An Examination of Theories of Health Behaviors for Guiding Research on Mammogram Screening Practices for Korean Immigrant Women (건강행위이론의 사회문화적 적합성에 대한 논의;한인여성의 유방암 방사선 검사 행위와 관련하여)

  • 연구 목적: 1980년대 이후 미국 여성들의 유방암 조기 진단을 위한 방사선 검진율은 급속히 증가하였음에도 불구하고 유색 인종의 여성들은 여전히 조기 검진의 혜택을 받지 못하고 있다. 유색인종 여성들의 낮은 검진율을 설명하기 위해 여러 건강행위이론을 이용한 관련 요인들이 연구되어 왔다. 이 논문은 미국 보건 의료관련 연구에서 가장 많이 쓰이는 다섯 건강행위 이론을 유색 인종 여성, 특히 한국 이민 여성들의 유방암 조기검진 이행에 적용하기 위해 사회문화적 적합성을 평가하기 위해 고안되었다. 연구 방법: 네 종류의 데이터 베이스(CINAHL, MEDLINE, PsycINFO, Sociological Abstracts)를 이용한 심층적 문헌 고찰을 통해 각각의 이론으로 유방암 조기 검진을 설명한 연구들을 모두 분석하였다. 연구 결과: 각 이론들의 배경, 주요 요인, 그리고 유색인종의 유방암 조기 검진에서의 적용 연구 등을 분석하였다. 결론: 서양 문화권 속에서 개발된 각 이론들이 한국적 정서와 행동을 설명하는데 명확한 한계가 있으며 이러한 한계를 극복하기 위해서는 기존의 이론들을 면밀하게 재분석하여 한국적 특성을 담아낼 수 있는 새로운 이론의 도출이 요구된다.

Cancer Health Literacy and Cancer Screening Behaviors: A Systematic Review (암 건강정보문해력과 암 조기진단행위: 체계적 문헌고찰)

  • The purpose of this paper is to systematically evaluate the relationship between cancer health literacy and cancer screening behaviors using global data. Following the preferred reporting items for systematic reviews and meta-analyses review guidelines, full-text articles published on PubMed, CINAHL, Embase, and Cochrane Library were reviewed until 31 January 2021. The reference lists of all selected studies have been also included. The cancer health literacy and cancer-screening behaviors of the participants aged ≥18 years were assessed. Seventeen studies that met the inclusion criteria were included. The cancer screening behaviors included mammograms, clinical breast cancer examination, Papanicolaou test, colonoscopy, and PSA screening. Eleven studies revealed a statistically meaningful relationship between the cancer health literacy and cancer screening behaviors. Further research should focus on developing effective interventions and guidelines on cancer health knowledge. The research scope of all areas of cancer health literacy and cancer screening behaviors should be extended to improve the cancer screening rates and public health.

A Study on the Effect of Info Seeking on Breast Cancer Screening Intention: Focusing on HBM and Autonomous Motives (건강 정보 추구가 유방암 검진행동에 미치는 영향 연구: 건강신념모형과 자율적 동기를 중심으로)

  • This study was conducted to find ways to improve people's perceptions to prevent and detect breast cancer. Health belief model(HBM) was used to examine the relationship between. risk perception of breast cancer, and benefit perception, and information seeking. In addition, the role of autonomous motives was included in the model. The results shows that the more people perceived cancer as dangerous, or perceived cancer screeing as beneficial, the more likely they seek relavant health information. Also, high autonomous motives showed positive effects on info seeking. Information seeking behaviors also had a positive impact on health screening intentions.

Factors Affecting Early Detection Behaviors of Breast Cancer (외래내원여성의 유방암 조기검진행위에 영향을 미치는 요인)

  • Purpose: The purpose of this study was to identify factors affecting early detection behaviors of breast cancer such as breast self examination(BSE), breast physical examination, mammography. Method: The subjects were 141 women on an island and materials were collected through an organized questionnaire from March, to August 2002. The data were analyzed by using descriptive statistics, $x^2$ and logistic analysis by SAS program. Results: 52.7% of the subjects performed breast self examination, 67.2% did breast physical examination and 67.7% did mammography. That is, about 60% of the subjects performed early detection behavior to find the breast cancer. Practice of breast self examination was significantly correlated with experience of physician examination and mammography. The most significant factor on BSE was a normal salted diet, and the most significant factor on physical examination and mammography was the high education level of subjects. Conclusions: According to the results of this study, it suggests that intensive education and information strategies for breast cancer early detection need to be developed. In particular, early detection programs for lower educated women should be activated.

  • Purpose: The aims of this study were to evaluate early breast cancer screening rate of occupational health nurses and to analyze factors affecting their behavior. Method: We reviewed and analyzed the structured questionnaires of III respondents from 230 participants in continuing education of occupational health nurses. Result: Breast self examination was performed in 72.1%, clinical breast examination was received in 30.6% and mammography was received in 40.5% significant factors affecting breast self examination were age and the age of menarche. significant factors affecting clinical breast examinations for breast cancers were age and the place of residence, and factors affecting mammography were age, monthly income, alcohol intake, the age of menarche, and attitude about early screening for breast cancers. Conclusion: Early breast cancer screening rate of occupational health nurses was higher than that of general population of women. But the rate of clinical breast examination and mammogaphy were lower of less than 50% of respondents. Therefore occupational health nurses who are responsible for health management of working women need more education for themselves to promote the motivation and to increase the rate of participation in early breast cancer screening.

Psychosocial Factors Predicting Delayed Diagnosis of Breast Cancer : The Role of Marital Relationship Functioning (지연된 유방암 진단을 예측하는 정신사회적 요인 : 부부관계기능의 역할)

  • Objectives : Breast cancer has been the most prevalent female cancer in South Korea since 2001. Early detection of this disease is the most effective strategy for reducing mortality. The objective of this study was to identify factors which could predict advanced stage at diagnosis of breast cancer. Methods : Participants who were initially diagnosed with breast cancer and referred to the Stress Clinic of the Breast Cancer Center at Kyungpook National University Hospital were included. Through a semi-structured interview, the authors investigated psychosocial variables such as the extent of marital and family functioning and emotional-economic family burden as well as sociodemographic and health behavior-, health characteristic- and cancer-related variables. Results : Data were collected from 219 participants. One hundred and twenty(54.8%) subjects were diagnosed with advanced-stage breast cancer. Variables that were significantly different between the advanced-stage and early-stage groups included : monthly breast self examination(p<0.000), annual mammographic screening(p<0.000), mode of tumor detection(p<0.000), nature of the first symptoms(p<0.000), time to treatment after diagnosis(p<0.000), overloaded economic and family burden(p=0.018), marital functioning(p<0.000) and family functioning(p<0.00). Logistic regression analysis indicated that irregular annual mammography screening(OR=7.431 ; 95% CI 2.407-22.944) or a lack of screening(OR=25.299 ; 95% CI 7.855-81.482) and a dysfunctional marital relationship(OR=4.772 ; 95% CI 2.244-10.145) were significantly associated with advanced stage at diagnosis of breast cancer. Conclusions : We reconfirmed screening behavior to be a risk factor for delayed diagnosis of breast cancer. Our findings also emphasized the importance of psychosocial factors such as marital functioning in early detection of breast cancer. Psychiatric consultation in the area of martial functioning could be beneficial for increasing early detection in breast cancer.

Factors Affecting Active Early Detection Behaviors of Breast Cancer in Outpatients (외래내원 여성의 적극적 유방암 조기검진행위 영향 요인)

  • Purpose: This study was done to evaluate factors affecting active early detection behaviors of breast cancer and performance rate of breast self examination (BSE), physical examination and mammography. Methods: The participants were 264 women from an outpatient breast clinic of a university hospital and materials were collected from March 2007 to February 2008 using a structured questionnaire. The data were analyzed using $x^2$ test, logistic analysis. Results: The rate for BSE was 58.3%, for physical examination, 55.3% and for mammography experience, 63.4%. Women with all of these active early detection behaviors accounted for 31.8% of the participants. Various factors such as age, income, marital status, and menopause showed increased significant performance rate. The explanation power of logistic model was 48.5%, and was significant for age, income and health belief. Factors related to high performance rate were being over 40 years of age, high income and high health belief score. Conclusion: Active early detection behaviors were not high in spite of marked increases in breast cancer incidence. Encouragement for women practicing early detection behavior is important, but there is also a need to develop interest and support for the low performance group. More sustained education and public relations are needed to further improve active early detection behavior.

Factors Affecting the Frequency of Breast self-examination in Korean Immigrant Middle-aged Women in the U.S. (미국 이민 한국 중년 여성의 유방자가검진 빈도에 영향을 미치는 요인)

  • 본 연구의 목적은 유방자가검진(BSE)의 지식정도, 훈련도, 숙련도 및 빈도와의 관계를 파악하고, BSE 빈도를 가장 잘 예측하는 변수를 결정하기 위함이다. 연구대상자는 미국에 이민 온 40세-60세 사이의 중년 여성 58명으로 와싱톤주의 시애틀과 타코마 지역에 거주하고 있었다. 연구도구는 미국암협회의 BSE 안내책자를 기초로 하여 개발하였다. 자료는 자가보고형 질문지를 이용하여 한국교회 4곳으로부터 연구의 취지를 설명하고 연구대상자들의 연구동의를 얻은 후 우편으로 질문지를 회수하였다. 자료수집 기간은 2000년 3월 6일부터 2000년 5월13일이었으며. 질문지 회수율은 77%이었다. 수집된 자료는 SPSS를 이용하여 Frequency, Percentage, Pearson correlation, Regression analysis로 분석하였다. 연구 결과는 주요 변수사이의 관련성에서 그 이전연구 결과와 대개 일치하였다. 대상자의 BSE 빈도는 년간 10회-12회 실행이 21.08%이였고, 전혀 실행하지 않는 정도가 4.80%를 차지했다. BSE 빈도와 관련변수와의 상관관계에서 나이는 BSE 빈도와 역상관 관계(r=-.289, p=.038)를 나타냈고, BSE 훈련에 대한 지도(r=0521, p=.000). BSE 지식에 대한 기술(r=.573, p=.000) 및 BSE 숙련도(R=.694 P=.000)는 BSE 빈도와 정상관 관계를 보였다. 회귀분석 결과는 BSE 빈도의 가장 유의한 예측변수로서 BSE 지식에 대한 기술(beta=.5813. p=.003)로 지지되었다. 이 연구의 의의는 지역사회간호사가 이민 온 한국 중년여성들의 BSE실행에 대한 이해를 높임으로 유방암의 조기발견에 대한 예방적 행위를 증진시키는 데 유용한 기초 자료로 제공될 것이다.

Risk Factors and Early Screening Behavior for Breast Cancer in Rural Women (일 지역 여성의 유방암 위험요인과 유방암 조기검진 행위 분포)

  • Purpose: The aim of this study was to identify risk factors for breast cancer and early screening behavior in women in the community. Method: The participants were 125 women residing in W city. Data was collected using an instrument developed by the researchers. Analysis was done using descriptive statistics, and the $x^2$ test. Result: For risk based on the Gail Model, age (above 50 years) had a distribution of 24.8%, first degree family history, 4.9%, age at first full term pregnancy, 13.8%, and benign breast cancer history, 4.9%. For risk based on other common risk factors, menopause had a distribution of 20.7%, did not breast feed, 15.4%, history of HRT, 7.3%, meat preference, 35.0%, and history of smoking or drinking, 2.4% and 43.5%, respectively. There was a significant difference in BSE and mammography screening behavior ($x^2=22.5$, p<.00), but no difference in distribution of risk factors and screening behavior. Conclusion: For effective prevention of breast cancer, it is necessary to develop an instrument for risk assessment and, through assessment, select women at high risk. It is also necessary to provide education and appropriate recommendations on screening behavior.

Effect of an Integrated Breast Health Program for Pregnant Women on Knowledge, Attitude, and Early Screening Practice Related to Breast Cancer

  • Purpose: Breast cancer is becoming increasingly prevalent among young Korean women. During pregnancy, women's concern regarding their breasts heightens. Thus, pregnancy provides a window of opportunity for breast cancer prevention and management along with antenatal care. This study developed and evaluated an integrated breast health program for pregnant women. Methods: This study employed a non-equivalent control group and non-synchronized design (22 experimental, 29 control). Women pregnant for over 28 weeks participated. The two-session integrated breast health program focused on breast management during breastfeeding and education about breast cancer prevention and early screening. Results: During the early postpartum period (within three months after the program), there were statistically significant differences in knowledge and attitude about breast cancer and breast self-examination before and after the program. There were also statistically significant differences in BSE at 6 and 12 months after the program and mammography at 12 months after the program. However, there were no statistically significant differences in clinical breast examination and breast ultrasonography at 6 and 12 months after the program. Conclusion: The integrated breast health management program was effective in increasing knowledge and improving attitudes regarding breast cancer, BSE, and early screening practices among pregnant women. Further studies should consider providing breast health programs differently for each phase of pregnancy and continuing the same after delivery.
