A Systematic Review of Clinical Trials on Alternative Medicine (대체의학의 임상실험 현황에 대한 체계적 고찰)

  • 본 논문은 근거중심 대체의학의 현황과 범위를 알아보기 위한 것이다. 연구방법: 2000년부터 2010년 3월까지 Pubmed에 게재된 논문 중 alternative medicine으로 검색하여 체계적 고찰을 하였다. 2000년부터 2010년 현재까지 근거중심 대체의학에 대한 다양한 연구결과을 통합하기 위해 체계적 문헌고찰을 실시하였다. 682편의 연구 중 대체의학에 관한 임상연구는 19개였으며 연구전체 대상자는 2,573명이었다. 논문분석 결과 RCT(Randomized Controlled Trial) 실험 설계연구가 44%였으며 CCT(Controlled Clinical Trials)가 16%, OD(efficacy studies with either a controlled or an Other than controlled Design) 40%였다. 가장 많은 임상실험이 시행된 중재 방법은 Acupuncture과 Massage 였다. 대상 질병군 분포는 근골격계 및 결합조직의 질환과 관련된 '통증 완화'에 대한 임상연구가 20%, 다음으로 특정 감염성 및 기생충성 질환, 신생물이 각각 15% 순이었다. 근거중심 대체의학 현황을 파악하고자 체계적으로 고찰한 결과 대체의학을 통한 중재가 질병치료에 효과가 있는지에 대한 과학적 근거를 제시한 임상연구가 부족하다. 대체의학의 품질, 안정성, 유효성 확보를 위해 대체의학에 대한 질 높은 연구필요하며 근거중심 대체의학의 활성화가 필요할 것으로 생각된다.

HGLM and EB Estimation Methods for Disease Mapping (HGLM과 EB 추정법을 이용한 질병지도의 작성)

  • For the purpose of disease mapping, we consider the four small area estimation techniques to estimate the mortality rate of small areas; direct, Empirical estimation with total moment estimator and local moment estimator, Estimation based on hierarchial generalized linear model. The estimators are compared by empirical study based on lung cancer mortality data from 2000 Annual Reports on the Cause of Death Statistics in Gyeongsang-Do and Jeonla-Do published by Korean National Statistical Office. Also he stability and efficiency of these estimators are investigated in terms of mean square deviation as well as variation of estimates.

Comparison of the Effectiveness of Dietary Intervention between Calorie Restriction Diet and Food Selective Method according to Yin and Yang Constitution in Obese Woman (열량제한식과 체질식에 따른 비만 여성의 체중감량 효과)

  • This study was conducted to evaluate the effects of constitutional diets in comparison with general low calorie diets in obese women. The subjects were 52 adult obese women that participated in a calorie restriction diet program (control group, n=16) or a constitutional diet program (experimental group, n=36) for 8 weeks. The subjects in the experimental group were classified as negative (Yin) or positive (Yang) constitution. The obesity management program focused on the calorie restriction diet (300~400 kcal reduction per day) for the control group, whereas, for a constitutional diet without calorie restrictions was allowed for the experimental group. Daily intakes of nutrient and food were assessed using a 24 hr recall method. Body measurements and blood biochemical parameters were measured at baseline and after 8 weeks. After the intervention, weight, body fat, BMI, waist circumference and hip circumference decreased significantly in both the control and experimental group. Energy intake increased from 1,679.6 kcal to 1,810.6 kcal along with significant increase in protein, carbohydrate, fiber, calcium, phosphorus, iron, zinc, vitamin A, vitamin $B_1$, vitamin $B_2$, niacin and vitamin C in the experimental group. Calcium intake increased from 54.0% to 72.4% of DRI in the experimental group. Hemoglobin concentrations were significantly decreased in the control group, but were increased in the experimental group. In addition, blood glucose, serum total cholesterol and triglyceride were significantly decreased in the experimental group. In conclusion, consumption of a constitutional diet without calorie restrictions for 8 weeks was effective for weight reduction with improvement of blood glucose, serum cholesterol and triglyceride.

  • 압타머칩은 (주)제노프라에서 개발한 새로운 개념의 바이오칩으로서, 압타머(aptamer)를 이용하여 혈액중의 특정 단백질군의 상대적인 양의 변화를 측정할 수 있으며, 질병 진단에 바로 응용할 수 있는 도구이다. 본 논문에서는 압타머칩 데이터 분석을 통해 심혈관 질환 환자의 질병 진행 단계를 예측할 수 있음을 보인다. 정상, 안정/불안정성 협심증, 심근경색의 네 단계로 표지된 환자의 혈액 샘플로부터 제작한 (주)제노프라의 3K 압타머칩 데이터를, 일반 DNA 마이크로어레이 분석과 동일한 과정을 거쳐 분류한 결과, 각 단계별 환자샘플이 확연히 구분되는 것을 확인하였다. 분산분석 결과 P-Value를 이용하여 자질 선택을 수행하고, 분류 알고리즘으로는 신경망, 결정트리, SVM, 베이지안망을 적용한 결과. 각 알고리즘별로 50대 남성환자 31개의 샘플에 대하여 $77{\sim}100%$의 정확도로 심혈관 질환의 단계를 구분해내었다.

Therapeutic Effect of Oyaksungisan in Dogs with Facial Nerve Paralysis (개 안면신경마비에 대한 오약순기산의 치료효과)

  • Herbal medicines are widely used to treat a variety of human diseases. However, therapeutic effect of herbal medicine on the canine facial nerve paralysis (FNP) has not been investigated. This study examined therapeutic effects of Oyaksungisan on the canine FNP. Ten dogs with the induced canine FNP were equally divided into the control and an herbal medicine-treated groups (5 dogs, Oyaksungisan-treated group), respectively. No treatment was given to the control group. In the Oyaksungisan-treated group, Oyaksungisan was administered orally, twice per day for 2 weeks (50 mg/kg) after inducing FNP. Significant differences in the clinical scores were detected between the Oyaksungisan-treated and control groups on day 14 (p < 0.05). The serum creatine kinase (CK) activities in the Oyaksungisan-treated group showed a tendency to decrease but there was no significant difference compared with those of the control group. In conclusion, Oyaksungisan was effective for the induced canine FNP.

A Study on Smoking among Korean Rural Residents (일부(一部) 농촌주민(農村住民)들의 흡연(吸煙)에 관(關)한 조사(調査))

  • The present study was conducted to investigate smoking status and the relationship of smoking to hypertension and diseases on 1,504 subjects who were over 30-year-old age in Kyungsan Gun, Kyungpook province, during the period of 12 months from March 1978 to February 1979. The results obtained are summarized as follows: Of 1,504 subjects, 52.2 percent smoked. The percentage of males smoking showed 84.8 and the percentage of females was 30.7. There were no statistically significant differences between the rate of smoking and educational levels. Protestants demonstrated lower rates of smoking compared with other groups. Regarding the age started to smoke, 66.4 precent of male smokers and 16.9 percent of female smokers commenced smoking before the age of 19. In the regard to the average number of cigarettes smoked daily, 45.8 percent of male smokers consumed some 20 cigarettes and 24.1 percent of female smokers consumed some 10 cigarettes. 3.2 percent of male smellers smoked 40 or more cigarettes a day. Alcohol consumption was closely associated with smoking. 90.7 percent of male alcohol drinkers and 60.1 percent of female drinkers smoked. Cigarette smokers have higher rates of ailments than nonsmokers as a whole. The complaints of cough with sputum and shortness of breath revealed higher rates among smokers than among nonsmokers. Frequency of illnesses showed higher for smokers than for nonsmokers. Smokers were more likely than nonsmokers to suffer from chronic obstructive lung diseases such as chronic bronchitis, asthma, and emphysema. Abnormal systolic blood pressure (>160mmHg) and diastolic (>100) revealed significantly higher among smokers than among nonsmokers.

Coronary Arterial Lesions of Kawasaki Disease Observed in a Mouse Model of Sepsis: A Pilot Study and a Review of the Literature (패혈증 마우스 모델에서 가와사키병의 관상동맥 병변 관찰: 예비연구와 문헌고찰)

  • Purpose: Coronary arterial lesions (CALs) were reported to have developed in children with systemic inflammatory diseases, as well as those with Kawasaki disease (KD). The purpose of this study was to confirm that the CAL development in children with KD occurs in a mouse model of sepsis presenting typical systemic inflammatory response syndrome (SIRS). Methods: To induce the sepsis mouse model with SIRS, 6-week-old C57BL/6 mice were intraperitoneally injected with endotoxin. We compared histological findings of the major organs between the control and the sepsis groups and examined CAL in the heart of the septic mice. Results: Infiltrating inflammatory cells were relatively increased in the heart, liver, and kidneys of the sepsis group, compared with those of the control group. We confirmed lymphocytic infiltration in the myocardium (myocarditis) and the pericardial soft tissue of the heart. Furthermore, coronary artery of the septic mouse was identified, but CAL was not observed. Conclusions: In this study, we failed to confirm the existence of CAL in a mouse model of sepsis. However, it is well-known that CALs are seen in many kinds of diseases that cause SIRS. Our findings suggest further investigation into the clinical significance of CAL in various systemic inflammatory diseases, including KD.

A Search of the Community between Eight Constitution Medicine and Disease Ecology: A Perspective from Medical Geography (8체질의학과 질병생태학의 공통성에 관한 연구: 의료지리학적 접근을 중심으로)

  • There is a considerable community between Eight Constitution Medicine and disease ecology in that they examine the relationship between humans and environment in terms of genetic, environmental and cultural factors, in addition to the factor of germs. In this respect, the purpose of this research aims to investigate the community between Eight Constitution Medicine, a branch of Eastern Medicine and disease ecology in geography. The research method is to analyse the clinical results from 647 patients and the data from a field survey on the Yeonggwang-gun County, Jeollanam-do Province. The results are summarized as the following: First, geographical distribution of patients varies depending on the type of physical constitution; it is also divided into two types, an oceanic type and an inland type. Second, it is highly probable that there is a significant relation between a vulnerable disease in association with a type of physical constitution and patient's native place; there are diseases that are not associated with all the patients who have the same constitution; they might be incurred by the eating or life habits in association with the characteristics of geographical environment. Third, the case study of Yeonggwang-gun County, Jeollanam-do Province, with a focus on the mutual relationship among the three factors. shows that patients, who share in common the eating or life habits that coincide with their own personal types of constitution, maintain a good health condition; if not, they tend to be exposed to a various kinds of disease. Because the study on the community between Eight Constitution Medicine and disease ecology is now at the early stage, diverse types of approaches should be tried to be applied in the future.

  • The late 18C Malthus studied population growth for the first time, Verhulst the logistic model in 19C and, after that, the study of the predation competition between two species resulted in the appearance of Lotka-Volterra model and modified model supported by Gause's experiment with bacteria. Instable coexistence equilibrium being found, Solomon and Holling proposed functional and numerical response considering limited abilities of predator on prey, which applied to Lotka Volterra model. Nicholson and Baily, considering the predation between host and parasitoid in discrete time, made a model. In 20C there were developed various models of disease dynamics with the help of mathematics and real data and named SIS, SIR or SEIR on the basis of dynamical phenomena.

A Study on the Growth and Disease of Chondrus ocellatus in Korea (한국산 진두발, Chondrus ocellatus의 생장과 질병에 대한 연구)

  • The growth, reproduction, gametophyte(G)/tetrasporophyte(T) ratio, and diseases of Chondrus ocellatus populations were examined at 3 sites (Samcheok, Youngduk, Pohang) of East coast and at 2 sites (Hakampo, Manripo) of West coast in between July and August, 2013. Average plant lengths were 6.10~9.69 cm and it was minimum at Manripo and maximum at Pohang population. In general, average plant length and weight of C. ocellatus were greater on East coast than West coast populations. The proportion of vegetative plant was between 26.7~66.7 %, and G/T ratio of total plants including vegetative plants after testing resorcinol method was 3:2 on the East coast where is gametophyte dominant area. However, G/T ratio was 1:1 at Hakampo and 1:2.3 at Manripo representing tetrasporophyte dominance. In the present study, Korean C. ocellatus have various diseases (white colour and green colour), an endophytic alga(Ulvella sp.), and many epiphytic macroalgae and diatoms. Healthy C. ocellatus plants were about 20~40 % in summer population and most of plants had disease. Chondrus ocellatus had a filamentous green alga, endophytic Ulvella sp. which was not identified. The endophyte is easily observed in C. crispus growing in Europe and Canada and it is recognised as a pathogen destroying population and reducing yield of C. crispus. Thus more interest and research on the endophytic algae and disease of C. ocellatus are required.
