The Study of Effects of Musculoskeletal Risk Factors on Farmer's Syndrome (근골격계 위험요인이 농부증에 미치는 영향)

  • This study was conducted to reveal the effects of musculoskeletal risk factor on farmer's syndrome. We sampled 97 farmers aged above 30 in 3 villages of Kyunggi-do. Self-administered questionnaire for general characteristics, farmer's syndrome, musculoskeletal risk factor, blood pressure, and laboratory tests were conducted. According to the score of musculoskeletal risk factor, we divide the subjects high risk group and low risk group. The most common musculoskeletal risk factor is repetitiveness, bending of waist and rapid movement in order. The prevalence of farmer's syndrome of all subjects is 28.9%. In female the prevalence of farmer's syndrome(45.2%) is higher than in male(16.4%) significantly. High musculoskeletal risk group had higher score of farmer's syndrome(5.9) than low musculoskeletal risk group(4.4). The most common symptom is lumbago(76.3%), the second was numb limb and shoulder stiffness(54.6%, 54.6%). The prevalence of numb limb and shoulder stiffness higher in high musculoskeletal risk group than low musculoskeletal risk group, but that of lumbago did not show significant differences. In linear regression, score of farmer's syndrome was related to musculoskeletal risk factor as well as gender. Blood pressure and laboratory test did not show significant differences between two groups. These results suggest that musculoskeletal risk factor would influence farmer's syndrome. Further ergonomic evaluation and intervention of farmer's works and musculoskeletal diseases are needed.

Integrative Review of Guidelines Related Symptom Management and Physical Activity for Developing of Self-Care Management Program for Cancer Survivors (암생존자의 자가관리 프로그램 개발을 위한 증상관리 및 신체적 활동 관련 가이드라인의 통합적 고찰)

  • The purpose of this study attempted to collect basic data for development of an integrated self-management program for cancer survivors who completed cancer treatment. Self-management programs are divided into symptom management and physical activity management. Symptom management includes fatigue, sleep disturbance, pain, depression and anxiety. PubMed, CINAHL and EMBASE were used for searching guidelines. Based on the guideline quality evaluation, the final 8 guidelines were analyzed. The structured table was used to extract the screening subjects, timing, contents, subjects for comprehensive assessment and contents, and summarized contents related to the physical activity and exercise in non-pharmacological approach. As a result, after the completion of cancer treatment, all cancer survivors should be screened regularly using reliable and validated tools. In the case of fatigue, physical activity was recommended as the primary intervention, but it was recommended for other symptoms as adjuvant therapy. Therefore, Cancer survivors should be encouraged to be active in their physical activity, and maintain the moderate intensity physical activity as long as they have no complication related to the cancer treatment. Motivation strategies for physical activity need to be developed and applied.

Assessment of subjective symptoms by 60Hz magnetic field in electromagnetic hypersensitivity (60Hz 자기장에 대한 과민 증후군의 자각증상 원인 평가)

  • As increasing of electrical device usage, social concerns about the possible effects of 60Hz electromagnetic fields (EMF) on human health have increased. The number of people with self-attributed electromagnetic hypersensitivity (EHS) who complain of subjective symptoms such as headache, insomnia etc. also increased. However, we don't know whether the EHS results from psychological factor or real perception to the electromagnetic field. In this study, we simultaneously investigated physiological changes(heart rate, respiration rate, heart rate variability, alpha and beta waves in EEG), subjective symptoms and perception accuracy to assess origins of subjective symptoms according to the EMF exposure. Experiment consists of real and sham sessions and 60Hz 12.5uT magnetic field was on(real) or off(sham) to 15 EHS and 16 nonEHS. As the results, EMF exposure did not have any effects on physiological parameters or subjective symptoms for both groups. There was also no evidence that EHS group perceived the EMFs correctly than the control group. Therefore, the origins of subjective symptoms is not the 60Hz magnetic field but psychological factors.

Subjective Symptoms and Physiological Changes of RF Exposure by a Cellular Phone (휴대전화 전자파에 의한 자각증상 및 생리학적 변화)

  • Due to the fast increase in cellular phone users, public interest on health effect of electromagnetic fields(EMFs) by cellular phonos is gradually increasing. Some EHS(electromagnetic hypersensitivity) patients complain of psycho-neurophysiological symptoms such as headaches, insomnia, memory loss resulting from RF radiation by CDMA cellular phones. However, EHS is difficult to diagnose and depends on the individual's subjective judgement. And we don't know clearly if the cause of EHS is uneasiness or real exposure. There have been various EHS volunteer studies on heart rate, blood pressure and subjective symptoms using GSM phones. But there are few studies on experimental case-control study investigating physiological parameters, subjective symptoms, and perception of EMFs. In this study, two volunteer groups of 17 self-declared EHS and 19 controls were exposed to both sham and real RF exposure by CDMA cellular phones for half an hour each. We investigated not only the physiological parameters such as heart rates, respiration rates and HRVs(hear rate variability), but also the perception of EMFs and subjective symptoms. As the results, EMF exposure did not have any effects on the subjective symptoms or physiological parameters for both groups. For the EMF perception, there was no evidence that EHS group perceived the EMFs correctly than the control group.

A study of prostate symptom, sleep quality and quality of life in the community-dwelling korean elderly (남성 노인의 전립선 증상, 수면의 질, 삶의 질에 관한 연구)

  • This study aimed to identify the severity and types of prostate symptom, to identify the relationship of prostate symptom, sleep quality and quality of life of the elderly in korea, so to provide basic data for developing nursing interventions for the elderly's prostate symptom management. This study used a descriptive correlational research design. The subjects of this study were 100 elderly men over 65yr in an urban city. Data were collected through personal interviews using questionnaires from September 2006 to March 2007. The IPPS, PSQI and SF-36-K were administered. The collected data were analyzed by descriptive statistics and Pearson's correlation. The major findings of this study were as follows: 1) 44% of the participants have moderate to severe Prostate symptom(IPSS>7). 2) Prostate symptom has positive relationship with sleep quality(r=.272, p=.006) and negative relationship with health related quality of life(r=-.197, p=.049). The results of this study indicate that prostate symptom is related to sleep quality and quality of life. So it is necessary to give information to the elderly about the effective coping method of prostate symptom and about the life style which have positive effect to prostate symptom.

Correlation among Heat and Cold, Life Style, Stress and Quality of Life for Adult Female with Constipation (성인 여성의 변비에 따른 한열(寒熱)증상, 생활습관, 스트레스, 삶의 질과의 관계)

  • Korean, and identified the correlation of a constipation with life style, heat and cold symptoms, stress and life quality of them. We also identified the relationships of the constipation with stress and life quality. A Survey was conducted from April 23, 2010 to October 25, 2010 for 464 women and the collected data was analyzed with computer software of SPSS 19.0. In the result, the constipation group appealed for cold stomach(p=.008), dyspepsia(p=.006), and tiredness(p=.001). more often than the normal group. The constipation group experienced Dreaming(p=.003), light sleeping(p=.007) and a trouble in sleeping(p=.001) more often, which impaired the quality of sleeping. Compared to the normal group, the constipation group also showed a tendency of high stress and low life quality. Based on the results, all the factors of heat and cold symptom, life style, stress and life quality should be considered when developing medications for the prevention and improvement of constipation.

Chronic Obstructive Pulmonary Disease in a Dressage Horse (마장마술마에서 발생한 만성폐쇄성폐질환 증례)

  • A 14-year-old gelded dressage horse weighing 500 kg was presented to the Equine Medical Center of the Seoul Racecourse of Korea Racing Authority (KRA) due to coughing and mucopurulent nasal discharge. The horse was initiated with empirical antibiotic in the first place. However, the clinical signs did not improve but were rather exacerbated even after 3 weeks of therapy. Extensive diagnostic procedures including transtracheal wash (TTW) fluid cytology were undertaken. The localized wheezes and crackles were auscultated and an increase in the amount of mucopurulent exudate in trachea was observed at endoscopy. Infiltration of neutrophils was observed in the TTW fluid cytology implying chronic obstructive pulmonary disease (COPD). Therefore, the systemic glucocorticoid therapy was to be given for 3 weeks with improved ventilation provided at the same time. The respiratory symptoms started to improve in 7 days of therapy and were fully resolved by when the therapy was terminated. The horse is clinically normal now and being monitored for development of any signs of chronic obstructive pulmonary disease.

Effect of Gabapentin for the Treatment of Burning Mouth Syndrome Comorbid with Depression in Postmenopausal Women (폐경 후 여성의 우울증에 수반된 구강 작열감 증후군(Burning Mouth Syndrome)에 대한 Gabapentin의 효과)

  • Burning mouth syndrome is characterized by intra-oral burning sensation without any organic abnormalities. This syndrome is associated with various etiological factors such as neuropathy, malnutrition, menopause and depression. Several medications have been tried for the treatment. Those are analgesics, hormones, anticonvulsants and antidepressants. However, optimal effective pharmacologic treatment remains still unknown. The purpose of this case study is to report the clinical effectiveness of gabapentin in the treatment of burning mouth syndrome in postmenopausal women with comorbid depression. We report two menopausal women. Antidepressants were effective for improving depressive symptoms, but it had no effects on intra-oral burning sensation. Gabapentin reduced intra-oral burning sensation effectively for all two patients. One patient reported 55% reduction(a decrease from 9 to 4 on VAS), the other patient reported 35% reduction(from 8 to 5) of the intra-oral burning sensation during 16 weeks. The minimal effective daily dose of gabapentin was 300mg. This study suggests that gabapentin might be a useful, effective therapeutic option for treating burning mouth syndrome in postmenopausal women with comorbid depression. Further prospective clinical studies are needed to investigate the effectiveness of gabapentin in patients with burning mouth syndrome.

  • Lumbar disc herniation in the adolescent is rare. The clinical features of adolescent lumbar disc herniation are typically different from those of adults. Many adolescent with that disease may be misdiagnosed as minor trauma, muscle sprain, tight hamstrings or scoliosis. Published series reiterate that adolescents, as opposed to adults, do not respond well to conservative treatment. In adolescent lumbar disc herniation with hamstring tightness, the neurologic defects are improved shortly after, discectomy, but the hamstring tightness continue and remain even after 1 year. Earlier discectomy and adjunct postsurgical conservative measures can provide a greater opportunity for correction and stabilization of scoliotic posture. We here report on a case of adolescent lumbar disc herniation that was misdiagnosed by knee injury during 2 years. After posterior partial discectomy, the hamstring tightness and scoliosis was disappeared without complication.

A Case Report of Transverse Myelitis with Tonic Spasm (Tonic Spasm을 호소하는 Transverse Myelitis 환자 증례보고)

  • Objectives : The purpose of this case is to report the improvement after treatment about tonic spasm of a transverse myelitis patient. Methods : We treated the patient with acupuncture therapy and herbal medicine from the 19th July 2006 to the 4th October 2006. At first we used Ssanghwa-tang until the 24th July 2006. After that we used Samul-tang until 2nd August. Lastly we used Yukmigihwang-tang until the 4th October 2006. And we used acupuncture at Asi points and used Sheng Ge of Liver from beginning to end. We checked the patient with frequency of tonic spasm and other symptoms. Results : After two months of treatments, frequency of tonic spasm decreased and abdominal muscle rigidity, visual disturbance, ocular discomfort and oppressed feeling in the chest were improved.
