제에 미치는 효과)

  • 최근 생약 추출물의 항균, 항염효과에 관한 관심이 증대되고 있으며 이를 이용한 치주질환의 예방 및 억제효과에 관한 연구들이진행되고 있다. 이 연구의 목적은 시험관 실험을 통하여 이미 밝혀진 후박 및 대조추출물의 치주질환 원인 균에 대한 항균, 항염효과를 성견에서 인위적으로 치주질환을 일으킨 후 실험 약제의 경구투여를 통하여 질환의 진행에 미치는 영향을 알아보는 것이다. 실험동물은 생후 18개월된 비글견 6마리를 이용하였다. 상악은 제2,3 소구치 및 제4소구치에 하악은 제2, 3, 4 소구치 및 제1 대구치를 실험대상 치아로 하였으며 교정용 철사와 봉합사를 이용하여 치경부주위에 결찰하여 인위적 치주질환을 일으켰다. 염증 유발 8주 후에 결찰을 풀고 실험동물을 아무 것도 투여하지 않은 군(음성대조군, 3마리), 후박 및 대조 추출혼합물 투여군(실험군, 3마리)으로 나누었다. 실험 시작 (-8주), 약제 투여 시작(0주), 투여 후 2, 4, 6, 8주째에 치태지수, 치은지수, Florida Probe를 이용한 치주낭깊이 및 치은열구액 등의 임상지수를 측정하였다. 염증유발 8주동안에 치태지수, 치은지수, 치주낭깊이, 치은열구액은 급속히 증가하였다. 결찰은 제거하고, 약제를 투여한 이후 실험군에서 치은지수, 치주낭깊이, 치은열구액은 가장 높은 값을 나타내고 있었다. 실험군에서는 8주가 될때까지 임상지수 수치가 낮아지고 있었으며, 8주째에는 대부분의 지수에서 음성대조군과 유의성 있는 차이를 보이고 있었다. 양성대조군에서 비해서 실험군은 대부분의 경우에 임상지수가 나이지고 있었지만 6주 또는 8주째의 치은지수, 치주낭깊이 중 일부만을 제외하고는 유의성 있는 차이를 보이지는 않았다. 이 연구결과 후박 및 대조 추출혼합물이 in vitro뿐만 아니라 in vivo 상태에서도 치주염의 치료 및 골재생의 효과가 있음을 알 수 있었으며, 이와 같은 동물실험의 결과를 바탕으로 임상시험을 거쳐 향후 사람에게 유용한 치주염치료제로 개발, 사용될 수 있을 것으로 여겨진다.

Antimicrobial and Antioxidant Activities and Inhibition of Nitric Oxide Synthesis of Oak Wood Vinegar (참나무 목초액의 항균 및 항산화 활성과 일산화질소 합성 저해연구)

  • This study was carried out to investigate the biological effects oak wood vinegar. Antimicrobial activity was tested in five microbial species at the concentration of 5 to $50{\mu}l$ of oak wood vinegar by paper disc method. Growth of P. oleovoranse, P. vulgaris, E. coli, S. aureus and Prevotella intermedia was inhibited at a dose of as low as $50{\mu}l$ of oak wood vinegar. Antioxidant activities were measured by using DPPH radical scavenging and SOD-like activity. DPPH radical scavenging and SOD-like activities were 90% and 65% at the concentration of $25{\mu}l\;and\;50{\mu}l$ of oak wood vinegar, respectively. Stimulation of the macrophages RAW264.7 cells with lipopolysaccharide (LPS) resulted in increased production of nitric oxide (NO) in the medium. However, the oak wood vinegar showed marked inhibition of NO synthesis in a dose-dependent manner. This result suggest that oak wood vinegar plays significant role for activation of immune system in the pathogenesis of inflammatory diseases.
