Psychiatric Evaluation of Maladaptive Male Conscripts in a Division of the Republic of Korean Army : Focus on Green Camp Participants (한국 육군 일개 사단에서 군 복무 부적응자들의 정신의학적 평가 : 그린캠프 참가자들을 중심으로)

  • Objectives : In this study, we aimed to elucidate the demographic and psychological characteristics of maladaptive soldiers in the Republic of Korean Army. Methods : Study participants included 110 male conscripts who had participated in the Green camp, which was a form of group psychosocial treatment program for maladaptive soldiers. All participants were interviewed and diagnosed by a psychiatrist, and classified into two groups according to whether they left or rejoined the army. We analyzed the differences between these two groups in terms of demographic features, psychiatric diagnosis and the Minnesota Multiphasic Personality Inventory (MMPI) subscores. Results : Compared with soldiers who rejoined their units, those who left military service exhibited significantly lower educational level (p=0.041) and a higher rate in psychiatric treatment prior to enlistment (p=0.011). Among the 106 subjects, 63 (59.4%) and 23 (21.7%) were diagnosed with personality disorder and adjustment disorder, respectively. Further, those who left military service were diagnosed more frequently with mood disorder (p=0.001) and schizophrenia (p=0.014) than those who rejoined their units. Additionally, the MMPI scores of soldiers who left military service were significantly higher on the psychasthenia (p=0.028) and schizophrenia (p=0.039) scales than those of soldiers who rejoined their units. Conclusion : Most of the maladaptive soldiers were diagnosed with mental disorders. The results of this study suggest that systematic, consistent psychiatric evaluation and intervention for maladaptive male conscripts is crucial in the Republic of Korean army.

Analysis of the Five-Factor Model of Personality in Obsessive-Compulsive Disorder (강박장애 환자에서의 성격 5요인 모델 분석)

  • Objective : The aim of this study is to evaluate the five-factor model of personality in patients with obsessive-compulsive disorder (OCD) related to obsessive-compulsive symptom severity and the distinct symptom subgroups. Methods : We recruited 95 patients with OCD and 116 normal controls in the study. We used the short version of Revised NEO Personality Inventory and the Yale-Brown Obsessive Compulsive Scale (Y-BOCS) for the assessment. Results : Patients with OCD showed significantly higher scores in neuroticism than normal controls. On multiple linear regression analysis, we found that agreeableness and conscientiousness were associated with the total Y-BOCS scores. On subscale analysis, agreeableness and neuroticism were associated with the obsession subscale scores and only conscientiousness was associated with the compulsion subscale scores. Furthermore, we found that patients who have contamination/cleaning or symmetry/ordering/counting/arranging as a main symptom presentation had significantly higher mean scores in conscientiousness than patients who have harm due to injury/violence/aggression as a main symptom presentation. Conclusion : In this study, we found that specific personality factors are associated with the obsessive-compulsive symptom severity. In addition, this is the first study to investigate the relationship between the personality factors in the five-factor model and the distinct symptom subgroups in OCD.

A Comparative Analysis on the Development and Maladaptive Behavior at Daycare Centers by Children of Multicultural and Non-multicultural Family Background (다문화 아동과 일반 아동의 발달 및 어린이집 부적응행동 비교 분석)

  • In this study, WSS developmental checklist and nursery maladaptive test were conducted and applied to 203 multicultural children and non-multicultural children. The children were 3-5years old and were attending 34 kindergartens located in Gimpo-si and Ansan-si, Gyeonggi-do to investigate the developmental level and status of adaptation to the nursery in multicultural children. The result shows that there is no difference in the development of scientific thinking and mathematical thinking while developmental level is lower in personality, sociality, language, reading and writing area in multicultural children compared to non-multicultural children. According to age, there is no difference in the development between 4~5-year-old multicultural children and non-multicultural children while there is a difference in the development between 3-year-old multicultural and non-multicultural children. It also shows that there are no significant differences in nursery adaptation between multicultural children and non-multicultural children.

MMPI Characteristics of the Sexual Offender (성적 가해자에서 MMPI 특성)

  • Objectives : The current study investigated the personality characteristics of the sexual offender using the Minnesota Multiphasic Personality Inventory (MMPI) test commonly used in clinical medicine. Methods : 30 sexual offenders (30 male) completed the Korean version of the MMPI. 20 of the 30 sexual offenders were patients with paraphilia. The sexual offenders had been admitted to the Institute of Forensic Psychiatry Ministry of Justice, Gongju, for sexual violence. Results : The MMPI scores of the sexual offenders were significantly higher on the psychopathic deviate (Pd), masculinity-feminity (Mf), paranoia (Pa), psychasthenia (Pt), schizophrenia (Sc) than the comparison group. And The MMPI scores of the sexual offenders had higher values for hypochondriasis (Hs), depression (D), hysteria (Hy), hypomania (Ma), social introversion (Si) than the comparison group, but were not significantly higher. Conclusion : Sexual offenders may be antisocial, paranoid, schizoid or psychasthenic. They seem to have psychotic psychopathology rather than neurotic psychopathology. These results suggested that the psychopatholgy of sexual offenders may be different to the control group.

A Study of the Relationship of the CSEI (The Core Seven-Emotions Inventory) and MMPI-2 (Minnesota Multiphasic Personality Inventory-2) (핵심감정척도와 다면성 인성검사의 상관성연구)

  • Objectives: The purpose of this study was to examine the correlation between CSEI (The Core Seven-Emotions Inventory) and MMPI-2 (Minnesota Multiphasic Personality Inventory-2). Methods: We analyzed the correlation between students' demographic characteristics, blood type, MMPI-2, and CSEI using the SPSS (Statistical Package for the Social Science) 24.0. Descriptive Statistical Analysis, independent t-test, analysis of variance (ANOVA), subsequent analysis duncan (post hoc multiple comparison), and correlation analysis were conducted. Results: 1. According to the demographic characteristics of 91 participants in this study: 60 males (65.9%), 88 unmarried (96.7%), 58 without religion (63.7%), 54 who have experienced stress in the last three months (59.4%), and 82 in their 20s (90.1%) comprised the majority. 2. As a result of verifying correlation by subfactors of CSEI, Hui (喜) showed statistically significant negative correlation with U (憂), Bi (悲), and Gong (恐). Six emotions except Hui (喜) showed statistically significant positive correlation, except for the relationship between U (憂) and Kyeong (驚). 3. Hui (喜) of CSEI had negative correlation with eight factors of MMPI-2 Clinical Scales except Pa (Paranoia) and Ma (Hypomania), whereas Bi (悲) had positive correlation with nine factors of MMPI-2 clinical scales except Mf (Masculinity-Femininity). Sa (思), Bi (悲), and Gong (恐) had similar tendency of positive correlation with six factors of MMPI-2 Restructured Clinical Scales. Conclusions: Based on the above results, we concluded that CSEI's sub factors had consistent correlations with MMPI-2. Thus, CSEI could contribute to psychiatry clinical use.

Diagnostic Utility of Minnesota Multiphasic Personality Inventory-2-Restructured Form Scales: Distinguishing Social Anxiety Disorder, Panic Disorder, and Major Depressive Disorder (다면적 인성검사 II 재구성판(MMPI-2-RF) 척도의 진단적 유용성: 사회불안장애, 공황장애, 주요우울장애 비교)

  • Objective : This study aimed to find out whether the Minnesota Multiphasic Personality Inventory-2-Restructured Form (MMPI-2-RF) scales are useful in distinguishing social anxiety disorder, panic disorder, and major depressive disorder. Methods : The study sample included 118 patients: 33 with social anxiety disorder, 53 with major depressive disorder, and 32 with panic disorder. Participants were classified according to the diagnosis indicated on their medical records. MMPI-2-RF scores were derived from MMPI-2 protocols. Results : The results of multivariate analysis of variance showed that the elevated scales were consistent with the diagnostic and clinical characteristics of each diafnostic group. Logistic regression analyses identified several scales that were useful in differentiating the diagnostic groups. The higher Cognitive Complaints (COG) scale significantly differentiated major depressive disorder from the other groups. The higher Self-Doubt (SFD) scale and Somatic Complaints (RC1) scale were useful in differentiating social anxiety disorder and panic disorder respectively. The lower Cynicism (RC3) scale was also useful in differentiating social anxiety disorder. Other scales that were useful in distinguishing between pairs of groups were also identified. Conclusion : The results of this study suggest that the MMPI-2-RF scales can be useful for discriminating anxiety disorders.

CLINICAL CHARACTERISTICS OF CHILDREN AND ADOLESCENTS WITH CONDUCT PROBLEMS ADMITTED INTO A PSYCHIATRIC UNIT (소아정신과 병동에 입원한 품행장애 소아 청소년의 임상 특성)

  • Objectives:Conduct disorder is one of the most frequent diagnosis in outpatient and inpatient mental health facilities for children and adolescents. This study investigated the clinical characteristics of children and adolescents with conduct disorder. Methods:The study employed a retrospective review of 45 inpatients diagnosed with conduct disorder by DSM-IV criteria at SNUH Child Psychiatric Unit from 1993, March to 1998, September. Results:The results were as follows:1) The sex ratio was 2:1, male dominant. Mean age on admission was $12.8{\pm}2.4$ years old(male:$12.2{\pm}2.3$, female:$14.1{\pm}2.1$). The mean age of male subjects was significantly lower than that of female subjects(p<.05). 2) We classified all subjects according to child- or adolescent-onset type by DSM-Ⅳ. Childhoodonset type was 42.2%, and adolescent-onset, 57.8%. The onset of male subjects were significantly earlier than that of female subjects(p<.05). 3) The most common complaint was 'serious violation of rules'(77.8%) by DSM-Ⅳ dimensions, while the most common single symtom was 'run away from home overnight at least twice while living in parental or parental surrogate home'(48.9%). 4) The comorbid psychopathology of the subjects were frequently described as follows, in order of frequency:substance use(42.2%), ADHD(35.6%), depression(20.0%), tic disorder(11.1%), bipolar disorder(4.4%). 5) On MMPI, both Pd and Ma subscales got the highest scores. The mean of total IQ by KEDIWISC was $100.0{\pm}15.1$. Female subjects' IQ was higher than that of male subjects. 6) Regarding the number of visits to the OPD after discharge, many subjects(33.3%) had visited OPD fewer than four times. Conclusions:In this study, the male to female ratio of adolescent type showed a decreasing trend. An interesting finding was the fact that socio-economic circumstances, as well as the level of education among patients, were higher than those of previous studies. The subjects' problem were also principally self contered and posed no threat to others.

Feasibility of Ultrasonic Inspection for Nuclear Grade Graphite (원자력급 흑연의 산화 정도에 따른 초음파특성 변화 및 초음파탐상의 타당성 연구)

  • Graphite material has been recognized as a very competitive candidate for reflector, moderator, and structural material for very high temperature reactor (VHTR). Since VHTR is operated up to $900-950^{\circ}C$, small amount of impurity may accelerate the oxidation and degradation of carbon graphite, which results in increased porosity and lowered fracture toughness. In this study, ultrasonic wave propagation properties were investigated for both as-received and degradated material, and the feasibility of ultrasonic testing (UT) was estimated based on the result of ultrasonic property measurements. The ultrasonic properties of carbon graphite were half, more than 5 times, and 1/3 for velocity, attenuation, and signal-to-noise (S/N) ratio respectively. Degradation reduces the ultrasonic velocity slightly by 100 m/s, however the attenuation is about 2 times of as-receive state. The results of probability of detection (POD) estimation based on S/N ratio for side-drilled-hole (SDHs) of which depths were less than 100 mm were merely affected by oxidation and degradation. This result suggests that UT would be reliable method for nondestructive testing of carbon graphite material of which thickness is not over 100 mm. In accordance with the result produced by commercial automated ultrasonic testing (AUT) system, human error of ultrasonic testing is barely expected for the material of which thickness is not over 80 mm.

A Study of Esophageal Acidity and Motility Change after a Gastrectomy for Stomach Cancer (위암 환자의 위절제술 후 식도산도의 변화와 운동장애)

  • Purpose: Some patients develop gastroesophageal reflux disease (GERD) after a gastrectomy for stomach cancer. Therefore, we conducted this research to gain an understanding of esophageal acidity and motility change. Materials and Methods: From July 2002 to March 2004, the cases of 15 randomized patients with stomach cancer who underwent a radical subtotal gastrectomy (RSG) with Billroth I(B-I) reconstruction (n=12) or a radical total gastrectomy (RTG) with Roux-en-Y (R-Y) gastroenterostomy (n=3) were analyzed. We investigated the clinical values of the ambulatory 24-hour pH monitoring and esophageal manometry in these patients, just before discharge from the hospital after an operation. Results: GERD was present in three patients ($20\%$). Compared with two reconstructive procedures, 3 of the 12 patients in the RSG with B-I group had GERD; however, none of RTG with R-Y group had GERD. Compared with pathologic stage, 2 of 9 patients in stage I, 1 of 2 patients in stage II, none of 3 patients in stage III, and none of 1 patient in stage IV had GERD. Esophageal manometry was performed in 10 patients. Nonspecific esophageal motility disorder (NEMD) was present in 7 patients. Conclusion: Some patients had GERD as a complication following a gastrectomy for stomach cancer. We suspect that the postoperative esophageal symptom is due to not only bile reflux but also gastroesophageal acid reflux. Therefore, careful observation is recommended for the detection of GERD.

Bronchial Hyperresponsiveness in Chronic Renal Failure Undergoing Hemodialysis (만성 신부전 환자에서 혈액투석 전후의 기관지 과민반응검사)

  • Background: Cardiogenic pulmonary edema increases nonspecific airway responsiveness in humans and animals. Increased extravascular lung water from overt pulmonary edema to subclinical interstitial edema is a common finding in patients with chronic renal failure. Several studies carried out to assess pulmonary function disturbances in this condition have documented a reduction in forced expiratory volume that usually reverses after hemodialysis, suggesting airway edema as the underlying mechanism. This interstitial edema may also lead to nonspecific bronchial hyperresponsiveness. We hypothesized that patients with chronic renal failure may present nonspecific bronchial hyperresponsiveness due to subclinical interstitial pulmonary edema. Methods: We studied 18 chronic renal failure undergoing regular hemodialysis 3 times a week(New York Heart Association Class II) without concomittent disease. These patients were checked pulmonary function test and metacholine provocation test before hemodialysis and same procedure was repeated if responsive, after hemodialysis. Results: 1) 12 out of 18 patients before hemodialysis were reactive in metacholine provocation test(66.7%) before hemodialysis. This airway hyperresponsiveness were decreased after hemodialysis. 2) Pulmonary function was improved after hemodialysis and change in $FEV_1$ was correlated with change in weight(r=-0.62, p<0.01). 3) There was a close correlation between log $PD_{20}$ and $FEF_{25}$, which is one of the variables of the peripheral airways(r=0.58, p<0.05). Conclusion: We speculated interstitial pulmonary edema may play a significant role in bronchial hyperresponsiveness and lung function impaired in patients with chronic renal failure.
