A Comparative Study on the Retention of Implant Overdenture According to the Shape and the Number of Magnetic Attachment (자성 어태치먼트의 형태와 수에 따른 하악 임플란트 피개의치의 유지력에 대한 비교 연구)

  • The aim of this study was to compare the retention and stability of implant overdenture according to the shape and the number of magnetic attachment. The experimental groups were designed for the number of implants(1, 2, 4) and shape of magnetic attachments(flat, cushion, dome type) resulting in 9 subgroups. 45 attachments were tested attached to $Br{\aa}nemark$ system implants which were planted on a mandibular model. Each attachment was composed of the magnet assembly embedded in a overdenture sample and the abutment keeper screwed into the implants. Dislodging tensile forces were applied to the overdenture samples using an Instron(cross-head speed 50.80mm/min) in 3 directions simulating function: vertical, oblique, and anterior-posterior. The loading was repeated 10 times in each direction for 45 samples. The values of maximum dislodging force of each subgroup were processed statistically using SPSS V. 12.0 at the 0.05 level of significance. The results of this study were as follows: 1. Flat type magnetic overdenture was the most retentive when subjected to vertically directed forces and dome type was the lest retentive when subjected to obliquely directed forces(p<0.05). 2. In case of planting one implant, flat type had a higher vertically retentive force than anterior-posteriorly retentive force. In case of planting two implants, flat type and dome type had a higher vertically retentive force and in case of planting four implants, flat type and cushion type had a higher vertically retentive force than anterior-posteriorly retentive force(p<0.05). 3. The incremental number of dental implant, without regards to the three types of magnetic attachment shapes, showed higher retention of overdenture(p<0.05). From the results, if a patient need much more retention of implant overdenture, flat type magnetic overdenture would be a good treatment. In case of the bruxism where excessive lateral forces are already present, dome type could be expected to produce better results. In case of planting one implant, flat type is more stable than the other shape of magnet and in case of two implant, flat type and dome type are more stable and in case of four implants, flat type and cushion type are more stable. Planting more than two implants and using flat type magnetic attachment would provide better retention and stability of implant overdenture

The effect of PostGraft$^{TM}$ on implants were installed at the tibia of ovariectomized rats (난소절제술한 백서에 식립한 임플란트에 대한 PostGraft$^{TM}$의 경구 투여 효과)

  • Purpose: This study evaluated PostGraft$^{TM}$ which enhances implant stability and bone density. Materials and methods: Forty eight implants were installed at the tibia of ovariectomized rats. The group administrated with PostGraft$^{TM}$ was the experimental group, and the control group was not administrated. Implant stability was evaluated at the $2^{nd}$, $4^{th}$ and $6^{th}$ week by Periotest value, bone mineral density, bone-to-implant contact. These values were analyzed statistically with Mann-Whitney U test (P<.05). Histological analysis was evaluated at the $2^{nd}$, $4^{th}$ and $6^{th}$ week. Results: According to the Periotest$^{(R)}$ measurement, both experimental and control groups showed decrease in values as time elapsed. Greater decrease was observed in the experimental group but there was no significant difference. By examining the radiographic images, both experimental and control groups showed tendency of increase in bone density. Greater increase was seen in the experimental group but there was no significant difference. According to the bone-to-implant contact measurement, both experimental and control groups showed increase in values as time elapsed. Greater increase was seen in the experimental group. At the $2^{nd}$ and $4^{th}$ week, there was no significant difference. But at the $6^{th}$ week, there was significant difference (P<.05). By histological analysis, both experimental and control groups showed increase in bone formation as time elapsed. In addition, greater increase was seen in the experimental group. Conclusion: It could be concluded that the PostGraft$^{TM}$ medicated group showed better results in the bone density and osseointegration.

Causes of failures of long-term used double crown denture and new rehabilitation with dental implant and tooth combined denture using remaining teeth and implants (장기간 사용한 이중관 의치의 실패원인 및 자연치와 임플란트를 이용한 이중관 의치 수복증례)

  • Hybrid telescopic double crown have ever been good treatment option for patient with periodontally unfavorable few remaining teeth for successful prognosis. Tooth and implant combined telescopic double crown can be used for improving retention and support for denture with strategically placed implants on edentulous ridge. In this case, an 55-year-old female patient had chronic periodontitis with few remaining teeth on maxilla and fixed partial prosthodontics on mandible. Treatment of hybrid telescopic double crown with friction pin lasted 9.5 years only with natural teeth. After 9.5 years, additional implants was installed due to fracture of cast-posted abutment teeth. After implantation, tooth and implant combined double crown had fabricated. Through re-treatment, no complications of new denture has been found during 2 years follow up. This case presents fair prognosis of tooth and implant combined double crown denture in periodontally unfavorable condition.

의료용 Ti-Ni 합금의 개발 동향

  • 형상기억합금은 합금이 갖는 형상기억효과 및 초탄성효과 때문에 학문적으로 공업적으로 큰 흥미를 끌어왔다. 형상 기억효과는 Au-Cd 합금과 Cu-Zn 합금에서 최초로 발견되었지만 당시에는 크게 주목을 받지 못하였다. 그 후 1962년 Ti-Ni 합금에서 형상기억효과가 발견된 이래로 크게 주목을 받게 되었고, 곧바로 상용화를 위한 노력이 이어져 리벳, 열엔진, 커플링, 회로차단기 등에 적용이 시도되었다. 현재까지 약 300여종에 이르는 형상기억합금이 개발되어 있지만 형상기억효과의 안정성, 우수성, 내식성, 가공성, 내피로성, 내마모성이 우수한 Ti-Ni 합금이 가장 실용화에 적합한 합금으로 인식되고 있다. Ti-Ni합금은 1960년대에 개발되었지만 의료분야에 적용되기 시작한 것은 1980년대이고, 그 후 미국의 FDA가 Ti-Ni 합금으로 제조된 몇몇 3급(Class III) 임플란트를 인증하면서 시장규모도 폭발적으로 증가하고 있다. 일본의 경우 1980년대 초부터 Ti-Ni 합금을 치과용 임플란트로써 사용하여 왔, 독일, 중국, 러시아도 1980년대부터 임상에 적용해 왔다. 우리나라는 2004년 식약청(식품의약안정청)으로부터 제조 및 판매가 허가되어 현재 실제 임상에 적용되고 있다. 이와 같이 Ti-Ni 합금이 의료용 금속재료로서 널리 쓰이게 된 가장 근본적인 이유는 말할 나위 없이 합금이 갖는 형상기억효과 및 초탄성효과 때문이다. 본 강연에서는 Ti-Ni 합금을 의료용 금속재료의 입장에서 재조명해보고, 본 합금이 의료용 금속재료로서 어떠한 장점이 있으며, 그러한 장점을 의료분야에서 어떻게 응용하고 있는가에 대해 소개하고자 한다. 또한 의료용 Ti-Ni 합금에서 향후 개선되어야 할 요소에 대해서도 논의하고자 한다.

The Biological Stability of Immediate Placement of Tapered Implants in Tooth Extraction Sites (발치와에 즉시 식립한 쐐기형 임플란트의 생물학적 안정성에 관한 전향적 연구)

  • Objective : To assess the biological stability of immediate transmucosal placement of tapered implants into tooth extraction sockets. Material and methods : Following tooth extraction, tapered implants were immediately placed into the sockets. Teeth with evidence of acute periapical pathology were excluded. After implant placement, sutured allowing a non-submerged, transmucosal healing. Standardized radiographs were obtained every visiting from baseline to 32 weeks after implant placment. Changes in depth of the distance from the implant shoulder (IS) and from the alveolar crest (AC) to the bottom of the defect (BD) were assessed. Results : Thirteen patients (10 males and 3 females) were enrolled and followed. They contributed with 15 tapered implants. extraction iste displayed sufficient residual bone volume to allow primary stability of all implants. The mean surgery time was $41{\pm}10.0$ mins. All implants healed uneventfully yielding a survival rate of 100%. Mean ISQ values were relatively stable. Interproximal crestal bone decreased $1.69{\pm}1.2mm$ (mesial), $1.65{\pm}1.2mm$ (distal) from baseline to 32-week follow-up. No statistically significant changes with respect to FMPS, FMBS, PPD and width of KG were observed. Conclusions: Immediate transmucosal implant placement represented a predictable treatment option for the replacement of teeth lost due to reasons including fractures, endodontic failures and caries.

Screw Joint Stability under Cyclic Loading of Zirconia Implant Abutments (지르코늄 임플란트 지대주의 나사결합부 안정성에 관한 연구)

  • Purpose: The purpose of this study was to evaluate the effect of abutment material on screw-loosening before and after cyclic loading. Among the different materials of abutments, zirconia and metal abutment were used. Material and methods: Two types of implant systems: external butt joint(US II, Osstem Implant, Korea) and internal conical joint(GS II, Osstem Implant, Korea) were used. In each type, specimens were divided into two different kinds of abutments: zirconia and metal(n=5). The implant was rigidly held in a special holding to device ensure fixation. Abutment was connected to 30 Ncm with digital torque gauge, and was retightened in 30 Ncm after 10 minutes. The initial removal torque values were measured. The same specimens were tightened in 30 Ncm again and held in the cycling loading simulator(Instron, USA) according to ISO/FPIS 1480. Cycling loading tests were performed at loads 10 to 250 N, for 1 million cycles, at 14 Hz,(by subjecting sinusoidal wave from 10 to 250 N at a frequency of 14 Hz for 1 million cycles,) and then postload removal torque values were evaluated. Results: 1. In all samples, the removal values of abutment screw were lower than tightening torque values(30 Ncm), but the phenomenon of the screw loosening was not observed. 2. In both of the implant systems, initial and postload removal torque of zirconia abutment were significantly higher than those of metal abutment(P<.05). 3. In both of the implant systems, the difference in removal torque ratio between zirconia abutment and metal abutment was not significant(P>.05). 4. In metal abutments, the removal torque ratio of GS II system(internal conical joint system) was lower than that of US II system(external butt joint system)(P<.05). 5. In zirconia abutments, the difference in removal torque ratio between the two implant systems was not significant(P>.05). Conclusion: Zirconia abutment had a good screw joint stability in the condition of one million cycling loading.

The effect of loading time on the stability of mini-implant (교정력 부하시기에 따른 교정용 미니 임플란트의 안정성)

  • Objective: The purpose of this study was to investigate the stability of mini-implants in relation to loading time. Methods: A total of 48 mini-implants (ORLUS, Ortholution, Korea) were placed into the buccal alveolar bone of the mandible in 8 male beagle dogs. Orthodontic force (200-250gm) was applied immediately for the immediate loading group while force application was delayed for 3 weeks in the delayed loading group. For the subsequent loading periods (3, 6, 12 weeks), BIC (bone implant contact) and BV/TV (bone volume/total volume) and mobility test were carried out. Results: The immediate loading group showed no changes in BIC from 3 to 12 weeks, while the delayed loading group showed a significant increase in BIC between 3 and 12 weeks (p<0.05). The BV/TO of the delayed loading group significantly increased from 6 to 12 weeks of loading (p<0.05), while the BV/TV of the immediate loading group decreased from 3 to 12 weeks of loading. However, there was no significant difference in BV/TV between experimental groups. The mobility of the immediate loading group was not significantly different from that of the delayed loading group after 12 weeks of loading (p<0.05). Conclusions: These results showed that immediate loading does not have a negative effect on the stability of mini-implants compared to the early loading method in both the clinical and histomorphometric point of view.

  • There are various approaches to treatment for patients with fully edentulous jaw. In maxilla, it is easy to obtain sufficient stability, support, and retention with the traditional full denture method. In mandible, however, there are difficulties caused by anatomical limitations. In this case, treatment with implants can be considered. If it is difficult to place implants in posterior area, a fixed complete denture can be considered, using 4 - 5 implants anterior to mental foramen. This approach has changed in many ways, such as surgical methods and materials used, over the years. This case report compares two approaches and their changes over time, especially in the cases of mandibular fixed prostheses using implants, paired with maxillary complete denture.

  • Anatomical changes in the facial and alveolar bones occur after multiple teeth are extracted. In the maxilla, the alveolar bone is absorbed in the direction and inclination of the root, and the remaining alveolar bone becomes shorter, reducing the diameter of the arch. In addition, as the nasolabial angle increases, the support of the lips and the aesthetics of the face are lost. This case reports a functional and aesthetically satisfactory results of full mouth rehabilitation with the implant-supported fixed prosthesis using a zirconia framework.
