A Study on the Prioritization of Medical Device using Fuzzy-AHP (Fuzzy-AHP를 활용한 미래유망 의료기기 우선순위 도출)

  • According to the aging, the medical device industry is focused as a future promising industry. However, Korea medical device industry is not enough market competitiveness due to a narrow domestic market and a small company structure. This study aims at evaluating medical device priorities following 3 steps. First, we classify the medical device into three hierarchy categories and AHP survey was conducted on 30 experts in order to extract medical device priorities. Second, priority scores of medical device are analysed using AHP and Fuzzy-AHP. Third, a most important medical device is selected by comparing the volume of medical device manufacture and priority scores. As a result, 'dental implant' is the most import medical device, and we suggest a strategy based on a positioning map. The proposed methodology will provide a inspiration for establish of R&D and support policy in the medical device industry.

Analysis of Maternal and Neonatal Transport by the 1339 Emergency Medical Information Center in Busan Area (부산 지역 응급의료정보센터를 통한 산모와 신생아 전원에 대한 연구)

  • Purpose: In relation to perinatal healthcare, medical institutions and resources are limitative and also are in a state of flux due to the therapeutic specialty. We analyzed requests for interhospital transfers received by Busan 1339 Emergency Medical Information Center (EMIC) to grasp the state of perinatal healthcare delivery system. Methods: This study was conducted on the basis of data inputted into the computing system of Busan 1339 EMIC, between January 1 and December 31, 2009. In connection with 378 pregnant women and 136 newborns who were required to transfer, retrospective analyses were made of the success rate of transfer (SR), the number of contacted hospitals, the time required for transfer and the reason of transfer and refusal. Results: In the case of pregnant women, the SR were 65.5%. They came in contact with 2.7 hospitals, and it took 24.4 minutes. As for the reason of transfer, preterm labor accounted for the highest proportion. In the case of newborns, the SR were 71.3%. They came in contact with 2.4 hospitals, and it took 15.6 minutes. The most common reason of transfer were respiratory symptoms. In the reason of refusal with pregnant women and newborn, the lack of medical staff, medical equipments and wards accounted for great. Conclusion: Many pregnant women and newborns have been transferred to hospitals by EMIC, but the SR has not been higher yet. Accordingly, there is a need to evaluate the propriety of perinatal treatment system, as well as to set up effective perinatal healthcare delivery system.

  • The purpose of this study is to contribute to developing psychological support system in disaster response of South Korea by examining that of Japan. Psychosocial support from the Red Cross with the psychiatric support by the DPAT from local government were achieved in the early stage of disaster psychology support system in Japan. In the long term intervention after disaster, psychiatric support from Kokoro-no care center has been established. In other words, not only traditional mental health approach but also disaster psychology support activities including psychosocial support are deployed. National Information Center of Disaster Mental Health, which is control tower of disaster psychology support, understand the disaster in the level of nationwide through collecting and analyzing the data related to disaster, and manage disaster psychological support activities. Disaster psychology support system in Japan modified problems immediately during large scale disaster that they are equipped with state of the current. Implications for disaster psychological support system and crisis counseling program in South Korea are discussed.

  • 최근 새로운 정보기술의 패러다임으로 이슈가 되고 있는 유비쿼터스 컴퓨팅이 기술은 행정, 경제, 의료, 교육, 문화 등 사회 곳곳에 적용될 것으로 예상되고 있으며, 그에 따라 비즈니스 모델에 대한 중요성도 증대되고 있다. 그러나 기초 기술, 표준화, 보안 등에 관련된 연구가 중심을 이루고 있을 뿐, 비즈니스 모델과 관련된 연구는 미흡한 실정이다 본 연구에서는 유비쿼터스 컴퓨팅 환경에서 등장하는 다양한 비즈니스 모델에 대해 사업화 이전 단계에서 보다 효과적으로 사업화 가능성을 평가함으로써 사업자가 유망한 비즈니스 모델의 도출이 가능하도록 하는 평가체계를 제시한다.

충북지역 보건진료원이 업무 분석적 연구

  • 본 연구는 보건진료원의 업무를 분석하기 위하여 충북에서 활동하고 있는 보건진료원 136 명을 대상으로, 설문지를 흥해 자료수집을 하였으며 백분률, 평균값, 표준편차로 자료처리를 하였다. 조사기간은 1968 년 8월 1일부터 11월 20일까지 했으며 그 결과를 요약하면 다음과 같다. 1. 보건진료원의 일반적 특성과 업무에 관련 된 배경을 살펴보면 다음과 같다. 보건진료원의 일반적 특성으로 연령 분포는 30 - 34세가 30.1 %로 가장 많았고 결혼 상태는 기혼자가 61.8 % 였다. 종교는 기독교가 42.6% 로 많았고 학력은 3 년제 간호전문대학 과정졸업 자가 78.9 %로 많았다. 현 가족과 동거상태에서 배우자와 자녀가 함께 사는 경우가 76.5% 였고, 보건진료원의 경력은 1 년 마만이 41.2 %로 가장 많았다. 보건진료원의 업무에 관련된 배경을 살펴보면, 근무지역조건은 갑지가 65.3 % 였고, 현 거주지는 보건진료소내 숙소가 대다수 였다. 보건진료소는 신축된 건물이 대다수였고, 보건진료소 시설은 '그저 그렇다'고 대답한 수가 가장 많았다. 보건진료소 운영협의회 수당은 월 $5\sim7$만원인 경우가 85.6 % 였다. 2. 보건진료원의 보건의료제공 실태는 다음과 같다. 보건진료원이 현재 담당하는 주민수는 $501\sim1,000$ 명이 35.3 % 이고, 1,500 명 이상의 주민을 담당하는 경우도 32% 였다. 보건진료소를 이용한 주만은 연평균 1,956 명이고, 보건진료소 평균 이용건수는 1,812건이다. 보건진료원의 직업만족도는 만족한다가 40.6%로 대체로 긍정적으로 나타났다. 3. 보건진료원과 보건의료 관련기관과의 관계는 다음과 같다. 지역주민의 보건의료자원 이용 정도는 보건소 진료소가 1 위로 가장 많았고, 약국, 약방, 의원, 보건지소의 순위로 나타났다. 보건의료 관련기관과의 협조가 잘 되고 있지 않다고 응답한 경우도 있는데, 보건소(19.7 %), 보건지소( 20.2%) 민간보건 의료기관(22.4 %)이며, 그 이유를 알아서 협조가 잘 이루어지도록 노력해야겠다. 원하는 감독체계는 보건소장(47.3 %), 보건간호 전문가(37.2 %)로 나타났으나 이는 현재있는 감독체제내에서 생각한 것이며 간호전문가에 의한 새로운 감독체제를 원하고 있었다. 많이 이용하는 의뢰 기관은 병원(52 %), 의원(24.4 %), 보건소(10.2 %) 순이었다. 4. 보건진료원의 업무 영역별 수행정도를 살펴보면 다음과 같다. 보건진료원의 업무 영역을 7개 영역으로 구분하여 수행평균 평점을 보면통상질환 관리가 3.70점, 사업운영 관리 및 지도가 3.44 점, 사업계획 수립이 3.19 점, 지역사회 조직 및 개발이 3.17 점, 모자보건 및 가족계획이 3.14 점, 지역사회 보건관리가 3.09 점, 보건 정보체계 개발이 3.02 점으로 나타났다. 위의 결과에서 지역사회 보건관리가 3.09점으로 통상질환 관리 3.70 점 보다 낮게 나타났는데 이는 일차보건의료 사업 정착의 과도기적인 현상으로 생각하며, 이 부분을 더 많이 수행해야 할 것으로 사려된다. 5. 보건진료원의 업무 영역별 수행 소요시간의 평균은 다음과 같다. 지역사회 조직 및 개발 3.48 시간, 사업계획 수립 3.55 시간, 지역사회보건 관리 6.74 시간, 모자보건 및 가족계획 5.55 시간, 통상잘환 관리 15.94시간, 사업운영 관리 및 지도 4.39시간, 보건정보체계 3.62 시간으로 나타났다. 보건진료원이 통상질환 관리와 더불어 예방활동을 효율적으로 할 수 있도록 적절한 행정적, 교육적 지원이 있어야 한다고 생각한다.

  • The threats to privacy and security have received increasing attention as ubiquitous healthcare applications over the Internet become more prevalent, mobile and universal. In particular, we address the communication security issues of access sharing of health information resources in the ubiquitous healthcare environment. The proposed scheme resolves the sender and data authentication problem in information systems and group communications. We propose a novel key management scheme for generating and distributing cryptographic keys to constituent users to provide form of data encryption method for certain types of data concerning resource constraints for secure communications in the ubiquitous healthcare domains.

  • 건강과 복지에 대한 사람들의 관심이 증대함에 따라 고령층 인구를 중심으로 병원에서 건강 점검 및 진료가 이루어졌던 기존의 Health산업 패러다임에서 연령층에 관계없이 언제, 어디서나 진료를 받을 수 있는 u-Health산업 패러다임으로 변화하고 있다. 이러한 흐름에 발맞추어 유비쿼터스 컴퓨팅 환경에서 혈압, 맥박, 심전도 등과 같은 의료정보뿐만 아니라 일상생활에서 획득한 정보를 통해 건강증진, 질병예방, 진료를 가능하게 하는 u-LifeCare 의료체계가 필요하다. u-LifeCare 를 지원하는 미들웨어는 유비쿼터스 환경에서 필수적인 기술 요소인 센서 네트워크 미들웨어와 상황인지 미들웨어를 통합하는 미들웨어이며, 라이프케어의 특성을 고려한 '초경량(LightWeight)의, 확장성(Scalability)있는, 보안성(Security)을 갖춘' 미들웨어이다.

  • Lots of costs threaten the sustainability of the national health-guarantee system. Despite research by the national center for disease control and prevention on health care dynamics with its auditing systems, there are still restrictions of time limitation, sample limitation, and, target diseases limitation. Against this backdrop, using huge volume of total data, many technologies could be fully adopted to the preliminary forecasting and its target-disease expanding of health. With structured data from the national health insurance and unstructured data from the social network service, we attempted to design a model to predict disease. The model can enhance national health and maximize social benefit by providing a health warning service. Also, the model can reduce the advent increase of national health cost and predict timely disease occurrence based on Big Data analysis. We researched related medical prediction cases and performed an experiment with a pilot project so as to verify the proposed model.

  • As the ARRA enacted recently in the United States, the interest in EHR systems have been increased in the field of medical industry. The passage of the ARRA presents a program that provides incentives to office-based physicians and hospitals adapting the EHR systems to guarantee interoperability with various medical standards. Thanks to the incentive program, a great number of EHR systems have been developed and lots of office-based physicians and hospitals have adapted the EHR systems certified by CCHIT. Keeping pace with the rapid changes in the market of healthcare, some enterprises try to push in to the United States healthcare market based on the experience acquired by developing EHR systems for hospitals in Korea. However, the developed system must be customized because of the different medical environment between Korea and the United States. In this paper, therefore, we design and develop an integrated EHR platform to guarantee the interoperability between different medical information systems based on medical standard technologies. In the developed platform, an integrated system has been composed by integrating various basic techniques such as data transmission standards and its methods, medical standard terminologies and its usage, and knowledge management for medical decision-making support. Moreover, medical data can be processed electronically by adapting an HL7 interface engine and the terminologies for exchanging medical information and the standardization of medical information. We develop SeniCare, an EHR system for supporting ambulatory care of the office-based physicians, based on the platform, and we verify the usability of the platform by confirming whether SeniCare satisfies the criteria of "meaningful use" issued by CMS or not.

  • Hospital information system has been widely used and increased recently for a variety of many aspects. And Order-Communicating system(OCS), as like as hospital information system, has been used in many medical care facilities, which is simple and easily accessible, useful system. Also then the use of OCS in emergency care center in YUMC has been introduced since 1996. 10, above 70% of availability is noticed at present and increasing in the use rate, is considered that it is very simple and accurate, time-saving, widely applicable system. So authors say that, after the use of OCS in emergency care center, interhospital exchange of the patient's information and also accomplishment of EMSS can be possible.
